BACKGROUND: As syphilis cases increase across the U.S., the emergency department (ED) has emerged as a critical venue for screening. However, little is known about the current ED screening landscape. This study aims to evaluate the national landscape of ED syphilis screening, identify barriers to implementation, and characterize existing infrastructure that may support expansion of syphilis screening in the ED.
METHODS: Electronic surveys were distributed via email to 281 institutions with emergency medicine residency programs. Survey responses were also solicited from institutions both with

BACKGROUND: Congenital syphilis (CS) is a preventable infection and an indicator of access to and quality of antenatal care. Several countries have reported an increase in cases associated with rising syphilis rates among women.
OBJECTIVE: To describe the epidemiological characteristics and trends of confirmed CS cases in Catalonia between 2010 and 2024.
METHODS: A descriptive analysis was conducted using data from the Catalan STI surveillance system. Clinical and epidemiological variables of live-born infants and their mothers were analysed, and annual incidence rates were calculated.

Aligned with the increasing trend observed across the EU/EEA, congenital syphilis (CS) cases have risen in Portugal, which has the third highest rate per 100000 live births in the EU/EEA. This study aimed to analyse CS cases reported in Portugal, focusing on pregnancy monitoring and antenatal screening to identify gaps in preventing vertical transmission of syphilis. We conducted a descriptive study, including confirmed CS cases reported in Portugal from 2015 to 2024. We calculated annual incidence per 100000 live births and the proportion of pregnancies monitored and antenatal screenings perf

The global resurgence of syphilis, characterized by a precipitous rise in incidence and the persistence of congenital transmission, has necessitated a critical re-evaluation of traditional diagnostic paradigms. This review synthesizes recent technological innovations in the detection of Treponema pallidum, spanning from molecular precision to decentralized point-of-care (POC) accessibility. This review synthesizes the paradigm shift from traditional darkfield microscopy (DFM) toward advanced nucleic acid amplification tests (NAATs). By evaluating innovations such as CRISPR-based assays and tra

PURPOSE: To evaluate the presence of hearing loss in the first months of life in infants treated for congenital syphilis at birth.
METHODS: Prospective study including all neonates born in a University public hospital in South Brazil, who needed treatment for congenital syphilis after birth in the Neonatal Unit, between 2021 and 2022. Otorhinolaryngologic and audiologic evaluations were performed at a mean age of 3.3 months, concomitantly with the first post-discharge appointment in the congenital syphilis outpatient clinic, through frequency-specific auditory brainstem response.
RESULTS: Si

BackgroundSyphilis remains a significant global public health challenge, particularly in Sub-Saharan Africa. As part of the sustainable development goals, Ethiopia has aimed to eliminate vertical transmission of syphilis by 2030. Thus, this study aimed to assess the prevalence and factors associated with syphilis infection among pregnant people attending antenatal care (ANC).MethodsAn institution-based cross-sectional study was conducted and public hospitals were selected using simple random sampling, and 488 study participants were recruited via systematic random sampling. Data were collected

The reemergence of congenital syphilis, a preventable condition, carries significant implications for both maternal and fetal health. This resurgence is closely linked to the increasing rates of primary and secondary syphilis among women of reproductive age, inadequate access to antenatal care, absence of routine maternal serological testing, and insufficient treatment even when a timely diagnosis is made. In this report, we present the diverse clinical manifestations of syphilis in both mothers and neonates observed at our institution over a one-year period.

OBJECTIVE: To evaluate the odds of congenital syphilis in neonates of mothers with a history of adequately treated prepregnancy syphilis compared with those with no prepregnancy history of syphilis.
METHODS: We conducted a retrospective cohort study of deliveries within a large multihospital health care system in the southern United States from January 2015 to December 2023. Pregnant individuals were stratified based on whether there was a prepregnancy history of adequately treated syphilis infection. Propensity score full matching was performed, retaining all 382 patients with prior treated

BACKGROUND: The number of syphilis-related infections has increased annually worldwide and especially in the Americas, with emphasis on Brazil, although trends may vary according to country and period, with fluctuations occurring at different temporal intervals. The objective of this study was to characterize the cases of acquired (AS), gestational (GS), and congenital (CS) syphilis and to identify the multiple spatial and spatiotemporal clusters of these diseases, considering the period from 2018 to 2022, in order to analyze changes in the epidemiological pattern of syphilis during this timef

Ceftriaxone is a critical antibiotic for treating severe infections during pregnancy, including community-acquired pneumonia, urinary tract infections, gonorrhea, and syphilis, yet optimal dosing remains undefined due to limited pharmacokinetic data in this population. This study developed and validated a physiologically based pharmacokinetic model for ceftriaxone in pregnant women using the open-source Open Systems Pharmacology Suite. A healthy adult model was first calibrated using data from 193 subjects across multiple studies with doses ranging from 500 to 3000 mg, demonstrating acceptable

Congenital syphilis is a leading cause of preventable stillbirth, yet the mechanisms that enable Treponema pallidum subsp. pallidum, the etiological agent of syphilis, to traverse and persist within the human placenta are virtually unknown. This knowledge gap reflects, in part, the fastidious nature of T. pallidum, which has historically only been propagated in rabbit epithelial cells. Here, we demonstrate that T. pallidum replicates and can be propagated long-term in human placental trophoblast models. We identified three human trophoblast cell lines, JEG-3, BeWo, and HTR-8, that sustained ro

Congenital syphilis (CS) cases have increased nearly eightfold during the past decade in the United States. CS occurs when Treponema pallidum, the causative bacteria of syphilis, is transmitted from an infected pregnant person to their fetus. Untreated maternal syphilis is linked to stillbirth, preterm birth, and neonatal death. Clinical bundles (small sets of evidence-based interventions that are implemented together) are described in this article to improve maternal and infant health outcomes related to CS. These components include opt-out screening with provider scripting, reverse syphilis

With increasingly limited funding availability for global health programmes including the elimination of mother-to-child transmission of HIV, syphilis, and hepatitis B, cost-effective and cost-saving service delivery options such as service integration strategies are vital. The introduction of dual HIV/syphilis rapid diagnostic tests (RDTs) has helped bridge testing gaps by enabling immediate integration of testing for HIV and syphilis. First prequalified by WHO in 2013, widescale adoption of the dual RDTs required extensive global and national advocacy and coordination, including evidence gen

Despite available guidelines, congenital syphilis (CS) continues to burden low- and middle-income countries (LMICs), with failures occurring across multiple points. Two prior case reports from India described symptomatic infants with delayed diagnoses resulting from absent or ignored antenatal screening. We add five neonates identified prospectively at birth at a tertiary care centre in northern India (September 2024-January 2026), born to mothers with reactive syphilis serology. All were asymptomatic, but maternal treatment was adequate in only two cases. Neonatal RPR was reactive in one infa

Congenital syphilis (CS) has been rapidly re-emerging as a public health crisis across North America. The present standard of care treatment for CS involves a 10-day hospitalisation for intravenous penicillin G, which is costly for healthcare systems and families. Given these costs and the frequency of their application, the purpose of this study was to locate the evidence upon which the standard of care for CS has been founded and to assess its quality.A search of Embase and MEDLINE was conducted, and a total of 398 publications were evaluated for inclusion in this study. 54 publications met

BACKGROUND: Congenital syphilis is a significant public health problem and can lead to several unfavorable perinatal outcomes and severe sequelae in children. In Brazil, the number of cases of congenital and gestational syphilis continues to increase, highlighting the need for timely diagnosis, treatment, and prevention. A lack of adequate follow-up during prenatal care may result in unwanted outcomes for the neonate, such as prolonged hospitalization and impacts on neurodevelopment. Although many studies have focused on the clinical aspects of congenital syphilis, there is a gap in understand
