Surveying Correctional Health Staff on Medication for Opioid Use Disorder Diversion: Barriers and Mitigation Strategies.

Diversion of medications for opioid use disorder (MOUD) is a well-documented concern among correctional staff and leadership and is cited as a barrier to evidence-based treatment. In this Viewpoint, we explore the perceptions of medication diversion among correctional health care professionals (HCPs) as it pertains to access to care, availability of contraband MOUD in the facility, strategies to reduce diversion, and barriers to such strategies. Data come from a survey administered to 180 correctional HCPs employed within a jail or prison facility in the United States. Among respondents, the m

Identification of common spontaneous brain activity alterations across psychiatric disorders.

Many mental illnesses share behavioral, genetic and imaging features. However, common resting-state functional abnormalities across diagnoses remain unclear. This study aimed to investigate shared spontaneous functional alterations in neural circuitry across psychiatric disorders through a transdiagnostic quantitative meta-analysis of published neuroimaging data. A voxel-wise meta-analysis was conducted to investigate amplitude of low-frequency fluctuation (ALFF) differences between patients with major psychiatric disorders (including bipolar disorders, major depressive disorders, schizophreni

Prevalence of Psychiatric Diagnoses and Preoperative Opioid Use Among Patients Undergoing Hip Arthroscopy: A Systematic Review and Meta-analysis.

BACKGROUND: Psychiatric morbidity (anxiety, depression, prior opioid use) is variably reported among patients undergoing hip arthroscopy, with prevalence estimates influenced by heterogeneous case definitions and ascertainment methods. Clarifying true prevalence and method-related differences is needed to inform preoperative screening and comparative research. PURPOSE: To quantify the pooled prevalence of psychiatric diagnoses and prior preoperative opioid use among adults undergoing hip arthroscopy and to evaluate how prevalence varies by ascertainment method. STUDY DESIGN: Systematic revie

Pain Medicine and the Opioid Crisis: Time to Own Our Responsibility!

Pain remains a major unmet health need. Rising opioid prescribing has not reduced population pain and has directly contributed to increased morbidity and mortality. Opioid use disorder (OUD) and pain frequently coexist. Many people with OUD experience acute and chronic pain and a substantial proportion of patients prescribed opioids develop OUD. We have contributed to this problem. In this Position Paper, we defend the idea that we must urgently redefine pain medicine as a frontline discipline in opioid use disorder prevention and recovery. Pain management must be reframed as part of the solut

Local chemoarchitecture explains widespread lower cortical thickness associated with clinical high risk for psychosis.

The Clinical High Risk (CHR) state for psychosis is consistently associated with widespread cortical thinning. However, the underlying mechanisms driving this neuroanatomical phenotype remain poorly understood. Here, we integrated the ENIGMA CHR Working Group's large pooled dataset (N = 1782 CHR, N = 1333 healthy controls) with an open-source PET molecular atlas to identify, for the first time, potential neurochemical drivers of cortical thinning associated with psychosis risk, transition, and its core symptoms. Using multilinear model analysis, we show that local chemoarchitecture significant

Sociodemographic Factors Associated with Opioid Agonist Treatment Receipt and Retention at an Integrated Safety-Net Health System.

INTRODUCTION: Sociodemographic differences in opioid use disorder (OUD) treatment engagement have implications for population health. Continuum of care models can guide the opioid epidemic response by identifying differences in treatment patterns. We evaluated one such model at an integrated, safety-net health system in Denver, Colorado and examined the association of patient characteristics with opioid agonist treatment (OAT) receipt and retention. METHODS: We identified a cohort of patients with OUD from 2023 to 2024 from electronic health records. We used multivariable regression to estima

Primary Care Practitioners' Approaches to Deprescribing Opioids for Older Adults With Chronic Pain: A Qualitative Analysis.

BACKGROUND: Risks related to long-term opioid therapy for chronic pain are high and may increase over time with aging. Deprescribing may be a beneficial intervention for older adults prescribed chronic opioids. METHODS: Semi-structured interviews with hypothetical clinical cases of older adults prescribed opioids for chronic pain: (1) low-risk case: a patient prescribed low-dose opioids without concerns; (2) moderate-risk case: a patient with multimorbidity and concurrent benzodiazepine use prescribed moderate opioid doses; (3) high-risk case: a patient prescribed high-dose opioids with signs

Association between early methadone dose titration and treatment discontinuation and opioid toxicity: A retrospective cohort study.

BACKGROUND: Clinical guidance in Canada and the United States recommends faster methadone dose titration for individuals using fentanyl. An initial dose increase is recommended between days 4 and 6, but the impact on patient outcomes remains unclear, particularly when provided in an outpatient setting. Therefore, we evaluated the association between early methadone dose titration (i.e., within treatment days 4-6) and treatment discontinuation and opioid toxicity. METHODS AND FINDINGS: We conducted a retrospective propensity-score weighted cohort study of Ontario residents who initiated methad

Patterns of polysubstance use among people who use drugs on an American Indian reservation of a southern plains Tribal Nation.

BACKGROUND: Polysubstance use is a key driver of overdose and infectious disease risk among people who use drugs (PWUD). Polysubstance use has not been well characterized in Indigenous American communities, which experience a disproportionate overdose burden. OBJECTIVE: To characterize polysubstance use patterns among 192 PWUD on reservation lands of a southern Midwest tribe. METHODS: We conducted a latent class analysis to identify profiles of polysubstance use in the past 6 months and assessed associations between latent classes, sociodemographic correlates, overdose, naloxone receipt, awa

Buprenorphine Microinduction Treatment in Tri-Diagnosis Patients with Opioid Use Disorder and Chronic Non-Cancerous Pain.

Tri-diagnosis patients cause prevalent economic and healthcare burdens. A tri-diagnosis patient presents with three concurrent conditions: a mental illness, a substance use disorder, and an additional chronic medical condition. The medical conditions of interest include chronic non-cancerous pain (CNCP) and co-occurring with opioid use disorder (OUD). These patients are complex to treat, and an intensive outpatient program (IOP) would be appropriate to address the severity of their symptoms. OUD is prevalent among this population, especially for those in poverty, and in the United States gener

"People who do this work care deeply… and that makes it harder": A qualitative study of harm reduction workers' occupational stressors, stress reactions, and coping responses.

BACKGROUND: Harm reduction workers are on the frontlines of the U.S. opioid crisis, providing life-saving non-clinical services to highly marginalized people who use drugs. Despite mounting evidence of occupational stress and burnout among helping professionals, community-based workers, such as harm reduction workers, are largely excluded from such conversations, further perpetuating inequities within the substance use and behavioral health workforce. METHODS: In-depth virtual interviews were conducted with N=32 harm reduction workers to understand their perceptions, experiences, consequences

Factors Associated With Opioid-Involved Overdose: Descriptive Data From a Randomized Controlled Trial Evaluating Extended-Release Buprenorphine for Perinatal Opioid Use Disorder.

BACKGROUND: Opioid use disorder (OUD) during pregnancy is a leading contributor to peripartum morbidity and mortality, with overdose deaths rising significantly in recent years. Despite the identification of various factors associated with overdose events, including social, demographic, psychiatric, and neonatal outcomes, the relative contributions of these factors to peripartum overdose history (or lack thereof) remain unclear. Thus, this study aims to characterize factors associated with lifetime opioid-involved overdose events among currently pregnant individuals receiving buprenorphine (BU

Actigraphy-based sleep outcomes in substance use disorders: A protocol for a systematic review and meta-analysis.

INTRODUCTION: Individuals with substance use disorders (SUDs) are at increased risk for sleep disturbances, creating a bidirectional relationship that may heighten relapse risk. While polysomnography is the gold standard for measuring sleep, many studies have used actigraphy, a noninvasive, wrist-worn device that estimates rest-activity patterns and sleep-wake characteristics. Despite its utility, the use of actigraphy in populations with SUDs remains limited, and findings vary across substances and methodologies. This protocol outlines a systematic review and meta-analysis aiming to synthesiz
