Comparative Efficacy of Insulin Delivery Modalities on Glycaemic Control in Adults With Type 2 Diabetes: A Systematic Review and Network Meta-Analysis.

AIMS: To compare insulin delivery modalities for glycaemic control, body weight and insulin dose in adults with insulin-treated Type 2 diabetes. MATERIALS AND METHODS: We searched eight databases and two trial registries until 27 April 2026 for parallel-group randomised trials comparing pre-specified insulin delivery modalities. The primary network meta-analysis excluded the clinically heterogeneous residual Other node. Outcomes were changes in HbA1c, time in range (TIR), time above range (TAR), time below range (TBR), body weight and insulin dose; certainty was assessed with CINeMA. RESULTS

Discovering the Molecular Secrets of Traditional Chinese Medicine: An Interpretation of Herbal Therapeutics Using Systems Biology.

Systems biology is primarily concerned with the dynamic study of biochemical metabolic processes in physical systems, the interplay of multiple signal transduction routes, gene regulatory networks, and disease causes, among other things. Its most notable aspect is that it merges individual genes with protein studies into thousands of genes or proteins, resulting in "three-dimensional" research. The action advantage of Traditional Chinese Medicine (TCM) is in the integration and modulation of the impact of each individual medication. TCM active components are compatible and blended according to

Expanded detection of early fibrotic phenotypes using lobar traction bronchiolectasis in lung cancer screening.

RATIONALE: Lung cancer screening regularly identifies participants with interstitial lung abnormalities (ILA). Existing classification methods may underestimate the prevalence of clinically relevant ILA phenotypes. OBJECTIVES: Can a classification system for ILAs developed in a lung cancer screening setting identify clinically relevant phenotypes? METHODS: Classification criteria based on the presence and lobar extent of traction bronchiolectasis (TBe) were developed internally by expert consensus. Categories included: no ILA, non-fibrotic ILA (NF-ILA), fibrotic ILA (F-ILA), and undiagnosed

Anticholinergic burden and bone health in rheumatoid arthritis.

OBJECTIVE: Rheumatoid arthritis is associated with osteoporosis and falls. While anticholinergic burden increases fracture risk in older adults, its contribution to bone health in rheumatoid arthritis is unclear. The aim of this study was to determine if anticholinergic burden is an independent risk factor for osteoporosis and fracture in rheumatoid arthritis. METHODS: This retrospective cohort study included 120 adults with rheumatoid arthritis. Cumulative anticholinergic burden was quantified using eight validated scales (anticholinergic burden, Anticholinergic Risk Scale, Anticholinergic D

Review Article: Accelerating Coeliac Disease Clinical Trials-Beyond Celiac Meeting With Industry, Academics, Patients and the FDA.

BACKGROUND: Coeliac disease is due to an acquired, lifelong gluten-specific immune response resulting in a gluten-dependent chronic immune-mediated enteropathy associated with gastrointestinal and extraintestinal morbidity. Strictly avoiding dietary gluten is the only available management but has never been subjected to rigorous efficacy assessment now required for a pharmaceutical agent. Gluten-free diet (GFD) is partially and inconsistently effective, but it places a high burden on patients, families, the food industry, and regulators and is defined inconsistently around the world, highlight

Association between multiple urinary metals and visceral obesity: a large cross-sectional study in South China.

BACKGROUND: The majority of studies have predominantly used body mass index or waist circumference as measures to establish a link between urinary metals and obesity, leading to inconsistent outcomes. Visceral fat index was a simple, practical and non-invasive physical examination indicator for measuring visceral obesity, and the association between urinary metal and VFI is unclear. METHODS: This study utilized a cross-sectional design and based on the baseline data of the Prospective Cohort Study of Chronic Diseases in Ethnic Minority Natural Population in Guangxi from May 2019 to December 2

Evidence on the Relationships Between Total Fat Intake and Health Outcomes Other Than Unhealthy Weight Gain in Adults and Children: A Scoping Review.

INTRODUCTION: Global guidance provides evidence-informed recommendations on total fat intake to prevent unhealthy weight gain in adults and children, however, evidence regarding total fat intake and other health outcomes, particularly non-communicable diseases (NCDs), also exists. This scoping review aimed to map and identify gaps in this body of evidence. METHODS: Four databases were searched from 2014 to 2024 for published systematic reviews (SRs), in any population, assessing the effects or associations of reduced total fat intake (e.g., ≤ 30% total energy [TE]), compared with usual fat in

Diagnostic coding combination patterns, CHS-DRG severity stratification, and hospitalization expenditure in pneumonia with coexisting hypertension or diabetes mellitus: a single-center cautionary case study of definitional circularity.

BACKGROUND: China's Healthcare Security Diagnosis-Related Groups (CHS-DRG) framework anchors inpatient payment to a relative weight (RW) set by each hospitalization's severity stratum. That stratum depends on the complications and comorbidities (CC) and major complications and comorbidities (MCC) identified among recorded secondary diagnoses, so ICD-10 documentation carries direct payment consequences. Evidence on diagnostic coding combination patterns in clinically complex populations, and on the methodological hazards of analyzing an exposure defined by grouper logic, remains limited. METHO

Seroprevalence of Celiac Disease in Mexican Mestizo Patients With Autoimmune Thyroid Disease.

GOALS: This study aimed to assess the seroprevalence of celiac disease in Mexican patients with autoimmune thyroid diseases, hypothesizing that prevalence would align with rates observed in other populations. BACKGROUND: The association between celiac disease and autoimmune thyroid diseases has been documented globally, with varying seroprevalence rates. Historically, Mexican Mestizos were considered at low risk for celiac disease, yet recent findings suggest similar prevalence to other regions. However, data regarding Hispanic patients with autoimmune thyroid diseases are limited. STUDY: Th

Development and validation of the CARE-DM model to predict the cardiovascular risk in older persons with type 2 diabetes.

AIMS: No cardiovascular risk prediction model dedicated to individuals aged ≥70 years with diabetes is currently recommended by the European Society of Cardiology. We aimed to develop a new model, CArdiovascular Risk Estimation-Diabetes Mellitus (CARE-DM), to predict the risk of cardiovascular disease (CVD) in older adults with type 2 diabetes. METHODS AND RESULTS: We developed a model to predict the risk of incident CVD in participants aged ≥65 years with diabetes using data from four population-based prospective cohorts, accounting for the competing risk of non-cardiovascular death. The effect of bright light therapy on glycemic control and cortisol rhythmicity in depression: a randomized controlled trial.

BACKGROUND: Depressed patients with comorbid metabolic disorders have poorer quality of life and prognosis. Pharmacological interventions carries risks of liver and kidney toxicity, which highlights the need for safer non-pharmacological alternatives. Experimental data suggest that light exposure modulates cortisol secretion, thereby influencing metabolic outcomes in depression. We hypothesized that bright light therapy may ameliorate metabolic disturbances by modulating cortisol secretion. METHODS: In this randomized controlled trial, hospitalized patients with depression were assigned to re

High- versus low-dose phototherapy for neonatal jaundice.

RATIONALE: Phototherapy is the mainstay of treatment for neonatal hyperbilirubinaemia. Increasing the dosage or intensity of phototherapy by various means is commonly practised for infants with high levels of serum bilirubin. It is unclear whether these measures of increasing phototherapy dosage are beneficial and safe for neonates. OBJECTIVES: To assess the effects of high-dose versus low-dose phototherapy on bilirubin level and associated clinical outcomes that constitute the major conditions in kernicterus spectrum disorder (KSD), such as acute bilirubin encephalopathy and kernicterus, as

Association between diabetes and sarcopenia in US adults and the role of adiposity: a survey-weighted analysis of NHANES 2011-2018.

BACKGROUND: Sarcopenia and diabetes are major public health burdens. Body mass index (BMI) represents a complex factor in diabetes-sarcopenia relationship, implicated in both low muscle mass and the high-risk phenotype of sarcopenic obesity, but its role as a statistical mediator in this association remains unclear in population-based studies. METHODS: This cross-sectional study included 7,037 U.S. adults aged 20–59 years from National Health and Nutrition Examination Survey (NHANES) 2011–2018. Sarcopenia was primarily defined by low appendicular skeletal muscle mass index (ASM/height2; sex-sp

Effect of a behavioral counseling for adoption and maintenance of a physically active lifestyle on long-term mortality in people with type 2 diabetes: post hoc analysis of the Italian Diabetes and Exercise Study_2.

An inverse relationship between physical activity and mortality has been observed in epidemiological surveys, but not in randomized clinical trials. This post hoc, not pre-specified analysis of the Italian Diabetes and Exercise Study_2 randomized clinical trial assesses the effect of a physical activity-based counseling intervention on long-term mortality in people with type 2 diabetes. Three hundred physically inactive and sedentary individuals are recruited in three outpatient diabetes clinics in Rome from October 2012 to February 2014. Participants are randomized 1:1 to a Control group rece
