Deep Brain Stimulation: Mechanisms, Cost-Effectiveness, and Precision Applications Across Neurology and Psychiatry.

In less than 30 years, Deep Brain Stimulation (DBS) has evolved from an antiparkinsonian rescue intervention into a flexible neuromodulatory therapy with the potential for personalized, adaptive, and enhancement-focused interventions. In this review we collected evidence from seven areas: (i) modern eligibility criteria, and ways to practically improve on these, outside of 'Core Assessment Program of Surgical Interventional Therapies in Parkinson's Disease' (CAPSIT-PD); (ii) cost-effectiveness, where long-horizon models now show positive incremental net monetary benefit for Parkinson's disease

Transcranial magnetic stimulation and symptom management in palliative care.

Background: Transcranial magnetic stimulation (TMS) is a non-invasive neurostimulation technique currently approved by the Food and Drug Administration for the treatment of depression, migraines with aura and obsessive-compulsive disorder. While there is ample evidence supporting the safety and efficacy of TMS in treating these conditions, there is limited exploration of the application of TMS in the context of palliative care. Methods: In the wake of a preliminary study of 'accelerated' repetitive TMS promising symptomatic improvement in depression and anxiety in palliative care patients, we

The Frequency, Nature, Impact, and Coping Strategies of Nonconsensual Intimate Image Dissemination Victimization: A Scoping Review.

Young adults increasingly initiate, maintain, and end sexual relationships online, an evolution that has also transformed how sexual violence may be perpetrated. Nonconsensual intimate image dissemination (NCIID) has gained attention in research, policy, and media. Yet, to date, there has been no synthesis of the literature on NCIID victimization. The goals of this review were to: (a) describe the frequency and nature of NCIID victimization, (b) examine the impacts of experiencing NCIID, and (c) identify survivor coping strategies. Following the Preferred Reporting Items for Systematic Reviews

Association of educational attainment with mental disorders and the mediation role of lifestyle factors: a Mendelian randomization study.

BACKGROUND: Educational attainment (EA) was associated with lifestyle behaviors and underlying mental disorders. However, the causality remained poorly understood. This study aimed to elucidate the causal association between EA and mental disorders while exploring the mediation role of lifestyle factors. METHODS: A two-step Mendelian randomization (MR) analysis was performed to evaluate the mediation role of lifestyle factors in the relationship between EA and mental disorders. The total effects of years of education (EduYears) and college completion (College) on 31 lifestyle factors and 7 me

Association Between Maternal Genome-Wide Polygenic Scores for Psychiatric and Neurodevelopmental Disorders and Adverse Perinatal Events: A Danish Population-Based Study.

BACKGROUND: Phenotypic links between psychiatric disorders and adverse perinatal events are increasingly being reported, but the mechanisms remain unclear. In this study, we aimed to assess how polygenic scores (PGSs) for 8 psychiatric conditions influence perinatal risk. METHODS: The main analysis included 13,085 mothers and their corresponding birth information. PGSs for psychiatric conditions were estimated using genome-wide association study data (excluding the iPSYCH cohort) via LDpred2 and used as exposures. Ten adverse perinatal events from Danish national registers served as outcomes.

A Comparative Study of the Effect of Anxiety, Stress and Poor Sleep on Multipara and Nullipara Pregnant Women.

INTRODUCTION: Sleep is an essential component of well-being. It is defined as a reversible behavioral state of perception disengagement from unresponsiveness to the environment. Sleep deprivation leads to stress, leading to mental issues like mood disorders and depression. This study has a prime focus on how the psychological parameters, like anxiety and stress, are affected by sleep deprivation during pregnancy and differentiates these in nullipara and multipara women. AIM AND OBJECTIVES: The aim of this study was to study the effect of sleep health on the psychological health of pregnant wo

A 47-Year-Old Female With a Tubular, Nonenhancing Structure in the Left Lower Lobe.

A 47-year-old woman presented to the emergency department with sudden-onset dull nonradiating chest pain and palpitations. She denied other symptoms such as shortness of breath, dizziness, or diaphoresis. Her medical history was significant for generalized anxiety disorder, obesity, and former tobacco use (less than 1 pack per day for 5 years, quit more than 20 years ago). Her only medications included daily multivitamins and ibuprofen as needed for knee pain.

Lifetime Sexual Revictimization in Military Populations: A Scoping Review.

There are multiple patterns of sexual revictimization specific to individuals who serve in the military, and each pattern may be associated with unique risk factors and outcomes. Our goal was to determine which aspects of those patterns have been studied and what is known. We followed Preferred Reporting Items for Systematic Reviews and Meta-Analyses and Joanna Briggs Institute guidelines and adhered to Arksey and O'Malley's methodological framework for scoping reviews. To meet inclusion criteria, articles had to focus on sexual victimization among military personnel, be original research publ

Thyroid-stimulating hormone is associated with differential antidepressant and anti-anhedonic response to ketamine in bipolar depression but not major depressive disorder.

BACKGROUND: No clinically useful biomarkers currently predict antidepressant response to ketamine in treatment-resistant major depressive disorder (MDD) or bipolar disorder (BD). This study investigated whether baseline thyroid-stimulating hormone (TSH) levels are associated with ketamine's antidepressant, anti-anhedonic, and anti-suicidal effects. METHODS: This was a secondary exploratory analysis of data drawn from four NIH-sponsored, randomized, placebo-controlled, double-blind crossover studies of ketamine in inpatients with current MDD (n = 39) or bipolar depression (n = 44). Exploring early add-on celecoxib treatment in OCD: Results from a preliminary randomized, double-blind, placebo-controlled trial.

BACKGROUND: Despite multiple treatment options available for obsessive-compulsive disorder (OCD), the response still remains unsatisfactory due to various factors. Hence, there is a continuous need to explore newer treatment modalities for OCD. Neuroinflammation might be one such contributing factor. AIM: We aimed to study the clinical outcome of early Celecoxib (Selective Cyclooxygenase-2 Inhibitor) add-on therapy to fluoxetine treatment in OCD over 8 weeks and explored the association of the outcomes with biochemical parameters, serum interleukin-6 (IL-6) and brain-derived neurotrophic fact

Altered Kynurenine Pathway Metabolism in Drug-naive Children and Adolescents with Generalized Anxiety Disorder.

OBJECTIVE: Generalized anxiety disorder (GAD) is among the most common psychiatric conditions in children and adolescents. Although its aetiology remains unclear, emerging evidence highlights the role of immune-metabolic pathways in its development. This study aimed to investigate alterations in the kynurenine pathway (KP) metabolites in drug-naive paediatric patients with GAD compared to healthy controls. METHODS: Twenty-six drug-naive children with GAD and thirty-two age- and sex-matched healthy controls were enrolled. Serum levels of tryptophan (TRP), kynurenine (KYN), kynurenic acid (KYNA
