Biomarkers of bipolar disorder in omics and neuroimaging.

Bipolar disorder (BD) affects 1% of the global population. BD is a group of chronic psychiatric disorders characterized by recurrent manic or hypomanic episodes that may alternate with depressive episodes. Given the current diagnostic modalities, accurately diagnosing BD, particularly distinguishing it from unipolar depression (UD), is challenging. Biomarkers have emerged as potent instruments for establishing objective diagnostic criteria for BD, and their identification, which reflects the pathophysiological processes of BD, can facilitate the precise diagnosis of the disorder. In this revie

Mood states recognition based on Mandarin speech and deep learning in patients with bipolar disorder.

BACKGROUND: Current assessments of bipolar disorder mainly rely on clinician’s experience and patients’ self-reporting, which has the risk of observer bias and subjective cognitive bias. There is an urgent need for more objective indicators to improve the accuracy of recognizing and monitoring mood states in patients with bipolar disorder. Speech might be a promising marker for recognizing different mood states of bipolar disorder, because of its accessible, objective, and non-invasive features. This study aims to construct a deep learning model based on Mandarin speech to recognize depressive

Advancing the prediction of factors associated with bipolar disorder risk: utilizing early recognition tools and polygenic risk scores.

Bipolar disorder (BD) is a highly heritable mental illness that affects ∼ 1-2% of the world's population and has complex genetic and environmental underpinnings. Early detection is critical to improving treatment outcomes, but current strategies have limited predictive power. Early detection tools such as the Early Phase Inventory for Bipolar Disorder (EPIbipolar) and the Bipolar At-Risk (BARS) criteria assess phenotypic risk factors, including family history (FH) and subthreshold mood problems. Polygenic risk scores (PRS) are a quantitative metric of genetic susceptibility. This study examine

A Critical Overview of the Validity of the Current Concept of Bipolar Disorder.

Objectives: The main aim was to evaluate the origin and empirical support of the current diagnostic criteria for (hypo)manic and depressive episodes in BD focusing on their nosological (i.e., is it a real entity?) and diagnostic validity (i.e., how well do the criteria for the category portray the entity?). Methods: A narrative review of relevant textbooks/reports and articles published in peer-reviewed English-language journals (from the online databases PubMed and PsycInfo), covering the period 1900-2024 and using the terms "validity" OR "diagnosis" AND "manic-depressive"; "mania"; "hypomani

Associations between bereavement and (hypo-)mania: An updated mini-review.

Loss and bereavement may precede mood episodes in bipolar disorders (BD). When a loved one dies, this is a stressful life event with a negative connotation that can trigger the onset of symptoms of both polarities (depressive or manic) of BD or lead to the emergence of the first mood episodes. Even though the relationships between death and depression are better known in the literature and clinical practice, the associations between bereavement and (hypo-)mania are less clear. To date, information on "funeral mania", also called "grief mania" or "bereavement mania", is rather limited, remainin

Symptom modulation and tolerability of intravenous ketamine in treatment-resistant bipolar depression: A retrospective study.

Ketamine's use in treating bipolar depression must account for risks, such as switching to manic episodes or worsening symptoms. This study examines ketamine's impact on depressive symptoms, focusing on 'inner tension,' 'sleep reduction,' and 'suicidal ideation' over four weeks in treatment-resistant bipolar disorder (TR-BD) patients. Fifty-nine patients with TR-BD were treated consecutively with ketamine (avg dose 0.8 mg/kg). Results showed significant reductions in MADRS scores without manic switches. Ketamine was well-tolerated despite polypharmacy. Antidepressant treatment of bipolar depre

"Bad timing for illness relapse!" Mood symptoms, challenges and strategies for wellbeing in the first year postpartum among infant mothers with bipolar disorder: a mixed-methods study.

BACKGROUND: The postpartum period is associated with a high risk of illness episodes in women with bipolar disorder (BD) and is a critical developmental phase for both a new mother and her infant. This mixed-methods study aimed to investigate the occurrence of mood symptoms among infant mothers with BD in the first year postpartum, as well as their perceptions of the first year, their challenges and their strategies for wellbeing. METHODS: Twenty-six women with BD participated. Mood symptoms were assessed at 3 and 12 months postpartum with the Inventory of Depressive Symptomatology and Young

Sex differences in pediatric major depressive episodes: a cross-sectional study on psychiatric symptoms in early-onset mood disorders.

BACKGROUND: Sex differences in psychiatric symptoms among children and adolescents with a major depressive episode (MDE) are less studied than among adults. Previous non-recent studies reported a greater severity in adolescent girls and small differences between sexes in specific symptoms. We aim to explore the differences between male and female patients in the diagnoses, comorbidities, and psychiatric symptoms in a large cohort of pediatric patients referred to a tertiary center for child and adolescent psychiatry. METHODS: We collected cross-sectional data on 382 consecutively referred pat

Bipolar II disorder: a state-of-the-art review.

Bipolar II disorder (BD-II) is currently identified by both the DSM-5 and ICD-11 as a distinct subtype of bipolar disorder, defined by at least one depressive episode and at least one hypomanic episode, with no history of mania. Despite its prevalence and impact, the literature on BD-II remains relatively sparse. This paper provides a comprehensive overview of the available research and current debate on the disorder, including its diagnostic criteria, clinical presentations, comorbidities, epidemiology, risk factors, and treatment strategies. Patients with BD-II often present with recurrent d

Where is mania in the meta-structure of psychopathology?

BACKGROUND: The nosology of mania has long been a conundrum. Prior studies have alternately concluded that it is an internalizing disorder, a thought disorder, or a unique condition. Unfortunately, nearly all existing studies assessed symptoms cross-sectionally. This is problematic for syndromes that follow a more episodic course, such as mania. Here, we test whether including a history of episodes, not simply current symptoms, can help resolve the placement of mania in the meta-structure of psychopathology. METHODS: First-admission patients with psychosis from the Suffolk County Mental Healt

How variants in inflammatory mediator genes influence symptom severity of psychiatric disorders: Findings from the PsyCourse study.

Alterations in glial cell function and cytokine levels in the central nervous system may be influenced by neuroinflammatory processes, which have a pathogenic role in psychiatric disorders. Variability in genes that encode inflammatory mediators is associated with risk of developing mental disorders. Therefore, by analyzing data from the transdiagnostic PsyCourse Study, we aimed to investigate whether variations in inflammatory mediator genes are associated with current symptom severity. We used cross-sectional data from 1320 individuals with a psychiatric disorder and 466 neurotypical individ

Creatine prevents ouabain-induced manic-like behavior by modulating GSK-3β via PI3K/AKT/mTOR pathway.

Bipolar disorder is a chronic psychiatric condition marked by alternating episodes of mania and depression. Current pharmacological treatments show limited efficacy and are often associated with incomplete remission. Creatine, a compound involved in energy buffering, mitochondrial function, and neuromodulation, has demonstrated neuroprotective and potential mood-stabilizing properties. However, the underlying mechanisms remain poorly understood. This study evaluated the effects of acute oral creatine administration (300 mg/kg) on behavioral and molecular changes in a validated rat model of man

[Lithium treatment for affective disorders in old age].

BACKGROUND: Lithium is considered the gold standard for the treatment of bipolar affective disorder for the prevention of recurrence of manic and depressive episodes and for augmentation treatment in unipolar severe depressive episodes. The indications for treatment with lithium do not differ for older or younger patients. Nevertheless, there are a number of aspects to be considered with respect to drug safety in the group of old patients. OBJECTIVE: The aim was to give an overview of the current literature on lithium treatment in old age and from this to derive recommendations for action. M

Serum neurofilament light as a potential marker of illness duration in bipolar disorder.

INTRODUCTION: Investigation on specific biomarkers for diagnostic or prognostic usage in mental diseases and especially bipolar disorder BD seems to be one outstanding field in current research. Serum neurofilament light (sNfL), a marker for neuro-axonal injury, is increased in various acute and chronic neurological disorders, but also neuro-psychiatric conditions, including affective disorders. The aim of our study was to determine a potential relation between a neuron-specific marker like sNfL and different clinical states of BD. METHODS: In the current investigation, 51 patients with BD an

Limited evidence of autocorrelation signaling upcoming affective episodes: a 12-month e-diary study in patients with bipolar disorder.

BACKGROUND: Increased autocorrelation (AR) of system-specific measures has been suggested as a predictor for critical transitions in complex systems. Increased AR of mood scores has been reported to anticipate depressive episodes in major depressive disorder, while other studies found AR increases to be associated with depressive episodes themselves. Data on AR in patients with bipolar disorders (BD) is limited and inconclusive. METHODS: Patients with BD reported their current mood via daily e-diaries for 12 months. Advances in functional MRI research in bipolar disorder: from the perspective of mood states.

Bipolar disorder is characterised by recurrent and alternating episodes of mania/hypomania and depression. Current breakthroughs in functional MRI techniques have uncovered the functional neuroanatomy of bipolar disorder. However, the pathophysiology underlying mood instability, mood switching and the development of extreme mood states is less well understood. This review presents a comprehensive overview of current evidence from functional MRI studies from the perspective of mood states. We first summarise the disrupted brain activation patterns and functional connectivity that have been repo
