Sex differences in prevalence and clinical correlates for psychotic symptoms in first-episode drug-naïve patients with bipolar disorder: a cross-sectional study.

BACKGROUND: Psychotic symptoms, which affect a significant proportion of patients with bipolar disorder (BD), are associated with adverse prognostic outcomes. Currently, sex-specific patterns of psychotic symptoms remain inadequately characterized in BD. This study seeks to investigate sex-based differences in psychotic symptoms among first-episode drug-naïve (FEDN) BD patients among Han Chinese population. METHODS: A total of 577 FEDN BD patients, comprising 271 males and 306 females, were recruited in this study. Demographic characteristics were collected using a specialized questionnaire.

Comparison of demographic and clinical features in adolescent-onset vs. adult-onset bipolar disorder an analysis of BIPAS Phase II data.

OBJECTIVE: The literature on large-scale studies of Chinese patients with adolescent-onset bipolar disorder (adolescent-onset BD) was limited. Based on the analysis of the National Bipolar Mania Pathway Survey (BIPAS) Phase II data, we examined the demographic and clinical characteristics of adults with adolescent-onset BD. METHODS: Among 899 participants diagnosed with BD from 20 mental health services, demographics and clinical data were collected at screening. Comparisons were made using chi-square (or Fisher's exact) tests and ANOVA. Multivariate logistic regression identified independent

Mood switching in 1629 antidepressant-treated bipolar disorder patients.

Mood-switching from depression into [hypo]mania is prevalent in bipolar disorder (BD) patients, particularly during treatment with an antidepressant, but many characteristics associated with such responses require clarification. Using standard bivariate and multivariate measures we compared characteristics of 1629 prospectively evaluated BD patients who did or did not become [hypo]manic during antidepressant treatment. Mood-switching occurred in 27.6% of BD patients (type II [BD2]: 31.4%; type I [BD1]: 24.5%) observed prospectively for 4.98 years, most often (65.5%) with new hypomania. Factors

Comparative effectiveness of mood stabiliser monotherapy versus second-generation antipsychotic monotherapy versus combination therapy for continuation and maintenance treatment after a first episode of psychotic mania in North America, Chile, and Spain: a target trial emulation using data from the BD-CAUSAL Collaboration.

BACKGROUND: The early stage of bipolar I disorder is considered a crucial period for improving long-term outcomes, yet evidence guiding treatment selection after a first manic episode is scarce. Using data from the BD-CAUSAL Collaboration, we aimed to emulate a pragmatic target trial among individuals aged 16 years or older presenting with a first manic episode with psychotic features. METHODS: For the target trial, individuals were eligible if they were aged 16 years or older, treated at a first-episode psychosis clinic with a diagnosis of bipolar I disorder between Jan 1, 2006, and Dec 31,

Under the Surface: A Case of Secondary Delusional Infestation With Mood-Congruent Psychotic Features in a 56-Year-Old Woman.

Delusional infestation (DI) is an uncommon phenomenological syndrome characterized by the fixed, false conviction of being infested by living organisms, which often results in significant disease burden through social isolation and self-destructive behaviors. This case explores a 56-year-old woman with HIV and a history of bipolar disorder who presented with suicidal ideation and profound distress attributed to a believed parasitic infestation following a recent dose escalation of her prescribed amphetamine. The patient exhibited classic manic features, including hypergraphia and pressured spe

Late-Onset Mania With Psychotic Features Precipitated by Umrah Pilgrimage: A Place- and Culture-Associated Psychiatric Syndrome.

Place- and culture-associated psychiatric syndromes describe acute psychiatric presentations precipitated by emotionally and spiritually intense experiences in culturally significant settings. We report the case of a 59-year-old widowed Tajik woman with no prior psychiatric history who developed an acute manic episode with psychotic features during her first Umrah pilgrimage to Makkah. The episode was preceded by approximately five days of prodromal decreased sleep, irritability, and repetitive ritualistic behaviors, progressing to elevated and labile mood, pressured speech, flight of ideas, s

Concurrent Presentation of Bipolar Affective Disorder and Recent Alcohol Cessation: Diagnostic Challenges in Resource Limited Setting.

BACKGROUND: Bipolar Affective Disorder (BPAD) frequently coexists with Alcohol Use Disorder (AUD), creating diagnostic complexities, particularly when manic symptoms occur close to alcohol cessation. Accurate differentiation between primary mania and substance induced symptoms is critical for appropriate management. CASE PRESENTATION: We describe a male in his early 30s with a two week history of elevated mood, irritability, reduced need for sleep, increased goal directed activity, and psychotic features, occurring shortly after abrupt cessation of chronic alcohol use. Although the patient in

Cannabis and tobacco use in bipolar disorder: Associations with early onset, psychotic symptoms, and relapse risk (2015-2019).

BACKGROUND: Substance use disorders (SUD) frequently occur alongside bipolar disorder (BD), with some studies indicating a 29 % comorbidity rate in Western populations [1]. The relationship between BD and SUD is intricate and bidirectional - drug misuse can increase the risk of developing BD, and individuals with BD have a higher risk of developing SUD. This complex interplay often leads to earlier BD onset, more hospitalizations, and reduced effectiveness of pharmacological treatments, particularly mood stabilizers. This work aims to describe the impact of drugs in the risk of relapse in BD a

Where is mania in the meta-structure of psychopathology?

BACKGROUND: The nosology of mania has long been a conundrum. Prior studies have alternately concluded that it is an internalizing disorder, a thought disorder, or a unique condition. Unfortunately, nearly all existing studies assessed symptoms cross-sectionally. This is problematic for syndromes that follow a more episodic course, such as mania. Here, we test whether including a history of episodes, not simply current symptoms, can help resolve the placement of mania in the meta-structure of psychopathology. METHODS: First-admission patients with psychosis from the Suffolk County Mental Healt

How variants in inflammatory mediator genes influence symptom severity of psychiatric disorders: Findings from the PsyCourse study.

Alterations in glial cell function and cytokine levels in the central nervous system may be influenced by neuroinflammatory processes, which have a pathogenic role in psychiatric disorders. Variability in genes that encode inflammatory mediators is associated with risk of developing mental disorders. Therefore, by analyzing data from the transdiagnostic PsyCourse Study, we aimed to investigate whether variations in inflammatory mediator genes are associated with current symptom severity. We used cross-sectional data from 1320 individuals with a psychiatric disorder and 466 neurotypical individ

Serum neurofilament light as a potential marker of illness duration in bipolar disorder.

INTRODUCTION: Investigation on specific biomarkers for diagnostic or prognostic usage in mental diseases and especially bipolar disorder BD seems to be one outstanding field in current research. Serum neurofilament light (sNfL), a marker for neuro-axonal injury, is increased in various acute and chronic neurological disorders, but also neuro-psychiatric conditions, including affective disorders. The aim of our study was to determine a potential relation between a neuron-specific marker like sNfL and different clinical states of BD. METHODS: In the current investigation, 51 patients with BD an

New onset gender dysphoria in bipolar affective disorder - A case series.

Gender confusion in the context of mania is very less frequently described in the literature. The actuality of a primary psychiatric condition in gender identity complaint has significant bearing on the applicable operation and prognostic. This case series describes cases of bipolar affective complaint presenting in a manic occasion whose mania was marked by hypersexuality and the desire to be of opposite gender. Both of these symptoms resolved with treatment of the manic occasion. Case 1 describes a 17-year-old male presenting with an episodic illness, with current manic episode. He is curren

Empirical validity of Leonhard's psychoses: A long-term follow-up study of first-episode psychosis patients.

The validation of nosological diagnoses in psychiatry remains a conundrum. Leonhard's (1979) nosology seems to be one of the few acceptable alternative categorical models to current DSM/ICD systems. We aimed to empirically validate Leonhard's four classes of psychoses: systematic schizophrenia (SSch), unsystematic (USch), cycloid psychosis (Cyclo), and manic-depressive illness (MDI) using a comprehensive set of explanatory validators. 243 patients with first-episode psychosis were followed between 10 and 31 years. A wide-ranging assessment was carried out by collecting data on antecedent, illn

Suicidal risk and protective factors in major affective disorders: A prospective cohort study of 4307 participants.

BACKGROUND: Suicidal behavior is strongly associated with major affective disorders, but there is a need to quantify and compare specific risk and protective factors in bipolar disorder (BD) and major depressive disorder (MDD). METHODS: In 4307 extensively evaluated major affective-disorder participants with BD (n = 1425) or MDD (n = 2882) diagnosed by current international criteria, we compared characteristics among those with versus without suicidal acts from illness-onset through 8.24 years of follow-up. RESULTS: Suicidal acts were identified in 11.4 % of participants; 25.9 % were violent

Diagnostic deep learning algorithms that use resting EEG to distinguish major depressive disorder, bipolar disorder, and schizophrenia from each other and from healthy volunteers.

BACKGROUND: Mood disorders and schizophrenia affect millions worldwide. Currently, diagnosis is primarily determined by reported symptomatology. As symptoms may overlap, misdiagnosis is common, potentially leading to ineffective or destabilizing treatment. Diagnostic biomarkers could significantly improve clinical care by reducing dependence on symptomatic presentation. METHODS: We used deep learning analysis (DLA) of resting electroencephalograph (EEG) to differentiate healthy control (HC) subjects (N = 239), from those with major depressive disorder (MDD) (N = 105), MDD-atypical (MDD-A) (N

Immune Cell Ratios Are Higher in Bipolar Affective than Unipolar Depressive Disorder and Modulated by Mood Episode: A Retrospective, Cross-Sectional Study.

Immune dysregulation is implicated in the pathophysiology of both bipolar and major depressive disorder, while immune cell ratios (IRCs) have recently been proposed as clinically applicable immune biomarkers. We investigated IRCs differences in affective disorders and their association with current mood episodes and clinical features. This retrospective cohort study analyzed neutrophil-lymphocyte (NLR), monocyte-lymphocyte (MLR), and platelet-lymphocyte (PLR) ratios upon admission in 135 affective disorder in-patients with mania (MA, n = 36), bipolar depression (BiD, n = 38), and unipolar depr

The Diagnostic Concept of Unipolar Mania: The Missing Entity of the Affective Spectrum.

Although the traditional nosological view interprets mania as strictly connected with depression in the context of manic-depressive illness, unipolar mania (UM), that is, the lifetime occurrence of mania or hypomania in the absence of depression, has been proposed as a necessary diagnostic entity. Epidemiological data estimate that at least 5% of people with type I bipolar disorder never experienced depression. These subjects are more often males with a hyperthymic temperament, a younger age at onset, and more psychotic features. Current classification systems do not formally recognize UM, but

Relationship between polygenic risk scores and symptom dimensions of schizophrenia and schizotypy in multiplex families with schizophrenia.

BACKGROUND: Psychotic disorders and schizotypal traits aggregate in the relatives of probands with schizophrenia. It is currently unclear how variability in symptom dimensions in schizophrenia probands and their relatives is associated with polygenic liability to psychiatric disorders. AIMS: To investigate whether polygenic risk scores (PRSs) can predict symptom dimensions in members of multiplex families with schizophrenia. METHOD: The largest genome-wide data-sets for schizophrenia, bipolar disorder and major depressive disorder were used to construct PRSs in 861 participants from the Iris
