Hepatic Steatosis and Cardiovascular Risk in Obesity: A 10-Year Follow-Up Study From Childhood Into Young Adulthood.

BACKGROUND: The early vascular impact of metabolic dysfunction-associated steatotic liver disease (MASLD) remains unclear. This study examined associations of steatosis in childhood and young adulthood with cardiovascular risk in adulthood. METHODS: Children with severe obesity were investigated for MASLD and cardiovascular risk, with 10-year follow-up. Hepatic steatosis was measured using proton magnetic resonance spectroscopy and cardiovascular risk by ultrasound carotid intima-media thickness (cIMT). Participants were categorised into four trajectory groups based on presence of steatosis i

Metabolic Dysfunction-Associated Steatotic Liver Disease and Mitochondrial Quality Control: From Laboratory to Clinical Practice.

Metabolic dysfunction-associated steatotic liver disease (MASLD) affects more than 1.66 billion people globally. This metabolic disorder shows strong connections with mitochondrial dysfunction and disrupted mitochondrial quality control (MQC) processes. Our review systematically analyzes the molecular mechanisms underlying the relationship between MQC and MASLD development. Research evidence indicates that impaired MQC regulation worsens three key pathological conditions: it intensifies liver fat accumulation, increases oxidative damage, and promotes inflammatory cell death through activating

Impact of Combination Therapy With Caffeine and Naringin on Insulin Resistance and Renal Function in a Nonalcoholic Fatty Liver Disease Rat Model.

INTRODUCTION: Chronic kidney disease (CKD) is a major global health concern, and evidence indicates that non-alcoholic fatty liver disease (NAFLD) independently increases its risk and progression. This connection involves shared pathways such as chronic inflammation, oxidative stress, and insulin resistance, which can worsen renal injury. Caffeine (CAF) and naringin (NAR), with their antioxidant and anti-inflammatory properties, may provide protective effects on both NAFLD and CKD. This study evaluated the impact of combined CAF and NAR treatment on insulin resistance and renal function in a h

[Elucidating mechanism of Biejia-Ruangan Compound Tablets against alcoholic liver disease through gut-liver axis using integrated multi-omics].

Based on the gut-liver axis, this study integrated multi-omics and network pharmacology strategies to explore the mechanism of Biejia-Ruangan Compound Tablets(BRC), a preferred Chinese patent medicine for anti-hepatic fibrosis, in alleviating alcoholic liver disease(ALD). The Lieber-DeCarli ethanol liquid diet was used to establish the ALD model, and the pharmacodynamic effects of BRC were evaluated. Non-targeted metabolomics and network pharmacology were employed to screen key metabolites and pathways, while multiple technical methods such as immunohistochemistry were used to verify key molec

Admission ionised calcium disturbances and adverse outcomes in paediatric major trauma: a UK multicentre retrospective cohort study.

BACKGROUND: Calcium disturbances are increasingly recognised as detrimental in adult trauma patients; both ionised hypocalcaemia (iHypoCa) and ionised hypercalcaemia (iHyperCa) are associated with increased mortality. Paediatric data is limited and heterogeneous, with suggestions that iHypoCa may be associated with haemodynamic instability and increased treatment requirements. No paediatric studies have explored the effect of iHyperCa. OBJECTIVES: To examine the association between admission ionised calcium (iCa) disturbances and hypotension, treatment requirements, poor functional outcome an

Decoding the CRH system from clinical failures to therapeutic translation.

The corticotropin-releasing hormone (CRH) system orchestrates stress adaptation, integrating neuroendocrine, autonomic, immune, and behavioural responses to internal and external challenges. For over 2 decades, CRH receptor type 1 (CRH-R1) has been pursued as a therapeutic target for neuropsychiatric disorders. Despite compelling biological rationale and extensive preclinical validation, most CRH-R1 antagonist programmes have failed in depression, anxiety, post-traumatic stress disorder, and alcohol use disorder. In contrast, CRH-R1 antagonism has demonstrated clinical proof-of-concept in cong

Clinical characteristics and outcomes of acute liver failure in Vietnam: A prospective cross-sectional observational study.

Understanding the etiologies, clinical characteristics, and outcomes of acute liver failure (ALF) is important for guiding management and evaluating prognosis; however, evidence from Vietnam remains limited. We conducted a prospective single-center cross-sectional observational study involving patients with ALF admitted to Bach Mai Hospital, Vietnam, between January 2020 and July 2021. The study included 60 patients from January 2020 to July 2021 who fulfilled the selection criteria for ALF diagnosis. The mean age of patients was about 53 ± 15 years (range = 21-85 years). Most complications we

Milk-derived extracellular vesicles in cardiovascular therapy: emerging biological functions and therapeutic perspectives.

Cardiovascular diseases remain a leading cause of morbidity and mortality worldwide and impose a substantial clinical and socioeconomic burden. Despite advances in medical, interventional, and surgical treatments, considerable residual cardiovascular risk persists, highlighting the need for effective and sustainable therapeutic approaches. Milk-derived extracellular vesicles (MDEVs) are heterogeneous lipid-bilayer vesicles present in human and bovine milk and have attracted increasing interest as potential orally administered biological nanocarriers. MDEVs carry diverse bioactive cargo, includ

Overcoming Therapy Resistance in Ovarian Cancer: From Molecular Mechanisms to Emerging Therapeutic Strategies.

Background/Objectives: Epithelial ovarian cancer (EOC) remains a gynecologic malignancy with a poor prognosis, with a 5-year survival of approximately 29% in advanced-stage disease. Despite cytoreductive surgery and platinum- and taxane-based chemotherapy, most patients relapse within 2 years. Major therapeutic barriers include chemoresistance, molecular heterogeneity, and an immunosuppressive peritoneal microenvironment. This review summarizes emerging therapeutic strategies for EOC, their mechanisms of action, and their potential to overcome treatment resistance. Methods: PubMed/MEDLINE was

Targeting Nrf2 in oxidative liver injury: expanding the role of gut microbiota and metabolites.

BACKGROUND: Liver diseases are a major cause of illness and death worldwide. Oxidative stress is a pivotal driver in the pathogenesis of a spectrum of liver diseases, including alcoholic liver disease (ALD), metabolic dysfunction-associated fatty liver disease (MAFLD), drug-induced liver injury (DILI), and hepatocellular carcinoma (HCC). The transcription factor Nrf2, a master regulator of cellular antioxidant responses, plays a central yet context-dependent role in modulating this injury. Additionally, the gut-liver axis is a critical regulator of hepatic homeostasis. AIM OF REVIEW: This rev

ILC plasticity in intestinal immune barrier remodeling: current evidence and therapeutic implications for inflammatory bowel disease.

Inflammatory bowel disease (IBD), which includes Crohn's disease (CD) and ulcerative colitis (UC), is a group of chronic gastrointestinal inflammatory diseases characterized by persistent mucosal inflammation and intestinal immune barrier dysfunction. Innate lymphoid cells (ILCs) are key regulators of mucosal immunity; however, the contribution of their context-dependent plasticity to IBD-associated immune barrier remodeling remains incompletely defined. This narrative review summarizes the intestinal distribution and functional specialization of ILC subsets, their dysregulation in the IBD mic

Evidence on the Relationships Between Total Fat Intake and Health Outcomes Other Than Unhealthy Weight Gain in Adults and Children: A Scoping Review.

INTRODUCTION: Global guidance provides evidence-informed recommendations on total fat intake to prevent unhealthy weight gain in adults and children, however, evidence regarding total fat intake and other health outcomes, particularly non-communicable diseases (NCDs), also exists. This scoping review aimed to map and identify gaps in this body of evidence. METHODS: Four databases were searched from 2014 to 2024 for published systematic reviews (SRs), in any population, assessing the effects or associations of reduced total fat intake (e.g., ≤ 30% total energy [TE]), compared with usual fat in

Reduced R-loop abundance at proinflammatory loci: a shared epigenetic mechanism in inflammatory and metabolic diseases.

INTRODUCTION: R-loops, RNA-DNA hybrid structures with a displaced single-stranded DNA loop, are key regulators of transcriptional control, chromatin architecture, and genome stability and have emerging roles in inflammatory signaling. However, the relationship between R-loop abundance and strongly modulated inflammatory effector genes in metabolic inflammation and influenza virus infection remains underexplored. METHODS: We performed a locus-centric integrative analysis combining robust differentially expressed genes (DEGs) from multiple inflammatory and infection-related murine and human tra

Pan-PPAR agonists: Emerging therapeutic strategies for metabolic dysfunction-associated steatotic liver disease.

Metabolic dysfunction-associated steatotic liver disease (MASLD), formerly known as non-alcoholic fatty liver disease (NAFLD), has emerged as a leading global chronic liver disorder, affecting approximately 30% of the general population. Characterized by the presence of hepatic steatosis along with at least one metabolic comorbidity, MASLD represents a progressive disease spectrum ranging from simple steatosis to hepatocellular carcinoma. The pathogenesis of MASLD involves a complex interplay of metabolic dysregulation, insulin resistance, oxidative stress, and inflammation, which remains inco

[Research progress on the applicability of diagnostic criteria for metabolic and alcohol-related liver disease in patients with cirrhosis and after liver transplantation].

Metabolic and alcohol-related liver disease (MetALD) is a key subtype within the new classification system of fatty liver disease, and its diagnosis relies on three core elements: hepatic steatosis, metabolic dysfunction, and alcohol consumption. However, the applicability of current diagnostic criteria is limited in patients with cirrhosis and liver transplant recipients due to structural destruction of the liver, remodeling of metabolic status, and difficulties in accurately assessing alcohol intake. This article systematically reviews the current application of MetALD diagnostic criteria in

AMPK-PP1/PP2 axis regulates GSDMD-mediated pyroptosis via phosphorylation.

Gasdermin D (GSDMD), the most important executor of pyroptosis, is activated through caspase cleavage to release the N-terminal fragment (GD-NT), which oligomerizes and forms pores in the plasma membrane to induce pyroptosis. We previously reported that AMP-activated protein kinase (AMPK) phosphorylates GD-NT, thereby inhibiting its cytolytic activity. Here, we further investigated the role of phosphorylation in GD-NT-mediated pyroptosis and found that the specific AMPK agonist A-769662 protected tumor cells from GD-NT-mediated pyroptosis and that metformin significantly reduced interleukin 1β

[Considerations on clinical trial issues related to new drug development for metabolic and alcohol-associated liver disease and fatty liver disease].

The global incidence of alcohol-associated liver disease(ALD) is rising. Beyond glucocorticoids, effective pharmacological therapies for severe ALD remain limited. With the introduction of the novel concept of "metabolic and alcohol-associated liver disease (MetALD) ", it has emerged as a distinct clinical condition characterized by synergistic liver injury resulting from the interplay between metabolic dysfunction and alcohol consumption. In contrast to the substantial progress in drug development for metabolic associated steatotic liver disease (MASLD), clinical research on ALD and MetALD ha
