Co-occurrence of dissociation and anxiety and depressive disorders in adults: a scoping review.

BACKGROUND: Dissociation, including depersonalization, derealization, dissociative identity disorder or dissociation in the context of functional neurological disorder, is classically associated with trauma-related disorders but is increasingly reported in anxiety and depressive disorders. Despite growing evidence linking dissociation to affective symptom severity, functional impairment, and treatment outcomes, its role in anxiety and depression remains insufficiently synthesized. OBJECTIVE: This scoping review aimed to map the extent, nature, and characteristics of dissociation in adults wit

Visual, vestibular, and ocular motor changes during nitroglycerin-triggered vestibular migraine.

BACKGROUND: Intravenous nitroglycerin (NTG) is a well-established model for provoking migraine attacks, but it has not previously been used to characterize vestibular migraine (VM). Different types of nystagmus have been reported during spontaneous VM attacks. We aimed to measure changes in nystagmus during NTG-induced migraine in participants with VM. METHODS: We studied 20 participants with definite VM (Barany-International Classification of Headache Disorders, 3rd edition criteria) undergoing NTG provocation in a prospective, within-subject repeated-measures pharmacological provocation stu

Longitudinal Outcomes of Oral Glutamatergic Augmentation in Trauma-Related Dissociation: A Six-Month Case Study.

Trauma-related dissociation can present with derealisation, depersonalisation, abrupt lapses in awareness, memory gaps, and fugue-like states, and it may be difficult to treat with standard approaches alone. Glutamatergic interventions have been proposed as neuroplasticity-focused strategies because N-methyl-D-aspartate (NMDA) receptor antagonism and downstream alpha-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid (AMPA)-linked signalling may influence synaptic plasticity. A 25-year-old woman with documented complicated grief related to her paternal grandfather's sudden death, longstanding

Deeper into dissociative Amnesia: A perspective from Higher-Order theories of consciousness.

Memory is a fundamental brain function relying on complex neurological mechanisms that involve the processes of experiencing, encoding, consolidating and retrieving. A disorder primarily characterized by the loss of (autobiographical) memory owing to stressful or traumatic events is dissociative amnesia, the underlying mechanisms of which are poorly understood and further research is warranted. In this review, we first outline the memory systems and associated brain areas, then introduce the neural suppression pathways of memory retrieval and discuss the neural correlates and influential theor

Rebuttal to "Questioning the recovery of dissociated traumatic memories under psilocybin".

In our original case report we provide detailed accounts of two research participants who reported the emergence of spontaneously recovered, previously forgotten traumatic memories of sexual assaults during psilocybin treatment. In their commentary of this article, Kangaslampi et al. argue that we preemptively label the experiences as dissociated traumatic memories in the absence of corroborating evidence and consideration of alternative explanations. Here we further address potential therapeutic effects associated with the classification of experiences and provide a rebuttal to the authors' c

Mild Traumatic Brain Injury and Functional Amnesia: When Concussion Becomes a Gateway to Functional Cognitive Disorder.

Mild traumatic brain injury (mTBI) is typically associated with transient cognitive disturbance, particularly involving attention and new learning, with most patients demonstrating full recovery within weeks. Memory impairment in uncomplicated mTBI generally reflects reversible neurometabolic dysfunction and is limited to a brief period of post-traumatic amnesia and restricted retrograde loss surrounding the injury. However, a subset of patients develop persistent and disproportionate autobiographical memory disturbance that exceeds expected neuroanatomical limits and lacks structural correlat

Differentiating Suicide Attempt History Among Individuals with Borderline Personality Disorder Engaging in Non-Suicidal Self-Injury.

OBJECTIVE: Suicidal behaviors and non-suicidal self-injury (NSSI) are highly prevalent and often co-occur among individuals with borderline personality disorder (BPD). Individuals with both a BPD diagnosis and NSSI are at heightened risk for suicide. However, few studies have examined which factors differentiate those with BPD and NSSI who have attempted suicide from those who have not. This study aimed to identify such clinical factors to inform suicide risk assessment within this vulnerable population. METHODS: Participants were 78 individuals with BPD and a history of NSSI. Fifty-four had

Dissociative amnesia with fugue following a suicide attempt: a rare clinical intersection of identity loss and suicidal ambivalence: a case report.

BACKGROUND: Dissociative amnesia with dissociative fugue is a rare dissociative disorder characterized by abrupt loss of autobiographical memory and disruption of personal identity, most often emerging in the context of severe psychological stress. Although dissociative phenomena are increasingly recognized in individuals with suicidal behavior, the development of fugue states in the immediate aftermath of a suicide attempt remains rarely documented and poses significant diagnostic and therapeutic challenges. CASE PRESENTATION: We report the case of a 50-year-old man with no documented psychi

[Functional cognitive disorders and dissociative amnesia: the contribution of double listening].

Cognitive neurofunctional disorders (NFDs) and dissociative amnesia (DA) are distinct clinical entities, although they may share common pathophysiological mechanisms, vulnerabilities, and risk factors. When carefully assessed, cognitive neurofunctional disorders appear to be relatively common in memory clinics, accounting for up to 20% of consultations. The widespread adoption of the term "functional" over the past decade reflects the non-organic nature of these disorders and is generally well understood by patients. This terminology has progressively replaced earlier terms such as "psychogeni

Client-reported prevalence of repressed memory recovery in therapy: a representative U.K. sample.

The prevalence of recovered memories of previously unknown abuse is important because of the risk of memory construction. Asking therapists about such prevalence comes with inherent biases, a problem bypassed by asking a representative sample of therapy clients directly. This study examined client-reported prevalence in a representative U.K. sample of 1507 adults. Of the total sample, 40% reported receiving therapy. Among these, 4.9% reported that a therapist discussed repressed memories of abuse (12% of those who received therapy), with half indicating that the therapist raised it first (2.5%

Exploring Psychopathology with Brain Fingerprinting Technique.

In the exploration of psychopathology, the most discussed anecdote which always remained contradictory was the notion of 'functional vs biological' perspective of psychopathology. With the same anecdote, the present study attempted to reflect the clarity on the issue by taking a paradigmatic move in exploring the psychopathology with the help of Brain Fingerprinting Technique (BFT). BFT computes a mathematical and statistical algorithm for determination of information through presentation of stimuli. This presentation of stimuli leads to generation of P300-MERMER brainwave to determine the inf

Evidence-based consensus guidelines for the pharmacological management of substance dependence: Recommendations from the British Association for Psychopharmacology.

The British Association for Psychopharmacology guidelines for the management of substance dependence focus primarily on the pharmacological aspects of treatment. A group of international experts from a wide range of disciplines reviewed the current evidence in their field, considered the strength of the evidence and discussed the clinical implications at a consensus meeting. The guidelines focus on the pharmacological management of dependence on alcohol, benzodiazepines, 'z-drugs', γ-hydroxybutyrate (GHB), gabapentinoids, opioids, nicotine, cannabis and synthetic cannabinoids, cocaine, ampheta

The Effect of Esketamine Combined with Pulsed Radiofrequency on Neuropathic Pain in Herpes Zoster Patients: A Randomised Clinical Trial.

INTRODUCTION: Zoster-related neuralgia (ZRN) remains challenging to treat with current analgesics and interventional therapies. Esketamine, an N-methyl-D-aspartate receptor antagonist, offers potent analgesic effects. This study evaluated the efficacy and safety of low-dose esketamine combined with pulsed radiofrequency (PRF) for ZRN. METHODS: In this randomized controlled trial, 60 patients with ZRN scheduled for PRF were assigned to receive either continuous esketamine (0.02 mg/kg/h for 48 h) or saline alongside standard PRF and anticonvulsants. Pain (NRS, ID-pain), anxiety (HAMA), depressi

Characteristics of patients hospitalized for diabetic foot in a national hospital in Peru, 2024.

INTRODUCTION: Expansive processes around the third ventricle have always been a major neurosurgical challenge. Despite all the technological and scientific progress recorded over the last few years, third ventricle tumors are still a very difficult pathology to approach due to their formidable complexity.Treating such a tumor demands a good knowledge of local anatomy, pathophysiology, pathology and a good capacity to integrate all the data gathered from the patient prior to and during surgery. Last but not least, the correct means to approach such a tumor involves using modern neuronavigation

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Quantum transport in the presence of a chiral molecular potential.

We investigate quantum transport in a two-dimensional electron system coupled to a chiral molecular potential, demonstrating how molecular chirality and orientation affect charge and spin transport properties. We propose a minimal model for realizing true chiral symmetry breaking on a magnetized surface, with a crucial role played by the tilt angle of the molecular dipole with respect to the surface. For non-zero tilting, we show that the Hall response exhibits clear signatures of chirality-induced effects, in both charge- and spin-resolved observables. Concerning the former, tilted enantiomer

Dissociative Symptoms and Interoceptive Integration.

Dissociative symptoms and disorders of dissociation are characterised by disturbances in the experience of the self and the surrounding world, manifesting as a breakdown in the normal integration of consciousness, memory, identity, emotion, and perception. This paper aims to provide insights into dissociative symptoms from the perspective of interoception, the sense of the body's internal physiological state, adopting a transdiagnostic framework.Dissociative symptoms are associated with a blunting of autonomic reactivity and a reduction in interoceptive precision. In addition to the central fu

Pharmacological characterization of cannabidiol as a negative allosteric modulator of the 5-HT2A receptor.

Promising clinical evidence suggests that psychedelic compounds, like lysergic acid diethylamide (LSD), have therapeutic value for treatment of psychiatric disorders. However, they often produce hallucinations and dissociative states, likely mediated by the serotonin (5-HT) receptor 5-HT2A, raising challenges regarding therapeutic scalability. Given the reported antipsychotic effects of cannabidiol (CBD) and its promiscuous binding at many receptors, we assessed whether CBD could modulate 5-HT2A signalling. Activation of the 5-HT2A intracellular signalling events were assessed using resonance

An intersectional examination of the relationship between racial/ethnic discrimination and psychotic-like experiences: the role of other psychiatric symptoms.

BACKGROUND: Racial and ethnic experiences of discrimination (EODs) are associated with numerous psychiatric symptoms, including outcomes along the psychosis spectrum; however, less is known about mechanisms by which EODs confer risk for psychotic-like experiences (PLEs; common subthreshold psychotic symptoms). Furthermore, work on gendered racism asserts that the intersection of race and gender impacts the nature of EODs experienced and, in turn, may impact the relationship between EODs and PLEs. AIMS: To utilize an intersectional lens (race and gender) to examine whether psychological correl
