Treatment outlines for paranoid, schizotypal and schizoid personality disorders. The Quality Assurance Project.

Treatment outlines for paranoid, schizotypal and schizoid personality disorders were developed by having nominated experts consider their own views in the light of the treatment literature and the responses of practising psychiatrists. In the detailed recommendations it is clear that while patients with all three disorders often present for treatment in a crisis and often see no issue other than the resolution of the crisis, patients with schizoid personality disorder can use long-term psychotherapy to develop and change to the extent of no longer being handicapped.

Dimensions of personality disorder. The DSM-III-R cluster A diagnoses.

The authors adopt a construct validity approach to examine the structural validity of diagnoses of paranoid, schizoid, and schizotypal personality disorders. Systematic descriptions of these diagnoses were developed based on features identified from the literature that were organized using the clinicians' ratings. Each diagnosis was described in terms of a series of behavioral dimensions. Sets of behavioral exemplars were developed to assess each dimension. Dimensions and diagnoses were found to exhibit satisfactory levels of internal consistency that were cross-validated in a general populati

The effect of personality on placebo response in panic patients.

Twenty-eight patients who participated as a placebo group in a treatment study of panic disorder were studied to determine the effect of personality on study completion and outcome. Those subjects who completed only 3 weeks had significantly more pathological personality traits than those who continued in the study. For those continuing beyond 3 weeks, there were negative correlations between the paranoid and borderline personality traits and a global outcome measure. Spontaneous panic attacks were not affected by personality.

Personality styles of patients asserting environmental illness.

Case reports and chart reviews of patients asserting environmental illness suggest that they suffer from psychiatric difficulties, typically somatization disorder. We assert that viewing these patients solely as somatizers or hysterical characters searching for a nurturant relationship will undermine the doctor-patient relationship. Rather, many of these patients are obsessive/paranoid characters searching for a medical explanation to their physical symptoms. This distinction is highlighted by contrasting the clinical presentations of hysteric/somatizing patients with those environmental illne

Paranoia and narcissism in psychoanalytic theory. Contributions of self psychology to the theory and therapy of the paranoid disorders.

Many factors go into a choice of a therapeutic focus: the patient's psychopathology; the therapist's training, countertransference reactions, and ideological beliefs; and, importantly, a decision about what seems most amenable to treatment and change. As a theory, self psychology describes one aspect of the paranoid process; as such, it is an incomplete theory that complements rather than invalidates more classical theories. As a technique, however, it suggests a style and focus conducive to working with paranoid patients, one that is markedly supportive, nonconfrontational, yet also interpret

The influence of paranoic parents on the education of children.

The influence of paranoic parents on the education of children is often discovered during the expertise - examinations. These problems are not so simple. There are some intersteps of parent's influences on the children: from natural to pathological one. The children who are exposed to the influences of paranoic parents are strongly influenced by them; the child gets an emotional uncertainty and this can happen a negative effect on his personality.

Primitive personality organization as an interpersonally adaptive modification of cognition and affect.

I have attempted a new description of the salient clinical phenomenology of the primitive personality disorders (borderline, narcissistic, paranoid and schizoid), focusing in particular on absence of personality integration, sensori-motor-affective thinking, and an inability to recognize and to own significant emotional predispositions and institute appropriate behavioral adaptations. I conclude that these phenomena cannot adequately be comprehended either from the classical intrapsychic perspective and the cognitive and affective capabilities assumed to exist in normal and neurotic individual

Personality disorder in the families of depressed, schizophrenic, and never-ill probands.

In a blind family study of 176 probands with nonpsychotic major depression, psychotic major depression, schizophrenia, or no history of DSM-III disorders, only the relatives of depressed probands with mood-incongruent psychotic features had a risk for personality disorders higher than that for the relatives of never-ill probands. The authors did not find a high rate of borderline personality in relatives of depressed probands or of schizotypal personality disorder in relatives of probands with schizophrenia or any psychosis. However, depressed probands with normal dexamethasone test results ha

Comorbidity for borderline and schizotypal personality disorders. A study of alcoholic women.

Twenty-six women alcoholics who fulfilled the criteria for a pure borderline personality disorder (BPD) were compared with 16 women alcoholics who also fulfilled the criteria of a schizotypal personality disorder (called mixed borderline disorder, MBD) based on personal interviews including SADS, SIB, Childhood Environment Scale (CES), and the Premorbid Adjustment Scale (PAC). The MBD women had fewer alcoholic relatives, poorer relationships with parents and siblings, a more deviant score on CES and PAC, a higher incidence of nervous children, and a poorer social network. They also reported mo

Pre-morbid psychopathology in schizophrenia spectrum.

In a prospective, longitudinal study of high-risk offspring of schizophrenic mothers, of several dimensions of pre-morbid behaviour 'peculiarity' predicted subsequent schizophrenia or schizotypy. Peculiarity/eccentricity may represent a subtle marker of the schizophrenic genotype. 'Pre-schizophrenics', as compared with 'pre-schizotypes', were characterised by affective dyscontrol, reflected in less introverted and more disturbed behaviour.

DSM-III personality disorder diagnoses in a nonpatient sample. Demographic correlates and comorbidity.

Seven hundred ninety-seven first-degree relatives of normal controls and patients with a variety of psychiatric disorders were interviewed with the Diagnostic Interview Schedule and the Structured Interview for DSM-III Personality Disorders. Slightly more than one sixth of the sample received a personality disorder (PD) diagnosis, and of those with a PD, almost one fourth had more than one. The most prevalent diagnoses were mixed, passive-aggressive, antisocial, histrionic, and schizotypal PD. The demographic correlates and frequency of Axis I disorders in individuals with each specific PD wer

[Personality and psychopathology in patients with primary Sjögren's syndrome].

Twenty-one patients with primary Sjogrens' syndrome were investigated for hostility structure and psychiatric symptomatology. It was observed that they manifested high levels of hostility with an intropunitive direction in combination with highly scored delusional items of guilt and paranoid hostility. Acting-out hostility was in low levels. From the psychopathology point of view the majority of patients reported mainly depression and somatization, as well as anxiety and obsessive-compulsive symptoms. Unexpectedly paranoid symptoms were revealed in a substantial number of these patients. SS pa

Familial aggregation of schizophrenia and schizophrenia spectrum disorders. Evaluation of conflicting results.

In this issue, Coryell and Zimmerman report no significant increase in the risk of schizophrenia or "schizophrenia spectrum" personality disorders in relatives of schizophrenics vs never-ill controls. This article attempts to evaluate critically and interpret their findings. Given the modest number of relatives studied, their negative findings regarding schizophrenia could easily be the result of sampling variation. Relatives were assessed using a general personality disorder instrument, and over two thirds were interviewed by telephone; this method may have a low sensitivity to detect key fea

The heritability of schizophrenia and schizoaffective disorder. A family study.

Ninety-one consecutively admitted patients with schizophrenia (n = 21), schizoaffective depression (n = 43), or psychotic depression (n = 27) entered a blind family study along with 36 never-ill controls. Though schizophrenia spectrum disorders clustered within families, they were not significantly more prevalent in the families of schizophrenic probands. In contrast, morbid risks for affective disorder clearly separated the families of psychotically depressed probands from the families of both schizophrenics and controls. Family study data for schizoaffective probands indicated links to both

Hypochondriacal psychoses: a long-term follow-up.

From a large series of delusional subjects 23 patients demonstrated hypochondriacal delusions as the main delusion during the long-term course of approximately 30 years. An extensive study of these cases reveals that the majority had abnormal premorbid personality, especially somatization, paranoid and/or avoidant traits. Previous and/or precipitating somatic illnesses, if present, were not very serious. The hypochondriacal preoccupation diminished in the long term, but the overall prognosis varied, as the patients did not belong to a uniform diagnostic category. Implications for the clinician

Delusional (paranoid) disorders.

The group of paranoid or delusional disorders, although not nearly as common as the mood and schizophrenic disorders, may be much more frequent than has usually been thought. DSM-IIIR has made a decisive step in recognizably defining at least one group of them. Interestingly, this change partly came about because the advent of an effective treatment helped to define that group more clearly. Nevertheless, DSM-IIIR's classification is too restrictive, and it was wrong to exclude the diagnosis of paraphrenia. Cases fitting this description will have to be consigned to the category of Psychotic Di
