Quantitative motor activity differentiates schizophrenia subtypes.

BACKGROUND: Motor symptoms are frequent in schizophrenia and relevant to the diagnosis of subtypes. However, the assessment has been limited to observations recorded in scales and experimental designs. The aim of this study was to use wrist actigraphy to obtain motor activity data in 3 schizophrenia subtypes. METHODS: In total, 60 patients with schizophrenia (35 paranoid, 12 catatonic, 13 disorganized) were investigated using continuous wrist actigraphy over 24 h in an inpatient setting on average 38 days after admission. Data of the wakeful hours of the day were analyzed. RESULTS: The activ

[Mental retardation as a risk factor to develop a psychotic disease].

INTRODUCTION: One of the main aims of research on schizophrenia has been to pinpoint the early symptoms and signals of the disease before its appearance. OBJECTIVES: We have examined the diagnoses previously given to patients before they were diagnosed of schizophrenia. METHOD: This is a case-control study in which we used a data register including the fields of minimum basic data set (MBDS) whose time period included 1999 to 2005. RESULTS: In our study, there was a 3.6% frequency of mental retardation and 2.1% one of behavioral and emotional disorders with onset usually occurring in childh

Creating a map of psychiatric patients based on psychopathological symptom profiles.

BACKGROUND: In the current debate about the categorical or dimensional classification of mental disorders many fruitful methods to illustrate one or the other aspect are employed, and suggestions are made to combine the two perspectives. METHODS: We present such an approach to combine both perspectives at the same time. Based on psychopathological AMDP-symptom profiles, a map of psychiatric patients was calculated by robust nonmetric multidimensional scaling (NMDS). RESULTS: The sample from the Ludwig-Maximilians University in Munich included the records of patients, who were admitted and di

Are there valid subtypes of schizophrenia? A grade of membership analysis.

BACKGROUND/AIMS: Cluster analysis has had limited success in establishing whether there are subtypes of schizophrenia. Grade of membership (GoM) analysis is a multivariate statistical technique which has advantages when, as in schizophrenia, individuals conforming to pure types are uncommon and mixed forms are frequent. METHODS: GoM analysis was applied to 118 chronic schizophrenic patients. The patients were of all clinical subtypes, including 13 with simple schizophrenia. Both current and 'lifetime' symptoms were assessed, and two different rating systems were used. RESULTS: Specifying 3 p

Psychosis following anti-obesity treatment with rimonabant.

BACKGROUND: In this report, we present the case of a patient with a relapse of schizophrenia following an episode of depression and increased anxiety after antiobesity treatment with rimonabant, a cannabinoid type 1 receptor antagonist. CASE REPORT: After 4 weeks of treatment the patient developed psychiatric symptoms, i.e. depressed mood and elevated anxiety. Four months after the discontinuation of rimonabant, the patient presented with psychotic symptoms fulfilling ICD-10 criteria of paranoid schizophrenia. Antipsychotic treatment with quetiapine was initialized. A stable recovery took fur

Altered resting-state functional connectivity and anatomical connectivity of hippocampus in schizophrenia.

Hippocampus has been implicated in participating in the pathophysiology of schizophrenia. However, the functional and anatomical connectivities between hippocampus and other regions are rarely concurrently investigated in schizophrenia. In the present study, both functional magnetic resonance imaging (fMRI) during rest and diffusion tensor imaging (DTI) were performed on 17 patients with paranoid schizophrenia and 14 healthy subjects. Resting-state functional connectivities of the bilateral hippocampi were separately analyzed by selecting the anterior hippocampus as region of interest. The for

Racial and ethnic effects on psychotic psychiatric diagnostic changes from admission to discharge: a retrospective chart review.

OBJECTIVE: Different cultural norms for paranoia that exist among African Americans may be misconstrued and fuel the overdiagnosis of schizophrenia. The present study examined whether the frequency of psychotic psychiatric diagnoses differs by race/ethnicity, particularly with regard to paranoid schizophrenia. We examined the frequency upon admission and at discharge and further explored the pattern of diagnostic changes that occurred by racial/ethnic group. METHOD: The present study is a secondary analysis of diagnostic data obtained on inpatients admitted to a research unit from 1990 to 200

Do psychiatric advance directives protect autonomy?

A psychiatric advance directive (PAD) is a legal resource that allows a person to document his or her wishes about receiving mental health services in the event that he or she is unable to provide consent at a future time. Advantages for completing a PAD include promoting the person's autonomy and authentic self, minimizing harms, and promoting beneficence. However, more research is needed to describe the positive and negative consequences associated with completing and implementing a PAD in the clinical setting.

Biochemical changes in the cingulum in patients with schizophrenia and chronic bipolar disorder.

Biochemical changes have been reported in vivo in the brain in schizophrenia patients using 1H-magnetic resonance spectroscopy (MRS). The aim of this study was to assess the specificity of biochemical changes occurring in schizophrenia patients, in a direct comparison with bipolar disorder patients. Fourteen patients with chronic paranoid schizophrenia, 17 euthymic type I bipolar patients with no previous history of psychotic symptoms and 15 healthy controls were included, most of them were female. They underwent a study with MRS: proton spectra were acquired using a Signa 1.5 T CVI scanner, w

[Selecting material to develop an emotional Stroop test adapted to schizophrenia and bipolar disorders].

OBJECTIVE: The emotional Stroop test evaluates the influence of the emotional valence of stimuli on cognitive inhibition processes. In subjects with psychiatric disorders, interference increases in this test when valence refers to their specific psychopathology. This study aims to develop a version of the emotional Stroop test adapted to paranoid schizophrenia and bipolar disorder. METHOD: The emotional valence and the number of times patients used 200 words related to schizophrenia and bipolar disorder psychopathology were assessed by 25 clinicians; then a principal component analysis was pe

Diabetic control and atypical antipsychotics: a case report.

INTRODUCTION: People with schizophrenia are at increased risk of developing metabolic disturbances. This risk may be further exacerbated by the use of antipsychotic agents. Research is still ongoing to determine the metabolic impact of antipsychotics on glucose regulation. In this case report we review some of the possible mechanisms of action of antipsychotic medication on glucose regulation. CASE PRESENTATION: We present the case of a 50-year-old man diagnosed with paranoid schizophrenia who developed type 2 diabetes mellitus whilst on treatment with second generation antipsychotics (SGA).

Ethnicity effects on clinical diagnoses compared to best-estimate research diagnoses in patients with psychosis: a retrospective medical chart review.

OBJECTIVE: Ethnicity effects on diagnoses are frequently reported and have variably been attributed to diagnostic biases versus ethnic differences in environmental exposures, and other factors. METHOD: We compared best-estimate gold standard research diagnoses to clinical diagnoses (DSM-III-R and DSM-IV criteria) among 129 white, 57 African American, and 50 Hispanic patients with psychosis admitted to an inpatient research unit from 1990 to 2003. RESULTS: Clinical and research diagnoses showed greater agreement in Hispanic than in African American patients (white patients were intermediate).

Differential synaptic changes in the striatum of subjects with undifferentiated versus paranoid schizophrenia.

Subjects with schizophrenia (SZ) have an increased density of synapses characteristic of corticostriatal or thalamostriatal glutamatergic inputs in the caudate matrix and putamen patches. SZ is a heterogeneous disease in many aspects including symptoms. The purpose of the present study was to determine if the synaptic organization in two different DSM-i.v. subgroups of SZ was differentially affected. Postmortem striatal tissue was obtained from the Maryland Brain Collection from normal controls (NC), chronic paranoid SZs (SZP), and chronic undifferentiated SZs (SZU). Tissue was prepared for ca

A case of aripiprazole and tardive dyskinesia.

This is a case report of a 24-year-old African lady, who developed symptoms of tardive dyskinesia (TD) following 9 months of treatment with aripiprazole 15 mg. She has no family history of mental illness, not used illicit substances and has a medical diagnosis of idiopathic hypertension which is well controlled. The patient has an 18-month history of paranoid schizophrenia with three psychotic episodes, one severe enough to warrant admission. Upon discontinuation of aripiprazole and switch to quetiapine, the symptoms of TD disappeared rapidly. A PubMed search revealed one previous case report

Neuropsychological relationships in paranoid schizophrenia with and without delusional misidentification syndromes. A comparative study.

Delusional misidentification syndromes (DMSs) and schizophrenia are strongly associated, since the former occur predominantly in the context of paranoid schizophrenia. However, the possible underlying neuropsychological relationships between DMSs and paranoid schizophrenia have not been thoroughly investigated. The aim of the present study was to investigate whether DMSs in paranoid schizophrenia are associated with a distinct neuropsychological substrate indicative of differential bilateral frontal and right hemisphere dysfunction. We compared two matched groups of paranoid schizophrenic pati

An exploratory analysis of neurocognition in methamphetamine-induced psychotic disorder and paranoid schizophrenia.

OBJECTIVE: This exploratory study compared the neurocognitive performances of individuals with methamphetamine-induced psychotic disorder (MA psychosis) and paranoid schizophrenia. BACKGROUND: MA psychosis presents very similarly to paranoid schizophrenia and the relationship between the 2 disorders is not well-understood. Neurocognitive functioning is one potential factor for comparison, as deficits are documented in both schizophrenia and methamphetamine dependence. METHOD: Participants were selected through chart review. Neurocognitive test scores for participants with paranoid schizophre

Preliminary study of the expression of genes connected with the orexigenic and anorexigenic system using microarray technique in anorexia nervosa.

The pathogenesis of anorexia nervosa (AN) is still poorly understood. The Diagnostic and Statistical Manual of Mental Disorders (4th edition) classification differentiates 2 AN types: the restricting type (AN-R) and the binge eating/purging type (AN-BP). We investigated 4 young women suffering from AN (2 with AN-R and 2 with AN-BP). Four women, age matched, with other psychiatric disorders (paranoid schizophrenia, adjustment disorder, mental retardation) served as the reference group. The oligonucleotide microarray method (HG-U133A, Affymetrix) was used to determine the expression profile of 1
