Consequences of Maternal Antidepressant and Anticonvulsant Use During Pregnancy: Is Postnatal Inpatient Monitoring Required? - A Retrospective Cohort Study.

INTRODUCTION: Prenatal exposure to antidepressants and anticonvulsants is steadily increasing in parallel with rising prescription rates. Our study focused on potential postnatal adjustment disorders in newborns and their appropriate monitoring after birth. OBJECTIVE: To descriptively assess clinically relevant complications in neonates after intrauterine exposure to antidepressants and/or anticonvulsants and to evaluate the need for routine postnatal monitoring. METHODS: We conducted a retrospective single-center cohort study at the University Hospital Erlangen (01/2021-12/2023). The study

Effect Modification of Cigarette Smoking on the Relationship between Post-traumatic Stress Disorder, Other Mental Health Conditions and Rheumatoid Arthritis Risk: A Large Case-Control Study in U.S. Veterans.

OBJECTIVE: We examined associations of post-traumatic stress disorder (PTSD) and other mental health disorders (OMH) with rheumatoid arthritis (RA), accounting for effects of smoking. METHODS: We conducted a matched case-control study, identifying incident RA cases and controls using national Veteran Health Administration data (2006-2019). PTSD/OMH exposures prior to RA diagnosis were identified using administrative/diagnostic codes. Conditional logistic regression examined associations of RA (vs. no-RA) with PTSD (±OMH) and OMH. Relative Excess Risk of Interaction was calculated between OMH/

The role of the renin-angiotensin-aldosterone system in the relationships between stress exposure, stress-related psychopathology, and cardiovascular disease: a rapid scoping review.

BACKGROUND: The renin-angiotensin-aldosterone system (RAAS) is widely recognized for its role in maintaining fluid and electrolyte balance, as well as its involvement in the pathogenesis of certain forms of cardiovascular disease (CVD). Recent research suggests that, in addition, the RAAS may play a role in the body's response to stress, making activity of the RAAS a potential mechanism linking stress exposure or psychopathology to cardiovascular disease risk. PURPOSE: This rapid scoping review synthesizes the relevant literature that has been published on this topic over the past 16 years.

Psychological factors as mediators of the effectiveness of telehealth mindfulness-based interventions for pain and pain-related conditions.

IMPORTANCE: Mindfulness meditation is an evidence-based therapy for chronic pain for which some have explored possible mechanisms of action. However, these mechanisms can vary widely depending on the mindfulness-based intervention (MBI). OBJECTIVE: To study possible mechanisms of action of 2 similar MBIs for pain. DESIGN: We used data from our learning to apply mindfulness to pain (LAMP) pragmatic randomized controlled trial comparing usual care with virtual group or self-paced MBIs. The LAMP MBIs included education on adaptive pain behaviors and mindfulness and used shorter, less formal min

Treating PTSD in a cognitive-behavioural paradigm.

Significant advances have been made over the past two decades, associated with an increase in methodologically advanced research, into the treatment of post-traumatic stress disorder (PTSD). However, interest in the topic of PTSD has not only not waned, but seems to be gaining strength. The main reason for this is the difficulty in clearly defining the phenomenon, which is related to the constant updating of the criteria for post-traumatic stress disorder in medical classifications. The search for general standards and methods of measuring this disorder in relation to the age of the patient is

Functional Neurological Symptom Disorder Obscured by Incomplete Diagnostic Communication: A Case Report.

Functional neurological disorder (FND) is a diagnosis grounded in positive clinical signs, yet it remains vulnerable to underdocumentation and incomplete communication across care transitions, particularly when it is suspected but not formally recorded. In this case, a 24-year-old woman with a history of major trauma (three gunshot wounds sustained four years earlier) presented with sudden-onset left lower extremity (LLE) weakness, pain, and tingling coinciding with the anniversary month of her injury. Extensive neuroimaging and serologic workup were unrevealing aside from an incidental disc p

Psychosocial interventions for military personnel with PTSD: protocol of a systematic review and network meta-analysis.

INTRODUCTION: Post-traumatic stress disorder (PTSD) is a serious mental health condition. It may develop following exposure to an extremely threatening event. Military personnel face an increased likelihood of developing PTSD because they sometimes bring experiences of difficult events from before they joined the military and because they have higher odds of being exposed to potentially traumatic events during service. To support the mental health of military personnel, several psychological interventions have been developed to help them manage PTSD symptoms. Although there is a lot of researc

Possible drivers of self-harm severity - relative contributions of post-traumatic stress and levels of personality functioning - results from the Extreme Challenges project.

BACKGROUND: Self-harm (SH) is common, but its severity varies. Severe cases may involve extensive hospitalization and high suicide risk. Although SH is associated with post-traumatic stress disorder (PTSD), personality disorder (PD), and complex cases of PD-PTSD comorbidity, factors related to SH severity are sparsely researched. AIMS: The study aims to investigate how SH severity relates to PTSD symptoms and different domains of personality functioning. The study also aims to explore indirect associations of interpersonal functioning, PTSD symptoms and SH severity. METHODS: In a multicenter

Interventions to Reduce Burnout for Military Physicians: A Scoping Review.

INTRODUCTION: Physician burnout is an increasingly researched topic, particularly since the COVID-19 pandemic. Burnout reduction interventions, both organizational and individual, have been studied in the civilian healthcare setting. Few have been conducted in military settings. A 2022 United States (U.S.) Department of Defense (DOD) Inspector General report identified staff burnout and fatigue as a serious concern for Military Treatment Facilities. Other studies identified a significant association between burnout and suicidal ideation. From 2011 to 2022, the suicide rates in the Active compo

Enhancing astrocytic GAT-3 in zona incerta diminishes fear memory generalization through remodeling neural network.

Subpopulations within the zona incerta (ZI) have been implicated in the encoding of post-traumatic stress disorder (PTSD)-associated phenotypes, including fear response and fear memory generalization. However, its neural integration mechanism underlying ZI-mediated regulation of these PTSD-related pathophysiological processes remains poorly elucidated. Here, we systematically investigated the role of astrocytes in encoding neural dynamics in an odor-cue related mouse model of PTSD. We observed aberrant astrocyte-neuron correlative dynamics in the ZI of the PTSD mice. Moreover, neuronal hypoact
