Journal of vascular surgery cases and innovative techniques2026eng
Hemangiomas are primarily associated with neovascularization and the abnormal proliferation of blood vessels. No cases of hemangioma combined with actinomycosis have been reported in the literature. We present a case of a 54-year-old man with a rapidly enlarging left upper limb mass after two traumatic injuries. Pathological examination performed after surgical resection of the mass at our center revealed a hemangioma complicated by actinomycosis. This case indicates that actinomycetes may colonize hemangiomas and contribute to their continuous enlargement, with excellent prognosis after surge

BACKGROUND: Mycetoma is a chronic granulomatous subcutaneous infection caused by fungi (eumycetoma) or bacteria (actinomycetoma). The classic triad includes a mass, sinus tracts, and discharge containing granules. We present an atypical case of actinomycotic mycetoma of the hand without sinus formation.
CASE PRESENTATION: An 85-year-old male farmer presented with an 8-year history of a progressively enlarging, painful mass on his left hand; he recalled that the initial lesion began as a painless pustule on the palm approximately 9 years earlier. Prior surgeries and short-course antibiotics fa

BACKGROUND: Osteomyelitis of the hand is relatively uncommon and often results from penetrating trauma or postoperative infection. Rare pathogens such as Actinomyces radingae and Finegoldia magna pose diagnostic and therapeutic challenges due to their slow growth, frequent involvement in polymicrobial infections, and limited antibiotic susceptibility data.
CASE PRESENTATION: A 66-year-old male sustained a crush injury being caught in a door to his right thumb over two years prior, leading to repeated episodes of redness, swelling, pain, and restricted motion. Despite undergoing surgical inter

ABSTRACT: BACKGROUND: Canaliculitis is often mis- or underdiagnosed, leading to delayed and sometimes inadequate treatment.
ABSTRACT: PATIENTS AND METHODS: Thirteen patients with in total of 17 affected canaliculi were analysed. Collected data included demographic characteristics, the affected canaliculus (upper or lower, right or left), duration of symptoms until the correct diagnosis, and previous misdiagnoses and treatments. The objectives of this study were to determine the diagnostic delay and frequency of misdiagnosis, to identify characteristic clinical features, and to evaluate treatm

Intrauterine contraceptive devices (IUCDs) are widely used, highly effective, and reversible methods of contraception. Despite their overall safety profile, IUCD malposition and complications May occur and are frequently asymptomatic or clinically unsuspected, posing a diagnostic challenge in daily practice. Imaging plays a central role in evaluating IUCD location, integrity, and associated complications, often in patients undergoing ultrasound, computed tomography (CT), or magnetic resonance imaging (MRI) for unrelated indications. Familiarity with the different types of IUCDs and their expec

This case discusses a 76-year-old female patient with a large mass at the major papilla of Vater, initially suspected to be an ampullary adenoma. The lesion, identified during an upper endoscopy for anemia and fatigue, showed chronic inflammation but no malignancy. The patient's medical history included breast cancer and a partial distal pancreatectomy for an intraductal papillary mucinous neoplasm. Further imaging and biopsies did not confirm malignancy or adenoma but suggested actinomycosis. Actinomycosis, caused by Actinomyces species, presents challenges due to its rarity, slow progression

Actinomycosis is a rare chronic bacterial infection, with pelvic involvement accounting for ~5% of cases. Surgery may be required if there is intestinal obstruction, abscess formation, severe infection, or if malignancy cannot be excluded. We report a 46-year-old woman with a 12-year history of an intrauterine device, presenting with abdominal pain, vomiting, constipation, lethargy, and malaise. Examination revealed abdominal distension, tenderness, and a left iliac fossa mass. Computed tomography imaging showed a complex, infiltrative pelvic mass involving the sigmoid colon and adjacent struc

INTRODUCTION: Cutaneous infections contribute to an important cause of all dermatological lesions. Most of the superficial cutaneous infections are diagnosed based on clinical findings and with adjunct diagnostic modalities like potassium hydroxide mounts, tzanck smears, Gram stain, and other microbiological investigations and are treated without the necessity of histopathological examinations. Histopathology is indispensable in some instances, especially when the above-mentioned investigations are non-contributory for diagnosis, and in situations where there is a doubt that the organisms isol

OBJECTIVE: Perigraftitis is a biologic complication associated with dental implants placed in grafted bone, distinct from peri-implant mucositis and peri-implantitis. This report further characterizes its clinical, radiographic, and histopathologic features.
CLINICAL CONSIDERATIONS: Three patients presented with discomfort around a single implant placed following tooth extraction and alveolar ridge preservation. Surgical access was obtained, and the implant fixture and surrounding defect were thoroughly debrided. Tissue fragments were submitted for histology, and sections from two previously

INTRODUCTION: Tropheryma whipplei (TW), which causes Whipple disease, has recently been associated with respiratory diseases, particularly pneumonia. To understand its role in pediatric pneumonia, this study analyzed the clinical and pathogenetic characteristics of TW in pediatric pneumonia patients.
METHODS: We utilized metagenomic and targeted next-generation sequencing (mNGS/tNGS) data from 3,759 pediatric bronchoalveolar lavage fluid (BALF) samples (2023-2024). This case-control study included 103 TW-positive pediatric pneumonia patients (59 with severe pneumonia, SPTW+; 44 with mild pneu

Actinomycosis is a subacute to chronic bacterial infection. Central nervous system (CNS) involvement is very rare. Clinical symptoms are nonspecific and vary depending on the topography and number of lesions. Cross-sectional imaging assists in both the positive and topographic diagnosis, and the assessment of locoregional aggressiveness may help identify the portal of entry. We present a case of cerebellar actinomycosis in a 52-year-old female patient. The diagnosis was suggested by brain MRI and confirmed by histopathological examination. This case highlights the possibility of CNS involvemen

OBJECTIVES: Cardiac involvement is a leading cause of morbidity and mortality in systemic sclerosis (SSc) yet is often underdiagnosed. Early recognition is crucial but published data remains limited. This study aimed to characterize the clinical presentation, diagnostic features, and outcomes of SSc patients with acute primary heart involvement.
METHODS: This multicentre retrospective case series included patients meeting the 2013 ACR/EULAR SSc classification criteria with clinically significant pericardial effusion or myocardial involvement, identified across tertiary rheumatology centres in

BACKGROUND: Whipple's disease, caused by Tropheryma whipplei, is a rare multisystem infectious disorder with diverse clinical manifestations. Typical symptoms include arthralgia, nausea, vomiting, diarrhoea, and weight loss, while nonspecific features such as fever, anaemia, and lymphadenopathy may also occur. Presentations with multiple serous effusions are exceedingly rare. Diagnosis remains particularly challenging in resource-limited regions because of nonspecific symptoms and limited access to advanced diagnostic techniques.
CASE PRESENTATION: A 34-year-old male presented with fever, vom

A middle-aged man presented with an indolent growth on his chest wall, features of sepsis, and constitutional symptoms. He was subsequently found to have raised inflammatory markers and pleural thickening on imaging which, coupled with a history of asbestos exposure, was concerning for malignancy. While there was clinical improvement with empirical intravenous penicillin antibiotics, the diagnosis was not certain until an excisional biopsy of the mass was performed, with histopathology showing Splendore Hoeppli material ('sulfur granules') leading to the diagnosis of an Actinomyces infection.

BACKGROUND Temporal bone necrosis is a rare condition, most commonly caused by radiation therapy, bisphosphonate treatment, and osteomyelitis. Cholesteatoma can contribute to temporal bone necrosis, particularly in the presence of secondary ear infections. We report a case of extensive temporal bone necrosis with intracranial involvement in a patient with mastoid and petrous apex cholesteatoma, further complicated by actinomycosis. CASE REPORT A 47-year-old woman presented with persistent otorrhea and right-sided conductive hearing loss. Imaging demonstrated diffuse necrotic tissue extending f

Pelvic actinomycosis is an uncommon infection caused by Actinomyces israelii, often linked to prolonged intrauterine device (IUD) use. Its clinical and radiologic presentation may closely mimic pelvic malignancies, leading to misdiagnosis and inappropriate management. We report two cases of pelvic actinomycosis initially mistaken for cervical cancer. The first involved a 50-year-old woman with pelvic pain and imaging suggestive of advanced cervical neoplasia; histology revealed Actinomyces infection, and antibiotic therapy led to complete resolution. The second concerned a 76-year-old woman wi

Tonsillar actinomycosis is an uncommon chronic infection that can clinically and radiologically mimic malignancy. Because of its nonspecific presentation and tumor-like appearance, particularly when associated with cervical lymphadenopathy, it often poses a diagnostic challenge. A 20-year-old patient presented with a four-month history of progressive left tonsillar hypertrophy. Nasofibroscopy showed an irregular tonsillar surface with nasopharyngeal bulging, raising suspicion of a neoplastic lesion. Contrast-enhanced CT revealed asymmetric left tonsillar enlargement with homogeneous thickening
