Clinical and diagnostic complexities of adipsic arginine vasopressin deficiency.

Adipsic arginine vasopressin deficiency (AAVP-D) is a rare hypothalamic disorder, with inadequate vasopressin production and impaired thirst, resulting in sodium and water dysregulation. It poses multiple challenges, including increased morbidity and mortality. We present a 24-year-old female who developed confusion after neurosurgical debulking of a central neurocytoma. Investigations revealed hypernatremia alongside raised plasma osmolality, low urine osmolality, polyuria, and adipsia. Transient AAVP-D was suspected; it was successfully treated with intravenous desmopressin, which resolved p

Severe Hypernatraemia Secondary to Adipsia Associated With a Pituitary Macroadenoma in a Dog: A Case Report.

BACKGROUND: Adipsia is a rare disorder characterised by absence of thirst despite hyperosmolality, resulting from dysfunction of the hypothalamic thirst centre. In veterinary medicine, most reported cases of adipsia are associated with congenital forebrain abnormalities, whereas acquired causes in adult dogs are uncommon. CASE PRESENTATION: This report describes a 9-year-old neutered male mixed-breed dog presented with lethargy, anorexia, vomiting, recumbency, complete absence of voluntary water intake (adipsia) and intermittent tremors. Severe hypernatraemia (192 mmol/L) and hyperchloremia w

Intraoperative Diagnosis and Management of Arginine Vasopressin Disorder During Pituitary Tumor Resection via Transsphenoidal Endoscopic Navigation.

Arginine vasopressin (AVP) disorders (previously called diabetes insipidus) lead to excessive urination due to reduced antidiuretic hormone (ADH) secretion or kidney resistance to ADH. This results in decreased water reabsorption, causing dehydration and electrolyte imbalances. Diagnosing these disorders during general anesthesia is challenging, but close monitoring of electrolytes and urine output, especially during high-risk surgeries such as intracranial procedures, is crucial. A 64-year-old woman with a history of asthma presented with severe bifrontal headaches and left-eye medial gaze pa

Ifosfamide-Induced Partial Arginine Vasopressin Resistance Responsive to Vasopressin/Desmopressin and Amiloride.

This is a case of partial arginine vasopressin resistance following the sixth cycle of doxorubicin-ifosfamide-mesna therapy for recurrent spindle cell sarcoma of the thigh. Polyuria and symptomatic hypernatremia started by the second day of the two-day chemotherapy cycle. The diagnosis was confirmed with serum and urine chemistry testing showing urine hypo-osmolality (161 mOsm/kg) with polyuria (4.8 L urine output) in 24 hours, serum hyperosmolality (355 mOsm/kg), and an elevated baseline plasma copeptin of 82 pmol/L. Treatment with intravenous vasopressin followed by amiloride and supraphysio

Gastric mucosal necrosis caused by ingestion of crystalline sodium chloride: A case report.

Acute sodium chloride (NaCl) poisoning is a life-threatening condition primarily known to cause systemic hypernatremia. However, the local gastrointestinal effects of ingesting a large mass of crystalline NaCl are not well-documented. This case illustrates that crystalline NaCl can cause a severe corrosive injury pattern akin to that of classic caustic agents. We present a 42-year-old woman with abdominal pain and hematemesis after ingesting 200 g of crystalline NaCl in a suicide attempt. She had hypernatremia (155 mmol/L) and hyperosmolality (serum osmolality 324 mOsm/kg). An upper endoscopy

A Quick Reference on Hypernatremia.

Hypernatremia occurs when water loss exceeds sodium, leading to increased serum sodium and hyperosmolality, primarily affecting the brain. It often results renal losses, water evaporation, hypodipsia or restricted water intake. Clinical signs range from subtle neurologic changes to severe neurologic impairment. Management requires slow and careful correction of water deficits using hypotonic fluids, with frequent monitoring of serum sodium and osmolality to prevent complications like brain edema. Accurate diagnosis involves assessing water and sodium balance, understanding underlying causes, a

Pathophysiology and aetiologies of hypernatremia.

Hypernatremia is a disorder of water balance defined by a serum sodium concentration above 145 mEq/L. It reflects a relative deficiency of free water rather than sodium excess. Under physiological conditions, hypothalamic osmoreceptors maintain plasma osmolality through stimulation of arginine vasopressin release- which promotes renal water conservation- and stimulation of thirst, which drives fluid intake. Hypernatremia develops when these defences fail due to impaired vasopressin secretion or action, diminished thirst, or inability to access water. The resultant hyperosmolality causes osmoti

Hyperosmolar hyperglycemic state with severe hypernatremia coexisting with central diabetes insipidus: A case report and literature review.

Diabetes insipidus is characterized by polyuria and polydipsia, often resulting from central or nephrogenic causes. In diabetic emergencies, hyperosmolar hyperglycemic state (HHS), severe hypernatremia, and ventricular fibrillation are life-threatening conditions that require prompt intervention. This report describes a 47-year-old male with poorly controlled diabetes mellitus, who developed coma, excessive thirst, polyuria, hyperglycemia (47.29 mmol/L), hypernatremia (195.6 mmol/L), and plasma hyperosmolality (385 mOsm/kg). Despite fluid resuscitation and insulin therapy, refractory hypernatr

Osmotic Demyelination Syndrome in Hyperosmolar Hyperglycemic State With Contrasting Initial Sodium Levels: A Report of Two Cases.

Osmotic demyelination syndrome (ODS) is typically associated with rapid correction of severe hyponatremia but is a rare complication of hyperosmolar hyperglycemic state (HHS). We describe two cases of ODS developing in HHS with contrasting initial sodium levels: one with profound hyponatremia (Na 112 mmol/L, corrected 167 mmol/L) and the other with hypernatremia (Na 149 mmol/L, corrected 177 mmol/L). Both patients exhibited severe hyperglycemia (2,398 and 1,277 mg/dL, respectively) and marked hyperosmolality (387 and 421 mOsm/kg) and were managed with cautious correction rates (1.2 and 1.0 mOs

When Thirst Ceases to Exist: A Case Report and Literature Review of Adipsic Diabetes Insipidus Following Coil Embolization of a Ruptured Anterior Communicating Artery Aneurysm.

Diabetes insipidus is a condition characterized by inappropriately dilute urine in the setting of serum hyperosmolality. The two predominant subtypes include central (from lack of vasopressin production) and nephrogenic diabetes insipidus (from renal resistance to circulating vasopressin). A common manifestation is the significant pursuant thirst from excessive polyuria. We present a case report and literature review of an infrequent variation of central diabetes insipidus known as adipsic (hypothalamic) diabetes insipidus, characterized by the absence of thirst, secondary to coiling of a rupt

Hyponatremia due to preserved non-osmotic arginine vasopressin secretion in adipsic diabetes insipidus: a case report with review of literature.

Adipsic diabetes insipidus (ADI) is characterized by central diabetes insipidus and an impaired thirst response to hyperosmolality, leading to hypernatremia. Hyponatremia observed in patients with ADI has been considered a complication of desmopressin therapy. Herein, we present a case of impaired thirst sensation and arginine vasopressin (AVP) secretion without desmopressin therapy, in which hyponatremia developed due to preserved non-osmotic AVP secretion. A 53-year-old woman with hypopituitarism, receiving hydrocortisone and levothyroxine, experienced hyponatremia three times over 5 months

Nephrogenic diabetes insipidus: a comprehensive overview.

Nephrogenic diabetes insipidus (NDI) is characterized by the inability to concentrate urine that results in polyuria and polydipsia, despite having normal or elevated plasma concentrations of arginine vasopressin (AVP). In this study, we review the clinical aspects and diagnosis of NDI, the various etiologies, current treatment options and potential future developments. NDI has different clinical manifestations and approaches according to the etiology. Hereditary forms of NDI are mainly caused by mutations in the genes that encode key proteins in the AVP signaling pathway, while acquired cause

[Family form of nephrogenic X-linked diabetes indiabetes].

There is a global trend towards an increase in the prevalence of diabetes insipidus. Symptoms of nephrogenic diabetes insipidus with X-linked inheritance appear in men, in women with heterozygous mutations, are characterized by an isolated symptom complex of polyuria, polydipsia, hypostenuria. In children, more often than in adults, with fluid restriction, a clinic of water-deficient dehydration develops with hypernatremia, hyperthermia, and plasma hyperosmolality. Animal models for diabetes insipidus.

The hormone arginine vasopressin (AVP) is a nonapeptide synthesized by hypothalamic magnocellular nuclei and secreted from the posterior pituitary into the bloodstream. It binds to AVP receptor 2 in the kidney to promote the insertion of aquaporin channels (AQP2) and antidiuretic responses. AVP secretion deficits produce central diabetes insipidus (CDI), while renal insensitivity to the antidiuretic effect of AVP causes nephrogenic diabetes insipidus (NDI). Hereditary and acquired forms of CDI and NDI generate hypotonic polyuria, polydipsia, hyperosmolality, and hypernatremia. The AVP mutant (
