ADHD-Like Traits Reshape the Balance Between Inhibitory Control and Predictive Processes.

OBJECTIVE: Adaptive behavior relies on a dynamic balance between flexible, goal-directed control, and efficient, automatic processes. This equilibrium is often disrupted in ADHD, a condition understood to exist along a continuum of traits in the general population. While ADHD is consistently linked to response inhibition deficits, its relationship with statistical learning (a mechanism for habit learning) and, crucially, the interaction between these functions, remains underexplored. METHODS: Here, using a novel paradigm in a non-clinical sample of university students (n = 226), we investigat

Accidental Ingestion of Highly Toxic Drugs in Young Children: A Multicenter Study From Spanish Pediatric Emergency Departments.

OBJECTIVES: Highly toxic medications are thought to lead to fatal overdose even after single-dose ingestions. Recent evidence has questioned some of the assumptions underlying this concept. In this context, this study aimed to evaluate the incidence, management, and outcomes of accidental ingestions involving highly toxic drugs in young children presenting to pediatric emergency departments (EDs). METHODS: We performed a descriptive and analytical study using a prospective, nationwide, multicenter pediatric poisoning registry including 58 Spanish EDs from 2008 to 2024. We analyzed accidental

CLCN6 mutation in an Indian patient with rapidly progressive dementia.

The CLC family of chloride channels and transporters plays crucial roles in endosomal/lysosomal function, with dysfunction contributing to protein aggregation and dementia development. CLCN6 mutations are recognized in early-onset neurodegenerative disorders, but reports from India remain limited. We report the first case from India of frontotemporal dementia associated with a novel CLCN6 mutation. A 56-year-old woman presented with rapidly progressive behavioral changes, cognitive decline, and frontotemporal atrophy. Genetic testing revealed a novel heterozygous missense variant c.1615A>G (p.

Two siblings presented with spastic paraplegia and psychomotor retardation with or without seizures with HACE1 variants: A case report and literature review.

RATIONALE: Spastic paraplegia and psychomotor retardation with or without seizures (SPPRS) is a rare autosomal recessive neurodevelopmental disorder caused by biallelic HACE1 variants. Its long-term clinical course and genotype-phenotype spectrum remain incompletely characterized. PATIENT CONCERNS: Two siblings from a nonconsanguineous Chinese family presented with infantile-onset hypotonia, global developmental delay, markedly delayed motor milestones, gait impairment, language delay, and dysarthria. The older sister experienced recurrent febrile seizures, whereas her younger brother had no

Cervical Sympathetic Blockade for Psychogenic Nonepileptic Seizures Associated With Post-traumatic Stress Disorder: A Case Report and Neurobiological Framework.

Psychogenic nonepileptic seizures (PNES), also termed functional seizures, are episodes that resemble epileptic seizures but occur without the abnormal ictal electrical activity characteristic of epilepsy. PNES is frequently associated with trauma exposure, post-traumatic stress disorder (PTSD), and abnormalities in autonomic regulation. Emerging evidence suggests that altered interactions among limbic, autonomic, and motor networks may contribute to symptom generation. Cervical sympathetic blockade (CSB), including stellate ganglion block techniques, has been investigated as an intervention f

Bavachin Attenuates Cognitive Impairment in Scopolamine-Induced Amnesia: Behavioral, Biochemical and Molecular Insights.

Cholinergic dysfunction is a key feature of cognitive impairment, and currently available acetylcholinesterase (AChE) inhibitors provide limited symptomatic benefit and are often associated with adverse effects. This study evaluated the cognitive-protective effects of bavachin (BVC), a prenylated flavonoid from Psoralea corylifolia, and its interaction with Donepezil (DNP) in a scopolamine (SCP)-induced amnesia model. Male Swiss albino mice were pretreated with BVC (10-40 mg/kg), DNP (2.5 mg/kg), or their combination for 7 days prior to SCP challenge, followed by assessment using multiple beha

The value of timely treatment with anti-amyloid therapy for Alzheimer's disease.

INTRODUCTION: Anti-amyloid therapies can slow Alzheimer's disease (AD) progression; diagnostic and treatment delays might reduce their value. METHODS: Using dynamic microsimulation, we estimated the gross societal value of anti-amyloid treatment for Americans with early symptomatic AD. A treatment scenario slowed the rate of clinical progression by 29% over individuals' remaining lifetime (baseline mean age: 73.0); earlier treatment initiated 2 years earlier. RESULTS: Treatment generated $104,885 (95% CI: $99,291, $110,479) in gross societal value over an individual's lifetime, including qua

Disordered eating behavior and adolescent social health: evidence of interpersonal disruption across time and relationships.

BACKGROUND: Disordered eating during adolescence is common and can interrupt daily participation in family, peer, and school life. However, prior studies typically treat social relationships as precursors to disordered eating rather than as health outcomes that may deteriorate once symptoms appear. PURPOSE: To test whether adolescents with disordered eating symptoms show poorer social health across multiple relational settings, and whether impairment is stronger for multi-symptom and more persistent symptoms and changes over time. METHODS: Data came from 11 868 adolescents in the Adolescent

Efficacy of a Prescription-Based Mobile Digital Therapeutic as an Adjunct to Pharmacotherapy for the Acute Phase of Panic Disorder: Multicenter Randomized Controlled Trial.

BACKGROUND: Although pharmacotherapy is the primary treatment for patients with acute-phase panic disorder, its incomplete efficacy causes them to experience frequent panic attacks and severe anticipatory anxiety for a considerable period. A prescription-based mobile digital therapeutic (DTx) that integrates self-guided cognitive behavioral therapy (CBT), real-time symptom management, and lifestyle tracking can be used as an adjunct to pharmacotherapy for these patients, helping them achieve rapid symptom recovery. OBJECTIVE: This study aimed to evaluate the efficacy of this adjunctive DTx in

Post-traumatic stress disorder: outline and updated insights focusing on occupational factors.

BACKGROUND: Posttraumatic stress disorder (PTSD) is a stress-related disorder associated with substantial psychological distress and functional impairment, and it carries major relevance for occupational health because it frequently follows traumatic exposure in both high-risk professions and occupations not traditionally classified as high risk. This narrative review focuses on occupational risk factors, prevalence, work capacity, and implications for workers' compensation and return to work. METHODS: A non-systematic (narrative) literature search was conducted in PubMed/MEDLINE and Google S

Association between polygenic liability to depression and lifetime depression in at-risk adolescents.

Internalising symptoms including depression, anxiety and somatic symptoms in adolescence are common. They can be transient, but they can also persist and progress to major depressive disorder (MDD) Polygenic risk scores for major depression (MD-PGS) may help identify which symptomatic adolescents are at greatest risk of later disorder. In a mega-analysis of two longitudinal population-based cohorts from Brisbane, Australia, we investigated whether polygenic liability to depression improves risk stratification among adolescents with elevated internalising symptoms. Internalising symptoms were a

Recent advances of GPR40 agonists as potential anti-Alzheimer's agents.

Alzheimer's disease (AD) is a progressive and multifactorial neurodegenerative disorder involving metabolic dysfunction, neuroinflammation, mitochondrial impairment, synaptic failure, and neuronal loss. Although amyloid-beta deposition and neurofibrillary tangles composed of hyperphosphorylated tau remain central pathological hallmarks, interconnected mechanisms contribute to disease progression and limit the effectiveness of single-target therapies. Current small-molecule drugs mainly provide symptomatic relief, whereas recently approved monoclonal antibodies have broadened the therapeutic la

Serum cannabinoid profiles of emergency department patients presenting with cannabinoid hyperemesis syndrome and acute cannabis intoxication.

INTRODUCTION: The underlying pathophysiology of cannabinoid hyperemesis syndrome remains poorly understood. Proposed mechanisms include "hyperintoxication" with delta-9-tetrahydrocannabinol, accumulation of delta-9-tetrahydrocannabinol metabolites, and differential exposure to non-psychotropic phytocannabinoids. This study sought to examine quantitative differences in serum concentrations of delta-9-tetrahydrocannabinol, delta-9-tetrahydrocannabinol metabolites, delta-8-tetrahydrocannabinol, and common non-psychotropic phytocannabinoids between patients presenting to the emergency department w

Physiologic and symptomatic burden across sleep-disordered breathing severity in men and women.

BACKGROUND: Sex differences in sleep-disordered breathing (SDB) are well recognized, but whether equivalent disease severity confers similar physiologic and symptomatic burden remains uncertain. METHODS: Data from a community-based cohort of 826 adults free of major cardiopulmonary and metabolic comorbidity were analyzed. Study participants were classified as having no SDB (apnea-hypopnea index [AHI] <5 events/h), mild SDB (5.0-14.9 events/h), or moderate-to-severe SDB (≥15 events/h). Within each stratum, men and women were matched 1:1 on age, race, body mass index, and AHI. Objective polysom

Therapy for Myhre Syndrome: Goals, Misconceptions, and Current Agents.

Myhre Syndrome (MYHRS, MIM #139210) is a rare, multisystem connective tissue disorder caused by recurrent heterozygous gain-of-function pathogenic variants in the SMAD4 gene, a key player in TGF-β signaling and a regulator of extracellular matrix homeostasis. MYHRS is characterized by a progressive fibrotic phenotype affecting multiple organ systems, including the skeletal, cardiovascular, respiratory, and integumentary systems. MYHRS individuals often present with short stature, joint contractures, cardiac valve defects, subglottic stenosis, and skin thickening. Neurodevelopmental disorders,

From Symptomatic to Pre-Symptomatic: Adaptive Knowledge Distillation for Early Alzheimer's Detection Using Functional MRI.

Alzheimer's disease (AD) progresses from asymptomatic changes to clinical symptoms, underscoring the critical need for early detection to facilitate timely treatment. Functional magnetic resonance imaging (fMRI) offers non-invasive biomarkers for detection, but current methods fail to reliably identify pre-symptomatic individuals due to two key challenges: (1) Subtle, anatomically distinct fMRI patterns in pre-symptomatic cases that resemble healthy controls more than symptomatic patients, and (2) Severe class imbalance in real-world data, where healthy controls vastly outnumber pre-symptomati

Accelerated long-term forgetting as a predictor of clinical onset in presymptomatic autosomal dominant Alzheimer's disease.

BACKGROUND: In Alzheimer's disease (AD), sensitive measures of cognitive decline prior to overt symptoms are urgently needed. Accelerated long-term forgetting (ALF), where new information is retained normally over conventional testing intervals but is then lost at an accelerated rate over the following days and weeks, has been identified cross-sectionally in presymptomatic autosomal dominant and sporadic AD cohorts. We aimed to assess whether ALF testing is predictive of proximity to future symptom onset. METHODS: 20 asymptomatic autosomal dominant AD mutation carriers who performed normally

History, Challenges, and Perspectives of CNS-Targeted Transdermal Formulations.

Central nervous system (CNS) disorders, such as Alzheimer's disease (AD), Parkinson's disease (PD), and Schizophrenia (Sch) present significant challenges for healthcare systems, both in terms of prevalence and the complexity of pharmacological treatment. While current therapies offer symptomatic relief, there is a high rate of failure in addressing the full spectrum of clinical symptoms and patient adherence issues, especially in long-term care. Since ancient times, various civilizations, including the Chinese, Egyptians, and indigenous South African cultures, have investigated and utilized t

Immune alterations in schizophrenia and the effects of a therapeutic antibody: a neuroimaging study.

Immune dysfunction is implicated in the pathophysiology of schizophrenia. The 18 kDa translocator protein (TSPO), expressed by various cell types, including microglia and astrocytes, is widely used as a marker for neuroinflammation and can be quantified in vivo using PET. However, findings from TSPO PET studies in recent-onset psychosis have been inconsistent, and it remains unknown whether TSPO levels can be modified in schizophrenia. We addressed these questions with a baseline case-control comparison of patients with a first-episode psychotic disorder who were symptomatic despite antipsycho

The Therapeutic Potential of MicroRNAs Delivered By Mesenchymal Stem Cells in Parkinson's Disease, Alzheimer's Disease, and Stroke: A Systematic Review of Preclinical Studies.

Population aging has led to a substantial increase in the prevalence of neurological disorders, including Parkinson's disease (PD), Alzheimer's disease (AD), and stroke. As current therapeutic strategies are largely limited to symptomatic management, the identification of disease-modifying treatments remains a major unmet clinical need. Mesenchymal stem cells (MSCs) and MSCs-derived microRNAs (miRNAs) have emerged as promising therapeutic candidates for these conditions. A systematic literature search was conducted in the PubMed and Scopus databases in accordance with the PRISMA guidelines. Th
