When Drainage is Not Enough in Emphysematous Pyelonephritis Complicated by Necrotizing Fasciitis: A Case Report.

Necrotizing fasciitis (NF) is a rapidly progressive, life-threatening soft tissue infection characterized by widespread fascial necrosis, systemic toxicity, and high mortality without prompt surgical intervention. Emphysematous pyelonephritis (EPN) is a rare necrotizing infection of the kidney and perirenal tissues caused by gas-producing microbes, most often Escherichia coli, in patients with uncontrolled diabetes mellitus. We report the case of a 69-year-old man who developed NF with contiguous EPN. Despite aggressive surgical and medical management, he rapidly progressed to refractory septi

Intravenous Immunoglobulin for Refractory Streptococcal Toxic Shock Syndrome With Purpura Fulminans: A Case Report and Literature Review.

Streptococcal toxic shock syndrome (STSS) is a rare but life-threatening condition characterized by rapid progression to septic shock and multiorgan dysfunction driven by toxin-mediated immune activation. We report the case of a previously healthy 39-year-old man who presented with acute encephalopathy and rapidly deteriorated into refractory septic shock requiring mechanical ventilation, maximal vasopressor support, and continuous renal replacement therapy. Blood cultures identified Streptococcus pyogenes, and the clinical course was complicated by severe metabolic acidosis, acute liver injur

A Case of Mediastinitis Due to Fulminant Group A Streptococcal Infection.

INTRODUCTION: Mediastinitis caused by group A Streptococcus (GAS) is extremely rare and often progresses rapidly, with high mortality. CASE PRESENTATION: A 43-year-old man with no significant medical history developed a sore throat. Several days later, he presented to a local clinic with a high-grade fever of 40°C and chest pain. He was initially advised to rest under observation, but his symptoms persisted. Two days later, he sought care at a nearby hospital, where chest CT revealed findings suggestive of mediastinitis. He was subsequently transferred to our hospital for emergency treatment.

Computational and immunoinformatics approaches for designing phytocompound-based drugs and a multi-epitope vaccine targeting FemA, a cell wall protein of Staphylococcus aureus.

Staphylococcus aureus, a bacterial pathogen, is increasingly linked to severe healthcare-associated diseases, from mild skin infections to toxic shock syndrome. Due to limitations in conventional methods, we use computational techniques to screen potential phytocompounds for new drug development and to construct a multiepitope vaccine. Aminoacyltransferase FemA, essential for peptidoglycan formation, was chosen as the target protein for drug and vaccine development. Meanwhile, 1100 phytocompounds were retrieved from 54 plants using the NPASS database and analyzed for drug-likeness and ADMET pr

Intrauterine Tampon Identified on CT as an Intrauterine Foreign Body Associated With Bilateral Leg Edema: A Case Report.

BACKGROUND: Foreign bodies retained within the vaginal canal are relatively common in gynecological practice, particularly forgotten vaginal tampons, which are well known to cause localized pelvic infection and life-threatening toxic shock syndrome. However, the transcervical intrauterine migration and retention of a tampon is exceptionally rare, as the cervical canal typically acts as a robust mechanical barrier. CASE PRESENTATION: I report the case of a 32-year-old multiparous woman presenting with lower abdominal pain and acute bilateral leg edema. Due to the presence of systemic edema, co

Improving menstrual and vaginal health for all (IMVAHA): protocol for a randomised cross-over trial assessing the impact of menstrual products on the vaginal microbiome of women aged 18-35 years in Cameroon, Peru and Switzerland.

INTRODUCTION: Menstrual health is critical to achieving gender equity and reaching the 2030 Sustainable Development Goals, yet evidence on the health impacts of menstrual products-particularly on the vaginal microbiota-is limited. The Improving Menstrual and Vaginal Health for All (IMVAHA) project aims to address this knowledge gap through qualitative exploration, a health survey and clinical trial embedded in three sister projects: Laura (Peru), Leke (Cameroon) and Marie (Switzerland). This paper outlines the protocol for the IMVAHA health survey and clinical trial studies, which aim to (1) a

Clonal structure, resistome and virulence gene profiles of methicillin-resistant Staphylococcus aureus from retail chicken meat in the United Arab Emirates.

Methicillin-resistant Staphylococcus aureus (MRSA) is an important public health hazard at the human-food-environment interface. Poultry meat has been increasingly implicated as a potential source of MRSA exposure, yet molecular data from the Middle East remain scarce. This study aimed to characterize MRSA isolated from retail chicken meat in the United Arab Emirates (UAE) using a DNA microarray platform and to compare these profiles with recent UAE clinical MRSA data. A total of 34 non-duplicate MRSA isolates recovered from retail chicken meat were analyzed using a high-resolution DNA microar

Machine Learning-Based Predictive Model for Fever and Adverse Clinical Events in Hospitalized Pediatric Burn Patients.

Systemic inflammation after pediatric burn injury frequently causes fever, complicating early recognition of infectious complications. Improved risk-stratification may help identify patients at risk for adverse clinical events during hospitalization. This study aimed to develop and validate a machine learning (ML)-based model using a Random Forest (RF) algorithm to predict fever and related adverse outcomes in hospitalized pediatric burn patients. We conducted a retrospective analysis of 595 pediatric burn patients admitted to a tertiary center between 2012 and 2022. Extracted data included de

Periorbital necrotizing soft tissue infection due to Streptococcus pyogenes with streptococcal toxic shock syndrome: successful early structured multidisciplinary management.

Periorbital necrotizing soft tissue infections (NSTIs) are rare but potentially life-threatening, particularly when complicated by streptococcal toxic shock syndrome (STSS). Rapid systemic deterioration can occur, even in previously healthy individuals. We report a case of a 57-year-old woman who developed periorbital NSTI, complicated by STSS, secondary to Streptococcus pyogenes. Emergency eyelid debridement was performed within 24 h of symptom onset, followed by intensive care management and empirical broad-spectrum intravenous antibiotics, which were subsequently adjusted to pathogen-direct

Genomic Characterization of Selected ESKAPEEE Clinical Isolates From Mbarara Regional Referral Hospital, Southwestern Uganda: Resistome, Multilocus Sequence Typing, Mobilome, and Virulome.

BACKGROUND: The global dissemination of Escherichia coli, Staphylococcus aureus, Klebsiella pneumoniae, Acinetobacter baumannii, Pseudomonas aeruginosa, Enterococcus faecium, Enterococcus faecalis, and Enterobacter cloacae pathogens is increasingly driven by the convergence of multidrug resistance (MDR) and hypervirulence. These opportunistic pathogens are characterized by their ability to "escape" the bactericidal effects of conventional antibiotics through a refined repertoire of resistance mechanisms. Infections related to these pathogens are responsible for the majority of hospital-acquire

[Phenotype and Genotype of Staphylococcus aureus Isolated from the Nostrils of Elderly Adults in the Valparaíso Region, Chile, from June 2019 to January 2020].

UNLABELLED: Staphylococcus aureus (S. aureus) persistently colonizes the nares of approximately 20-30% of healthy individuals, and the nasal carriage constitutes a risk of infection and transmission. This pathogen causes mild to severe infections, different strains show variable virulence, and methicillin-resistant S. aureus (MRSA) is a global concern. Due to the worldwide aging of the population, further research in older adults is necessary, such as the study of their microbiota. AIM: To determine the carriage of S. aureus in older adults in Valparaíso, Chile, and to characterize the phenot

Transcriptomic profiling of TSST-1-induced cytotoxicity and inflammatory responses in human vaginal epithelial cells.

OBJECTIVE: Toxic shock syndrome toxin-1 (TSST-1) is a critical superantigen produced by certain staphylococcal strains, closely associated with menstrual toxic shock syndrome and mucosal inflammation. While the systemic superantigenic function of TSST-1 is well-established, its early cytotoxic and inflammatory effects on human vaginal epithelial cells (HVEC) remain poorly characterized. This study, therefore, sought to investigate the initial temporal dynamics of TSST-1-induced changes in cell viability, inflammatory cytokine responses, epithelial stress-related responses, and barrier-associat

Fulminant streptococcal toxic shock syndrome presenting as cervical epidural abscess: a case report.

Streptococcal toxic shock syndrome (STSS), caused by Group A Streptococcus, is a life-threatening condition that can lead to organ failure within several days of symptom onset. Fulminant presentations resulting in death within hours of emergency department arrival are exceedingly rare and have a nearly universally fatal prognosis. Prompt recognition is critical; however, early clinical features may closely mimic those of other surgical emergencies, creating diagnostic challenges. A 75-year-old male was transferred who had posterior neck and bilateral shoulder pain for several days and a suspec
