Development of the new CPTSD diagnosis for ICD-11.

BACKGROUND: The diagnosis of complex post-traumatic stress disorder (CPTSD) was proposed several decades ago by scientist-practitioners, almost parallel to the first description of the diagnosis of post-traumatic stress disorder (PTSD). In the previous International Classification of Diseases, version 10 (ICD-10) issued by the World Health Organization (WHO), this symptom constellation was termed 'enduring personality change after catastrophic experience'. This diagnosis has not been clinically influential, nor has it been subjected to much research. Thus, in a multi-stage process of ICD-11 de

Prevalence of complex post-traumatic stress disorder in refugees and asylum seekers: systematic review.

BACKGROUND: Refugees and asylum seekers often report having experienced numerous complex traumas. It is important to understand the prevalence of complex post-traumatic stress disorder (CPTSD), which can follow complex traumas. AIMS: This systematic review aims to summarise the available literature reporting the prevalence in refugees and asylum seekers of three operationalised definitions of CPTSD: the ICD-11 diagnostic criteria, the ICD-10 criteria (for enduring personality change after catastrophic experience) and the DSM-IV criteria (for disorders of extreme stress not otherwise specified

PTSD with secondary psychotic features among trauma-affected refugees: The role of torture and depression.

This cross-sectional study examined the prevalence of PTSD with secondary psychotic symptoms (PTSD-SP), its comorbidities, and its association with torture and depression in treatment-seeking refugees. Data were pooled from the Danish Database on Refugees with Trauma (DART). The sample represents approximately 90% of trauma-affected refugee-patients (N = 627) attending a Danish psychiatric outpatient clinic from 2008 to 2013. PTSD, secondary psychotic symptoms, and comorbidities were assessed with structured and routine clinical interviews. The association of PTSD-SP with torture and depressio

Sex differences in trauma exposure and symptomatology in trauma-affected refugees.

The aim of this cross-sectional study was to examine associations between sex, exposure to potentially traumatic events and pre-treatment symptomatology in trauma-affected refugees. We analyzed pre-treatment self-report questionnaires and clinical interviews from 318 refugees with post-traumatic stress disorder (PTSD) in a specialized mental health clinic. Results show that twice as many females reported exposure to family violence (p = .001) and sexual abuse (p = .004), whereas men were at least three times as likely to report exposure to torture (p <0.001), imprisonment (p < .001), and comba

Secondary psychotic features in refugees diagnosed with post-traumatic stress disorder: a retrospective cohort study.

BACKGROUND: A substantial amount of refugees (10-30%) suffer from Post-Traumatic Stress Disorder (PTSD). In Denmark there are different facilities specialised in psychiatric treatment of trauma-affected refugees. A previously published case report from such a facility in Denmark shows that some patients suffer from secondary psychotic symptoms alongside their PTSD. The aim of this study was to illustrate the characteristics and estimate the prevalence of psychotic features in a clinical population of trauma-affected refugees with PTSD. METHODS: Psychiatric records from 220 consecutive patient

Comparison of ICD-10 diagnostic guidelines and research criteria for enduring personality change after catastrophic experience.

Enduring personality change after catastrophic experience (EPCACE) is a diagnostic category included in the International Statistical Classification of Diseases and Related Health Problems, 10th revision (ICD-10), as one of the adult personality disorders. Preliminary investigation suggests there is considerable endorsement in principle for this new category amongst experts in the field of intentional human trauma, yet many aspects of the diagnosis remain contentious. EPCACE is described in 2 sets of ICD-10 criteria: the Diagnostic Criteria for Research (DCR) and the Clinical Description and D

[Process--development--personality change. Psychotraumatological considerations on basic concepts of psychopathology].

The"erlebnisbedingte Persönlichkeitswandel" (reactive personality change) described by Venzlaff in 1958 is characterized here as a prototype of "enduring personality change after catastrophic experience" (ICD-10 F62.0). The symptoms of this post-traumatic syndrome indicate profound and irreversible changes in the individual's view of the world and herself. Referring to more anthropologically oriented psychotraumatological studies by Straus and Müller-Suur, Venzlaff interprets this syndrome as an "incurable disruption of the patient's existence" ("unheilbarer Bruch der Daseinsordnung"). Clinicians' understanding of International Statistical Classification of Diseases and Related Health Problems, 10th Revision diagnostic criteria: F62.0 enduring personality change after catastrophic experience.

International Statistical Classification of Diseases and Related Health Problems, 10th Revision (ICD-10) has included a diagnostic category of "enduring personality change after catastrophic experience" (EPCACE). Preliminary investigation suggests that there is considerable endorsement in principle for this new category among experts in the field of intentional human trauma, yet many aspects of the diagnosis remain contentious. Criticisms leveled at EPCACE raised issues that question the reliability, validity, and clinical utility of this diagnostic category. In ICD-10, there are 2 sets of doc

Pharmacotherapy of post-traumatic stress disorder.

In the Diagnostic and Statistical Manual of Mental Disorders (DSM)-III, DSM-III-R and DSM-IV, the diagnosis of post-traumatic stress disorder (PTSD) requires the presence of three symptom clusters: re-experiencing, avoidance and hyperarousal. The selective serotonin reuptake inhibitors (SSRIs), in particular sertraline and paroxetine, have emerged as the treatment of choice for trauma victims experiencing these three symptom clusters. While not approved by the U.S. Food and Drug Administration, other pharmacological agents are often used, some for symptoms found in victims of early, chronic or

[Post-traumatic stress disorders: diagnostic and therapeutic principles ].

Over 50 percent of the general population are exposed to a traumatic event at least once in their lives. Some but not all trauma survivors go on to develop posttraumatic stress disorder (PTSD). The diagnostic criteria of acute stress reaction, posttraumatic stress disorder, and enduring personality change after catastrophic experience are presented. PTSD should primarily be treated by means of psychotherapy. Cognitive-behavioral approaches, exposure therapy in particular, currently show the best evidence as effective treatments for PTSD. In addition to psychotherapy, psychopharmacological trea

[Reactions to torture and persecution. Traumatized refugees in the Danish health service].

The Danish health services encounter a growing number of refugees who suffer from the effects of persecution and torture. To be a traumatised refugee in a foreign culture often causes psychological reactions and biological changes, and, at the same time, adaptation to a new culture is a demanding existential challenge. The condition is rather poorly described by the diagnoses "post-traumatic stress disorder" and "enduring personality change after catastrophic experience". These conditions can cause difficulties both diagnostically and treatment-wise, as the trauma story can awaken violent reac

Expert opinions about the ICD-10 category of enduring personality change after catastrophic experience.

The inclusion of enduring personality change after catastrophic experience (EPCACE) as a diagnostic category in the ICD-10 represents a turning point in the evolution of the nosology of traumatic stress syndromes, yet many aspects of the diagnosis remain contentious. Given the absence of published research concerning this category, an exploratory survey of international experts was conducted using a questionnaire focusing on key aspects of the category, namely whether respondents used the diagnosis of EPCACE in their practice; which features were most salient in making a diagnosis of posttraum
