The treatment of secondary orgasmic dysfunction II.

This study investigated the impact of group treatment on women with secondary orgasmic dysfunction and their partners. Eleven couples received 4 hours of basic sex education, after which seven of the couples received a Combination treatment format consisting of communication skills followed by sexual skills, while four of the couples received the opposite sequence. No significant differences were found between the two sequences. When compared with similarly recruited control couples in a previous study, a significantly greater number of treated women reached or exceeded the 50% criterion for c

Perspectives of sex therapy outcome: a survey of AASECT providers.

Randomly selected AASECT members who reported that they conduct sex therapy (N = 289) responded to a questionnaire about their sex therapy caseloads. Desire discrepancies between partners was the most common problem (31%). Least commonly reported problems were vaginismus (5%), ejaculatory inhibition (5%), and primary erectile dysfunction (2%). Highest success rates (client satisfaction with sexual functioning) were for premature ejaculation (62%), secondary orgasmic dysfunction (56%), and desire discrepancies (53%). Primary erectile dysfunctions had the lowest success rate (25%). The most comm

Measurement of therapy outcome and maintenance of gains in the behavioral treatment of secondary orgasmic dysfunction.

Choice of therapeutic goals and criteria used for evaluation of therapeutic outcome represent fundamental conceptual and methodological issues. The present investigation examined the relationship between how data were obtained (by retrospective questionnaire or ongoing daily self-monitoring), what outcome criteria were used (behavioral or cognitive-affective), whose data were being analyzed (the female or male partner), and when measurements were taken (at posttherapy or follow-up) in a sample of 23 couples with the problem of secondary orgasmic dysfunction in the wife. The results indicate th

Treatment of secondary orgasmic dysfunction: an outcome study.

This study investigated the differential effectiveness of various treatment formats for 55 couples in which the woman reported secondary orgasmic dysfunction (defined as the inability of the woman to reach orgasm during intercourse, with or without clitoral stimulation, more than 50% of the time for the last 5 months). Couples received two 2-hr sessions of sexual reeducation and were then assigned into one of four treatment conditions (a communication skills format, a sexual skills format, one of two combination formats) or into one of two control conditions (an attention-comparison control co

Effects of penile implant surgery on ejaculation and orgasm.

Thirty-five patients who had received a penile implant were interviewed to assess their pre- and postsurgical ejaculatory and orgasmic experience. Prior to surgery, most men were able to experience ejaculation (71%) and orgasm (80%) even though erectile capability was impaired or lost completely. The implantation of a prosthetic device did not interfere with ejaculation or orgasmic functioning. Two patients who lost their ejaculatory functioning following surgery did so because of advances in their illnesses. Five men who had been non-orgasmic earlier reported having orgasm after surgery. In t

Sexual dysfunction in younger insulin-treated diabetic females. A comparative study.

The presence of sexual dysfunction was studied comparatively in 80 insulin treated diabetic women, 26-45 years of age, and in an age-matched control group of women without chronic somatic disease or psychiatric illness. We found no significant difference in the incidence of sexual dysfunction between the two groups (27.5% and 25% respectively). In diabetics and controls, the most common symptom of sexual dysfunction was reduced libido (22.5% in both groups) while only 18% complained of orgasmic dysfunction. In diabetics, sexual dysfunction was not correlated to age and diabetes duration. Sexua

A review of referrals to the psychosexual clinic at the Belfast City Hospital.

The number of patients seen at the psychosexual clinic has more than doubled in eight years. The diagnostic categories of the patients referred have also changed, especially in the area of homosexuality, and now show greater similarity to figures obtained in two reports from Great Britain, apart from a lower referral rate for orgasmic dysfunction. There is also an overall lower rate of referral in Northern Ireland. This raises the question as to whether or not there is a real difference in the prevalence of these disorders in the Province, compared with Great Britain.

A comparison of three therapeutic formats in the treatment of secondary orgasmic dysfunction.

The goal of the present study was to compare the effectiveness of three therapeutic formats: Standard Couple Therapy, Group Therapy, and Minimal Contact Bibliotherapy (self-help) in the treatment of 23 couples in which the wife was suffering from secondary orgasmic dysfunction. The results indicate that a cognitive-behavioral sex therapy program is clearly effective in changing a wide range of subjective satisfaction and behavioral measures, with concurrence of the husbands' and wives' data providing further strength to these findings. Differences in outcome which were elicited in the three ex

The sexual interaction of women with secondary orgasmic dysfunction and their partners.

This paper reports the sexual interaction of 48 couples in which the women had secondary orgasmic dysfunction and the men reported no difficulties in sexual functioning. The results were contrasted with a sample of 63 "sexually satisfied" couples. On the Sexual Interaction Inventory, male and female members of the dysfunctional couples reported greater dissatisfaction with the frequency and range of specific sexual activities engaged in than their counterparts in the normative sample. Males in the dysfunctional group rated their sexual pleasure slightly higher than the norm, whereas the dysfun

Sexual dysfunction in multiple sclerosis.

A questionnaire study on sexual problems occurring with multiple sclerosis (MS) was carried out with 217 patients who had previously participated in the University of Washington Multiple Sclerosis Project. More than one-half of the participating subjects were ambulatory without aids and nearly 75% did not use a wheelchair. Sexual dysfunction was reported by 56% of the women and 75% of the men. Among the women, the most commonly occurring sexual symptoms (in decreasing order of frequency) were fatigue, decreased sensation, decreased libido, decreased frequency or loss of orgasm and difficulty w

Inhibited sexual desire and orgasmic dysfunction in women.

Desire phase dysfunction and orgasmic dysfunction are frequently brought to the attention of the gynecologist. While a variety of psychosocial and organic factors may be involved, the clinician can be helpful in a number of ways. Specific techniques of sexual counsel for both ISD and orgasmic dysfunction have been presented. Recognition and treatment of sexual dysfunction can be a somewhat time-consuming but rewarding aspect of comprehensive women's health care.

Methodological issues in the study of sex therapy: effective components in the treatment of secondary orgasmic dysfunction.

Three components commonly utilized in sex therapy for secondary orgasmic dysfunction (Sensate Focus I, Sensate Focus II, and ban on intercourse) were studied, with the aim of not only exploring their effects on therapy outcome but also, in part, of sensitizing investigators to the benefits of incorporating component analyses within larger investigations of therapy outcome. The subjects were 23 married couples with the problem of secondary orgasmic dysfunction in the wife. Subjects were administered a multicomponent therapy program over a 14-week period. Daily self-monitoring data were analyzed

A comparison between the effects of Kegel's exercises and a combination of sexual awareness relaxation and breathing on situational orgasmic dysfunction in women.

Several authors suggest that women should use pubococcygeal muscle exercises to increase their probability of experiencing orgasm during sexual activities. However, few experimental studies support the efficacy of these exercises. In the present study a comparison was made between the effects of pubococcygeal exercises (PC group) and a combination of sexual awareness, relaxation and breathing (SARB group) on situational orgasmic dysfunction in women. No significant changes occur in orgasmic responsivity in these two groups. Significant differences were obtained in questionnaires and these diff

The effects of sex education on women with secondary orgasmic dysfunction.

This study evaluated the effects of sex education on 48 couples in which the women reported secondary orgasmic dysfunction. None of the males had a problem with premature ejaculation or with erectile dysfunction. Couples received two, two-hour sessions of sex education during a one-week period. From measures administered before and after treatment, the women reported significantly increased orgasmic frequency and decreased sexual anxiety. The males reported a significant increase in the duration of intercourse and in oral-genital sexual stimulation. None of the subject characteristics signific

Primary orgasmic dysfunction: diagnostic considerations and review of treatment.

As a diagnostic category, primary orgasmic dysfunction includes all women who have never experienced orgasm under any circumstances except sleep or fantasy. However, the research samples of nonorgasmic women in clinical reports and empirical investigations are heterogeneous with regard to disruption of earlier phases of the sexual response cycle and emotional concomitants of the dysfunction. The major treatment models—systematic desensitization, sensate focus, directed masturbation, and hypnosis—are presented, and empirical support is reviewed. Separate discussion is included for investigation
