A Twist in the Tale: Rare Diagnosis of Lipomatosis of the Appendix.

A 66-year-old white male with a history of diverticulosis and prediabetes presented with right lower quadrant (RLQ) abdominal pain lasting 1 month. On physical examination, the abdomen was soft without tenderness or palpable masses. CT imaging showed an enlarged appendix with a diameter of 1 cm with no surrounding inflammation. A laparoscopic appendectomy was performed, and the patient's symptoms resolved within 3 weeks. Histopathology revealed lipomatosis of the appendix, an extraordinarily rare diagnosis which has only been reported five times prior to our case. This case study aims to incre

Lipomatous pseudohypertrophy of pancreas with chronic calcific pancreatitis presenting with nutritional dermatosis and type 3c diabetes.

A woman in her early 50s presented with a 4-month history of erythema and scaling of the limbs, weight loss, anasarca and chronic foul-smelling diarrhoea, suggestive of nutritional dermatosis with steatorrhoea due to malabsorption. She was non-obese and had no prior history of diabetes or exocrine deficiency. During admission, she experienced abdominal pain and hyperglycaemia consistent with diabetic ketoacidosis. CT of the abdomen revealed a diffusely enlarged, entirely fatty pancreas with preserved shape and a dilated main pancreatic duct containing intraductal calculi, indicating lipomatous

Madelung Disease of the Scrotal Region: A Case Report.

INTRODUCTION: Madelung disease is a rare disorder characterized by symmetrical fat accumulation, typically around the neck, shoulders, and trunk. CASE PRESENTATION: A 56-year-old man presented for 10-month history of dysuria and painless swelling of the lower abdomen and scrotum. A large scrotal mass completely buried the penis, and cranially displaced both testes. Contrast-enhanced magnetic resonance imaging suggested a well differentiated liposarcoma but whole-body computed tomography revealed symmetrical fat accumulation in the upper body trunk, raising suspicion of Madelung disease. Prenatal Shwachman-Diamond Syndrome: Diagnostic Challenges in Two Unrelated Cases With a Rare Clinical Presentation and Pseudogene Interference, and a Review of the Literature.

This report describes two unrelated prenatal cases of Shwachman-Diamond syndrome (SDS) presenting primarily with severe skeletal anomalies. SDS is a rare autosomal recessive disorder characterized by a triad of bone marrow dysfunction, skeletal abnormalities, and exocrine pancreatic dysfunction. The most common postnatal features include faltering growth, short stature, and neutropenia resulting in recurrent infections. Prenatal presentations could be scarce as the most common features are typically not apparent before birth. Molecular diagnosis of SDS relies on the identification of biallelic

Risk Factors and an Interpretable Machine Learning Model for Predicting Spinal Epidural Lipomatosis: A Multicenter Study.

STUDY DESIGN: A retrospective multicenter study. OBJECTIVE: To identify independent risk factors for spinal epidural lipomatosis (SEL) and to develop and validate an interpretable machine learning-based predictive model. SUMMARY OF BACKGROUND DATA: SEL is an underdiagnosed yet clinically significant cause of debilitating lumbar spinal stenosis. Robust tools for early identification and risk stratification of at-risk patients are currently lacking. METHODS: Using data from 774 patients with low back and leg pain who underwent lumbar MRI at five institutions, we applied LASSO regression for v

Diffuse lipomatosis of the thyroid and thyroid papillary carcinoma, lipomatous type: A case report.

Diffuse lipomatosis, characterized by the widespread presence of mature adipose tissue within the thyroid, is a rare condition. The presence of adipose tissue may vary within the neoplastic nodules in cases of diffuselipomatosis. Although the etiology remains unclear, it is generally regarded as an embryological developmental anomaly. This case report presents a 68-year-old female patient presented to the Ear, Nose, and Throat (ENT) clinic with complaints of neck swelling. Ultrasonographic examination revealed an enlarged thyroid gland and a 2 cm nodule. The patient subsequently underwent the

Resolution of severe lumbar spinal epidural lipomatosis following tirzepatide-induced weight loss: illustrative case.

BACKGROUND: Spinal epidural lipomatosis (SEL) is a rare disorder characterized by pathological accumulation of adipose tissue in the spinal epidural space, leading to signs and symptoms of lumbar stenosis. SEL is increasingly linked to obesity, and treatment requires surgical decompression if conservative measures fail. The role of modern pharmacological weight-loss agents in reversing SEL has not been previously described. OBSERVATIONS: A 35-year-old man with severe, symptomatic lumbar SEL and class II obesity (BMI 37.8 kg/m) presented with progressive neurogenic claudication and functional

PIK3CA Mutations Downregulate PPT1 to Promote Adipogenesis by Suppressing P300 Depalmitoylation and Phase Separation.

PIK3CA mutations drive benign adipose overgrowth in facial infiltrating lipomatosis (FIL), but the downstream molecular mechanisms remain incompletely understood. This study investigated the role of palmitoyl-protein thioesterase 1 (PPT1)-mediated depalmitoylation in regulating aberrant adipogenesis induced by mutant PIK3CA. Using single-cell RNA-seq, molecular dynamics simulations, and functional assays in primary human FIL adipose-derived stem and progenitor cells (ASPCs), immortalized cell lines, and mouse models, we dissected the signaling pathway linking PIK3CA mutation to adipogenesis. SRP54-related congenital neutropenia: a multidisciplinary effort.

A previously healthy Caucasian man in his 30s presented with a 9-week history of productive cough, fever and dyspnoea, initially treated as pneumonia with transient improvement. Examination revealed coarse right-sided crepitations and finger clubbing. Laboratory testing demonstrated persistent severe neutropenia and elevated inflammatory markers, while imaging showed persistent right middle lobe consolidation. Further history revealed recurrent respiratory infections, diarrhoea and poor dentition since childhood, with medical records showing possible neutrophil migration defect. The patient re

Minimally Invasive Selective Decompression for Sagittal Malalignment due to Steroid-Induced Spinal Epidural Lipomatosis: A Case Report.

CASE: Spinal epidural lipomatosis (SEL) is the epidural accumulation of adipose tissue, sometimes associated with steroid use. There are limited reported cases of pediatric SEL. The purpose of this case report was to describe a 15-year-old girl with severe sagittal malalignment secondary to steroid-induced SEL associated with cardiac transplant rejection immunotherapy. She underwent multilevel decompression without traditional deformity correction and fusion, which improved her spinal deformity and symptoms. She had improved sagittal alignment and clinical outcomes at 2-year follow-up. CONCLU

Genetic diagnosis of macrotia in PIK3CA-Related Overgrowth Spectrum (PROS) and long-term outcome of otoplasty: a case report and literature review.

OBJECTIVES: PIK3CA-Related Overgrowth Spectrum (PROS) is a highly heterogeneous disease. Facial Infiltrating Lipomatosis (FIL) is a rare PROS subset, and its atypical phenotypes, such as macrotia, present diagnostic and therapeutic challenges due to limited data. This study aims to detail the diagnostic and long-term management procedures for an extremely rare case of Macrotia associated with PROS/FIL. METHODS: To confirm the underlying etiology, Whole Exome Sequencing (WES) was performed, complementing routine clinical and pathological examinations. Otoplasty was used as the intervention to

Ptch2 Deficiency Triggers Lipoma Formation and Adipogenic Transcriptome Reprogramming in Nile tilapia (Oreochromis niloticus).

The Hedgehog (Hh) signaling pathway is a key regulator of adipogenesis and lipid metabolism. However, the specific role of its receptor, Patched2 (Ptch2), in these processes remains unclear. Here, using a CRISPR/Cas9-mediated ptch2 homozygous mutation model in Nile tilapia (Oreochromis niloticus), we found that Ptch2 deficiency induced visceral and perirenal lipomatosis characterized by small, multinucleated adipocytes. Comparative adipose transcriptomics revealed pronounced adipogenic reprogramming, with marked upregulation of genes governing de novo lipogenesis (e.g., acaca, fasn), fatty aci

Madelung Deformity: A Current Concepts Review.

Madelung deformity is a rare congenital wrist deformity characterized by growth disturbance of the volar-ulnar aspect of the distal radial physis, leading to progressive wrist deformity, pain, and stiffness. Madelung deformity is frequently associated with genetic conditions such as Leri-Weill dyschondrosteosis and Turner syndrome, and more recently, mutations in the GNAS gene have been implicated. Diagnosis is based on clinical findings and radiographic criteria, with plain radiographs demonstrating characteristic volar-ulnar radial curvature, positive ulnar variance, and carpal subluxation.

Complete radiologic and clinical reversal of lumbar spinal epidural lipomatosis via GLP-1 agonist.

Spinal epidural lipomatosis (SEL) is a rare cause of lumbar canal stenosis, typically linked to obesity or corticosteroid use. Severe cases often require decompression. We report a 48-year-old man with chronic low back pain and morbid obesity (153 kg) who presented with extensive SEL and critical stenosis. Previous conservative therapies failed. A non-surgical strategy using GLP-1-induced weight loss (semaglutide) and medical cannabis for pain control was initiated. Over a follow-up of one year, his weight dropped to 93.8 kg, with major improvement in pain and mobility. MRI demonstrated near-c
