The efficacy and safety of cannabinoids for the treatment of mental disorders and substance use disorders: a systematic review and meta-analysis.

BACKGROUND: Mental disorders and substance use disorders (SUDs) are among the leading reasons for which the medical use of cannabinoids has been approved, but their efficacy and safety in treating these conditions is yet to be established. We conducted a systematic review and meta-analysis of randomised controlled trials (RCTs) testing the efficacy and safety of cannabinoids as the primary treatment for mental disorders or SUDs. METHODS: We searched Ovid MEDLINE, PsychINFO, Cochrane Central Register of Controlled Clinical Trials, Cochrane Database of Systematic Reviews, and Embase for peer-re

Childhood ADHD and Its Adult Mimics: When First-Time ADHD Diagnoses in Adulthood May Reflect Other Conditions-A Narrative Review.

Background/Objectives: Attention-deficit/hyperactivity disorder (ADHD) begins in childhood, but its presentation changes over time. Adult ADHD is increasingly recognized; in the United States, 2023 survey data indicate that approximately 6% of adults reported a current diagnosis. This narrative review examines the developmental course of ADHD from childhood to adulthood and the controversial phenomenon of apparent late-onset presentations. Broadened diagnostic criteria, greater public awareness, and expanded access through telehealth may contribute to the rise in adult diagnoses, but these fac

Plasma fractionation in Latin America: a path towards sovereignty and sustainability in the supply of plasma-derived medicines.

PURPOSE: Increased reactive oxygen species (ROS) released by NADPH oxidase and inflammation are associated with arterial hypertension and eye diseases associated with high blood pressure, including glaucoma, retinopathies (e.g., age-related macular degeneration), and choroidopathies affecting ocular function; however, the mechanisms underlying these adverse outcomes remain undefined. The present study was designed to highlight the importance of oxidative stress in severe hypertension-related eye damage. METHODS: Male Wistar rats (n = 7, unless otherwise specified for specific experiments) wer

Concurrent Presentation of Bipolar Affective Disorder and Recent Alcohol Cessation: Diagnostic Challenges in Resource Limited Setting.

BACKGROUND: Bipolar Affective Disorder (BPAD) frequently coexists with Alcohol Use Disorder (AUD), creating diagnostic complexities, particularly when manic symptoms occur close to alcohol cessation. Accurate differentiation between primary mania and substance induced symptoms is critical for appropriate management. CASE PRESENTATION: We describe a male in his early 30s with a two week history of elevated mood, irritability, reduced need for sleep, increased goal directed activity, and psychotic features, occurring shortly after abrupt cessation of chronic alcohol use. Although the patient in

Metacognitive Training for Subjects with Bipolar Disorder: A Randomized Controlled Trial.

INTRODUCTION: Impairments in social cognition in bipolar disorder (BD) have been extensively described in the last decade but few treatment strategies have been studied to address this issue. This study presents findings from a randomized controlled trial (RCT) investigating the efficacy of metacognitive training for bipolar disorder (MCT-BD) compared to Treatment as Usual (TAU) among individuals with BD in remission. The aim was to determine whether MCT-BD could improve social cognition and overall functioning in this population. METHODS: Participants (N = 56) were recruited via social media

Deviant sound frequency and time stimuli in auditory oddball tasks reveal persistent aberrant brain activity in patients with psychosis and symptomatic remission.

The detection of rare or deviant stimuli shares common brain circuits involved in temporal processing and salience, critical for cognitive control. Disruption in these processes may contribute to the mechanisms of the disease and explain cognitive deficits observed in psychosis and related disorders. We designed a neuroimaging study, using oddball task-based functional sequences (fMRI) and diffusion tensor imaging (DTI), comparing healthy controls (HC, n = 14, 7 females) and patients with stable psychosis (PSY, n = 20, 10 females). The PSY individuals had schizophrenia or bipolar disorder diag

Polygenic contributions to lithium augmentation outcomes in antidepressant non-responders with unipolar depression.

OBJECTIVE: Lithium augmentation (LA) is an effective treatment for patients with major depression after inadequate antidepressant response, but therapeutic outcomes vary considerably between individuals. Molecular studies could yield novel insights into treatment prediction to enable personalized therapy choices. Here, we investigated the effects of polygenic risk scores (PRS) for schizophrenia (SCZ), major depressive disorder (MDD), and bipolar disorder (BIP) on clinical outcomes following LA. METHODS: Recent GWAS summary statistics were used to construct disorder-specific PRS in lithium-aug

Light therapy for bipolar disorders: Clinical recommendations from the international society for bipolar disorders (ISBD) Chronobiology and Chronotherapy Task Force.

The International Society for Bipolar Disorders (ISBD) Chronobiology and Chronotherapy Task Force conducted a comprehensive review to deliver concise evidence-based recommendations on the use of bright light therapy (BLT) for bipolar disorder (BD). Adjunctive BLT is likely an efficacious acute treatment for bipolar depression as implicated by higher quality evidence. The position of maintenance BLT for relapse prevention awaits further investigation. Protocols of effective BLT in BD are similar to parameters indicated for treatment of seasonal and non-seasonal major depressive disorder. Anti-m

Episode-specific cortical functional connectome reorganization and neurobiological correlates in bipolar disorder: a cross-sectional study.

BACKGROUND: Bipolar disorder (BD) is a heterogeneous psychiatric condition characterized by distinct episodes: manic (BipM), depressive (BipD), mixed (mBD), and remission (rBD). Current evidence indicates alterations in brain functional connectivity in BD, yet a comprehensive understanding across all episodes remains incomplete. METHODS: Here, to investigate how different BD episodes alter brain functional organization, we calculated the sensory-association axis using diffusion map embedding on the functional connectome matrix and compared this axis between the four BD groups and neurotypical

Deep learning based treatment remission prediction to transcranial direct current stimulation in bipolar depression using EEG power spectral density.

Bipolar disorder is characterized by marked changes in mood and activity levels and is a leading cause of disability worldwide. We sought to investigate the application of deep learning methods to electroencephalogram (EEG) signals to predict clinical remission after 6 weeks of home-based transcranial direct current stimulation (tDCS) treatment. Pre-treatment resting-state EEG acquired from 21 bipolar participants was used for this work. A hybrid 1DCNN and GRU model, with input from power spectral density values of theta, beta and gamma frequency bands of the AF7 and TP10 electrodes, achieved

The DEXA-PSYCH Study-dexamethasone for moderate-to-severe depression: study protocol for a double-blind, randomized, parallel-group, placebo-controlled trial.

BACKGROUND: Globally, depression represents one of the leading causes of years lived with disability. The effects of current pharmacological treatments are small-to-moderate and often delayed by weeks. Immunological disturbances have been associated with depression and meta-analyses have suggested that anti-inflammatory agents have moderate-to-large anti-depressant effects. The largest effects were reported for glucocorticoids. However, previous trials were too small to legitimize standard use of glucocorticoids as add-on treatment in depression. The purpose of the DEXA-PSYCH study is to inves

Disease progression in bipolar disorder in relation to white matter microstructure: A comprehensive approach based on staging models.

BACKGROUND: Bipolar disorder (BD) is assumed to follow a progressive course, conceptualized through staging models. It is unclear whether white matter (WM) microstructure abnormalities, central to BD pathophysiology, parallel this development throughout disease progression. This study explored the link between WM and disease progression in BD, using a comprehensive approach based on clinical staging models. METHODS: This cross-sectional diffusion tensor-imaging study included 153 BD patients and 153 healthy controls (HCs) matched for age, sex, and study site. Using tract-based spatial statist
