Nocardia Otomastoiditis in a Chemotherapy-Induced Neutropenic Patient With Diffuse Large B-cell Lymphoma: Successful Medical Management.

Nocardia species are aerobic, filamentous, weakly acid-fast, Gram-positive bacteria. They cause opportunistic infections, particularly in patients receiving chemotherapy or corticosteroids. Otologic involvement is extremely rare, with only a few cases of Nocardia otomastoiditis reported worldwide.  We report a 31-year-old woman with gastric and pancreatic diffuse large B-cell lymphoma who presented with fever and a one-week history of worsening purulent discharge from the left ear during chemotherapy-induced neutropenia. She had a 15-year history of intermittent otorrhea, previously treated wi

Synergistic and additive in vitro interactions of imipenem, trimethoprim-sulfamethoxazole, amikacin, and clofazimine against clinical isolates of Nocardia wallacei.

Nocardia wallacei presents a therapeutic challenge due to its frequent in vitro resistance to commonly used antimicrobials, including trimethoprim-sulfamethoxazole (TMP-SMX), imipenem, and amikacin. Current nocardiosis treatment guidelines provide general recommendations but lack species-specific guidance for N. wallacei. Given the limited therapeutic options for this species, evaluating combination therapies may support treatment optimization. We assessed the potential synergistic and additive effects between TMP-SMX, imipenem, amikacin, and clofazimine using the checkerboard titration method

Nocardia farcinica Brain Abscess Mimicking Acute Ischemic Stroke in an Immunocompetent Elderly Patient: A Case Report.

Central nervous system (CNS) infections caused by Nocardia species are rare but potentially life-threatening. Even more unusual is Nocardia infection in immunocompetent patients. Most patients initially present with a primary respiratory or cutaneous infection that may progress to disseminated disease involving the CNS; however, primary CNS infection is rare. Nocardia farcinica is frequently associated with brain abscesses. We report the case of an 81-year-old immunocompetent man who presented with acute focal neurological deficits and seizures, initially managed as an ischemic stroke. Subsequ

A critical anthropological evaluation of digital media based interventions for mycetoma in Sudan; the case of "Hope for a Devastating Neglected Disease" documentary film.

Mycetoma, a disfiguring and debilitating neglected tropical disease (NTD), long languished in obscurity. Prior to 2013, limited research, ineffective treatments, and inadequate global recognition plagued the disease. However, a pivotal 2013 meeting in Geneva marked a turning point. Recognizing advocacy as a critical missing link, the Drugs for Neglected Diseases Initiative (DNDi) spearheaded efforts to raise awareness and address knowledge gaps. Scientific symposia, media coverage, and research collaborations, including the DNDi trial, paved the way for mycetoma inclusion in the 2016 WHO NTD l

Diagnostic value of repeat deep sampling and cytocentrifugation in disseminated nocardiosis due to Nocardia brasiliensis with intramuscular and prostatic abscesses in a patient with myelodysplastic syndrome.

Nocardiosis remains diagnostically challenging in immunocompromised patients. We report disseminated nocardiosis due to Nocardia brasiliensis, supporting the role of repeat deep sampling and specimen concentration by cytocentrifugation in enhancing diagnostic yield. An 80-year-old man with myelodysplastic syndrome with low blasts and ring sideroblasts and type 2 diabetes mellitus presented with a three-week history of progressive pain and swelling of the right thigh and left lower leg. One week before symptom onset, the patient received a five-day course of oral levofloxacin for urinary freque

Nocardia farcinica infection after facial lipolysis injection in an immunocompetent patient.

INTRODUCTION: Injection lipolysis is a widely used technique for the rapid reduction of localized fat deposits. Although complications such as non-tuberculous mycobacterial infections have been reported, Nocardia infections following this procedure remain rare. CASE PRESENTATION: This report details a case of Nocardia infection in an immunocompetent patient after facial lipolytic agent administration. The patient exhibited amelioration of facial edema and resolution of chest rash following a course of amoxicillin-clavulanate treatment, ultimately achieving recovery and discharge. CONCLUSIONS

Fatty acid synthase-II is required for the synthesis of mycolic acid and essential for pathogenicity of Nocardia seriolae.

Nocardia seriolae is the etiological agent of fish nocardiosis. Fatty acid synthase type II (FAS-II) system is involved in the biosynthesis of mycolic acid, a key virulence factor of N. seriolae. In this study, genes associated with mycolic acid biosynthesis within the FAS-II system (MabA, HadAB, InhA and KasA) were identified in the wild-type strain ZJ0503. Targeted knockout of these FAS-II enzyme genes yielded the mutants ΔMabA, ΔHadAB, ΔInhA and ΔKasA, which exhibited loss of acid-fast staining and a complete absence of mycolic acid derivatives. Virulence of these mutants was significantly

Mechanistic Analysis of Programmed Iteration by Module 5 of the Nocardiosis-Associated Polyketide (NOCAP) Synthase.

Assembly line polyketide synthases (PKSs) possess multimodular architectures in which each module harbors the requisite protein domains to catalyze a single round of polyketide chain elongation and postelongation modifications. Exceptions to this paradigm are modules that catalyze multiple elongation cycles, a phenomenon referred to as "programmed iteration". The molecular mechanism that allows PKS modules to iterate remains poorly understood. For example, Module 5 of the nocardiosis-associated polyketide (NOCAP) synthase catalyzes three elongation cycles during the biosynthesis of its undecak

Case Report: diagnosis of mixed Scedosporium apiospermum infection assisted by bronchoalveolar lavage fluid morphology.

BACKGROUND: Scedosporium apiospermum is a highly aggressive opportunistic pathogen, widely distributed in natural environments. Infections predominantly occur in immunocompromised individuals but may also affect immunocompetent individuals with predisposing factors such as trauma, drowning, exposure to contaminated water, diabetes, or malnutrition. CASE PRESENTATION: In the reported case, morphological abnormalities were initially identified in bronchoalveolar lavage fluid (BALF). Subsequent microbial culture identification and metagenomic next-generation sequencing (mNGS) confirmed a mixed in

Advances in the treatment of mycetoma: the emerging role of nanomedicine.

Mycetoma is a chronic, debilitating, neglected tropical disease characterized by a progressive subcutaneous infection caused by either fungi (eumycetoma) or bacteria (actinomycetoma), typically affecting the limbs, but no body part is immune. Current treatments remain limited, relying on long-term antifungal or antibiotic regimens that offer only modest efficacy, carry significant toxicity; and often surgical intervention is required. Drug-repurposing efforts, including computational screening and phenotypic assays, have identified several FDA-approved compounds with potential activity against

Whole-genome sequencing and analysis of Nocardia veterana isolates from keratitis patients.

In this study, we conducted whole-genome sequencing and comparative genomic analysis of two Nocardia veterana isolates (JJAEH1 and JJAEH2) from severe corneal ulcers to elucidate their pathogenic potential, antimicrobial resistance determinants, and genomic features associated with steroid–antibiotic interactions in keratitis. Genome assemblies, annotated with RAST and Prokka, ranged from 7.2 to 7.5 Mb with a GC content of ~ 68.2%. Pan-genome analysis (Panaroo) revealed conserved virulence determinants, including fbpA, fbpB, and fbpC (fibronectin-binding mycolyltransferases), stp (serine/threo

Elusive Diagnosis of Recurrent Subcutaneous Emphysema: Nocardia farcinica Infection in an Immunocompetent Female Patient.

This case report describes an immunocompetent female with recurrent subcutaneous emphysema and refractory soft tissue infections involving multiple non-contiguous sites-bilateral breasts, chest wall, and upper limb-over seven years, consistent with disseminated nocardiosis. Initial presentations mimicked bacterial mastitis, with localized swelling, erythema, crepitus, and elevated inflammatory markers. Despite repeated incision and drainage procedures, antibiotic therapies, and bilateral mastectomies, symptoms recurred persistently. Conventional microbial cultures repeatedly failed to identify

Molecular mechanisms underlying Nocardia host interactions.

Nocardia species are opportunistic pathogens that cause localized and disseminated infections, particularly in immunocompromised individuals. Despite their clinical importance, the molecular mechanisms underlying Nocardia pathogenicity remain incompletely understood. This review summarizes current advances in Nocardia virulence factors, host immune responses, and intracellular survival strategies. A diverse array of virulence factors enables Nocardia to invade host cells, circumvent immune defenses, and maintain persistence within host tissues, including mammalian cell entry (Mce) proteins, an

Nocardiosis: clinical and pathologic examination of 140 cases representing 18 species in 6 complexes.

OBJECTIVES: Before molecular taxonomy, Nocardia species infections were predominantly attributed to Nocardia asteroides; now, more than 50 species are known to infect humans. To better study the range of associated clinicopathologic features, Nocardia species can be grouped into complexes based on gene sequencing, antibiotic susceptibilities, and matrix-assisted laser desorption/ionization with time-of-flight mass spectrometry profiling. METHODS: A 20-year retrospective study of laboratory-detected Nocardia species cases from 2 US tertiary-care academic medical centers was performed by review

Clinical and microbiological characteristics of Nocardia infections: a 10-Year multicentre study from Türkiye.

PURPOSE: Multicentre studies from countries such as Spain, Australia, Japan, China, and Taiwan have revealed significant geographic variation in Nocardia species patterns, resistance profiles, and patient outcomes. In Türkiye, however, most available data originate from single-centre reports with limited species-level identification and antimicrobial resistance profiling, highlighting a significant gap in the current understanding of nocardiosis at the national level. Our aim was to evaluate the clinical and microbiological characteristics of Nocardia infections in Türkiye. METHODS: In this m
