People with Huntington's disease (HD) commonly experience anxiety, yet access to psychological interventions remains limited. Guided self-help is a low-cost, scalable, evidence-based approach with the potential to improve access to psychological support. This qualitative study aimed to explore participants' experience of GUIDE-HD, a remote guided self-help intervention designed to address anxiety in people with HD based on cognitive behavioural therapy and acceptance and commitment therapy, by examining: (i) the acceptability of the intervention, (ii) any perceived benefits or challenges, and

BackgroundHuntington's Disease (HD) is a hereditary neurodegenerative disorder caused by a cytosine-adenine-guanine (CAG) repeat expansion (CAG > 35) in the Huntingtin (HTT) gene. Intermediate alleles (IAs, CAG = 27-35) are generally not associated with HD. However, IA carriers with symptoms have been reported in literature.ObjectiveTo review the existing literature on IAs, in order to provide an overview of the clinical phenotype of IA carriers.MethodsPeer-reviewed articles published between 1993 and July 2024 from three databases (Embase, PubMed, and Web of Science) were included.ResultsIn c

ObjectiveExamine cortical activation patterns in Huntington's disease (HD) under single-task (ST) and dual-task (DT) balance conditions compared to controls using portable functional near-infrared spectroscopy (fNIRS).BackgroundIndividuals with HD have difficulty multitasking while performing balance tasks, so previously automatic tasks may require more attentional resources to maintain stability and prevent falls. Our understanding of the neural mechanisms underlying the relationship between impaired cognition and balance in HD is minimal. fNIRS provides a noninvasive means to functionally im

Huntington's disease (HD) is a neurodegenerative disorder caused by an expanded trinucleotide repeat in the huntingtin gene (HTT) that subsequently leads to aggregation of the mutant huntingtin (mHTT) protein. Thus, lowering mHTT is a key therapeutic approach used by several candidate therapeutics currently under investigation. Visualization of the efficiency of these therapeutics through in vivo mHTT quantification rises in importance. For positron emission tomography (PET) imaging of mHTT aggregates, it is critical to characterize the in vivo kinetic profile of newly identified mHTT binders

Progressive neurodegeneration with the movement disorder can be challenging to diagnose. In this article, we present a 12-year-old female child with stroke and progressive neurocognitive decline that was followed by choreo-athetoid movements and worsening dystonia and epilepsy. There was diffuse cortical involvement with predominant frontal-parietal lobes involved, affecting speech and bladder bowel control. Despite no significant family history, the genetic evaluation helped us to diagnose the rare condition. We also discuss the various challenges faced while managing and diagnosing the patie

BACKGROUND AND OBJECTIVES: Huntington disease is an autosomal dominant neurologic disorder caused by an unstable cytosine-adenine-guanine (CAG) expansion (>35 CAG) in the HTT gene. The CAG repeat length is the major determinant of disease onset and penetrance. In addition, sequence variants within the HTT repeat, not detectable by currently used fragment size-based assays in diagnostic laboratories, are potent modifiers of disease onset and penetrance, particularly in carriers of small repeat expansions (<40 CAG). However, differences regarding severity, progression, and phenotype are poorly c

Previous studies investigating language deficits in Huntington's disease (HD) have reported relatively preserved lexical and semantic systems. Nevertheless, some aspects such as semantic word learning have never been previously explored. With this aim, in two experiments we used a well-known contextual word-learning paradigm to study two groups of early-stage HD patients with matched controls. The task required participants to infer the meaning of new words by extracting and integrating semantic cues from different sentences in a self-paced reading paradigm. The results showed that patients di

Huntington's disease (HD) is a severe neurodegenerative disorder that causes motor impairment and ultimately death. Yet, safe and effective disease-modifying treatments of HD are scarce, and delivery to the HD brain is challenging. Here, we present a small, ∼11 nm polyethylene glycol-coated gold nanoparticle (NP) for brain delivery and treating HD in R6/2 mice. Upon intravenous injection, this NP crosses brain barriers, preferentially accumulates in the brain of R6/2 mice than healthy littermates without pronounced liver or kidney sequestration, diffuses to the cortex and striatum, and enters

BACKGROUND: Impaired awareness occurs in people with dementia (DEM; Alzheimer’s disease (AD), including mixed AD/vascular), Huntington’s disease (HD) and Korsakoff’s syndrome (KS). We explored the experiences of these people and their informal caregivers, to gain a better understanding of this phenomenon. METHODS: We conducted 28 in-depth, semi-structured interviews with 14 dyads, consisting of people with DEM, HD, or KS who had impaired awareness, and their informal caregivers. Topics discussed included impaired awareness of diagnosis and functioning, as well as the consequences of impaired a

The identification of the repeat expansion which causes Huntington's disease in 1993 soon led to a clinical genetic test for the condition, enabling people at risk to have a test to determine whether they will get the disease. The primary determinant of age at onset in Huntington's disease is CAG repeat length, but in recent years there have been advances in identifying and characterising genetic modifiers which influence age at onset. This has led to the question of whether these data may be applied clinically to improve clinical practice. Here, on behalf of the European Huntington's Disease

BackgroundRecent evidence suggests that accumulation of mutant exon 1 protein (HTT1a) may be critical to HD pathogenesis, but the relation of this to differential regional and cellular vulnerability in HD is unknown.ObjectiveWe assessed the contribution of the accumulation of the mutant huntingtin HTT1a to the regional and cellular variation in HD brain pathology by determining if more vulnerable regions and neuron types were relatively enriched.MethodsWe performed immunolabeling using the novel monoclonal antibodies 11G2 and 1B12 against the C-terminal proline 90 (P90) neoepitope of huntingti

OBJECTIVES: Huntington's disease (HD) is a hereditary neurodegenerative disorder characterized by a range of motor, cognitive, and psychiatric symptoms. Psychological symptoms can arise from being at risk for the disease and from its manifestation, necessitating psychological interventions to address the evolving burden, even before onset. This qualitative study explores the psychological interventions used for HD within the Dutch health care context and identifies barriers and facilitators for their implementation across the different HD disease stages.
METHODS: A qualitative approach using

BACKGROUND: Huntington's disease (HD) is a progressive neurodegenerative disorder. Models of brain biological age have shown evidence of accelerated aging relative to chronological age, but they typically rely on a single whole-brain measure. While studies in other neurodegenerative diseases suggest region-specific brain age models can provide deeper insights, this approach remains underexplored in HD. Such regional models could benefit clinical trials, which depend on sensitive biomarkers to monitor therapeutic effects.
OBJECTIVES: This study aimed to characterize region-specific patterns of

Huntington's disease (HD) is a progressive neurodegenerative disorder characterized by motor, cognitive, and metabolic dysfunction, largely driven by mitochondrial impairment and defective energy metabolism. Altered signaling through hypoxia-inducible factor-1α (HIF-1α) and PI3K/AKT cascades contributes to neuronal vulnerability. Canagliflozin (Cana), a sodium-glucose cotransporter-2 inhibitor, has shown cognitive benefits in experimental studies. Here, we evaluated whether Cana mitigates 3-nitropropionic acid (3NP, 10 mg/kg, i.p., 14 days)-induced HD-like neurotoxicity in rats. Animals receiv

The authors report the case of a patient diagnosed and treated with schizophrenia for nearly 20 years. A rapid change in the neurological status of the patient necessitated a diagnostic revision. The patient's additional medical history included narcolepsy and psoriasis. With a stable maintenance antipsychotic combination regime, one and a half years after the last inpatient treatment for a psychotic relapse, initially mild lower limb hyperkinesis developed. At first, this hyperkinesis was hypothesized to be antipsychotic side effects. The decrease of the antipsychotic daily dose resulted in n

Protein palmitoylation in the Golgi apparatus is critical for the appropriate sorting of various proteins belonging to secretory and lysosomal systems, and defective palmitoylation can lead to the onset of severe pathologies. HIP14 and HIP14L ankyrin repeat-containing palmitoyl transferases were linked to the pathogenesis of Huntington's disease, however, how perturbation of these Golgi resident enzymes contributes to neurological disorders is yet to be understood. In this study, we investigated the function of Hip14 and Patsas - the Drosophila orthologs of HIP14 and HIP14L, respectively - to

BACKGROUND AND AIM OF THE STUDY: Although next-generation cardiac prostheses have shown favorable results in transcatheter aortic valve implantation (TAVI), these have mostly been documented in intermediate-risk patients. Whether this could be translated to high-risk patients is not known. Hence, the safety and clinical performance of the new, repositionable CoreValve Evolut R-System (ERS) was evaluated by comparison with a non-repositionable CoreValve-System (CVS), in 96 high-risk/inoperable (HRI) patients.
METHODS: The primary safety end points were mortality and stroke, defined by VARC-2 c

OBJECTIVE: Irritability has been reported as a common psychological co-occurrence with Huntington's disease (HD). Irritability associated with HD is variously attributed to neurological changes, an understandable response to increasing difficulties with cognition and communication, or a multifactorial combination of the two or more causes. To improve our understanding, we aimed to explore perspectives and lived experiences of irritability among individuals affected by HD.
METHODS AND MEASURES: Individual semi-structured interviews were conducted with 12 participants (50% female) who were gene

Movement disorders are a group of heterogeneous neurological conditions associated with alterations of tone, posture and voluntary movement. They may either occur in isolation or as part of a multisystemic condition. More recently, the advent of next generation sequencing technologies has facilitated better understanding of the underlying causative genes and molecular pathways, thereby identifying targets for genetic therapy. In this review, we summarize the advances in genetic therapy approaches for both hyperkinetic and hypokinetic movement disorders, including Parkinson's Disease, Huntingto
