Rapid and Efficient Genetic Manipulation of Intestinal Organoids Under Transient Two-Dimensional Culture Conditions.

Genetic manipulation of cells represents one of the fundamental techniques in current biological research, essential for elucidating the regulatory mechanisms underlying various biological phenomena, modeling many diseases, and investigating their therapeutic interventions. However, in the context of organoid biology, the manipulation of target genes and the selection of resultant genetically modified clones often encounter significant technical challenges due to the typically low gene introduction efficiency. This difficulty is largely attributed to the three-dimensional culture of organoids

Mass Spectrometry-Based Proteomics of Intestinal Organoids.

Proteomics, the large-scale study of proteins, enables the identification, quantification, and functional characterization of proteins, revealing post-translational modifications and protein interactions that are not apparent from transcriptomic data. Human organoids, which recapitulate the structural and functional complexity of native epithelial tissues, provide powerful tools to study disease mechanisms and personalize therapies. However, their culture poses challenges for efficient protein extraction and reproducible analysis. Here, we present a proteomics workflow optimized to maximize pr

Methods for Processing Stem Cell-Derived Organoids for Histological and Immunofluorescence Staining.

Intestinal stem cell (ISC)-derived organoids provide a physiologically relevant 3D culture system to model intestinal biology, regeneration, and disease mechanisms. Their complex architecture, recapitulating crypt structures, poses challenges for downstream histological and immunohistochemical analyses, necessitating optimized processing, and staining protocols. Here, we describe robust workflows for the fixation, embedding, sectioning, and staining of ISC-derived organoids. Each method is detailed stepwise, with emphasis on preserving morphology and antigenicity. We discuss reagent choices, c

Deep Insights Without Clearing: An Optimized Protocol for Patient-Derived Whole-Organoid Immunofluorescence Imaging.

Here, we present an optimized whole-organoid immunofluorescence staining protocol that preserves morphology, achieves uniform antibody penetration, and enables deep-tissue imaging without the need for tissue clearing. Organoids are three-dimensional, self-organizing structures that replicate many of the cellular and architectural features of their tissue of origin, making them powerful preclinical models for studying development, disease, and therapeutic responses. Most organoid research is performed on formalin-fixed, paraffin-embedded (FFPE) samples, in which organoids are sectioned across t

Diagnostic performance and testing costs of pooled testing for molecular detection of tuberculosis in seven countries: a prospective, observational, diagnostic accuracy study.

BACKGROUND: Globally in 2024, only 54% of people diagnosed with tuberculosis received an initial WHO-recommended rapid molecular diagnostic test. Pooled testing could expand access to molecular detection using existing laboratory capacity. We aimed to evaluate the performance of pooled tuberculosis testing in a multicentre diagnostic accuracy study. METHODS: In this prospective diagnostic accuracy study, we enrolled individuals with presumptive tuberculosis through facility-based case finding (district hospitals and primary health centres) and community-based case finding in Bangladesh, Brazi

Comprehensive Isolation Strategy for Intestinal Epithelial and Fibroblast Cells from Healthy and Tumor Human Tissues.

Organoids and fibroblasts derived from patient tissue serve as physiologically meaningful in vitro models to investigate tissue biology, diseases, and treatment responses. In this study, we present a robust protocol for the simultaneous isolation and long-term culture of patient-derived organoids (PDOs) and fibroblasts from both healthy and colorectal cancer samples. The workflow uses enzymatic dissociation followed by efficient separation into epithelial and stromal cell fractions. Organoids are embedded in Matrigel or Matrigel-Collagen I mixtures to support three-dimensional growth, whereas

A Straightforward and Reliable Mouse Model of Colorectal Cancer by Orthotopic Transplantation.

Colorectal cancer (CRC) is a major malignancy with significant global implications for human health. The development of a reliable and pathologically relevant orthotopic CRC model is essential for advancing our understanding of its molecular mechanisms and for developing more effective therapeutic interventions. However, the construction of such models is fraught with challenges primarily because of the technical complexities involved in the transplantation of CRC cells into the intestinal epithelium. In this chapter, we describe a recently developed method for generating an orthotopic CRC mod

Dysregulated AKT signaling reprograms osteosarcoma to drive selective reliance on EP300.

Cancers depend on transcriptional control to maintain the malignant cell state. EP300 and CBP are paralogous master epigenetic enzymes that control normal and malignant transcription. Using a cancer-wide integrative chemical-genetic analysis, we demonstrate enhanced dependency on EP300, compared with CBP, in cell lines from most cancer lineages, including lethal osteosarcoma (OS). Using a selective EP300 degrader (JQAD1), we identify that a genetically defined subgroup of OS with dysregulation of the PI3K-AKT-mTOR pathway is genetically and chemically sensitive to loss of EP300. Activated AKT

Artificial Intelligence-Driven Cost Savings from Emergency Department Chest Pain Patient Evaluation: A Monte Carlo Simulation.

INTRODUCTION: Chest pain patients presenting to the emergency department (ED) often require risk stratification through stress testing, which may necessitate hospitalization. This practice contributes to ED crowding by occupying beds with low-risk patients who may derive limited benefit from testing, thereby increasing bed occupancy, prolonging ED boarding times, amplifying crowding, and resource strain. Use of an artificial intelligence (AI)-driven clinical decision support tool improves the detection of obstructive coronary artery disease and focuses diagnostic testing on those most likely t

Resident Exposure to Acutely Ill Patients Over the Course of Residency.

INTRODUCTION: Emergency medicine literature has long debated the "black cloud" and "white cloud" phenomena, which suggest individual variability in exposure to acutely ill patients. If true, this variation may have significant implications for resident training. This study investigates whether emergency medicine (EM) residents consistently encounter differing volumes of high-acuity clinical encounters throughout their training. METHODS: This retrospective cohort study analyzed 11 years of electronic health record (HER) data from a Midwestern academic emergency department to quantify the numbe

Climate Change and Heat-related Illness in Older Adults: Implications for Emergency Medicine.

Climate change is increasing the frequency, duration, and intensity of extreme heat events, creating a growing public health threat for older adults. Age-related physiologic decline, comorbid chronic diseases, polypharmacy, and social vulnerability all contribute to reduced heat tolerance and higher rates of heat-related morbidity and mortality. Emergency departments (ED) are increasingly encountering older adults presenting with minor heat illness, heat stroke, and exacerbations of chronic illnesses triggered by heat exposure. This review synthesizes current evidence describing why older adul

Identifying Social Need Trends Among Emergency Department Super-Users: A Latent Class Analysis.

INTRODUCTION: Frequent emergency department (ED) users, particularly those with high rates of use, often experience complex social determinants of health (SDOH) that contribute to their repeated visits and may lead to adverse health outcomes. Our objective in this study was to characterize the needs most commonly affecting this population, which may support the creation of tailored interventions with a higher chance of success. METHODS: This was a retrospective analysis of patients evaluated by an ED social medicine team in a large urban academic ED from May 2021-December 2023. Patients were

Protective Effect of COVID-19 Vaccination on Post-Infection Visuospatial Deficits.

OBJECTIVE: COVID-19 has been associated with persistent cognitive deficits months after SARS-CoV-2 infection. Vaccination reduces severe disease, but its effect on post-infection cognition remains uncertain. We evaluated whether COVID-19 vaccination attenuates cognitive deficits after SARS-CoV-2. METHODS: We assessed 237 adults with COVID-19 and 105 controls. Patients were grouped by vaccination status at infection (vaccinated n=87; unvaccinated n=150). The neuropsychological assessment covered language, visuospatial ability, visual memory, verbal memory, attention, executive functions, and w

Machine learning predicts treatment response to nusinersen in non-sitter spinal muscular atrophy (SMA).

BackgroundNusinersen has substantially increased survival and improved disease progression in Spinal Muscular Atrophy (SMA) patients. However, treatment response is heterogeneous, with some patients gaining the ability to sit and walk whilst others display less obvious changes.MethodFrom the SMA Reach UK and the Italian Telethon Network, 124 non-sitter SMA patients treated with nusinersen under 4 years were included and randomly allocated to training and testing (80/20%). Tree and regression-based machine learning for survival outcomes were compared, and oblique random forests were selected, w

Orofacial neuropathic pain and enhanced excitability of non-nociceptive-like and nociceptive-like trigeminal Aβ-afferent neurons in mice following infraorbital nerve chronic constrictive injury.

Orofacial neuropathic pain is a debilitating pain condition. It is poorly treated, partially due to an incomplete understanding of its underlying neurophysiological mechanisms. Orofacial neuropathic pain is often caused by injury or dysfunction of trigeminal nerve branches, including the infraorbital nerve (IoN). Clinically, a main symptom of orofacial neuropathic pain is mechanical allodynia, excruciating pain induced by gentle touch. While previous studies on neuropathic pain have mainly focused on the involvement of C- and Aδ-afferents, the potential contribution of Aβ-afferents to orofacia

Risk Factors for Early Intestinal Resection in Pediatric Crohn's Disease: A Nationwide Cohort Study.

INTRODUCTION: Complicated disease affects a substantial proportion of pediatric Crohn's disease (CD) patients and leads to surgery. A subset of children requires intestinal resection shortly after diagnosis. Several risk factors for surgery are known; however, the predictors of these early resections have not been well defined. We aimed to characterize this patient group and identify risk factors associated with early intestinal resections. METHODS: We performed a retrospective analysis of patients under 18 y diagnosed with CD between 2010 and 2019, based on the Hungarian Pediatric inflammato

Ubiquitin-specific proteases and the immune microenvironment in cervical cancer: mechanistic evidence, immunotherapy resistance, and therapeutic perspectives.

Cervical cancer remains a major global health burden, and resistance limits the benefit of immune checkpoint inhibitors (ICIs). Ubiquitin-specific proteases (USPs) regulate protein homeostasis and may connect human papillomavirus-associated oncogenesis with immune evasion. This narrative review evaluates USP-dependent mechanisms relevant to cervical cancer, distinguishing disease-specific findings from evidence in other tumor types and proposed pathways. Cervical cancer studies support USP7-associated immune-checkpoint regulation and USP14-dependent MCT4 stabilization and lactate export, altho

CHEmical-shift selective Adiabatic Pulse (CHEAP): fast and high resolution downfield 3D 1H-MRSI at 7T.

PURPOSE: The downfield resonances in the human brain remain elusive or challenging to detect with traditional 1H-MRSI. Conventional in vivo 3D 1H-MRSI faces several challenges, including low signal-to-noise ratio (SNR) and magnetization transfer effects with water, which can lead to prolonged measurement times and reduced spatial resolution. To address these limitations, we propose a downfield 3D-MRSI method aimed at measuring downfield metabolites with enhanced spatial resolution, and speed acceptable for clinical practice at 7T. METHODS: The CHEmical-shift selective Adiabatic Pulse (CHEAP)
