Depression and risk of future suicidal behaviour in patients with deliberate self-harm.

BACKGROUND: Depression is strongly associated with suicidal behaviour, but whether major depressive episode (MDE) diagnosis and depressive symptom severity provide equivalent prognostic information after self-harm remains unclear. METHODS: 739 adults were interviewed at three Swedish hospitals after an episode of deliberate self-harm; 503 women and 236 men, age range 18-95, mean age 38.4, 610 suicide attempts (SA) and 129 non-suicidal self-injuries (NSSI). The Montgomery-Åsberg Depression Rating Scale (MADRS-S), the Suicide Assessment Scale (SUAS) and the Suicide Intent Scale (SIS) were admin

The Role of Social Adversity in the Association Between Autistic Traits and Borderline Personality Disorder Symptoms.

Autism and personality disorders, including borderline personality disorder (BPD), demonstrate high levels of co-occurrence. Autistic individuals are more likely to experience social adversity and exhibit heightened reactivity to stressors, while social adversity is a well-established precursor to BPD. The current study investigated the role of social adversity in the association between autistic traits and BPD symptoms, and the moderating role of autistic traits in the association between social adversity and BPD symptoms. Data of 7403 individuals from the 2007 Adult Psychiatric Morbidity Sur

Perinatal mental health practitioners' experiences supporting mothers with Emotionally Unstable Personality Disorder: a qualitative study.

BACKGROUND: Emotionally Unstable Personality Disorder (EUPD) is a controversial and potentially stigmatising diagnosis, especially in women. Limited research exists on how mothers diagnosed with EUPD engage with perinatal mental health services and no United Kingdom studies have explored perinatal mental health staff perspectives on working with this population. AIM: To explore perinatal mental health staff's experiences of working with mothers diagnosed with EUPD and their views on service provision. METHOD: Reflexive Thematic Analysis was used to analyse semi-structured interviews with 13

Personality disorder diagnoses in UK Autistic people: Evidence from a matched cohort study.

Clinical accounts and cohort studies suggest that Autistic people are disproportionately likely to be diagnosed with personality disorder. We conducted a cohort study of adults diagnosed Autistic drawn from the IQVIA Medical Research Database, with follow-up from 1 January 2000 to 16 January 2019. We included a comparison group without diagnosed autism, matched (1:10) by age, sex and primary care practice. We included 22,112 Autistic adults, of whom 6437 (29.1%) had a diagnosis of intellectual disability. Median age was 20.36 (interquartile range: 18.0-28.5), and 16,881 (76.3%) were men. The r

Rapids Crisis Service for Children and Adolescents 2019-2024: 6-Year Changes in Crisis Service at Lambeth Children and Adolescent Mental Health Services in London, United Kingdom.

OBJECTIVE: The demand for children and adolescent mental health crisis services has risen dramatically in recent years, yet evidence on real-world implementation models remains limited. Rapids is a community-based crisis intervention service developed within Lambeth Children and Adolescent Mental Health Services (CAMHS) at South London and Maudsley NHS Foundation Trust. The primary aim of this study was to examine how clinical presentations, risk profiles, and service outcomes changed over time among children and adolescents accessing Rapids over a 6-year period. We also aimed to describe the

'The true me': Unravelling the dual narrative of borderline personality disorder and autistic spectrum disorder.

OBJECTIVES: There is growing recognition that some individuals who receive a diagnosis of borderline personality disorder (BPD) are later diagnosed with autism. However, existing literature on this topic remains limited. This study aimed to explore the experiences of individuals diagnosed with BPD prior to autism, how they made sense of these diagnoses, navigated clinical systems and learnt to manage the complex challenges associated with this diagnostic sequence. METHODS: Thirteen (6 male, 6 female, 1 non-binary) adult participants took part in semi-structured interviews exploring their diag

[Is it all bipolar? Differential diagnosis of mood swings in young adulthood].

BACKGROUND: Mood swings can represent a key symptom of various psychiatric disorders in young adulthood. Due to substantial symptom overlap, clinicians often face diagnostic uncertainty, increasing the risk of misdiagnosis and inappropriate treatment. OBJECTIVES: Description of qualitative and quantitative characteristics of mood swings in the most relevant psychiatric disorders. MATERIALS AND METHODS: Narrative review based on the International Classification of Diseases, 11th Revision (ICD-11) and pertinent literature. RESULTS: Bipolar affective disorders exhibit episodic, mostly non-reac

Investigating the neurodevelopmental consequences of anesthesia and surgical interventions on cognitive and behavioral outcomes: A comprehensive Mendelian randomization study.

Anesthesia is indispensable to modern surgical practice and clinical care; however, its potential impact on neurodevelopment and behavioral outcomes has raised growing concerns. Discrepancies among previous studies may partly arise from difficulties in disentangling the effects of anesthesia from those of surgical interventions. Mendelian randomization (MR) offers an approach to explore these associations while minimizing confounding and reverse causation. We analyzed genetic instruments related to exposure to different types of anesthesia and the number of surgical operations, in conjunction

"It All Exists Within Me": A Qualitative Study Exploring Mental Health Service Experiences in Adults With Comorbid Eating Disorders and Emotionally Unstable Personality Disorder.

OBJECTIVE: Comorbidity between Eating Disorders (ED) and Emotionally Unstable Personality Disorder (EUPD) is high. This comorbidity is also linked with worse outcomes for traditional evidence-based treatments. We aimed to improve mental health service experience for adults with comorbid EUPD and ED by: (1) understanding the service-user experience of receiving and accessing care; (2) identifying areas of unmet need; and (3) creating recommendations for improving care for this population. METHODS: The project comprised semi-structured interviews with nine service-users within the UK National H

Survival After Massive Potassium Cyanide Ingestion Without Antidote in a Tertiary Care Setting.

Cyanide is a rapidly acting cellular toxin that blocks mitochondrial oxidative phosphorylation, causing abrupt lactic acidosis, cardiovascular collapse, and death within minutes if untreated. Survival after massive oral ingestion is rare, and published cases almost always involve the use of specific antidotes such as hydroxocobalamin or sodium nitrite with sodium thiosulfate. A 29-year-old man with emotionally unstable personality disorder/bipolar disorder, on regular psychotropic medication, ingested approximately 8 g of potassium cyanide dissolved in water in an impulsive act of self-harm. Personality disorders and cardiometabolic diseases: A Mendelian randomization study.

Observational studies have suggested the associations between personality disorders (PDs) and cardiometabolic diseases (CMDs), though these may be biased by unmeasured confounding, including lifestyle factors or reverse causality. Mendelian randomization (MR) provides a novel approach to address these limitations by using genetic variants as instrumental variables, thereby minimizing environmental confounding and enabling causal inference. This study leverages MR to systematically evaluate causal relationships across 9 clinically relevant PDs and 8 major CMDs, offering genetic evidence to clar

Mental health and sentencing: How are judicial decisions made in light of the judgement of R v Vowles?

In England and Wales, s.45A of the Mental Health Act 1983 allows a judge to pass a sentence including both an immediate direction to hospital as well as a punitive custodial element. R v Vowles provides four specific considerations for judges to attend to when considering such sentences (referred to as the 'Vowles statements'). The section, however, remains infrequently used. The present study adopted an online experimental methodology to explore decision-making in relation to the Vowles statements. We used a proxy judicial sample who made decisions about the same criminal case vignette. The e

Factors That Influence Prescribing in Borderline Personality Disorder: A Systematic Review.

Borderline personality disorder (BPD) is a psychiatric condition characterised by pervasive patterns of thinking and feeling, which can lead to social dysfunction and poor mental health. BPD has a significant impact not just on individuals with the diagnosis but also on those around them. Currently, no medication is licenced for BPD. Despite this, it is common for people with BPD to be prescribed multiple psychotropics. All psychotropic medications are associated with adverse events. A systematic review was conducted to explore factors that influence prescribing in adult BPD patients. Searches

Implementation of the Collaborative Assessment and Management of Suicidality in a UK National Health Service Adult Mental Health Service.

Background: The Collaborative Assessment and Management of Suicidality (CAMS) is an evidence-based approach to reducing suicidal ideation. To date, there is limited evidence of the feasibility of implementing CAMS within the United Kingdom's National Health Service (NHS). Aims: The paper aims to describe the implementation of CAMS in an NHS adult mental health service and to report data on engagement and change in self-rated suicidal ideation. Method: Data on engagement, suicidal ideation, and care pathways were gathered for 88 people who started CAMS. Results: 75% of participants completed CA

Efficiency of AIMS: A 4-Week Recovery Oriented Suicide Prevention Pathway.

The global rise in presentations to emergency departments (EDs) for suicidal crises has created significant challenges for healthcare systems. Traditional approaches often emphasise containment and risk aversion, leading to prolonged ED stays and resource-intensive inpatient admissions. A large Australian metropolitan mental health service introduced a Wellbeing Team (WBT), and trialled the recovery-oriented AIMS (assessment, intervention, monitoring, step up/down) pathway. Grounded in the PROTECT framework, its focus is on person-centred care, integrating positive risk-taking and collaborativ

Feasibility randomised controlled trial of a brief psychological intervention for adolescents with borderline personality disorder symptoms delivered with schools and colleges.

BACKGROUND: There is an urgent need for accessible interventions to facilitate early intervention for young people with borderline personality disorder (BPD) symptoms. Existing evidence-based interventions for adolescent BPD are highly resource-intensive, and few young people with BPD symptoms have access to timely treatment. We adapted a brief psychological treatment for adolescent BPD symptoms previously provided within secondary mental health services for delivery within schools and colleges. This study aimed to assess the feasibility of evaluating the effectiveness and cost-effectiveness o
