Runx1 transcription factor modulates opioid analgesia and withdrawal in humans and rodents.

Opioid analgesia and adverse outcomes vary across individuals. We show that runt-related transcription factor 1 (Runx1) modulates the microglial transcriptome and is a genetic determinant of opioid antinociceptive responses and withdrawal. In mice, Runx1 deletion in microglia produces distinct ultrastructural and transcriptomic signatures, reducing morphine potency despite no prior opioid exposure. These mice also require greater post-operative morphine and display exacerbated morphine-induced hyperalgesia and withdrawal. Single-cell RNA sequencing (scRNA-seq) and chromatin immunoprecipitation

Early-life light exposure and mental health and substance use outcomes in adolescence and young adulthood.

BACKGROUND: Experimental models suggest that in utero photoperiod influences circadian regulation and systems tied to later-life anxiety, depression, and addiction. Whether in utero photoperiod is associated with mental health and substance use in adolescents and young adults remains unknown. METHODS: 10,721 full term born children from the GUTS cohort contributed to these analyses. Total photoperiod was calculated by summing daily light hours across 280 gestational days using each offspring's exact birth date and state. We used multivariable adjusted generalized estimating equations, logisti

Patient Profiles of Buprenorphine Initiators in General Healthcare Settings: A Latent Class Approach.

BACKGROUND: Buprenorphine is the most commonly used medication for treating opioid use disorder (OUD). It may also be beneficial for individuals with chronic pain who are physically dependent on opioids but do not meet the criteria for OUD. Therefore, the buprenorphine patient population may be heterogeneous in terms of OUD and comorbidities. OBJECTIVES: To characterize the heterogeneity of patient profiles among those initiating buprenorphine treatment in a large multi-state healthcare system, focusing on comorbid pain, psychiatric, and substance use disorder diagnoses, and other patient cha

Placebo Effects in Modern Medicine: Mechanisms, Clinical Evidence, Limitations, and Future Directions.

This narrative review traces the historical background of placebo use in medicine, from its early role as a consolatory intervention prior to evidence-based practice to its scientific reframing with the emergence of randomized controlled trials and the pivotal contributions of mid-20th-century research. The article examines the mechanisms underlying placebo effects, focusing on neurobiological processes involving pain-modulating brain networks, endogenous opioid and dopaminergic pathways, and emerging evidence for hormonal and immune system involvement, alongside psychological mechanisms such

Opioid use disorder of any severity is associated with increased suicidality.

BACKGROUND: Suicide represents an underappreciated contributor to drug-related mortality, with 20%-30% of opioid overdoses estimated to be intentional. However, suicide risk relative to opioid use disorder (OUD) severity has not been well characterized. Understanding suicidality risk across the OUD severity spectrum will inform which healthcare providers should be screening and treating self-harm. METHODS: We estimated the odds of past-year suicidal thoughts, plans, and attempts relative to OUD severity using data from the National Survey on Drug Use and Health (NSDUH), a cross-sectional, nat

Bringing MOUD "In House": Implementing an Internal MOUD Program During Pregnancy in a State Prison.

Pregnant individuals with opioid use disorder (OUD) are incarcerated at higher rates than the general pregnant population and are overrepresented within the carceral system. Although OUD treatment reduces overdose risk and improves perinatal outcomes, incarceration can be a barrier to timely access and continuity of care. We describe the implementation of an integrated perinatal medications for opioid use disorder (MOUD) clinic ("the clinic") within the single state prison facility housing pregnant individuals in North Carolina. The clinic provides prenatal care, screening for substance use di

Differences in take-home methadone receipt by state policy and individual social factors in a multistate survey of people who use drugs: A cross-sectional study.

BACKGROUND: Methadone is a highly effective, strictly regulated medication to treat opioid use disorder. COVID-19 flexibilities allowed for up to 28 days of take-homes versus daily travel to clinics for observed dosing, but receiving take-homes differed widely across clinics and individuals. We examined the relationship between state take-home policies and social vulnerability on take-home methadone receipt and days' supply. METHODS: Data were from the VOICES study, a telephone survey conducted between 1/2023-8/2024 of people who use drugs from Wisconsin, Michigan, New Mexico, and New Jersey.

A community conversation process to establish resident and service provider perspectives on needs related to use and treatment of opioids and substances.

INTRODUCTION: Rates of fatal overdose due to opioid and substance use in Richmond, Virginia increased from 44.6 in 2018 to 129.5 per 100,000 city residents in 2023. The underlying contexts surrounding the increase in substance use and overdoses in Richmond, Virginia remains poorly understood. METHODS: Using community based participatory research principles (CBPR), a series of "community conversations" with neighborhood residents were conducted between May-December 2023. These events included educational information, resource connection, and facilitated qualitative focus group discussion on fa

Cannabis Use Among Individuals Treated with Medication for Opioid Use Disorder: Correlates, Patterns, and Motivations for Use.

BACKGROUND: Cannabis use is prevalent among patients receiving medication for opioid use disorder (MOUD). However, little is known about factors associated with frequent cannabis use. OBJECTIVE: To describe correlates, patterns, and motivations for cannabis use among individuals treated with MOUD. METHODS: Participants from a randomized trial (n = 303) investigating a mind-body intervention adjunct to MOUD were compared based on frequent cannabis use (≥3 days/week) versus less frequent or no use, assessing demographic and clinical characteristics. Individuals with frequent cannabis use were

Temporal and regional associations between fentanyl concentrations in the unregulated drug supply and drug-related mortality in British Columbia, Canada.

BACKGROUND: British Columbia (BC), Canada, continues to experience persistently high rates of unregulated drug toxicity. While the presence of fentanyl in the drug supply is well-established, less is known about how fluctuations in its concentration may influence mortality at the population level. This study describes geographic variation and temporal trends in fentanyl concentrations across BC and assesses if they are associated with unregulated drug toxicity rates. METHODS: Using a validated machine learning model that quantifies fentanyl and fluorofentanyl concentrations in unregulated opi

Lifting eviction moratoria shifted treatment for substance use disorders.

INTRODUCTION: About 2.7 million U.S. households face an eviction filing each year. The effect of eviction policy on substance use disorder (SUD) treatment is uncertain. METHODS: We used IQVIA all-payer claims, Jan 2020-Dec 2021, in a difference-in-differences design. Phase 1 compared states where moratoria expired March-August 2020 with states that kept protections. Phase 2 examined the August 2021 end of the federal moratorium, contrasting states with ongoing state moratoria vs none. The population was people with a mental health or SUD diagnosis. Outcomes were state-week counts of unique pa

The role of preemptive analgesia with various analgesic drugs in patients undergoing orthopedic surgeries: A systematic review 2015-2020.

INTRODUCTION: Effective postoperative pain management remains a major challenge, particularly in orthopedic surgeries, as poor control can increase patient stress, prolong opioid use, and delay recovery and hospital discharge. This structured review aimed to evaluate the effectiveness of various analgesic agents in preventing or reducing postoperative pain in patients undergoing orthopedic procedures. METHODS: A comprehensive systematic search was conducted in the Web of Science, PubMed, and Scopus databases from 2015 up to the end of December 2020. Additionally, the reference lists of releva

Characteristics and predictors of antipsychotic medication off-label use among community-dwelling older people.

BACKGROUND: Concerns have been raised regarding the common off-label use of antipsychotic (AP) medications among older adults. However, comprehensive knowledge of the determinants of AP off-label use in this population remains limited. METHODS: We examined characteristics and predictors of AP off-label use in a nationwide Finnish cohort of community-dwelling older adults with psychiatric morbidity-but without Alzheimer's disease-using longitudinal register data on sociodemographic and illness-related factors. The sample comprised four groups: those prescribed APs off-label (n = 20,563), those

Experiences of People Who Discontinue Long-Acting Injectable Buprenorphine Treatment Recently Released From Prison in New South Wales, Australia.

INTRODUCTION: Among other benefits, opioid agonist treatment (OAT; methadone and buprenorphine) is protective against overdose for people with opioid use disorder following release from custody. Long-acting injectable buprenorphine (LAIB) is safe with an apparent low risk of diversion relative to other OAT formulations in correctional settings and may become a preferred OAT formulation in this setting. Factors that influence retention in LAIB post-release are unknown. METHOD: People receiving LAIB in nine New South Wales prisons were consented to participate in a survey designed to explore tr

Emergency health care providers' positive regard for patients with opioid use disorder: Exploring the intersectional influence of race and medical condition.

OBJECTIVE: The opioid epidemic continues to devastate communities throughout the United States; however, those in need of life-saving care for opioid misuse report many barriers to accessing and initiating treatment. Opioid-related stigmatization and racial biases undermine the provision of high-quality health care, reducing the likelihood of treatment-seeking behavior among those with opioid use disorder. While people experiencing opioid-related harms such as overdose frequently present to emergency departments, an experimental test examining attitudes of emergency department personnel in reg
