Computational phenotypes underlying effort-based decision-making and negative symptoms in a transdiagnostic severe mental illness sample.

Effort-based decision-making (EBDM) impairments predict negative symptoms across multiple psychiatric diagnoses. However, it is unclear whether equifinality is present and different disorders reach the same clinical endpoint of negative symptoms via different mechanistic EBDM processes. This study used computational modeling to isolate processes underlying EBDM in a large severe mental illness-spectrum sample. The Effort Expenditure for Rewards Task, negative symptom measures, and neuropsychological tests were administered to 920 participants: schizophrenia (SZ; n = 147), first-episode psychos

Hallucinations during bipolar depressive episode treated with bupropion after alcohol withdrawal surgery: a case report.

Alcohol Use Disorder (AUD) is a prevalent condition often co-occurring with affective disorders. Bupropion, a medication used to treat affective disorders, acts by inhibiting dopamine (DA) and norepinephrine (NE) reuptake, potentially helping to alleviate emotional dysregulation. However, the risk of delirium in AUD patients at therapeutic doses is poorly understood, despite reports linking bupropion overdose to delirium. We report here a rare case of a 49-year-old Chinese male with AUD who developed mood disturbances and relapsed following alcohol detoxification surgery. He was diagnosed with

Sex differences in prevalence and clinical correlates for psychotic symptoms in first-episode drug-naïve patients with bipolar disorder: a cross-sectional study.

BACKGROUND: Psychotic symptoms, which affect a significant proportion of patients with bipolar disorder (BD), are associated with adverse prognostic outcomes. Currently, sex-specific patterns of psychotic symptoms remain inadequately characterized in BD. This study seeks to investigate sex-based differences in psychotic symptoms among first-episode drug-naïve (FEDN) BD patients among Han Chinese population. METHODS: A total of 577 FEDN BD patients, comprising 271 males and 306 females, were recruited in this study. Demographic characteristics were collected using a specialized questionnaire.

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Cannabis and tobacco use in bipolar disorder: Associations with early onset, psychotic symptoms, and relapse risk (2015-2019).

BACKGROUND: Substance use disorders (SUD) frequently occur alongside bipolar disorder (BD), with some studies indicating a 29 % comorbidity rate in Western populations [1]. The relationship between BD and SUD is intricate and bidirectional - drug misuse can increase the risk of developing BD, and individuals with BD have a higher risk of developing SUD. This complex interplay often leads to earlier BD onset, more hospitalizations, and reduced effectiveness of pharmacological treatments, particularly mood stabilizers. This work aims to describe the impact of drugs in the risk of relapse in BD a

How variants in inflammatory mediator genes influence symptom severity of psychiatric disorders: Findings from the PsyCourse study.

Alterations in glial cell function and cytokine levels in the central nervous system may be influenced by neuroinflammatory processes, which have a pathogenic role in psychiatric disorders. Variability in genes that encode inflammatory mediators is associated with risk of developing mental disorders. Therefore, by analyzing data from the transdiagnostic PsyCourse Study, we aimed to investigate whether variations in inflammatory mediator genes are associated with current symptom severity. We used cross-sectional data from 1320 individuals with a psychiatric disorder and 466 neurotypical individ

Hyperuricemia in Chinese Patients with Mood Disorders: Prevalence, Related Factors, and Predictive Model.

PURPOSE: Previous studies have shown that serum uric acid (UA) levels are significantly higher in patients with bipolar disorder (BD) than in patients with depressive disorder (DD), schizophrenia, and healthy controls. Currently, studies generally report that there is a complex bidirectional interaction between mood disorders (MD) and hyperuricemia (HUA). We investigated the prevalence and related factors of hyperuricemia in Chinese patients with mood disorders to find out potential mechanisms and build a predictive model. PATIENTS AND METHODS: A total of 771 patients with mood disorders who

Prospective Acceptability of Digital Therapy for Major Depressive Disorder in France: Multicentric Real-Life Study.

BACKGROUND: Major depressive disorder is one of the leading causes of disability worldwide. Although most international guidelines recommend psychological and psychosocial interventions as first-line treatment for mild to moderate depression, access remains limited in France due to the limited availability of trained clinicians, high costs for patients in the context of nonreimbursement, and the fear of stigmatization. Therefore, online blended psychological treatment such as Deprexis could improve access to care for people with depression. It has several advantages, such as easy accessibility

EPA guidance on lifestyle interventions for adults with severe mental illness: A meta-review of the evidence.

There is growing interest in lifestyle interventions as stand-alone and add-on therapies in mental health care due to their potential benefits for both physical and mental health outcomes. We evaluated lifestyle interventions focusing on physical activity, diet, and sleep in adults with severe mental illness (SMI) and the evidence for their effectiveness. To this end, we conducted a meta-review and searched major electronic databases for articles published prior to 09/2022 and updated our search in 03/2024. We identified 89 relevant systematic reviews and assessed their quality using the SIGN

Exploration of cannabis use and polygenic risk scores on the psychotic symptom progression of a FEP cohort.

Cannabis use is highly prevalent in first-episode psychosis (FEP) and plays a critical role in its onset and prognosis, but the genetic underpinnings promoting both conditions are poorly understood. Current treatment strategies for cannabis cessation in FEP are clearly inefficacious. Here, we aimed to characterize the association between cannabis-related polygenic risk scores (PRS) on cannabis use and clinical course after a FEP. A cohort of 249 FEP individuals were evaluated during 12 months. Symptom severity was measured with the Positive and Negative Severity Scale and cannabis use with the

Anxious distress in people with major depressive episodes: a cross-sectional analysis of clinical correlates.

OBJECTIVE: Most people with major depressive episodes meet the criteria for the anxious distress (AD) specifier defined by DSM-5 as the presence of symptoms such as feelings of tension, restlessness, difficulty concentrating, and fear that something awful may happen. This cross-sectional study was aimed at identifying clinical correlates of AD in people with unipolar or bipolar depression. METHODS: Inpatients with a current major depressive episode were included. Data on socio-demographic and clinical variables were collected. The SCID-5 was used to diagnose depressive episodes and relevant s

Expression of ANK3 moderates the association between childhood trauma and affective traits in severe mental disorders.

Exposure to early life trauma increases the risk of psychopathology later in life. Here we investigated if ANK3 mRNA levels influence the relationship between childhood trauma experiences and clinical characteristics in mental disorders. A sample of 174 patients with bipolar disorder and 291 patients with schizophrenia spectrum disorder were included. Patients were diagnosed using the Structured Clinical Interview for DSM-IV, and childhood trauma was assessed using the childhood trauma questionnaire. Age at illness onset and number of psychotic and affective episodes were assessed from intervi

Exploration of mood spectrum symptoms during a major depressive episode: The impact of contrapolarity-Results from a transdiagnostic cluster analysis on an Italian sample of unipolar and bipolar patients.

BACKGROUND: Subthreshold hypomania during a major depressive episode challenges the bipolar-unipolar dichotomy. In our study we employed a cross-diagnostic cluster analysis - to identify distinct subgroups within a cohort of depressed patients. METHODS: A k-means cluster analysis- based on the domain scores of the Mood Spectrum Self-Report (MOODS-SR) questionnaire-was performed on a data set of 300 adults with either bipolar or unipolar depression. After identifying groups, between-clusters comparisons were conducted on MOODS-SR domains and factors and on a set of sociodemographic, clinical a
