Physical exercise as an adjunct to the pharmacological treatment of epilepsy: a priority for future research.

Medication adherence is a major determinant of seizure control in people with epilepsy, yet nonadherence remains frequent and is associated with breakthrough seizures, increased healthcare utilization, and higher mortality. Despite the central role of antiseizure medications (ASMs), adherence is a multifactorial behavior shaped by routine stability, health beliefs, self-efficacy, mood, and social support, and it cannot be improved by prescription alone. Physical exercise has emerged as a promising complementary strategy in the management of chronic diseases, with benefits that extend beyond ge

Quantifying the impact of pathogen interactions on vaccine effectiveness estimates in the test-negative design.

The test-negative design (TND) has been widely adopted for real-world vaccine effectiveness (VE) assessment, assuming that control selection is independent of vaccination status. However, pathogen interactions may contradict this assumption, posing challenges to VE evaluations. We developed a theoretical framework to quantify the pathogen-dependence-related biases in VE estimates. The framework was further empirically evaluated using data from 66,117 pediatric patients tested for multiple respiratory pathogens during 2023-2024, comparing influenza VE estimates using standard influenza-negative

Engineering biomaterials to improve cytokine therapies for wound healing.

Recombinant cytokines have been used as therapeutics for cancers, autoimmune diseases, and infectious diseases, but their application in wound healing and tissue regeneration remains relatively underdeveloped. Short half-lives, narrow therapeutic windows, and a myriad of adverse effects due to pleiotropy and redundancy have limited their clinical use. Biomaterial-based delivery platforms such as hydrogels, particles, and scaffolds have gained traction as engineering strategies to bridge this translational gap. Despite their popularity in preclinical research, the last FDA approved biomaterial-

An examination of the effect of dual task on gait in Parkinson's disease and REM sleep behavior disorder.

BACKGROUND: Gait variability is a hallmark of Parkinson's disease (PD) and has been linked to cognitive deficits and fall risk. Rapid eye movement sleep behavior disorder (RBD) is a strong predictor of synucleinopathies, yet evidence for gait changes in RBD is inconsistent. Performing a dual task increases gait variability, an effect that can be quantified using a cost function. OBJECTIVE: Determine the degree to which dual task cost differs between control, RBD, and PD participants at baseline, and between RBD converters versus non-converters at follow-up. METHODS: 46 RBD, 23 control, and 1

High-Resolution Manometry Assessment of Esophagitis Across Los Angeles Grades Validates Lyon 2.0 Criteria.

INTRODUCTION: Gastroesophageal reflux disease (GERD) with erosive esophagitis is driven by antireflux barrier (ARB) dysfunction, yet a comprehensive high-resolution manometry (HRM) assessment across Los Angeles (LA) grades is lacking. This study evaluates HRM metrics in patients with erosive esophagitis to validate Lyon 2.0 consensus criteria, hypothesizing that LA grade B esophagitis shares manometric features with severe GERD (LA grades C-D) and that HRM can stratify LA grade A patients for objective GERD risk. METHODS: A retrospective multicenter study (July 2022 to July 2025) enrolled 254

Comparison of the Efficacy of Dexrabeprazole and Rabeprazole in Inhibiting Intragastric and Intraesophageal Acidity in Gastroesophageal Reflux Disease.

BACKGROUND: Proton pump inhibitors (PPIs) are the cornerstone of gastroesophageal reflux disease (GERD) treatment, but concerns regarding their efficacy and side effects persist. Dexrabeprazole, the R(+) enantiomer of rabeprazole, may offer similar efficacy with a lower dose, potentially addressing these issues. AIM: This study aimed to compare the efficacy and safety of dexrabeprazole 10 mg and rabeprazole 20 mg in GERD patients with pathologic acid reflux. METHOD: A total of 48 GERD patients were included: 24 received dexrabeprazole 10 mg and 24 constituted the historical active-control gr

CD47 fuels intestinal fibrosis by inducing fibroblast phagocytic evasion and represents a therapeutic target.

Intestinal fibrosis is a major complication of Crohn's disease, for which targeted therapies remain unavailable. Fibrosis is defined by excess matrix deposition, which may result from inadequate phagocytic clearance, yet how phagocytosis shifts here is unclear. Using single-cell and spatial transcriptomics, we identify fibrosis-associated CD47hi fibroblasts coupled to SIRPAhi macrophages in fibrotic regions. Cell-cell communication analyses and neutralizing-antibody experiments identify interleukin-1β as a mediator of their crosstalk, forming a profibrotic niche characterized by phagocytic eva

Validation of a Rapid Loop-Mediated Isothermal Amplification-Based Test for the Detection of Shigella from Rectal Swab.

The rapid loop-mediated isothermal amplification-based diagnostic test (RLDT) has been validated as a simple, sensitive, and rapid tool for detecting Shigella spp. from fresh or frozen stool. When stool is unavailable or difficult to transport, rectal swabs (RSs) in Cary-Blair medium, stool collected in buffered glycerol saline (BGS), or stool-dipped swabs (SDSs) in Cary-Blair medium offer alternatives that can be easily collected and transported without maintaining a cold chain. This study evaluated the performance of RLDTs for detecting Shigella spp. from RSs, stool collected in BGS, and SDS

Global Perceptions of the Term "Tropical Medicine": Results from a Cross-Regional Multilingual Survey.

The term "tropical medicine" has sparked many discussions on its appropriateness because of its colonial history. The aim of this study was to analyze perceptions of the term among international medical professionals. An open online survey, conducted between August 2023 and September 2024, was distributed via professional networks and social media. Of 590 respondents from 66 countries, 80% were employed in the global north. Participants employed in the global south associated "tropical medicine" more with a geographical connotation (77/85 versus 346/471, P <0.001). Only 21% saw the need to rep

Local Tissue Damage Induced by Snakebite Envenoming: Mechanisms and Therapeutic Advances.

Snakebite envenoming is a serious and often neglected tropical disease associated with high mortality and disability rates. Local tissue injury is one of the main causes of long-term functional impairment after snakebite. Clinically, it may present as local hemorrhage, edema, myonecrosis, and tissue fibrosis. Accumulating evidence indicates that several venom components, including snake venom metalloproteinases, snake venom phospholipase A2s, three-finger toxins, and L-amino acid oxidases, are involved in local tissue injury. These toxins can induce direct cellular and microvascular damage. Th

Proteome-wide C-degron activity profiling connects conditional regulation of the CTLH E3 ligase complex to ribosome biogenesis.

Many E3 ubiquitin ligases recognize degrons located at protein termini, but the paucity of bona fide substrates of N-degron and C-degron pathways hampers our understanding of their physiological significance. Devising an expression screening strategy to assess the effect of C-terminal "capping" on the stability of thousands of human proteins, we systematically identify substrates harboring C-degrons. Interrogating one leading candidate, ZMYND19, we characterize a C-degron pathway governed via the C-terminal to LisH (CTLH) E3 ligase complex subunit Muskelin. Cell-to-cell variability in ZMYND19

Gut-derived glucagon-like peptides attenuate T cell-induced intestinal injury.

Glucagon-like peptide-1 (GLP-1) and glucagon-like peptide-2 (GLP-2) are produced in enteroendocrine L cells and control energy homeostasis. GLP-1 receptor (GLP-1R) activation improves glucose homeostasis and induces satiety and weight loss, whereas GLP-2 receptor (GLP-2R) signaling promotes intestinal growth and nutrient absorption. GLP-1R and GLP-2R agonism also reduce intestinal damage in preclinical studies. Here, we demonstrate that endogenous GLP-1R and GLP-2R signaling are essential for protecting the intestine from T cell-mediated enteropathy. Gut injury induced by T cell activation is

More than just accuracy: A pathway-level framework for implementing blood and digital biomarkers for Alzheimer's disease care.

Blood biomarkers are transforming Alzheimer's disease diagnostics by enabling a scalable, minimally invasive assessment of pathology. Food and Drug Administration clearances and European market certifications of different plasma tests mark a shift toward biological diagnosis in routine care. However, clinical implementation is not guided by diagnostic accuracy alone but will depend on improved patient-centered and pathway-level outcomes, including diagnostic confidence, the appropriate use of CSF and PET assessments, time to diagnosis, amyloid-targeting treatment eligibility assessment, and tr

The effect of food taxes in the UK on health and economic outcomes, and equity: a modelling study.

BACKGROUND: WHO recommends using fiscal policies (such as taxes on unhealthy foods) that discourage consuming foods that contribute to unhealthy diets. Food tax evaluations require consideration for sociodemographic differences, consumer and industry behaviour, reformulation, and long-term health-related outcomes and health inequalities. We aimed to model the potential benefits and harms of three food tax policy designs, to health, costs, and inequalities, in the UK. METHODS: Using a microsimulation model, we performed an economic evaluation of three UK food tax policy designs: (1) a Sugar Sn

Reimagining the aspirations of public health for the coming decade.

The past 10 years have challenged and reshaped public health in ways that were difficult to imagine more than a decade ago. This situation calls on public health to think clearly about where the field is headed and to anticipate where the field needs to be so as to emerge stronger from the moment. Five trends will be shaping health in the coming decades: population ageing, urbanisation, climate change, changing relationships with institutions, and the emergence of transformative technology. These trends are driving-and coinciding with-shifting disease burden, including the rise of non-communic

Care Outcomes Among Hospitalized Patients With Cancer: A Retrospective Cohort Study.

BACKGROUND: Patients with cancer often require hospital-level care to address urgent needs and symptom concerns. However, little research has sought to identify patient factors, particularly rural-urban residence, associated with care outcomes in this population. METHODS: We conducted a retrospective cohort study including adult patients with cancer admitted to the University of Oklahoma Medical Center from January 2017 through December 2022. We extracted patients' sociodemographic (ie, age, sex, race, ethnicity, rural-urban residence [Rural-Urban Continuum Codes]) and clinical (ie, Charlson

Major Plant Diseases and Regional Plant Health Priorities in Asia.

Asia supports major food, horticultural, and plantation crops, yet uneven loss data and surveillance capacity hinder continental-scale prioritization of plant diseases. This review integrates FAOSTAT crop statistics, EPPO pathogen occurrence and host records, Scopus affiliation- based publication data, and disease-specific literature to identify regional surveillance and management priorities. Wheat, rice, maize, and potato accounted for nearly three quarters of the documented crop-pathogen intersections, highlighting shared staple-crop vulnerabilities. Rice blast, bacterial blight, sheath bli

Circuit organization of Tac2-expressing neurons from established functions to testable hypotheses.

Tac2-expressing neurons participate in behavioral and neuroendocrine functions through regionally specialized circuits. Building on a 2021 review of the Tac2/neurokinin B (NKB) pathway, we examine how cell- and projection-specific studies have refined this organization. Fear-memory and isolation-stress studies provide convergent evidence for NKB-neurokinin 3 receptor (NK3R) signaling, whereas other studies establish functions of Tac2-expressing populations without demonstrating peptide dependence. We distinguish interventions targeting neurons, the Tac2-encoded peptide, NK3R, and NK3R-expressi

Effects of prolonged SARS-CoV2 spike protein exposure in long COVID and a mouse model of the disease.

We addressed the question: can the SARS-CoV2 spike protein persist in ACE2+ endothelia in the CNS/other sites and induce a prolonged microvasculature injury syndrome? People who died from COVID-19, with age-matched controls, and people with long COVID with symptoms persisting from 1-5 years, with controls of people who had recovered from COVID-19, were studied. Also compared were WT mice injected with the spike protein sacrificed at day 5 versus 1.5 months. The ACE2+ microvasculature density in the CNS increased 19-fold in the mice exposed to spike for 1.5 months. Human fatal COVID-19 cases sh
