Contextualizing Violence Risk Associated With Hallucinogens.

Psilocybin, lysergic acid diethylamide (LSD), and 3,4-methylenedioxymethamphetamine (MDMA) have remerged as agents of psychiatric and cultural relevance. However, public discourse has outpaced empirical understanding of their effects. Although hallucinogens have demonstrated a potential to treat certain mental disorders, their effect on aggression and violence risk remains inconclusive. This article reviews the historical, medical, legal, and cultural contexts of these popular psychedelic and entactogenic compounds. The impact of substance type, dose, set and setting, psychiatric comorbidity,

Psychedelics as a potential treatment for borderline personality disorder: A narrative review.

Borderline personality disorder (BPD) is a serious mental illness with high rates of morbidity and stigma; however, successful remission is frequently limited by a paucity of accessible treatment options. In an era of growing interest in psychedelics as novel psychiatric treatment modalities, patients with BPD are often excluded from research due to perceived safety risks, particularly pertaining to suicide and substance misuse. However, there is evolving evidence that psychedelic treatment may effectively target core BPD symptoms, in addition to those of the mood and anxiety disorders frequen

Not a Standalone Treatment: Considerations for Psychedelic-Assisted Therapy.

PURPOSE: Psychedelic-assisted therapy (PAT) is becoming a clinically available treatment for patients with treatment-resistant conditions such as depression and post-traumatic stress disorder (PTSD) in several countries. As PAT transitions from research settings into routine clinical practice, there is a growing need for coherent clinical frameworks and evidence-based guidelines to support its safe, effective, and integrated delivery. This paper examines how external therapists and psychiatrists can best support patients throughout the PAT journey; the different models for involving external t

[LSD revisited: Mechanisms of action and therapeutic potential in mental health].

Major depressive disorder (MDD) represents a colossal worldwide burden of health, affecting more than 350 million individuals. While traditional antidepressants with mechanisms of action at the serotonin, dopamine, or norepinephrine pathways are routinely prescribed, approximately one-third of patients fail to achieve adequate remission and develop treatment-resistant depression (TRD). This therapeutic shortfall underscores the necessity for novel approaches to treatment. Over the past two decades there has been a resurgence of scientific interest in psychedelics, such as lysergic acid diethyl

The Ethnopharmacological Use of Mescaline for Psychiatric Disorders: A Systematic Review.

Mescaline, the primary bioactive alkaloid found in Peyote and San Pedro cactus, has been used in traditional medicine for centuries and is now attracting renewed interest for clinical applications. The purpose of this systematic review was to search the literature for studies reporting the use of mescaline to address the gap in our understanding of mescaline use and its impact. References were exported from PubMed, Scopus, Embase, and Cochrane. Included studies contained patient data pertaining to mescaline, primary sources for beliefs on the use of mescaline as traditional medicine, and a ran

Blinding Integrity in Psychedelic Randomized Clinical Trials: A Systematic Review.

IMPORTANCE: Psychedelic drugs possess acute psychoactive effects that can compromise blinding integrity in randomized clinical trials (RCTs). Functional unblinding, when participants or raters correctly identify treatment allocation based on subjective effects, may bias outcomes through expectancy effects, challenging the validity of efficacy estimates and regulatory acceptance. OBJECTIVE: To systematically quantify the prevalence of blinding integrity assessment and the extent of functional unblinding in psychedelic RCTs for psychiatric disorders. EVIDENCE REVIEW: A systematic review was co

Psycholytic Therapy Using LSD: a Realist Review.

PURPOSE: Psycholytic therapy, utilizing psychoactive compounds such as d-lysergic acid diethylamide (LSD) to enhance psychoanalytically oriented therapy, was once the most common form of psychedelic-assisted psychotherapy. Despite its historical prominence, little attention has been given to the reasons behind its decline, and there is a lack of standardized guidance for new clinical trials. This review addresses the diversity of psycholytic methods and their association with effectiveness and safety. METHOD: A realist review was conducted in which studies were identified via a comprehensive

Media hype regarding psychedelic treatments for depression and PTSD from 2017 to 2024.

Enthusiastic media coverage of psychedelics as a treatment for mental health disorders is perceived as hyped compared to coverage of established treatments. To measure potential hype, we conducted pre-registered analyses comparing media coverage (e.g., newspaper articles) of psychedelics to FDA-approved antidepressants for major depressive disorder and posttraumatic stress disorder. We used LexisNexis to access publications from 2017 to 2024 and natural language processing to analyze 6,805 publications' sentiment and content. Binary logistic regressions compared the language in paragraphs rela

Trials, trips, and tribulations: pathways for implementing psychedelic therapy in Ireland.

Classical serotonergic psychedelics, such as psilocybin, show emerging evidence of therapeutic potential across a range of mental health disorders, including depression, anxiety, and substance use disorders, with indications of transdiagnostic efficacy. While early-phase studies yielded encouraging results, recent larger-scale phase 3 trials, such as those evaluating psilocybin for treatment-resistant depression, have shown more modest effects, and further findings from ongoing trials are awaited. Despite the absence of regulatory approvals from the U.S. Food and Drug Administration and Europe

Development of the MDMA-Assisted Psychotherapy Side Effects Tool (M-SET): a Delphi study.

BACKGROUND: Despite growing interest in the therapeutic potential of 3,4-methylenedioxymethamphetamine (MDMA), no targeted measure to systematically assess side effects of MDMA-assisted psychotherapy (MDMA-AP) exists. OBJECTIVE: Our aim was to develop an MDMA-Assisted Psychotherapy Side Effects Tool (M-SET) to capture side effects over the course of MDMA-AP. METHODS: Informed by a systematic review and a review of other relevant questionnaires, we drafted a list of potential side effects. Face and content validation were obtained via a modified two-round online Delphi process involving exper

From therapeutic promise to evidentiary discipline: Reassessing MDMA-assisted psychotherapy for posttraumatic stress disorder.

In this commentary on Morland et al. (2026)'s recent State of the Science article on 3,4-methylenedioxymethamphetamine (MDMA)-assisted psychotherapy for posttraumatic stress disorder (PTSD), we aim to emphasize both MDMA's therapeutic promise and its unresolved scientific challenges. Although recent trials on MDMA-assisted interventions have reported encouraging reductions in PTSD symptoms, the current evidence base remains constrained by several major limitations, including difficulties in blinding, expectancy effects, the absence of robust active comparator conditions, limited mechanistic cl

Emerging therapeutic opportunities for psychedelic and related drugs.

Increasing interest in psychedelic and related drugs as potential therapies for a wide spectrum of difficult-to-treat conditions, that extend beyond neuropsychiatric disorders, provided the impetus for this themed issue. This collection of reviews and original articles includes the mechanistic basis of how these drugs act, the current status of preclinical research and progress in clinical trials, and insight into the regulatory processes that determine clinical approval. In this editorial, we introduce these aspects and provide an overview of current controversies and challenges in the field,

Epigenome-wide association study of psilocybin-induced methylome changes in alcohol use disorder.

The serotonergic hallucinogen psilocybin has shown potential as a treatment for psychiatric conditions like alcohol use disorder (AUD) and depression in clinical studies. Epigenetic mechanisms, including DNA methylation, are hypothesized to contribute to its lasting therapeutic benefits. In this exploratory study, we present the first methylome-wide analysis of psilocybin-induced changes in a cohort of detoxified patients with AUD. The longitudinal study design included three assessment days in 37 patients with blood sampling and acquisition of psychometrics - at baseline, 24 h after administr

A Missing Voice? Peer Support Workers' Perceptions of Psychedelic-Assisted Therapy in Australia: A Cross-Sectional Survey.

INTRODUCTION: Peer Support Workers (PSWs) in mental health and substance use treatment draw on their lived experience (LE) to support clients through mutual understanding and trust. Psychedelic-assisted therapy (PAT) is emerging as a mental health and substance use treatment modality. As interest grows, understanding PSWs' perspectives on PAT is critical to inform safe and effective implementation, and the potential role PSWs can play in models of care. This study examined factors shaping PSWs' perspectives toward PAT and perceived barriers to its adoption and implementation. METHODS: A mixed

Simultaneous use of cannabis with other drugs: Identifying latent classes in a sample of Canadians.

BACKGROUND: Rates of cannabis use are rising, with potential public health implications. However, many people who use cannabis use it simultaneously with other psychoactive drugs such that their intoxicating effects overlap. Thus, it is important to understand cannabis use in the context of simultaneous use with other substances, especially in jurisdictions with legal access to cannabis. We used latent class analysis (LCA) to identify patterns of simultaneous use of cannabis with other drugs in a sample of Canadians who use cannabis and examined demographic and cannabis-related correlates of c

Short- and Long-Acting Psychedelics: Structure-Activity Relationships, Pharmacology, and Implications for Neuropsychiatric Therapeutics.

Psychedelics have re-emerged as promising therapeutics for neuropsychiatric disorders, including depression, anxiety, post-traumatic stress disorder, and substance use disorders. While their beneficial effects are largely attributed to serotonin 2A (5-HT2A) receptor activation, psychedelics exhibit substantial diversity in chemical structure, receptor binding kinetics, metabolism, and duration of action. These differences underpin the distinction between short-acting psychedelics like N,N-dimethyltryptamine (DMT) and 5-methoxy-DMT, and long-acting compounds like lysergic acid diethylamide (LSD
