Molecular profiling of virulence determinants and antimicrobial resistance in MRSA isolates from HIV-infected and non-HIV individuals.

BACKGROUND: Staphylococcus aureus is an important pathogen exhibiting antibiotic resistance and multiple virulence factors. This study analyses the antimicrobial resistance and virulence genes of Staphylococcus aureus isolates from HIV and non-HIV patients from southern India. METHODS: S. aureus strains from HIV (n = 125) and non-HIV (n = 100) patients were isolated using conventional bacterial culture techniques. Antimicrobial resistance and virulence genes were analysed using polymerase chain reaction (PCR). RESULTS: Methicillin resistance was detected in 88.8 % and 34 % of the S. aureus i

Evaluation of lawsone as a potential inhibitor of Staphylococcus aureus efflux pump mediated drugs resistance: An in-vitro and in-silico study.

Staphylococcus aureus (S. aureus) is a causative agent of a wide range of infections such as staphylococcal scalded skin syndrome, toxic shock syndrome, necrotizing pneumonia, endocarditis, and osteomyelitis, posing a significant challenge in clinical management. This study explores the antibacterial potential of lawsone on S. aureus using experimental and computational methods. The minimum inhibitory concentration (MIC) of lawsone (alone and in combination with erythromycin, tetracycline, norfloxacin and ciprofloxacin) on 8 clinical multi-drug resistant S. aureus was evaluated in vitro. The a

Menstrual toxic shock syndrome: A systematic review of imaging findings.

Menstrual toxic shock syndrome (mTSS) is a rare, life-threatening condition mainly affecting young menstruating women. While the clinical and microbiological aspects of mTSS are well-documented, consolidated evidence on its radiologic findings remains limited in the literature. This study aimed to systematically review published case reports of mTSS to describe the spectrum of radiologic findings and assess the role of imaging in diagnosis and management. A systematic search of Medline and Embase was conducted from the earliest available records to September 2024. Case reports were included if

Toxic Shock Syndrome Caused by Methicillin-resistant Staphylococcus aureus After Immediate Breast Reconstruction With Tissue Expander.

Toxic shock syndrome (TSS) is a severe postoperative infection characterized by rapidly progressing systemic symptoms, although local findings are typically unremarkable. Therefore, understanding its pathogenesis and ensuring early diagnosis are critical. To date, only 5 cases of TSS caused by methicillin-resistant Staphylococcus aureus have been reported in the context of breast reconstruction. We report a case of total mastectomy for breast cancer where a tissue expander was used. On postoperative day 12, the patient had hypotension, high fever, decreased consciousness, renal dysfunction, an

Endothelial NLRP3 inflammasome activation drives immunothrombosis in Streptococcus pyogenes infection.

Streptococcus pyogenes (Strep A), a formidable human pathogen, is notorious for causing life-threatening diseases such as necrotizing fasciitis and streptococcal toxic shock syndrome, often complicated by thrombosis and coagulation abnormalities. While macrophage inflammasome activation has been widely studied in Strep A pathogenesis, the contribution of vascular endothelial cells-key regulators of immunity and coagulation-remains largely unexplored. This study aimed to determine whether endothelial NLRP3 inflammasome activation drives immunothrombosis during invasive Strep A infection and to

Necrotizing Soft Tissue Infection at a Self-Administered Subcutaneous Etanercept Injection Site in an Immunosuppressed Patient With Rheumatoid Arthritis: A Case Report.

Necrotizing soft tissue infection (NSTI) is rapidly progressive and can be fatal; outcomes depend on early recognition and prompt surgical debridement. In immunocompromised patients, local findings and inflammatory responses may be atypical, increasing the risk of delayed diagnosis. Although injections breach the skin barrier, NSTI originating at injection sites can be overlooked as a benign injection-site reaction; procedure-associated NSTI has been reported to have poor outcomes. A woman in her 60s with rheumatoid arthritis was receiving methotrexate, tacrolimus, and prednisolone and had sta

Association of Staphylococcus aureus virulence factors with clinical and subclinical bovine mastitis.

Staphylococcus aureus is a major pathogen responsible for both clinical and subclinical bovine mastitis. Its ability to persist within the mammary gland and evade host immune responses makes it particularly significant for herd health and dairy production. So, this study aimed to evaluate whether the presence and diversity of S. aureus virulence genes mastitis-causing are associated with the onset and severity of mastitis in dairy cows. We analyzed 50 S. aureus isolates obtained from milk samples of cows with subclinical mastitis (SM) and 101 isolates from clinical mastitis (CM), including 28

Beta-Thalassemia Major Complicated by Streptococcal Toxic Shock Syndrome: A Rare Case of Survival and Successful Management.

Beta-thalassemia is an inherited blood disorder associated with defective hemoglobin production and impaired immunity, increasing susceptibility to severe infections. Streptococcus pyogenes poses a significant risk due to its potential to cause streptococcal toxic shock syndrome (STSS), a life-threatening condition characterized by fever, shock, and multiorgan failure. We report a rare survival case of a nine-year-old child with beta-thalassemia major who developed STSS secondary to Streptococcus pharyngitis. The patient presented with fever, neck swelling, hematemesis, and hypovolemic shock,

[Tampon use during menstruation among Japanese females].

Objectives In Japan, many females primarily use sanitary napkins during menstruation. However, growing awareness of environmental concerns and increasingly diverse lifestyles has led to the introduction of various menstrual products. Particularly, tampons, which are inserted intravaginally, are becoming more common; however, the prevalence of tampon use among Japanese females, the psychological and social factors influencing their use, and awareness of tampon-related adverse events remain unclear. Therefore, this study used an internet survey to examine tampon use among Japanese females.Method

Intravenous Immunoglobulin Treatment in Patients With Streptococcal Toxic Shock Syndrome in Southern Sweden: A Retrospective Population-Based Study.

BACKGROUND: Intravenous immunoglobulins (IVIGs) have been suggested as an adjunctive treatment in streptococcal toxic shock syndrome (STSS), but there are no conclusive trials. In southern Sweden, IVIG is routinely used in certain hospitals but not others. We hypothesized that this would resemble a natural experiment, and we aimed to evaluate the effect of IVIG in patients with STSS. METHOD: We conducted a population-based retrospective cohort study on STSS cases in southern Sweden from 2017 to 2024. The main exposure was any IVIG treatment, and the primary outcome was 30-day mortality. Cox r

Case Report: Fatal Streptococcus pyogenes infection secondary to closed femoral fracture.

BACKGROUND: Invasive Streptococcus pyogenes infection is exceedingly rare in patients with closed fractures. However, it can precipitate life-threatening necrotizing soft-tissue infection and streptococcal toxic shock syndrome once it occurs. Early recognition and aggressive multidisciplinary management are essential for survival. CASE SUMMARY: We presented a patient who sustained closed fracture of the upper segment of the left femur due to a traffic accident. He had a history of acute pharyngitis 10 days ago. The patient experienced fever on the second day after admission, with his body tem

When Drainage is Not Enough in Emphysematous Pyelonephritis Complicated by Necrotizing Fasciitis: A Case Report.

Necrotizing fasciitis (NF) is a rapidly progressive, life-threatening soft tissue infection characterized by widespread fascial necrosis, systemic toxicity, and high mortality without prompt surgical intervention. Emphysematous pyelonephritis (EPN) is a rare necrotizing infection of the kidney and perirenal tissues caused by gas-producing microbes, most often Escherichia coli, in patients with uncontrolled diabetes mellitus. We report the case of a 69-year-old man who developed NF with contiguous EPN. Despite aggressive surgical and medical management, he rapidly progressed to refractory septi

A Case of Mediastinitis Due to Fulminant Group A Streptococcal Infection.

INTRODUCTION: Mediastinitis caused by group A Streptococcus (GAS) is extremely rare and often progresses rapidly, with high mortality. CASE PRESENTATION: A 43-year-old man with no significant medical history developed a sore throat. Several days later, he presented to a local clinic with a high-grade fever of 40°C and chest pain. He was initially advised to rest under observation, but his symptoms persisted. Two days later, he sought care at a nearby hospital, where chest CT revealed findings suggestive of mediastinitis. He was subsequently transferred to our hospital for emergency treatment.

Computational and immunoinformatics approaches for designing phytocompound-based drugs and a multi-epitope vaccine targeting FemA, a cell wall protein of Staphylococcus aureus.

Staphylococcus aureus, a bacterial pathogen, is increasingly linked to severe healthcare-associated diseases, from mild skin infections to toxic shock syndrome. Due to limitations in conventional methods, we use computational techniques to screen potential phytocompounds for new drug development and to construct a multiepitope vaccine. Aminoacyltransferase FemA, essential for peptidoglycan formation, was chosen as the target protein for drug and vaccine development. Meanwhile, 1100 phytocompounds were retrieved from 54 plants using the NPASS database and analyzed for drug-likeness and ADMET pr

Intrauterine Tampon Identified on CT as an Intrauterine Foreign Body Associated With Bilateral Leg Edema: A Case Report.

BACKGROUND: Foreign bodies retained within the vaginal canal are relatively common in gynecological practice, particularly forgotten vaginal tampons, which are well known to cause localized pelvic infection and life-threatening toxic shock syndrome. However, the transcervical intrauterine migration and retention of a tampon is exceptionally rare, as the cervical canal typically acts as a robust mechanical barrier. CASE PRESENTATION: I report the case of a 32-year-old multiparous woman presenting with lower abdominal pain and acute bilateral leg edema. Due to the presence of systemic edema, co
