Impact of early oral antiviral use for outpatients with COVID-19 on healthcare utilization and recovery (ANCHOR-02).

OBJECTIVES: To evaluate the associations of early-phase oral antivirals with recovery in outpatients with COVID-19, regardless of baseline risk for severe disease, during the SARS-CoV-2 Omicron JN.1-/KP.3-dominant epidemic era. METHODS: This nationwide, multi-institutional (51 acute-care hospitals in Japan), prospective, registry-based cohort study enrolled outpatients aged ≥12 years with laboratory-confirmed COVID-19 within 5 days of symptom onset (February 1-October 31, 2024). Participants were compared between those with and without oral antiviral use at enrollment (ensitrelvir, nirmatrelv

Viral infections alter the human salivary volatilome.

The volatile metabolome, or volatilome, contains a wealth of information regarding individual health. Two parallel approaches can detect patterns of these volatile metabolites: chemometric analyses employing gas chromatography-mass spectrometry (GC-MS) or "noses" (trained animals or so-called electronic noses). While "noses" are exquisite odor discrimination tools, chemometric analyses are superior for determining the chemical identity of odorants related to disease conditions. Without identifying compounds altered by disease, it is difficult to uncover the responsible mechanism(s) and develop

A multi-phase project to develop a patient-reported measure of barriers to antiretroviral therapy adherence for use in HIV care: The 7-Item I-Score.

Adherence to antiretroviral therapy (ART) is a priority in HIV care. Our goal was to develop and validate a new, short patient-reported outcome measure to screen for barriers to daily oral ART adherence in HIV care in Canada and France. The overarching design was that of a multi-phase, multi-site instrument development project. It involved a previously published qualitative research synthesis (to develop a conceptual framework from which to generate items). Here, we present the results of cognitive interviews (for content validity testing of the preliminary items) and of a longitudinal survey-

Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Extensive Bone Destruction, Lymphovascular and Perineural Invasion, and Local Recurrence: Unusual Features in Salivary Gland Microsecretory Adenocarcinoma.

PURPOSE: Microsecretory adenocarcinoma (MSA) is a recently described salivary gland adenocarcinoma with distinctive histological features and novel genetic signature viz. MEF2C::SS18 fusion. Archival diagnoses are most frequently adenocarcinoma, not otherwise specified (NOS) and polymorphous adenocarcinoma (PAC). To date, most documented MSA cases have shown indolent behaviour, supporting their categorization as low-grade adenocarcinomas. METHODS: Molecularly confirmed cases of MSA with unusual clinical and histological features indicative of aggressiveness are described. Archival cases of ad

Mechanism of activation of an ancestral TEC kinase by PIP3.

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. In this conte

Demographic and clinical characteristics of participants in the Neuropsychiatric Genetics of African Populations-Psychosis (NeuroGAP-Psychosis) study: A descriptive analysis.

BACKGROUND: Approximately 1.3 million individuals in sub-Saharan Africa, as well as millions of others worldwide, live with psychotic disorders. However, there is limited mental health research based on these populations and even less of a focus on neuropsychiatric genetics research within these settings. METHODS: To address this gap, the Neuropsychiatric Genetics of African Populations-Psychosis (NeuroGAP-Psychosis), a case-control study, collected data identifying genetic and environmental risk factors associated with psychotic disorders in Ethiopia, Kenya, South Africa, and Uganda. Association between leukocyte telomere length and neurodegenerative diseases: a prospective cohort in the UK Biobank.

BACKGROUND: This study aims to investigate the association between leukocyte telomere length (LTL) and the risk of incident NDDs using a large-scale cohort from the UK Biobank. METHODS: Data from 459,902 subjects were analyzed using Cox proportional hazards models and machine learning (ML) algorithms to assess LTL's association with NDD risk. RESULTS: Shorter LTL was associated with an increased risk of NDDs, including Alzheimer's disease (HR: 0.52, 95% CI 0.40-0.67, P < 0.001), dementia in AD (HR: 0.53, 95% CI 0.39-0.73, P < 0.001), unspecified dementia (HR: 0.74, 95% CI 0.58-0.95, P < 0.05

[What do we know about compassion focused therapy for anxiety and depressive symptoms: A scoping review].

OBJECTIVE: Anxiety and depressive disorders are among the most common psychiatric conditions and have a high comorbidity rate. While cognitive behavioral therapy (CBT) remains the gold standard psychotherapeutic treatment, its protocols which focus on a specific disorder have limitations when faced with clinical complexity. Transdiagnostic approaches aim to overcome these constraints by targeting processes common to different mental disorders. Compassion-Focused Therapy (CFT) is part of this approach. It is particularly aimed at patients with overwhelming shame and marked self-criticism, often

Standard Therapy Plus Adjunctive Vegan Lifestyle Intervention in Metastatic Prostate Cancer: A Case Report.

Prostate cancer is among the most frequently diagnosed cancers in men globally, with metastatic disease contributing substantially to morbidity and mortality. While conventional therapies remain the standard of care, dietary and lifestyle behaviors are increasingly explored as potentially modifiable factors influencing clinical outcomes, although their role in advanced disease is not well established. We report a 60-year-old, physically active male, lifelong non-smoker without significant comorbidities, diagnosed with de novo metastatic prostate adenocarcinoma (Gleason score 9) and an initial

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Differences in mean age at diagnosis of invasive melanoma for men and women by anatomical site, thickness and subtype.

BACKGROUND: Understanding the factors that influence age at melanoma diagnosis by sex and anatomical site is crucial for developing effective prevention and early detection strategies. While previous research has highlighted sex-based differences in melanoma incidence by age and anatomical distribution, the underlying mechanisms remain unclear. OBJECTIVES: To investigate sex-specific patterns in melanoma age at diagnosis across different anatomical sites and thickness categories, considering the potential influence of disease progression and detection rates. METHODS: We conducted a registry-

User Experiences of and Preferences for Self-Guided Digital Interventions for the Treatment of Mild to Moderate Eating Disorders: Systematic Review and Metasynthesis.

BACKGROUND: Digital interventions typically involve using smartphones or PCs to access online or downloadable self-help and may offer a more accessible and convenient option than face-to-face interventions for some people with mild to moderate eating disorders. They have been shown to substantially reduce eating disorder symptoms, but treatment dropout rates are higher than for face-to-face interventions. We need to understand user experiences and preferences for digital interventions to support the design and development of user-centered digital interventions that are engaging and meet users'

Psychoneurological Links Contributing to Body Mass Index and Eating Disorder Severity.

BACKGROUND-OBJECTIVES: Multiple dynamic interacting factors contribute to the presence and progression of eating disorders (ED). Empirical research has provided mixed findings regarding the mechanisms explaining the contribution of body mass index (BMI) to the diverse ED endophenotypes. The present study aims to evaluate the underlying processes (direct and indirect effects) contributing to BMI and ED severity, considering the contribution of multiple neuropsychological constructs. METHOD: Path analysis, implemented through structural equation models (SEM), was applied to a sample of N = 193
