A New Frontier in CWD Detection: Antemortem Plasma Biomarkers and Behavioral Profiling in Transgenic Mouse Models.

Chronic Wasting Disease (CWD) is a fatal transmissible spongiform encephalopathy (TSE) that is confined to cervids (deer, moose, elk, and reindeer) but shares key properties with human neurodegenerative conditions such as Alzheimer's, Parkinson's, Huntington's disease and frontal-temporal dementia. CWD and other TSEs are caused by the misfolded prion protein (PrP). Although the identification of diagnostic and prognostic biomarkers at all stages of disease progression is becoming exceedingly critical as CWD continues to increase in prevalence, accurate antemortem testing techniques are extreme

A wearable accelerometry landscape of human health and disease.

Digital wearable devices provide objective characterization of human behaviors in real-world settings, yet their implications for health and diseases remain incompletely understood. Here, we analyzed a large-scale accelerometry-based cohort, including 70,473 participants with up to 10 years of follow-up, yielding a total of 11.8 million person-hours of data. From this cohort, we constructed a comprehensive atlas of 210 accelerometer-derived phenotypes (ADPs), encompassing physical activity, sedentary behavior, sleep, circadian rhythm, step counts, and step intensity. The study identified 8149

Amyloid, p-tau, neurofilament light, and α-synuclein cerebrospinal fluid biomarkers across Alzheimer's disease, dementia with Lewy bodies, and Parkinson's disease.

INTRODUCTION: Mounting evidence suggests overlapping pathology across Alzheimer's disease (AD), dementia with Lewy bodies (DLB), and Parkinson's disease (PD), yet its extent and clinical implications remain unclear. METHODS: Cerebrospinal fluid (CSF) amyloid beta (Aβ)42/40, phosphorylated tau 181 (p-tau181), neurofilament light chain (NfL), and total α-synuclein were centrally measured using fully-automated Elecsys immunoassays in 347 participants (European Platform for Neurodegenerative Diseases [EPND] cohort) and compared between groups. Associations of amyloid and tau positivity with cogni

Association of Skin Denervation With Brain Atrophy and Cognitive Impairment in Patients With Parkinson Disease.

BACKGROUND AND OBJECTIVES: Small fiber pathology is an intrinsic feature of Parkinson disease (PD), yet its relevance to central neurodegeneration remains unclear. Given that cognitive impairment is a major manifestation of disease progression closely linked to cortical structural changes, we aimed to determine whether peripheral skin denervation is associated with cognitive impairment and brain atrophy in PD. METHODS: In this cross-sectional study at a movement disorders center in China, patients with PD and healthy controls (HCs) were recruited. Key inclusion criteria included a clinical PD

Berbamine hydrochloride as a brain-penetrant galectin-3 inhibitor in a model of Huntington's disease.

Galectin-3 (Gal3) acts as an intracellular sensor of lysosomal damage and a key driver of microglial activation and neuroinflammation in multiple neurodegenerative disorders. Despite its biological importance, no inhibitors have been identified that both modulate intracellular Gal3 activity and effectively cross the blood-brain barrier. Here, we established an innovative high-content imaging platform using microglia expressing Gal3-GFP, allowing direct visualization of Gal3 puncta formation upon lysosomal damage as a phenotypic readout for compound screening. Through screening of ∼24 000 small

Individuals with dementia with lewy bodies exhibit worsened reactive walking balance responses to treadmill-induced perturbations than matched controls.

BACKGROUND: Dementia with Lewy Bodies (DLB) significantly increases older adults' risk of falling far more than other forms of dementia. This increased falls risk may be attributed to worse proactive balance adjustments due to reduced cognitive functioning and/or worse reactive responses due to symptoms such as parkinsonism. However, no studies have deployed balance perturbations in individuals with DLB to interrogate their balance integrity. METHODS: This study aimed to (1) determine the feasibility of deploying walking balance perturbations in individuals with DLB and (2) quantify differenc

Quadra-Brain: Quadruplet Higher-Order Topological Representation Learning for fMRI-Based Brain Disorder Diagnosis.

Accurately characterizing higher-order coordination among distributed brain regions is important for understanding brain disorders from fMRI data. Existing methods mainly rely on pairwise connectivity or low-order motifs, which may overlook disease-related alterations in collective multi-region communication and higher-dimensional topological organization. To address this limitation, we propose Quadra-Brain, a computational framework for modeling quadruplet higher-order interactions in fMRI-based brain disease diagnosis. Quadra-Brain first estimates time-resolved quadruplet co-fluctuations usi

A Live-Cell Imaging System for the Identification of Pro- and Anti-Aggregatory α-Synuclein Modulators.

α-Synuclein (αSyn) aggregation is a defining pathological feature of synucleinopathies, including Parkinson's disease, dementia with Lewy bodies, and multiple system atrophy (MSA). However, existing in vitro methods to monitor αSyn aggregation often rely on endpoint assays that cannot distinguish monomeric from oligomeric species or capture early aggregation dynamics. Here, we implement an αSyn bimolecular fluorescence complementation (αSyn-BiFC) strategy in a stable human glial-derived cell system optimized for real-time, high-content analysis of early αSyn self-association events and their m

Cognitive impairment in Parkinson disease correlates with loss of cholinergic axon projections to cortex in an autopsy study.

INTRODUCTION: People with Parkinson's disease (PD) frequently develop dementia. We investigated the relationship between cognitive impairment and the loss of cholinergic axon projections from the nucleus basalis of Meynert to neocortical regions in PD. METHODS: Levels of choline acetyltransferase (ChAT), the enzyme required for acetylcholine synthesis, were measured with a sandwich enzyme-linked immunosorbent assay (ELISA) in eight brain regions from 37 PD autopsy cases. Analyses focused on relationships between measures of global cognitive impairment and average ChAT levels in four neocortic

Extracellular Vesicles and Exosomes as Biomarkers in Huntington's Disease: Uncovering Pathogenic Insights and Pioneering Therapies.

Huntington's disease (HD) is a neurodegenerative disorder caused by the expression of the mutant huntingtin protein (mHTT), characterized by a progressive decline in motor function, cognitive abilities, and psychiatric symptoms. Exosomes, a distinct class of small extracellular vesicles, have emerged as promising diagnostic biomarkers and potential therapeutic tools for early disease detection and intervention. This is largely attributed to their molecular stability, cell type-specific cargo, and potential to cross the blood-brain barrier under optimized structural or delivery conditions. Extr

A Silent RAGE: The Inflammatory Pathway to Dementia in Parkinson's Disease.

Cognitive decline in Parkinson's disease (PD), progressing from mild cognitive impairment (PD-MCI) to severe dementia (PD-D), is a debilitating core feature driven by complex neuropathological interactions. While α-synuclein accumulation is the disease's hallmark, co-pathologies involving amyloid-β and tau, alongside chronic neuroinflammation, are critical accelerators of cognitive collapse. This review positions the receptor for advanced glycation end products (RAGE) as a key molecular orchestrator bridging these disparate pathological events. Functioning as a multi-ligand sensor, RAGE binds

Long-term exposure to ultrafine particles and mortality from neurodegenerative diseases: a population-based cohort study.

BACKGROUND: Transportation and industrial activities emit large quantities of air pollutants, including unregulated ultrafine particles (UFP, <0.1 um), which may adversely affect brain health. METHODS: We analyzed data from 2.1 million adults in the Canadian Census Health and Environment Cohorts (CanCHECs) living in Montreal and Toronto (2001-2019). Vital status and primary cause of death were obtained from the Canadian Vital Statistics- Death database. We assigned high-resolution estimates of UFP number concentrations and UFP size to participants' residential postal codes using three-year mo

SPORADIC CREUTZFELDT-JAKOB DISEASE: CLINICAL FEATURES, DIAGNOSIS AND THERAPEUTIC HORIZONS.

Sporadic Creutzfeldt-Jakob disease (sCJD) is a rare, rapidly progressive, and fatal prion disease that remains a major diagnostic and therapeutic challenge. Its presentation often overlaps with other causes of rapidly progressive dementia, making early recognition essential, particularly to exclude potentially reversible conditions. This review provides an updated overview of the diagnostic tools, cerebrospinal fluid (CSF) biomarkers, prognostic factors, clinical progression, differential diagnosis, and current management strategies in sCJD. Stage-dependent tau-PET signatures in Huntington's disease revealed by [¹⁸F]PI-2620.

PURPOSE: Neuropathological and biomarker evidence implicates tau dysregulation as a downstream component of Huntington's disease (HD) pathobiology, yet its in vivo distribution has not been characterised using second-generation tau-PET tracers. We aimed to define the regional organisation, stage dependence and clinical relevance of tau-sensitive PET signal across the HD disease spectrum. METHODS: Fifty-four participants (13 healthy controls, 9 premanifest mutation carriers and 32 manifest carriers) underwent 60-minute dynamic [¹⁸F]PI-2620 PET imaging. Tau-PET signal was quantified using distr

Nanomedicines for neurodegenerative ageing: nasal delivery innovations for Alzheimer's and Parkinson's disease.

Alzheimer's disease and Parkinson's disease are progressive, age-related neurodegenerative disorders with increasing global prevalence, yet their treatment remains challenging despite the availability of multiple therapeutic agents. Conventional formulations are often limited by poor solubility, restricted blood-brain barrier penetration, extensive first-pass metabolism, short elimination half-life, low brain bioavailability, and systemic adverse effects. The nose-to-brain route has emerged as a promising strategy for delivering therapeutics directly to the brain. This approach offers non-inva

Progressive Cognitive Decline and Pyramidal Signs in a Patient With a Novel Homozygous c.395A>T; p.Lys132Met Mutation in CHCHD2.

Biallelic CHCHD2 variants are rare. We report a consanguineous man with a novel homozygous CHCHD2 c.395A>T (p.Lys132Met) variant who developed progressive cognitive decline, apraxia, oculomotor impairment, and pyramidal signs without parkinsonism. Clinicians should consider CHCHD2 in familial dementia with pyramidal signs, even when motor parkinsonism is absent.

Anti-osteoporosis medications in persons with and without Parkinson disease in Finland.

UNLABELLED: Contrary to reports on under-treatment in persons with Parkinson's disease, our study found similar anti-osteoporosis medication usage before and after their hip fractures among Finns with and without Parkinson's disease. Additional preventive measures may be needed to mitigate the high risk of hip fractures among persons with Parkinson's disease. PURPOSE: Parkinson's disease (PD) is associated with a doubled risk of hip fracture. However, previous reports suggest under-recognition of osteoporosis among persons with PD. Our study aimed to assess the annual prevalence of anti-osteo
