Striatal astrocyte μ-crystallin (Crym) loss contributes to early-stage Huntington's disease pathophysiology.

A striatal astrocyte subpopulation expressing μ-crystallin (gene: Crym) regulates perseverative behaviors, but its role in disease is unclear. We found that astrocytic Crym and μ-crystallin protein expression levels are reduced in human Huntington's disease (HD) tissue, cerebrospinal fluid, and HD mouse models. Selective astrocytic μ-crystallin restoration in a HD mouse model improved astrocyte and neuronal morphology, electrophysiology, gene expression, and early-stage compulsive and repetitive behaviors. Late-stage motor deficits remained mostly unchanged. Approximately 32% of the μ-crystall

Brain weight across the adult lifespan in great apes.

Reduction in brain weight is a hallmark feature of human aging, which is exacerbated in pathologies, such as Alzheimer's disease and other dementias. However, it remains unclear whether a decrease in brain weight with age reflects a vulnerability unique to the human lineage or represents a consequence of an extension of aging processes shared with other primates. To address this question, we examined age-related changes in brain weight across our closest living relatives, the great apes (chimpanzees, bonobos, gorillas, and orangutans). The sample spanned the full adult age range in each specie

Obstructive sleep apnoea and Alzheimer's disease pathology: a review of plausible pathological mechanisms and potential modifying impact of hypertension.

Alzheimer's Disease (AD) is estimated to account for up to 75% of all dementia cases, of which there is a global prevalence of 50 million. There are no curative treatments available for AD dementia, therefore, early identification of risk factors is paramount. Up to 40% of all dementia cases may be attributable to modifiable risk factors. One such risk factor is poor sleep. Obstructive sleep apnoea (OSA) is a common sleep disorder characterised by repeated upper airway collapse, contributing to intermittent hypoxaemia and sleep fragmentation. Hypertension is frequently comorbid with OSA and is

Regional amyloid-PET clearance dynamics and anti-Aβ monoclonal antibody therapy: a preliminary real-world study.

INTRODUCTION: Amyloid positron emission tomography (PET) is the reference biomarker for treatment target engagement in anti-amyloid beta (Aβ) therapies in early symptomatic Alzheimer's disease (AD), yet global metrics may mask a regional heterogeneity of amyloid clearance. METHODS: We report real-world longitudinal amyloid-PET data from 26 patients treated with lecanemab or donanemab for 6 months. Global Centiloids and regional standardized uptake value ratios (SUVRs) were quantified at baseline and 6 months and examined according to treatment, clinical diagnosis, and clinical stage. RESULTS

Depression and anxiety in vascular dementia: an update.

Depression with a prevalence between 20% and 86% and anxiety with a prevalence up to 72% most frequently coexist with vascular dementia (VaD) and reciprocally increase disease risks. Both psychopathological disorders are usually more frequent and more severe in patients with VaD than in those with Alzheimer disease (AD). Although there is a significant association between depression and all-cause dementia, VaD with depression has more comorbidities and higher mortality than without depression. A history of depression prior to dementia is more common than in AD, while its pre-disease state is a

Spatiotemporal heterogeneity and dynamic evolution of microglial glucose metabolic reprogramming in Alzheimer's disease.

Alzheimer's disease (AD) is accompanied by persistent disturbances in brain energy metabolism, and microglial glucose metabolic reprogramming constitutes an important link among pathological protein deposition, neuroinflammation, and impaired clearance. This review examines these metabolic changes across disease stages, brain regions, and plaque-associated microenvironments and proposes a dynamic state-composition model. In this model, microglia in different metabolic states coexist, and their relative proportions and spatial distribution change as AD progresses. In early disease or during act

The problem with Alzheimer's disease.

Despite decades of intensive research, progress in treating late-onset Alzheimer's disease remains limited. A central problem may be conceptual: Alzheimer's disease may not be a single disease, but a heterogeneous syndrome arising from differing combinations of aging, genetic susceptibility, vascular injury, metabolic stress, inflammation, and other pathologies. Research has focused largely on biomarkers rather than the pathways leading to them. The dominance of amyloid- and tau-centered models, together with limited animal models, may constrain therapeutic progress. A more productive strategy

Exploratory associations between APOE-ε4 carrier status and facial neuromuscular activity in cognitively intact adults: a cross-sectional surface electromyography study.

INTRODUCTION: The APOE-ε4 allele is a genetic risk factor for late-onset Alzheimer's disease (AD). Beyond cognitive decline, APOE-ε4 affects motor function, reducing muscle strength and coordination, potentially through mitochondrial dysfunction and oxidative stress. This study examined the influence of the APOE-ε4 allele on neuromuscular function in oral muscles involved in speech production, using surface electromyography (EMG); and evaluated the preliminary discriminatory performance of EMG-based multivariable models in differentiating APOE-ε4 carriers from noncarriers. METHODS: Forty-two

Cell-based polygenic risk scores predict clinical progression and prioritize network-based therapeutic targets in Alzheimer's disease.

BACKGROUND: Late-onset Alzheimer's disease (AD) exhibits substantial biological heterogeneity. We developed a framework linking cell-type-specific polygenic risk profiles to precision medicine in AD. METHODS: Cell-based polygenic risk scores (cbPRSs) derived from single-nucleus RNA-seq co-expression networks were evaluated in Alzheimer's Disease Neuroimaging Initiative (ADNI) and Framingham Heart Study (FHS) cohorts. Network hubs were prioritized using a graph-based PageRank algorithm and candidate drugs were validated in human-induced pluripotent stem cell (hiPSC)-derived astrocytes. RESULT

Epigenetic Age and Resilience in the Framingham Heart Study.

BACKGROUND AND OBJECTIVES: Growing evidence suggests that decreased epigenetic age relative to chronological age may be protective against late-life cognitive decline and dementia, but the mechanism leading to this resilience is unknown. METHODS: DNA methylation (DNAm), plasma total tau (t-tau), MRI total cerebral brain volume (TCBV), and neuropsychological (NP) data were obtained from Framingham Heart Study Offspring cohort participants. Three epigenetic age measures, DunedinPACE, PCPhenoAge, and PCGrimAge, were estimated from DNAm data. DunedinPACE and residuals from regressing PCPhenoAge a

Extracellular exosome signature links hypertension and Alzheimer's disease.

Untreated hypertension is a risk factor for late-onset Alzheimer's disease (AD); however, this association is not well understood. The aim of this study was to reveal protein signatures that bridge the pathophysiological changes in hypertension to AD. Using untargeted proteomics, we analyzed brain samples from aged (30- and 40-week-old) spontaneously hypertensive rats (SHRs, n = 12), and age-matched normotensive Wistar Kyoto (WKY, n = 8) controls, and human AD patients (Braak stages 4-6) (n = 30), cerebral amyloid angiopathy (CAA) (n = 5), non-demented controls (Braak stages 0-3) (n = 37). Correction of saccadic decisions during active visual search in the monkey.

IMPORTANCE: Apolipoprotein E (APOE)*2 and APOE*4 are, respectively, the strongest protective and risk-increasing, common genetic variants for late-onset Alzheimer disease (AD), making APOE status highly relevant toward clinical trial design and AD research broadly. The associations of APOE genotypes with AD are modulated by age, sex, race and ethnicity, and ancestry, but these associations remain unclear, particularly among racial and ethnic groups understudied in the AD and genetics research fields. OBJECTIVE: To assess the stratified associations of APOE genotypes with AD risk across sex, a

Sleep arousals are associated with the polygenic risk for developing Alzheimer's disease and with cognitive change in healthy late middle-aged individuals.

STUDY OBJECTIVES: Sleep disturbances are increasingly recognized as early features of Alzheimer's disease (AD) neuropathology. Specifically, spontaneous arousals during sleep have been associated with the burden of Amyloid beta in the brain of healthy late middle-aged individuals. However, it remains unclear whether heterogeneity of arousals relates to genetic risk for AD in younger adults or to cognitive change later in life. Here, we evaluated the association between arousals, polygenic risk scores (PRS) for AD, and cognitive performance and change in healthy young and late-middle-aged indiv

Low-dose CT for MRI-free quantification of tau PET in Alzheimer's disease: a multicentre, cross-sectional, comparison study.

BACKGROUND: Tau PET quantification typically requires structural MRI for reference-region definition, normalisation, and segmentation. We evaluated whether low-dose CT can serve as a reliable MRI alternative for tau PET quantification when MRI is unavailable. METHODS: In this multicentre cross-sectional methods comparison study, we developed a PET-CT pipeline using low-dose CT as the sole anatomical reference for tau PET quantification. Participants were recruited from six sites in the multicentre Head-to-head Harmonisation of tau PET tracers (HEAD) study. The cohort comprised cognitively uni

The association of Alzheimer's disease genetic risk with social connectedness in middle- and older ages.

Observational evidence suggests that social connectedness protects against Alzheimer's disease (AD), but reverse causality has not been ruled out. We evaluated the potential for a reverse path by estimating associations between AD genetic risk score (AD-GRS) and social connectedness across mid and late-life. We used data from 487 194 UK Biobank participants aged 40+ years and considered social connectedness measures capturing social isolation, loneliness, relationship satisfaction, emotional support, and diverse social activity participation. Participants' mean age was 56.5 (SD: 8.2) years. Hi

Alzheimer and cardiovascular genetic scores and cognition: the FINGER randomized controlled trial.

Alzheimer's disease and coronary artery disease are common late-life chronic conditions and share multiple risk factors, including the apolipoprotein E (APOE) ε4 allele. A meta-analysis of two multidomain lifestyle intervention trials found greater cognitive benefits in APOE4 carriers compared with non-carriers. This study investigated the impact of genetic risk scores for Alzheimer's disease and coronary artery disease (AD-GRS, CAD-GRS) on cognition in the Finnish Geriatric Intervention Study to Prevent Cognitive Impairment and Disability (FINGER) randomized controlled trial. FINGER included

Microglia-derived extracellular vesicle content as a biomarker for early detection of Alzheimer's disease.

Alzheimer's disease (AD) is a progressive neurodegenerative disorder characterized by amyloid-beta aggregation, tau pathology, synaptic dysfunction, and neuronal loss. While current biomarker frameworks such as the A/T/N classification have advanced diagnosis, they predominantly reflect relatively late stages of disease, limiting opportunities for early intervention. This underscores the urgent need for biomarkers that are not only accessible and sensitive but also specific to the earliest pathogenic mechanisms of AD. Recent work has underscored the pivotal role of disease-associated microglia

Hybrid multimodal late fusion frameworks for bvFTD classification in imbalanced dementia datasets.

BACKGROUND: Behavioral variant frontotemporal dementia (bvFTD) is an irreversible neurodegenerative disorder characterized by progressive changes in personality and behavior. Magnetic Resonance Imaging (MRI) is widely used to detect and assess structural brain alterations associated with the disease. However, due to the low prevalence of bvFTD among neurodegenerative diseases causing the dementia syndrome, conventional machine learning approaches may struggle to capture comprehensive feature representations. Therefore, this study proposes two late fusion frameworks that integrate a 3D convolut

Exploring the role of statin interventions on cognitive function by Mendelian randomization study.

BACKGROUND: While statin use is linked to cognitive impairment, causal relationships with specific dementia subtypes remain unclear. OBJECTIVE: To investigate causal effects of statin medication, genetically mimicked statin effects, and plasma lipids on various cognitive impairments using Mendelian randomization (MR). METHODS: Utilizing summary genome-wide association studies data, we identified genetic variants associated with statin medication, cholesterol-dependent/independent statin effects, and dementia subtypes, such as Alzheimer's disease (AD), dementia in Alzheimer's disease (ADD), l
