Low-Dose Ketamine Infusions for Postoperative Analgesia in Patients on Chronic Opioids Therapy in General Inpatient Units: safety Signals and Monitoring Needs in a Retrospective Cohort.

Patients with chronic pain often have difficult postoperative pain and high opioid needs. Ketamine is a useful adjuvant, but uncertainty about monitoring requirements limits utility. To evaluate incidence and severity of ketamine-related adverse effects under a low-resource monitoring protocol on general inpatient units, we conducted a retrospective chart review at a tertiary center including adults with chronic pain on long-term opioids who received postoperative ketamine infusions (0.1-0.3 mg/kg/h). Monitoring included vital signs, sedation, and mental status at predefined intervals. Data in

Aberrant salience associated with menstrual cycle, mood, and sleep alterations - A cross-sectional study in female medical students during the COVID-19 pandemic.

BACKGROUND: The SARS-CoV-2 pandemic represented an unprecedented global context. Alterations in subjective distress and in psychological processes such as aberrant attribution of meaning - defined as aberrant salience, that is, the tendency to assign excessive or inappropriate significance to otherwise neutral internal or external stimuli - have been described in vulnerable populations. METHODS: In a cross-sectional survey administered in March 2021, 293 female medical students completed the Aberrant Salience Inventory (ASI), Beck Depression Inventory (BDI), Impact of Event Scale-Revised (IES

A comprehensive review and meta-analysis on the safety and efficacy of esketamine for emerging delirium in elderly patients.

BACKGROUND: Postoperative delirium (POD), a common complication in older surgical patients, has an incidence between 20 and 50%, highlighting the importance of effective intervention methods. Preventive methods available so far mainly focus on the reduction of well-known perioperative risks to prevent POD onset. Esketamine, a newly added therapeutic agent, has also exhibited the potential for POD incidence reduction and the improvement of patient outcomes. Previous meta-analyses and systematic reviews primarily focused on general adult patient groups; thus, the purpose of the current meta-anal

Effect of total intravenous opioid-free anesthesia on quality of recovery following gynecological laparoscopy: protocol for a multicenter, randomized, double-blind, controlled trial.

BACKGROUND: Whether opioid-free anesthesia (OFA) improves postoperative quality of recovery remains uncertain. This study aims to compare the effects of an intraoperative OFA protocol with traditional intraoperative opioid-based anesthesia (OBA) on recovery quality in patients undergoing gynecological laparoscopic surgery. METHODS: This multicenter, randomized, double-blind, controlled trial will include 300 adult women scheduled for elective gynecological laparoscopic surgery at five tertiary hospitals in China. Patients will be randomized in a 1:1 ratio to either the OFA group (dexmedetomid

Sleep effects of hydrophilic and lipophilic statins: a comparative narrative review of clinical and experimental evidence.

Statins inhibit 3-hydroxy-3-methylglutaryl CoA reductase to reduce cardiovascular risk, but may induce neuropsychiatric adverse effects. Sleep disturbances, especially insomnia and vivid dreams, are frequently reported, particularly with lipophilic statins that cross the blood-brain barrier (BBB). However, randomized trials often show minimal objective effects, highlighting the need for integrated evidence. This narrative review synthesized data from human clinical trials, pharmacovigilance databases, animal studies, and molecular investigations to clarify the relationship between statin lipop

Associations between Sleep Problems, Psychotic Experiences and Mental Disorders in Children at Familial High Risk of Schizophrenia or Bipolar Disorder and Population-based controls - The Danish High Risk and Resilience Study.

BACKGROUND: Sleep problems are linked to schizophrenia or bipolar disorder and often precede the onset of manifest illness. Psychotic experiences and mental disorders during childhood are risk factors for later development of schizophrenia and bipolar disorder. This study investigated whether nightmares, prolonged sleep latency, and nocturnal awakenings are associated with psychotic experiences and mental disorder in children at familial high risk of schizophrenia (FHR-SZ) or bipolar disorder (FHR-BP) and population-based controls (controls). METHOD: The study included 447 eleven-twelve-year-

Effects of Prazosin and Other Alpha-1 Adrenergic Antagonist Drugs on Nightmares and Sleep Disturbances in Posttraumatic Stress Disorder.

PURPOSE/BACKGROUND: Chronic insomnia causes malaise and is associated with depression, irritability, cognitive impairment, impaired alertness, and substance abuse. Individuals with posttraumatic stress disorder (PTSD) often have both inadequate and fragmented sleep, putting them at elevated risk for these consequences. Sleep abnormalities in patients with PTSD include frequent trauma-related nightmares, prolonged sleep latency, frequent interruptions, early awakening, nighttime behavioral disorders, and disruptions of sleep cycles. Here, we review the pharmacological treatment for nightmares w

DARC-NESS: a mastery-based cognitive-behavioral model for treating chronic nightmares in youth.

Theories of chronic nightmare maintenance highlight dysfunctional beliefs about sleep and nightmares, distress and arousal, anticipatory anxiety, maladaptive sleep habits, and sleep deprivation as perpetuating factors that maintain nightmare disorder over time. Theories of nightmare treatment suggest that self-efficacy is a common factor in nightmare mitigation. The current article introduces DARC-NESS, a multi-component theory of nightmare maintenance that emphasizes nightmare self-efficacy as a central mechanism influencing the maintenance cycle at multiple points. DARC-NESS is a mnemonic fo

Management of trauma-related nightmares in PTSD: Can doxazosin serve as a pragmatic alternative to prazosin?

OBJECTIVES: Trauma-related nightmares (TRNs), core features of post-traumatic stress disorder (PTSD), partially or fully replicate the traumatic event and are often associated with nocturnal awakenings and significant emotional distress. In light of the imminent withdrawal of prazosin in France, this review explores the potential of doxazosin as a pharmacological alternative while emphasizing the central role of psychotherapeutic interventions in TRNs management. METHODS: Following a brief overview of TRNs and existing therapeutic strategies, both pharmacological and non-pharmacological, this

The Distressing Dream Content Inventory: Development and Initial Validation in Acute Trauma Patients.

Distressing dreams shortly after trauma may reflect attempts at emotional processing that, if unsuccessful, could heighten the risk of posttraumatic stress disorder (PTSD). However, no measures are designed to assess the emotional content of dreams after trauma. The current studies aimed to develop and validate a clinician-rated inventory for assessing emotional dream content after trauma. In Study 1, we created 77 items targeting seven emotions in dreams: fear, anger, sadness, seeking, care, play, and lust. Two independent raters scored 100 randomly selected dreams using these items. In Study

Item Response Theory Analysis of the PCL-5 in Measuring Sexual Violence-Related PTSD Symptoms Among Bisexual+ Women.

Women with multi-gender attractions (i.e. bisexual+ women) experience alarmingly high rates of sexual violence (SV), which puts women at risk for posttraumatic stress disorder (PTSD). Bisexual+ women experience higher rates of PTSD compared to heterosexual and lesbian/gay people. The PTSD Checklist for DSM-5 (PCL-5) is a widely used self-report measure of PTSD symptoms. We conducted an item response theory (IRT) analysis to evaluate the item difficulty and discrimination of PCL-5 items in assessing PTSD symptoms related to adult SV among young bisexual+ women. We combined two samples of women:

Comparative pharmacovigilance of non-benzodiazepine receptor agonists versus dual orexin receptor antagonists for insomnia in older adults.

BACKGROUND: The accelerating global aging trend has positioned insomnia in the elderly as a major public health challenge. Although cognitive behavioral therapy is the first-line intervention, pharmacological treatment remains widely used. However, significant safety concerns exist regarding the use of traditional non-benzodiazepine sedative-hypnotics (nBZRAs) in older adults, while long-term real-world safety evidence for newer dual orexin receptor antagonists (DORAs) remains scarce. Addressing this evidence gap is critical for guiding safe medication use in the aging population. METHODS: We

Trauma-related nightmares: A Comprehensive and Integrative review.

Trauma-related nightmares (TRNs) are a phenomenon where memory, sleep and suffering converge in episodes marked by vivid imagery, abrupt awakenings and the echo of past trauma. Despite their frequency and clinical impact, TRNs have resisted precise classification, blurring lines between pathology, sleep disturbance and memory-resulting in conceptual inconsistencies this review aims to resolve. Given their complexity, TRNs must be investigated across clinical, phenomenological and physiological dimensions, combining patient narratives with objective sleep and autonomic markers. This review clar
