The rise in domestic shigellosis and the genomic characteristics of Shigella clones linked to men who have sex with men in Taiwan, 2015‒2022.

UNLABELLED: Since 2015, Taiwan has experienced a notable rise in domestic shigellosis cases, particularly among young adult males. In this study, we aimed to investigate this epidemiological trend through demographic analysis and genomic characterization of bacterial isolates. We analyzed demographic data on shigellosis cases from 2003-2014 to 2015-2022. For cases from 2015-2022, we conducted genomic analyses of Shigella isolates using pulsed-field gel electrophoresis (PFGE) and whole-genome sequencing (WGS). Antimicrobial resistance and plasmid profiles were examined to identify genetic deter

BECC-engineered live-attenuated Shigella vaccine candidates display reduced endotoxicity with robust immunogenicity in mice.

Shigella spp. infection contributes significantly to the global disease burden, primarily affecting young children in developing countries. Currently, there are no FDA-approved vaccines against Shigella, and the prevalence of antibiotic resistance is increasing, making therapeutic options limited. Live-attenuated vaccine strains WRSs2 (S. sonnei) and WRSf2G12 (S. flexneri 2a) are highly immunogenic, making them promising vaccine candidates, but possess an inflammatory lipid A structure on their lipopolysaccharide (LPS; also known as endotoxin). Here, we utilized bacterial enzymatic combinatori

Tebipenem pivoxil as an alternative to ceftriaxone for clinically non-responding children with shigellosis: a randomised non-inferiority trial protocol.

INTRODUCTION: Shigellosis is the second leading cause of diarrhoeal deaths among children worldwide. Oral azithromycin and intravenous ceftriaxone are the recommended first-line and second-line therapies for shigellosis in Bangladesh, respectively, but growing antibiotic resistance will require new antibiotic options. Tebipenem pivoxil, an orally administered carbapenem antibiotic with activity against many strains of antibiotic-resistant bacteria, may be a viable option. METHODS: A phase IIb randomised controlled trial was planned to determine the efficacy and safety of oral tebipenem pivoxi

Transposon mutagenesis identifies acid resistance and biofilm genes as Shigella sonnei virulence factors in Caenorhabditis elegans infection.

Identifying essential genes in bacterial pathogens during infection can enhance knowledge and provide novel targets for antimicrobial agents' development. Currently, only Shigella flexneri essential genes during in vitro growth have been experimentally identified. This study used transposon insertion sequencing (TIS) to identify Shigella sonnei essential genes during Caenorhabditis elegans infection. 498 genes were predicted to be essential in S. sonnei during growth on nutrient-rich media. Some genes previously predicted to be essential in Shigella were found non-essential in S. sonnei, such

Genetic characterization of extensively drug-resistant blaCTX-M-27Shigella sonnei clusters among men who have sex with men in a region of northern Spain.

OBJECTIVE: The convergence of globalization with increased sexual risk behaviours has significantly facilitated the dissemination of multidrug-resistant and extensively drug-resistant clusters of Shigella spp. among men who have sex with men, particularly Shigella sonnei and Shigella flexneri. A cluster of S. sonnei carrying blaCTX-M-27 caused a European outbreak in 2020-2021, with more than 30 cases in Spain, including two in our institution. In this study, we conducted a retrospective study from October 2022 to December 2023 that included five additional patients with shigellosis caused by a

Dynamics of the gut microbiome in subjects challenged with Shigella sonnei 53G in a controlled human infection model.

Shigella is a significant cause of diarrhea, predominantly affecting children in low- and middle-income countries, as well as international travelers. Not all individuals exposed to Shigella or other enteropathogens have symptomatic responses, and investigating the differences between symptomatic and asymptomatic individuals can further our understanding of enteropathogen proliferation and symptomatic responses. Here, we profiled the fecal microbiomes of 45 individuals infected with Shigella sonnei strain 53G through 16S rRNA sequencing in a controlled human infection model before and during i

Evaluation of extended-spectrum β-lactamase producing bacteria in feces of shelter dogs as a biomarker for altered gut microbial taxa and functional profiles.

BACKGROUND: The USA is home to 83-88 million dogs, with 3-7 million living in shelters. Shelter dogs move through the supply chain from their geographical origin to adoptive homes, with possible exposure to pathogens and shift in their gut microbiota. However, research in this area is limited. This study examined the effects of intestinal colonization by ESBL bacteria on gut taxa abundance, diversity, and functions in 52 shelter dogs of various ages, sexes, and fertility statuses. METHODOLOGY: We isolated fecal DNA, sequenced their 16S, processed the sequences using DADA2, identified taxa pro

Shigella flexneri evades LPS ubiquitylation through IpaH1.4-mediated degradation of RNF213.

Pathogens have evolved diverse strategies to counteract host immunity. Ubiquitylation of lipopolysaccharide (LPS) on cytosol-invading bacteria by the E3 ligase RNF213 creates 'eat me' signals for antibacterial autophagy, but whether and how cytosol-adapted bacteria avoid LPS ubiquitylation remains poorly understood. Here, we show that the enterobacterium Shigella flexneri actively antagonizes LPS ubiquitylation through IpaH1.4, a secreted effector protein with ubiquitin E3 ligase activity. IpaH1.4 binds to RNF213, ubiquitylates it and targets it for proteasomal degradation, thus counteracting

Multidrug-resistant Shigella flexneri outbreak affecting humans and non-human primates in New Mexico, USA.

Shigellosis is a gastrointestinal infection caused by species of Shigella. A large outbreak of Shigella flexneri serotype 2a occurred in Albuquerque, New Mexico between May 2021 and November 2023 that involved humans and non-human primates (NHP) from a local zoo. We analyzed the genomes of 202 New Mexican isolates as well as 15 closely related isolates from other states, and four from NHP. The outbreak was initially detected within men who have sex with men but then predominantly affected people experiencing homelessness. Nearly 70% of cases were hospitalized and there was one human death. The

Shigella flexneri Bacteremia in an Immunocompetent Woman With Socioeconomic Risk Factors: A Case Report.

Shigella species are responsible for millions of cases of shigellosis each year worldwide, particularly in underdeveloped countries. These bacteria invade the colonic epithelium and release enterotoxins, leading to watery or bloody diarrhea, fever, and abdominal pain. Bacteremia is a rare systemic complication of shigellosis, with only a few cases reported among immunocompetent adults. Here, we present an intriguing case of Shigella flexneri bacteremia in an elderly, immunocompetent woman. Uniquely, the patient's stool tested negative for Shiga toxin despite confirmed shigellemia. In addition

Shigella Mutant with Truncated O-Antigen as an Enteric Multi-Pathogen Vaccine Platform.

Background/Objectives: Rising antibiotic resistance underscores the urgent need for effective vaccines against shigellosis. Our previous research identified the Shigella flexneri 2a truncated mutant (STM), a wzy gene knock-out strain cultivated in shake-flasks, as a promising broadly protective Shigella vaccine candidate. Expanding on this finding, our current study explores the feasibility of transitioning to a fermentor-grown STM as a vaccine candidate for further clinical development. Methods: The STM and STM-Cj, engineered to express the conserved Campylobacter jejuni N-glycan antigen, wer

Accelerated electrochemical antibiotic susceptibility testing using disposable electrodes and Resazurin.

Stenotrophomonas maltophilia and Shigella sonnei are increasingly associated with serious infections and shigellosis, respectively, with conventional antibiotic susceptibility tests requiring 18-24 h or more, often delaying treatment. In this study, we present a novel electrochemical sensor employing the redox indicator Resazurin for rapid assessment of antibiotic susceptibility. Detection of live S. maltophilia and S. sonnei was achieved within 2 h as Resazurin, reduced to Resorufin by metabolically active bacteria, produced a measurable decrease in the reduction current. Following exposure t

Fecal microbiome profiling of children with Shigella diarrhea from low- and middle-income countries.

Shigella is one of the leading pathogens causing diarrhea in children globally. Stool culture remains the standard for diagnosing Shigella infections; however, quantitative PCR (qPCR) offers greater sensitivity. In this study, we evaluated the fecal microbiomes of 27 diarrheal children with and without Shigella identified by culture and quantitative PCR, respectively, aiming to characterize the fecal microbiome profiles of children with Shigella diarrhea and to explore the differences in microbiome and the mechanisms associated with the culturability of Shigella. Our results showed that Shigel

Inhibiting multidrug resistant Shigella flexneri using comparative genomics guided In vitro and In silico screening.

ETHNOPHARMACOLOGICAL RELEVANCE: The Gram-negative bacterium Shigella is the most widely documented etiologic factor for diarrheal mortality in infants, contributing to 13.2 % of diarrheal episodes across the globe. The present surge in antibiotic resistance, increasing numbers of non-typeable strains, and the disparate regional distribution of multiple Shigella variants has rendered it obvious that novel therapeutic alternatives are desperately sought after to address the growing threat of shigellosis. AIM OF THE STUDY: In the present investigation, we have assessed the antibacterial properti

Epidemiologic and Genomic Investigation of Sexually Transmitted Shigella sonnei, England.

Shigellosis is a bacterial infection that causes enteric illness and can be sexually transmitted, particularly among gay, bisexual, and other men who have sex with men. Multiple extensively drug-resistant Shigella strains have been detected through genomic surveillance and are associated with plasmids carrying the gene variant blaCTX-M-27 in the United Kingdom. We report an increase in possible sexually transmitted cases of Shigella bacteria carrying the blaCTX-M-15 gene variant, which was previously associated with travel. In 2023, there were 117 cases belonging to the 10 single-nucleotide po

Lactose fermenting enteroinvasive Escherichia coli from diarrhoeal cases confers enhanced virulence.

Enteroinvasive Escherichia coli (EIEC), known for causing bacillary dysentery akin to Shigella species, comprises both lactose-fermenting (LF) and non-lactose-fermenting (NLF) isolates. While NLF-EIEC is a well-established pathogen associated with acute dysentery and harbours classical Shigella-like virulence factors, the role of LF-EIEC in human disease remains underexplored. In this study, we sought to characterize LF-EIEC clinical isolates and assessed their pathogenic potential in comparison to NLF-EIEC. Among 13,682 diarrhoeal stool specimens, six LF and nine NLF-EIEC were isolated, predo

Cross-comparison study of three ELISA methodologies to measure Shigella Sonnei O-antigen serum IgG.

Measurement of serum immunoglobulin G (IgG) to Shigella lipopolysaccharide (LPS) has been proposed as a correlate of protection against shigellosis, the confirmation of which in the target population of infants in low- and middle-income countries will facilitate vaccine development and licensure. Data obtained from Shigella sonnei vaccine clinical studies have identified a protective serum LPS IgG threshold value that is assay-specific. In the interim period before an International Standard serum was available, a retrospective bridging study using a panel of human serum samples had been conduc

The emergence of sexually transmissible Shigella flexneri serotype 1b between 2019 and 2024 in England: a descriptive epidemiological study.

Background. Shigella species are pathogenic bacteria that cause gastrointestinal symptoms ranging from mild watery diarrhoea to bacillary dysentery. Transmission is faecal-oral and historically associated with international travel. Recently, sexual transmission has been documented among gay, bisexual and other men who have sex with men (GBMSM). Through routine surveillance, we observed an increase in notifications of S. flexneri serotype 1b. We investigated the emergence of this serotype and examined possible drivers of transmission.Methods. We used historical data and whole-genome sequencing
