Severe Orthopnoea With Positional Paradoxical Breathing: A Response to Continuous Positive Airway Pressure.

A 54-year-old man presented with an abrupt onset disabling orthopnoea preventing supine sleep for several months. Cardiopulmonary imaging excluded heart failure, pulmonary embolism, interstitial lung disease, and upper airway obstruction. Pulmonary function testing demonstrated restrictive ventilatory impairment, and arterial blood gas revealed hypercapnia suggestive of hypoventilation. Neurological examination in the supine position showed paradoxical breathing with inspiratory thoracic expansion and inward abdominal movement, raising suspicion of diaphragmatic dysfunction. Sleep study demons

[Energy drink-induced severe hypokalemia and rhabdomyolysis].

A 37-year-old man developed severe hypokalemia and rhabdomyolysis after daily intake of over three cans of energy drink (≥ 480 mg caffeine). Clinical features included muscle weakness, leg paralysis, and ECG changes. Blood tests revealed severe hypokalaemia (P-potassium 2.0 mmol/l; ABG 1.5 mmol/l) and elevated creatine kinase levels. Treatment with intravenous potassium and fluids led to recovery. This case report highlights the risk of electrolyte disturbances from high caffeine intake and the need to inquire about energy drink consumption in patients with unexplained hypokalemia.

Optimal serum potassium concentrations in heart failure: an individual patient data meta-analysis.

BACKGROUND AND AIMS: It remains unclear what the safe serum potassium range is in heart failure (HF) and whether it is the same in HF with reduced ejection fraction (HFrEF) and HF with preserved ejection fraction (HFpEF). METHODS: A patient-level pooled analysis from 12 randomized controlled trials including 32 346 HFrEF and 13 723 HFpEF patients was performed. Baseline serum potassium level was categorized into six groups (<3.5, ≥3.5-<4.0, ≥4.0-<4.5, ≥4.5-<5.0, ≥5.0-<5.5, and ≥5.5 mmol/L) and serum potassium level at baseline was also analysed as a continuous variable using restricted cubic

Extracellular ionic concentration modulates arrhythmias in a simulated population of human ventricular myocytes with long QT syndrome type 3.

Ventricular arrhythmias are a major cause of sudden cardiac death and can occur when repolarization reserve is reduced by electrolyte imbalances and natural variation in ion-channel expression. Long QT syndrome type 3 (LQT3), caused by a gain-of-function mutation in the voltage-gated sodium channel, is an established exemplar; however clinical presentation of disease-associated symptoms can vary substantially. Here we examined how electrolyte state and intrinsic variability in ionic current conductances collectively influence arrhythmia risk. We simulated a population of human ventricular myoc

Chloride at the spotlight-but not alone: towards a multi-ionic diuretic strategy.

Diuretic resistance in heart failure and the cardiorenal syndrome is traditionally managed by escalating diuretic doses, yet this approach often fails to address the underlying pathophysiology. Emerging evidence challenges the sodium-centred paradigm and identifies chloride as a key determinant of diuretic responsiveness, tightly interconnected with potassium, magnesium, and acid-base balance. Disturbances in this multi-ionic network promote maladaptive tubular responses, neurohormonal activation, and persistent sodium retention, ultimately leading to diuretic resistance. Clinical and mechanis

Dual Aetiology of Diabetes Insipidus in Pregnancy: Vasopressinase-Mediated and Central Mechanisms.

Diabetes insipidus (DI) is a heterogeneous disorder characterised by polyuria and polydipsia due to impaired arginine vasopressin (AVP) secretion or action. We describe a 36-year-old pregnant woman who presented at 28 weeks' gestation with weakness, severe hypokalaemia from renal potassium wasting, hypernatraemia and polyuria with dilute urine. Vasopressinase-mediated DI was suspected; there was no response to AVP, but desmopressin (dDAVP) produced a rapid clinical improvement, confirming the diagnosis. However, postpartum recurrence of symptoms prompted further evaluation, with imaging sugges

Thyrotoxic periodic paralysis presenting with severe hypokalaemia and cardiac involvement.

A Caucasian man in his 20s with no known medical history presented to the emergency department with acute flaccid quadriparesis following a high-carbohydrate meal and strenuous physical activity. Initial evaluation revealed profound hypokalaemia (1.7 mmol/L; reference 3.5-5 mmol/L), marked QTc prolongation (594 ms), and mild troponin I elevation (51 ng/L; reference <40 ng/L). He reported two similar episodes over the preceding year that had been attributed to dehydration. Thyroid function testing demonstrated severe hyperthyroidism with suppressed thyroid-stimulating hormone (<0.003 mIU/L; ref

High-dose insulin euglycaemic therapy (HIET): How high is high enough and what are the risks?

ABSTRACT: Calcium-channel blockers (CCBs) are widely prescribed in South Africa and are frequently implicated in overdose-related morbidity and mortality. We report a case of amlodipine overdose in a teenager. Her management is notable for the exceptionally high dose of insulin used as part of high dose insulin euglycaemic therapy (HIET), far exceeding standard protocol. Despite the aggressive dosing, no clinically significant hypoglycaemia or hypokalaemia occurred. The patient showed progressive improvement and was discharged from the intensive care unit on day 3, making a full recovery. This

Beyond adrenal suppression: a comprehensive post-marketing safety profile of etomidate from real world data.

BACKGROUND: Etomidate is a widely used intravenous anesthetic, yet concerns persist regarding its potential for adverse drug reactions (ADRs) beyond well-known adrenal suppression. Pre-marketing trials may not fully capture its long-term or rare safety profile, necessitating continuous post-marketing surveillance. METHODS: A retrospective pharmacovigilance study was performed based on the U.S. FDA Adverse Event Reporting System (FAERS) database (Q1 2004-Q2 2025). Disproportionality analysis using ROR, PRR, BCPNN and EBGM algorithms was conducted and the corresponding 95% CIs were calculated.

Home Parenteral Support in Severe Gitelman Syndrome: A Case Report.

Gitelman syndrome (GS), also known as familial hypokalaemia-hypomagnesemia, is a rare autosomal recessive renal tubular disorder characterized by hypokalaemia, hypomagnesemia, metabolic alkalosis, and hypocalciuria. Electrolyte replacement of potassium and magnesium remains the mainstay of management, with oral supplementation achieving this in the majority of patients. We report the case of a 65-year-old woman with a severe manifestation of GS, managed with high-dose intravenous electrolyte replacement as part of a home parenteral support (HPS) regimen in addition to oral supplementation. Active surveillance and risk prediction of piperacillin/tazobactam‑associated hypokalaemia: development and external validation of a clinical nomogram.

INTRODUCTION: Piperacillin/tazobactam (TZP) is widely used in hospitalised adults. Real‑world data suggest a higher risk of hypokalaemia than previously recognised. AIM: To investigate incidence, severity and risk factors of TZP‑induced hypokalaemia, and to develop and externally validate a nomogram for individualised risk prediction. METHOD: This retrospective study included hospitalised adults receiving TZP (2021-2025). TZP‑induced hypokalaemia was defined as serum potassium < 3.5 mmol/L ≥ 48 h after TZP initiation. Logistic regression identified predictors. A nomogram was constructed. Mod

When Treatment Becomes the Trigger: Fludrocortisone-Associated Hypokalaemia in an Older Adult.

Hypokalaemia is a common electrolyte abnormality in older adults and is frequently multifactorial in origin. Although often mild and asymptomatic, severe hypokalaemia can result in significant neuromuscular and cardiac complications. Medication-induced hypokalaemia remains an important and potentially reversible cause, particularly in frail elderly populations. We present the case of an 85-year-old woman who presented with generalised weakness, muscle cramps, reduced mobility, and a fall, and was found to have significant hypokalaemia in the context of long-term fludrocortisone therapy prescri

Prognostic Impact of Baseline Potassium Abnormalities in Patients with Heart Failure with Reduced Ejection Fraction: A Real-World Cohort Study.

INTRODUCTION: Abnormal serum potassium levels are a common issue in patients with heart failure with reduced ejection fraction (HFrEF), limiting the use of prognostically beneficial therapies. The primary aim of this study was to characterise the HFrEF population according to baseline potassium levels and to assess their impact on medium- to long-term prognosis. METHODS: A retrospective study was conducted on a cohort of consecutive patients with HFrEF recruited at our centre. Patients were classified according to baseline serum potassium levels into: Group 1 (<3.5 mEq/L), Group 2 (3.5-5.0 mE
