Multiple identity enactments and multiple personality disorder: a sociocognitive perspective.

People who enact multiple identities behave as if they possess 2 or more selves, each with its own characteristic moods, memories, and behavioral repertoire. Under different names, this phenomenon occurs in many cultures; in North American culture, it is frequently labeled multiple personality disorder (MPD). This article reviews experimental, cross-cultural, historical, and clinical findings concerning multiplicity and examines the implications of these findings for an understanding of MPD. Multiplicity is viewed from a sociocognitive perspective, and it is concluded that MPD, like other form

Charcot and Les Névroses Traumatiques: historical and scientific reflections.

Between 1878 and 1893, Jean-Martin Charcot published over twenty detailed case histories dealing with what he termed "traumatic hysteria" and what today would be labelled the psychoneurology of trauma. Charcot's cases record a highly diverse clinique tableau of symptoms. Etiologically, Charcot posited a dual model of a hereditary diathèse, or constitutional predilection to nervous degeneration, and an environment agent provocateur. Increasingly during the 1880s, he emphasized the role of "psychical shock". These writings of Charcot also exhibit many of the same, superb clinical qualities that

Multiple personality: a psychiatric misadventure.

Though multiple personality disorder has been diagnosed with increasing frequency over the past decade, its validity has been called into question. A brief review of the literature indicates a likelihood of socio-cultural influences and iatrogenicity in many if not most cases. Symptoms of multiple personality disorder parallel other psychiatric syndromes but are usually given diagnostic priority. The author has described five cases diagnosed multiple personality disorder by another health professional in which iatrogenic influences are clearly evident. All renounced both the diagnosis and the

Traumatic experiences and substance abuse: mapping the territory.

This article examines the relationships between various types of traumatic experiences and addictive behavior, with an eye to formulating effective treatment strategies. Interventions in the posttraumatic stress disorder (PTSD) and related fields are reviewed in an effort to understand how best to integrate them into substance abuse treatment. The recovery-oriented therapy model is used as a framework to define treatment tasks at each stage of the recovery process: how one addresses painful issues depends on the objective, given the recovery stage at hand. These tasks include making a commitme

Dissociated or fabricated? Psychiatric aspects of repressed memory in criminal and civil cases.

During the last decade, clinicians, courts, and researchers have been faced with exceedingly difficult questions involving the crossroads where memory, traumatic memory, dissociation, repression, childhood sexual abuse, and suggestion all meet. In one criminal case, repressed memories served as the basis for a conviction of murder. In approximately 50 civil cases, courts have ruled on the issue of whether repressed memory for childhood sexual abuse may form the basis of a suit against the alleged perpetrators. Rulings that have upheld such use underscore the importance of the reliability of me

Reactions to disclosure of childhood sexual abuse. The effect on adult symptoms.

This study examines the effect disclosing childhood sexual abuse had on adult psychological functioning as measured on symptom checklists. One hundred eighty-eight adults filled out questionnaires regarding childhood trauma experiences and the reaction they received from the first person they told of their sexual abuse, and symptom checklists measuring depression, trauma symptoms, posttraumatic stress disorder symptoms, and dissociation. Data were analyzed using multiple regression analysis and path modeling. Those who told in childhood (N = 66) reported a significantly worse reaction to discl

Rethinking the comparison of borderline personality disorder and multiple personality disorder.

This article has made a number of points that assert what is today a minority position within the fields of MPD/DID and BPD. We hope our views will stimulate attempts by others to rethink their positions and test our assertions, so that issues surrounding these two disorders can be sharpened. For the sake of the clarity of future work, we summarize in outline form the essence of our viewpoint. 1. BPD and MPD/DID have similar appearing symptoms, such as identity problems, unstable affect modulation, self-destructive behaviors, chaotic impulse control, and troubled interpersonal relationships, b

Multiple personality: self-rape.

Multiple personality disorder is the classification given a person for whom two or more distinct personalities are diagnosed. The personalities can be different and vary in character from aggressive to submissive (victimized). The victim alters can be abused and abuse or mutilate self to relieve anxiety or guilt (deserving punishment) or to exert control. Alters may provide a means of expressing anger or other feelings. Aggression towards the body may be sexually oriented, so one may ask whether aggression could make self-rape possible. If so, such expression of self-injuries may be observed i

Incest: transference and countertransference implications.

It has not been the attempt of this paper to discuss the overall treatment of adults with a history of incest, but rather to elucidate and identify the various transference and countertransference implications that are emergent in psychoanalytic therapy. Despite this, some statements can be made regarding the analysis of these themes and the overall effectiveness of treatment with these patients. Analytically oriented treatment with adults with a history of incest concerns itself with three intertwined aspects. They are the reconstruction, integration, and validation of a past history of traum

Recovered-memory therapy and robust repression: influence and pseudomemories.

A subset of the psychotherapists practicing trauma-focused therapy predicate their treatment on the existence of a newly claimed, powerful form of repression that differs from repression as used in the psychoanalytic tradition and from amnesia in any of its recognized forms. Recovered-memory specialists assist patients to supposedly retrieve vast quantities of information (e.g., utterly new dramatic life histories) that were allegedly unavailable to consciousness for years or decades. We refer to the hypothesized mental mechanism as "robust repression" and call attention to the absence of evid

Trauma symptoms in substance abusers with and without histories of childhood abuse.

This study was designed to provide a prevalence rate of childhood abuse in patients being treated for alcohol and other drug problems, and to compare the extent of trauma symptoms that were present in substance abusers with and without childhood histories of physical, sexual, and emotional abuse. One hundred male veterans completed a standard test battery that included the Trauma Symptom Checklist and the Dissociative Experiences Scale. Clinical information pertaining to history of childhood abuse was obtained from the subjects' medical charts. Results demonstrated that 34% of the sample repor

Homicidal somnambulism: a case report.

A case of a homicide and an attempted homicide during presumed sleepwalking is reported in which somnambulism was the legal defense and led to an acquittal. Other possible explanations including complex partial seizures, dissociative state, rapid eye movement sleep behavior disorder and volitional waking behavior are discussed. The evidence supporting the probability that this act occurred during an episode of somnambulism and sleep-related confusional arousal is reviewed and weighed. This evidence includes personal and family history of somnambulism and related disorders; neurological, psychi

Effects of childhood trauma on psychological functioning in adults sexually abused as children.

Standardized symptom measures were used to determine the effect of childhood trauma experiences on adults sexually victimized as children. One hundred eighty-eight sexually abused individuals were tested for mean scores for depression, self-esteem, general levels of trauma symptoms, sexual dysfunction, posttraumatic stress disorder symptoms, and dissociation. Childhood traumatic experiences (parents fighting, physical abuse by father or by mother, other childhood traumas) of a nonsexual nature correlated with increased symptom levels and accounted for significant changes in percentage of varia

Levels of dissociation and histories of reported abuse among women outpatients.

A total of 99 female patients consecutively admitted to an adult psychiatric outpatient clinic were surveyed about their history of physical and sexual abuse. Sixty-five percent of this sample reported having been physically abused, sexually abused, or both during their lifetimes. Scores on the Dissociative Experiences Scale were significantly higher among those reporting a history of sexual abuse than among those reporting a history of physical abuse or no history of abuse, who did not differ from each other. Dissociative Experiences Scale scores were important predictors of histories of sexu

The case of the amnesic intelligence officer.

This paper describes a patient whose amnesia for an offence (fraud) and two fugue episodes occurred against the background of an underlying organic amnesia. The fugue states conformed in their duration and precipitating factors to previous accounts in the literature. The organic, anterograde memory impairment was attributed to multiple small infarcts and a larger infarction in the left medial temporal lobe, which were evident on MRI and PET scans after the patient had developed transient neurological signs. At follow-up, the anterograde amnesia had persisted, and the patient also showed some d

Dissociative symptoms in media eyewitnesses of an execution.

OBJECTIVE: The first execution in California since 1976 took place recently in the San Quentin Prison gas chamber. Eighteen journalists were invited as media eyewitnesses. The authors postulated that witnessing this execution was psychologically traumatic and that dissociative and anxiety symptoms would be experienced by the journalists. METHOD: To investigate the prevalence and specific nature of these symptoms, questionnaires were sent to all the journalists about a month after the execution. The questionnaire contained 17 items assessing dissociative symptoms from the authors' questionnair

Multiple personality disorder in criminal law.

The authors review the recent literature on multiple personality disorder (MPD), the most severe and chronic of the dissociative disorders, in relation to court cases of competence to stand trial, the insanity defense, and research on malingerers feigning MPD. Issues relevant in the assessment of competency and insanity are described. Features characteristic of MPD, including amnesia and alterations in consciousness and personality, have varying degrees of influence over the criminal behavior of an individual with MPD. As in other psychiatric disorders, the influence of MPD on an individual's

Slippage in the unity of consciousness.

Many psychological studies assume a unity of consciousness. Doubt is cast on this assumption (a) by psychophysical studies in normal subjects and those with blindsight showing the simultaneous dissociation of different modes of report of a sensation, and (b) by clinical studies of anosognosic patients showing dissociations of awareness of their own states. These and other phenomena are interpreted to imply two kinds of division of consciousness: the separation of phenomenal experience from reflexive consciousness and the non-unity of reflexive consciousness. Reflexive consciousness is taken to
