Legionella pneumophila (L. pneumophila) persists in engineered water systems and is the causative agent of Legionnaires' disease. Despite its clinical importance, little is known about whether environmental populations are undergoing gradual changes in their antimicrobial susceptibility. Here, we compared the antimicrobial susceptibility of 95 environmental L. pneumophila isolates collected in 2013-2014 with those obtained in 2024-2025 from the same sites in a drinking water distribution system (DWDS) using Epsilometer (E-test). The analyses showed both temporal and spatial variation: Ciproflo

Background: Legionella pneumophila is a leading cause of community- and hospital-acquired pneumonia (HAP/CAP), although the clinical and radiological features associated with infection remain incompletely defined. This study aimed to identify clinical, comorbidity-related, and chest computed tomography (CT) features associated with L. pneumophila pneumonia in hospitalized adults with community-acquired pneumonia (CAP). Methods: A retrospective case-control study was conducted at a tertiary-care university hospital in Italy between January 2019 and August 2025. Eighty patients positive for L. p

Legionella pneumophila populations within environmental reservoirs can exhibit genomic diversity, complicating source attribution during outbreak investigations. Core genome single-nucleotide variant (cgSNV)-based approaches are widely used for identifying closely related lineages during molecular epidemiological investigations; however, they primarily focus on nucleotide-level variation and may overlook structural variation among closely related isolates. In this study, we investigated genomic diversity among clinical and environmental L. pneumophila isolates obtained during an epidemiologica

Legionella species are environmental Gram-negative bacteria capable of colonising artificial water systems and causing Legionnaires' disease, a severe form of atypical pneumonia. Healthcare facilities are particularly vulnerable to transmission because complex building water systems can support bacterial growth and aerosol generation, exposing highly susceptible patient populations. This article uses a case-based scenario to illustrate the investigation of suspected healthcare-associated Legionnaires' disease on a haematology ward. The recognition of clustered pneumonia cases prompted multi-di

Sequence-based typing (SBT) via Sanger sequencing has been the standard for describing Legionella pneumophila relatedness for two decades. SBT involves sequencing seven loci, identifying alleles using the United Kingdom Health Security Agency database and inferring the corresponding sequence type (ST). While similar SBT approaches for other organisms can be easily adapted to whole-genome sequencing (WGS), L. pneumophila presents two challenges for this adaptation: multiple copies of one locus (mompS) and extensive heterogeneity in a second locus (neuA/neuAh). Although several computational met

INTRODUCTION: Legionnaires' disease (LD) can cause severe pneumonia, requiring intensive care and frequently associated with multiorgan failure. The mechanisms and prognosis of kidney complications in LD remain poorly characterized. We aimed to describe the incidence, clinical characteristics, and prognosis of acute kidney injury (AKI) in patients who are critically ill with LD and identify factors associated with AKI in a multicenter cohort.
METHODS: This multicenter retrospective cohort study was conducted in 39 intensive care units (ICUs) in France between 2012 and 2024. Inclusion criteria

Legionella pneumophila is a well-recognized cause of community-acquired pneumonia, but its capacity to cause extrapulmonary complications, including myocarditis, remains underappreciated. Myocarditis is among the rarest and most serious cardiac manifestations of Legionella infection and may be underdiagnosed in critically ill patients, where troponin elevations are often attributed to sepsis-related myocardial injury. We report the case of a 51-year-old woman with diabetes mellitus, ulcerative colitis managed with adalimumab (a tumor necrosis factor-α inhibitor), and a significant smoking hist

Legionella longbeachae is an emerging respiratory pathogen primarily found in soil environments, yet its genomic architecture and evolutionary strategies remain poorly characterized. Here, through pan-genomic and functional analysis of 242 L. longbeachae isolates, we reveal an open pan-genome driven by extensive horizontal gene transfer and marked functional divergence between core and accessory genomes. We identify a phylogenetically distinct lineage, designated as putative serogroup 3 (sg3), recovered from Chinese environments. This lineage diverges from canonical serogroups 1 and 2 through

Legionella pneumophila is a Gram-negative opportunistic pathogen responsible for Legionnaires' disease, a severe form of atypical pneumonia associated with high morbidity and mortality, particularly in immunocompromised individuals. Despite its growing global burden and the persistence of environmental reservoirs, no licensed vaccine is currently available. In this study, an integrative immunoinformatics and reverse vaccinology approach was employed to design a multi-epitope vaccine candidate targeting L. pneumophila. The complete proteome was systematically analyzed to identify essential, vir

AIMS: The interpretation of cytokeratin 7 (CK7)-positive cells in the epidermis of the nipple has been controversial. These cells have been described in Paget's disease of the nipple, and they have also been cited as benign 'Toker' cells or as Merkel cells. Having observed CK7+ cells in histologically unremarkable nipple biopsies, we sought to assess the distribution of CK7+ cells in Paget's disease of the nipple and in histologically unremarkable nipple.
METHODS AND RESULTS: Representative sections from 37 cases of Paget's disease of the nipple and 32 cases of histologically unremarkable nip

INTRODUCTION: Clostridium perfringens (C. perfringens), a ubiquitous Gram-positive bacterium in the environment and mammalian gut flora, is a leading cause of enterotoxemia in animals, necrotizing enteritis in humans and animals, and gas gangrene in both, attributed to its diverse exotoxin profile. Alpha-toxin, a pivotal virulence factor produced by all C. perfringens serotypes, plays a central role in the pathogenicity of these diseases.
METHODS: Here, we engineered a lipid nanoparticle encapsulated CPA-CTD mRNA vaccine targeting the conserved C-terminal domain of C. perfringens alpha-toxin

UNLABELLED: Emphysematous gastritis is a rare, often fatal infection of the stomach wall characterized by gas formation and tissue necrosis, most frequently associated with polymicrobial infections. Cases with demonstration of tissue invasion by Clostridium species are exceptionally uncommon. We report the case of a 77-year-old woman with multiple comorbidities, including diabetes mellitus, obesity, atherosclerosis, and chronic kidney disease, who presented with severe abdominal pain, nausea, and vomiting. Imaging revealed gastric emphysema with pneumoperitoneum. Emergent laparotomy demonstrat

BACKGROUND: Transmetatarsal amputation (TMA) is a commonly performed procedure that can result in residual limb and phantom pain. Neuromodulation using pulsed radiofrequency (PRF) is a growing area of research in the management of refractory postamputation pain (PAP).
CASE REPORT: A veteran in his sixties with a history of diabetes required a left TMA due to gas gangrene, subsequently complicated by severe, prolonged postoperative pain. PRF was performed first on the saphenous nerve using ultrasound guidance, then 12 weeks later on the saphenous, peroneal, and sural nerves. This resulted in a

Gas gangrene, or clostridial myonecrosis, is a rare and life-threatening infection caused primarily by Clostridiumperfringens. Despite its reduced incidence due to advancements in wound care and antibiotics, the disease remains associated with significant morbidity and mortality, particularly in immunocompromised individuals. This report presents a clinical case of a 37-year-old male with esophageal carcinoma and a totally implantable venous access device (Port-a-Cath), who developed fatal gas gangrene following chemotherapy.

Inflammasomes are an essential component of the innate immune response against pathogen infections. However, the molecular mechanism regulating the inflammasome signaling in response to gangrenous Clostridium perfringens (C. perfringens) infection remains elusive. We herein report the unexpected discovery that IFI204 (the murine homolog of human IFI16)-dependent STING protects against C. perfringens gas gangrene via enhancing NLRP3 inflammasome signaling. In the C. perfringens gas gangrene model, compared with wild-type (WT) mice, Sting deficiency (Sting-/- ) mice displayed an increased suscep

BACKGROUND: Among pharmaceuticals currently in clinical use, few drugs directly target bacterial toxins. Clostridium perfringens α-toxin, a phospholipase C (PLC), is a major virulence factor responsible for gas gangrene caused by C. perfringens type A. There is a clinical need for small-molecule compounds that inhibit such bacterial toxins.
METHODS: A library of 764 FDA-approved drugs was screened to identify compounds that inhibit the PLC activity of C. perfringens α-toxin. Identified hits were further evaluated for their ability to inhibit α-toxin-induced cytotoxicity in human umbilical vei

We report a rare case of monomicrobial gas gangrene of the upper limb caused solely by Terrisporobacter glycolicus, an obligate anaerobe, in a 24-year-old male with a recent traumatic (burn) injury and history of substance abuse, who developed rapid-onset swelling, pain, and skin discolouration in the left arm, ultimately requiring surgical amputation. The pathogen was confirmed by matrix-assisted laser desorption/ionisation time-of-flight mass spectrometry (MALDI-TOF MS) and whole genome sequencing (WGS). This case emphasises the potential severity of anaerobic monomicrobial infections in tra
