An interferonopathy associated with a monoallelic variant in TRIM28.

BACKGROUND: In this work, we describe the clinical and functional changes in a family with a novel type I interferonopathy phenotype which we suggest could be associated with a TRIM28 variant, presenting with chilblains, skin ulceration, lipodystrophy and calcinosis. TRIM28 has multiple roles in regulating human health and disease, which include DNA repair and anti-viral response. The latter includes negative regulation of IRF7, one of the master regulators of the interferon response. METHODS: Whole exome sequencing and targeted panel sequencing was performed on DNA extracted from peripheral

[Advances in the application of Janus kinase inhibitors for the uncommon pediatric dermatological disorders].

Janus kinase (JAK) inhibitors have emerged as a research hotspot in pediatric inflammation-mediated autoimmune skin diseases, owing to the ability to target and block the JAK-signal transducer and activator of transcription (JAK-STAT) signaling pathway. This article summarizes the mechanism of action and approved indications of JAK inhibitors, and systematically reviews the efficacy and safety data in pediatric uncommon skin diseases such as lichen planus, connective tissue diseases, hidradenitis suppurativa, haploinsufficiency of A20, and chronic atypical neutrophilic dermatosis with lipodyst

Recognizing Mandibular Hypoplasia, Deafness, Progeroid Features, and Lipodystrophy Syndrome Through Its Cutaneous Manifestations: Distinction From Werner Syndrome.

Mandibular hypoplasia, deafness, progeroid features, and lipodystrophy syndrome (MDPL) is an extremely rare autosomal dominant progeroid disorder caused by pathogenic variants in POLD1. Because MDPL shares many clinical features with Werner syndrome (WS), distinguishing between the two disorders can be challenging. We report a Japanese man with genetically confirmed MDPL caused by the recurrent POLD1 p.Ser605del variant, highlighting its distinctive cutaneous manifestations. The patient exhibited mandibular hypoplasia, limb-predominant lipodystrophy, skin atrophy, painful plantar clavi and cal

Metreleptin improves glucose metabolism and hepatic steatosis in a patient with Werner syndrome and partial lipodystrophy: a case report with review of literature.

Werner syndrome (WS) is a rare autosomal recessive progeroid disorder caused by mutations in the WRN gene and is characterized by distinctive physical features and metabolic complications, including insulin resistance, diabetes, hypertriglyceridemia, and hepatic steatosis. Although lipodystrophy is a recognized component of WS, clinical reports of metreleptin use in this setting remain very limited. We report the case of a 38-year-old woman with WS complicated by partial lipodystrophy, diabetes with suboptimal glycemic levels, hypertriglyceridemia and severe hepatic steatosis. She presented wi

Reading the Lipid Code: Cooperative Domain Functions in Perilipin Targeting to Lipid Droplets.

Perilipins (PLINs) are the most abundant proteins coating the phospholipid monolayer of lipid droplets (LDs), regulating their biogenesis, stability, growth, and lipolysis. Most mammalian isoforms (PLIN1-3, PLIN5) share a modular architecture characterized by an N-terminal PAT domain (perilipin-adipophilin-TIP47), a central 11-mer repeat region forming amphipathic helices (hydrophobic and hydrophilic faces on opposite sides of the helix), and a C-terminal 4-helix bundle; PLIN4 is a notable exception that relies mainly on expanded amphipathic helices. In this review, we synthesize recent struct

Expert Recommendations on Insulin Injection Technique and Education for the Pediatric and Young Adult Population with Diabetes: FITTER Forward Youth.

Rates of type 1 diabetes (T1D) and type 2 diabetes (T2D) in the pediatric population have risen over the past two decades. Insulin therapy remains critical for diabetes management, and proper injection technique has been shown to improve injection experience, glycemic management, and health outcomes. Youth with diabetes (YWD), ranging from infancy to young adulthood, require unique considerations for injection technique and education best practices due to their distinct physiological, developmental, emotional, psychological, and psychosocial needs relative to adults. Considering these needs, c

Liraglutide use in congenital generalized lipodystrophy: glycemic and metabolic outcomes.

OBJECTIVE: Evidence on glucagon-like peptide 1 receptor agonists (GLP-1RAs) in lipodystrophies is limited. We evaluated the effects of liraglutide on metabolic control and satiety in patients with congenital generalized lipodystrophy (CGL). METHODS: In this case series, patients with CGL received liraglutide 1.8 mg/day for 12 weeks. Metabolic parameters, satiety, and body composition were assessed at baseline, week 12, and after a 12-week post-treatment follow-up (week 24). Liver imaging was performed at baseline and week 12. RESULTS: Eight patients were enrolled (aged 24-45 years; 75% femal

Reactive oxygen species-induced lipid peroxidation mediates Wnt-dependent lipodystrophy and associated skin fibrosis.

Skin fibrosis is marked by extracellular matrix accumulation and dermal white adipose tissue (DWAT), resulting in tissue stiffening. We show that sustained Wnt activation in mature dermal adipocytes induces lipodystrophy and promotes fibrotic remodeling via oxidative stress. Meta-analysis of early systemic sclerosis skin transcriptomes reveals increased reactive oxygen species (ROS) production and lipid peroxidation. In cultured mouse dermal adipocytes, Wnt activation triggers a robust increase in ROS and lipid peroxidation, which is suppressed by scavengers. In vivo, Wnt activation elevates o

Helios is critical for Treg effector differentiation in adipose autoimmunity.

The deletion of the transcription factor Helios in regulatory T cells (Tregs) in mice leads to systemic Th1 immune activation, decreased T follicular regulatory cells, and enhanced germinal center formation. We report here these mice also exhibit acquired lipodystrophy, hepatic steatosis, and insulin resistance. Adipose tissue exhibited lymphocytic infiltration followed by CD8+ T cell-mediated destruction of adipocytes. Helios-deficient Tregs were more frequent in adipose tissue and showed an enhanced effector Treg (eTreg) phenotype (CD62Llo Tcf1lo). The Helios-deficient eTregs in adipose tiss

Development of Antisense Oligonucleotide Gapmers for the Treatment of Dyslipidemia and Lipodystrophy.

Although technological advances in molecular genetics over the last few decades have greatly expedited the identification of mutations in many genetic diseases, the translation of the genetic mechanisms into a clinical setting has been quite challenging, with a minimum number of effective treatments available. The advancements in antisense therapy have revolutionized the field of neuromuscular disorders as well as lipid-mediated diseases. With the approval of splice-switching antisense oligonucleotide (AO)-based therapies for the treatment of spinal muscular atrophy (SMA) and Duchenne muscular

Development and Clinical Applications of Antisense Oligonucleotide Gapmers.

Antisense oligonucleotides (AONs) are synthetic oligonucleotides designed to bind target RNA and have opened new treatment possibilities for genetic diseases by regulating gene expression. AONs are often used to suppress the expression of mutated genes, which may interfere with essential downstream pathways. Since AONs have been introduced for clinical use, different chemistries have been developed to further improve efficacy, potency, and safety. One such chemistry is a chimeric structure of a central block of deoxyribonucleotides flanked by sequences of modified nucleotides. Referred to as a

Emerging livestock science and technologies can drastically alter the projections of global food security models.

The incidence and prevalence of all types of diabetes mellitus is increasing at an alarming rate. Modern therapy involves greater and earlier use of intensive insulin regimens in order to achieve better control of blood glucose levels and reduce the long-term risks associated with the condition. Insulin therapy is associated with important cutaneous adverse effects, which can affect insulin absorption kinetics causing glycemic excursions above and below target levels for blood glucose. Common complications of subcutaneous insulin injection include lipoatrophy and lipohypertrophy. The developme

Acquired generalized lipodystrophy complicated by fulminant type 1 diabetes mellitus: a case report with review of literature and immunological insights.

Acquired generalized lipodystrophy (AGL) is a rare disorder characterized by progressive loss of subcutaneous and visceral adipose tissue and severe metabolic disturbances, including diabetes mellitus (DM). Although AGL is typically associated with type 2 DM, clinical experience with patients affected by both AGL and Fulminant type 1 DM (FT1DM) remains limited. We report a case of a 50-year-old man with long-standing (4-year) FT1DM who developed AGL, with histopathological and immunological evaluation and long-term follow-up after metreleptin therapy. The patient presented with rapid deteriora

Refractory hypoparathyroidism in paediatric autoimmune polyglandular syndrome type 1: successful long-term use of teriparatide.

UNLABELLED: The concentration of calcium is carefully maintained under physiological conditions with parathormone, calcitonin and 1,25-dihydroxyvitamin D at appropriate levels. There are multiple causes that may bring about increased concentrations of calcium which exceed physiological values. Increased production of parathormone in parathyroid glands is only one of the possible causes. Malignant diseases are a very frequent cause of hypercalcemia, due to their creating mediators which stimulate osteoclasts and thereby osteolysis. A less frequent cause is represented by granulomatous processes

Serum Proteomics of Insulin Resistance Disorders Distinguish MASLD From Lipodystrophy and Insulin Receptor Defects.

BACKGROUND AND AIMS: Metabolic dysfunction-associated steatotic liver disease (MASLD) is linked to obesity, metabolic syndrome, and insulin resistance (IR). In IR associated with obesity or lipodystrophy (LD), caused by missing adipose tissue, hyperinsulinemia stimulates hepatic lipogenesis, causing steatotic liver disease (SLD). In contrast, insulin receptor pathogenic variants (INSR), despite hyperinsulinemia and IR, do not promote hepatic lipogenesis and steatosis. We aimed to understand SLD pathophysiology by comparing serum proteomic signatures of MASLD, LD, and INSR. METHODS: Single-cen
