Mild acute sleep restriction alters visual attention during driving: an eye-tracking study using a driving simulator.

OBJECTIVES: Safe driving requires cognitive resources such as attention, working memory, and decision-making. Sleep loss impacts these resources, affecting driving performance. Although severe or chronic sleep deprivation is well-studied, the impact of the far more common, mild and acute sleep loss remains less well understood. Similarly, current eye tracking technology in passenger vehicles relies on eye closures or head position as a marker of drowsiness. But these are downstream indicators of fitness to drive. Eye fixation patterns may be a more sensitive, early warning sign of drowsiness.

Preoperative Sedation via Cecostomy Tube in an Autistic Adolescent: A Case Report.

Preoperative sedation of autistic children is frequently complicated by challenges in communication, sensory processing, and participation in medical care. We report a 40-kg, 13-year-old autistic boy with a preexisting cecostomy tube for chronic constipation, in whom previous oral and intramuscular sedation had failed due to agitation and physical aggression. Midazolam 10 mg (0.25 mg/kg) and ketamine 200 mg (5 mg/kg) were administered via cecostomy tube, producing sedation within 20 minutes. Dental rehabilitation under general anesthesia proceeded uneventfully. Cecal administration of sedative

Comparison of long-term outcomes between opioid-free and opioid-based anesthesia in head and neck cancer surgeries: A pilot study.

UNLABELLED: Background, aim, and objective: Literature is replete with studies evaluating opioid-sparing strategies in head and neck cancer (HNC) surgeries; however, no study has attempted opioid-free anesthesia (OFA). Various long-term outcomes have not been evaluated in any cancer surgeries. This study aimed to compare OFA with standard opioid-based anesthe-sia (OBA) in HNC surgeries. The primary objective was the incidence of chronic post-surgical pain (CPSP), and the sec-ondary objective included Numeric Rating Scale (NRS)-Pain scores, Neuropathic Pain Symptom Inventory (NPSI), Douleur Neu

Low-Volume Superior Trunk and Superficial Cervical Plexus Block With Sedation as the Sole Regional Anesthetic for Proximal Humerus Fracture Fixation in a Patient With Alzheimer Disease.

BACKGROUND Proximal humerus open reduction and internal fixation (ORIF) is usually performed under interscalene brachial plexus block (ISB) or general anesthesia (GA). ISB causes ipsilateral phrenic nerve paresis in nearly all patients, and GA may raise the risk of postoperative neurocognitive disorders in elderly patients with cognitive impairment. We report the feasibility of a low-volume regional technique, used as the sole regional anesthetic with sedation, in a patient in whom GA avoidance was desirable. CASE REPORT A 68-year-old woman (American Society of Anesthesiologists physical statu

Evaluating sedation efficacy and perioperative safety of fospropofol disodium versus propofol in elderly patients undergoing elective TURP: study protocol for a randomized controlled trial.

BACKGROUND: Fospropofol disodium is the first novel water-soluble propofol prodrug in China. After administration, it is mainly converted to propofol by alkaline phosphatase on the surface of endothelial cells to exert its pharmacological effects. Compared with propofol, fospropofol disodium has the characteristics of longer duration of action, lower incidence of injection pain and adverse events related to the respiratory and circulatory systems, and no adverse reactions related to lipid metabolism. At present, there are few reports on the safety and efficacy of fospropofol disodium in the in

White noise as a non-pharmacological intervention for enhancing the recovery of rats with wound-associated sleep disorders.

OBJECTIVE: Sleep disorders are one of the common symptoms in patients with wounds, which can hinder recovery. Commonly used hypnotic medications in clinical practice are short-acting but carry risks of adverse reactions and drug abuse. As a non-invasive acoustic intervention, white noise can regulate brain electroencephalographic rhythms to improve sleep disorders. However, the interventional effects and rehabilitation outcomes of white noise on wounds with sleep disorders remain unclear. METHOD: A wound model with sleep disorders was established in rats by integrating para-chlorophenylalanin

Propofol at a Sedative Dose Potently Activates the G1-S Phase Transition to Promote Neural Stem Cell Proliferation via Down-regulated FoxO3a-p27 Pathway.

INTRODUCTION: Sedative doses of propofol have recently been reported to exert positive effects on the developing brain, potentially enhancing later cognitive function, but the underlying mechanism remains unclear. This study aims to investigate changes in cell cycle status and reveal the dominant upstream molecular cascade following sedative-dose propofol treatment of primary neural stem cells (NSCs) in vitro. METHODS: Primary C57BL/6 murine NSCs were isolated and cultured. Proliferation was assessed using CCK-8 assays, direct cell counting, and EdU labeling after exposure to 10 μM propofol.

Opioid-free versus opioid-based anesthesia for robot-assisted radical prostatectomy: a single-center randomized controlled trial.

Multimodal, opioid-sparing analgesia is standard in urological surgery, and there is additional interest in eliminating intraoperative opioids in prostate cancer surgery. Randomized evidence for opioid-free anesthesia (OFA) in robot-assisted radical prostatectomy (RARP) is lacking. In this single-center randomized controlled trial, adult men undergoing RARP were allocated 1:1 to OFA (dexmedetomidine plus ketamine, no intraoperative opioids) or a remifentanil-based technique. Both groups received identical multimodal premedication, a bilateral quadratus lumborum block, sevoflurane maintenance a

Assessment of lipemia interference on phenobarbital quantification using particle-enhanced turbidimetric inhibition immunoassay with Dimension clinical chemistry system and comparison to high-performance liquid chromatography.

Phenobarbital, an anti-seizure medication with a narrow therapeutic window, requires therapeutic drug monitoring (TDM) for safe and effective use. Lipemia is a well-known interferent in turbidimetric immunoassays, yet its impact on phenobarbital quantification compared with high-performance liquid chromatography (HPLC) has not been fully characterized. We evaluated the effect of lipemia on phenobarbital measurement using a particle-enhanced turbidimetric inhibition immunoassay (PETINIA; Dimension Xpand Plus clinical chemistry system, Siemens) versus HPLC. Simulated lipemic plasma was prepared

Discovery of an intravenous drug injection site: A meticulous cutaneous examination provides the forensic dermatologic clue to a diagnosis of homicide.

An unsuspected homicide in a decedent was considered when the meticulous cutaneous examination of the deceased resulted in the discovery of an intravenous injection site. A 24-year-old woman was found dead in her home. Initial assessment by the crime scene investigator and detective could not determine either the cause or the manner of death. Law enforcement personnel returned to the scene after the assistant medical examiner found a single intravenous needle puncture site during her cutaneous examination of the left antecubital fossa of the dead woman. Additional evidence located near the dec
