Autism and Rett syndrome: behavioural investigations and differential diagnosis.

The most common wrong diagnosis of children with Rett syndrome has been the infantile autistic syndrome, and it is probable that this false diagnosis continues to be made in many places. This paper is a follow-up to the authors' previous study concerning differential diagnosis between the two syndromes and is based on the observation of 63 patients over the age of 22 months in 10 stimulus situations. In Rett syndrome there were very small individual differences in the sensorimotor performances, which correspond only to the third and fourth stages of Piaget's 'sensorimotor intelligence' scale.

Rett syndrome--a gray matter disease? Electrophysiologic evidence.

Electrophysiologic dichotomy of abnormal EEGs, as reported by us previously, and normal evoked potentials, as reported in this communication, in patients with Rett syndrome, suggests a predominant gray matter pathophysiologic insult in this serious condition of unknown etiology.

Electroencephalographic abnormalities in Rett syndrome.

Electroencephalograms were performed on 11 girls with Rett syndrome between the ages of 4 and 14 years. Electroencephalographic abnormalities included slowing and disorganization of background activity while awake, multifocal epileptiform discharges maximal over the central and temporal regions, and intermittent, high-amplitude discharges followed by relative attenuation of background activity during sleep. Electroencephalographic patterns were remarkably similar in all patients and may be of diagnostic assistance in suspected cases.

Rett syndrome.

A Case of Rett syndrome in a 20-year old female patient in a mental retardation institution is described. Clinically and according to the case history she is very typical for the syndrome, and the different stages, the etiological theories and some efforts in order to find etiological factors are surveyed.

Rett's syndrome: characterization of respiratory patterns and sleep.

Rett's syndrome is a progressive disorder that occurs in females and is characterized by autistic behavior, dementia, ataxia, loss of purposeful use of the hands, and seizures. Patients with Rett's syndrome have been observed to have stereotyped hand movements (hand-washing) and to exhibit intermittent hyperventilation. To characterize more precisely the sleep and respiratory patterns associated with this disorder, polygraphic studies were made during sleep and wakefulness in 11 patients with this syndrome. These studies showed abnormal respiratory patterns during wakefulness, and abnormal sle

Breathing impairment in central alveolar hypoventilation and Rett syndrome.

Breathing is regulated by the behavioural and the automatic systems, the former mainly acting during wakefulness, the latter during sleep. Central alveolar hypoventilation and breathing disorders in Rett syndrome indicate a selective impairment of the two systems.

Differential diagnosis of syndromes with abnormal respiration (tachypnea-apnea).

Abnormal respiration with episodic tachypnea-apnea can occur in several syndromes (particularly, the Rett, Joubert, Mohr and Dandy-Walker syndromes). These syndromes are briefly reviewed. In the Rett syndrome, in contrast to in most other genetically determined syndromes, the onset of breathing abnormalities does not occur in the neonatal period.

The care of our children in everyday life.

The life within a family of a Rett syndrome child is usually in a constant state of changing emotion due to the overwhelming responsibility of caring for the afflicted child and meeting the needs of the remaining family. Personal philosophy and care techniques are related to give insight into one family's mixed blessing of having a Rett syndrome daughter and their struggle to maintain some normalcy within the seemingly incomprehensible malady. When families share with each other and with those who provide medical, educational and therapeutic care for their daughters, progress is made through m

Rett syndrome--a review and discussion of syndrome delineation and syndrome definition.

The current status of clinical genetic and pathogenetic knowledge in Rett syndrome is reviewed and updated. Some of the concepts which are highlighted include among others, casual homogeneity vs heterogeneity; the difficulty of dealing with potential variability while the possibility of heterogeneity still exists; progress in examining genetic hypotheses. We review our experience with 8 new patients, note the evidence for the presence of congenital hypotonia in all and several instances of minor anomalies and again raise the issue of whether girls with the Rett syndrome are in fact normal from

Rett's syndrome: progression of symptoms from infancy to childhood.

The results of studies of seven girls with Rett's syndrome and two additional cases suggestive of Rett's syndrome are presented. After normal neurological development up to the age of 7 to 20 months, there was a rapid regression of higher cortical function. Rett's syndrome was initially manifested by a delay of further motor development and the appearance of autistic traits. As the disease progressed, there was a loss of ability to crawl, loss of purposeful hand movements, abnormal respirations, truncal ataxia, seizures, and spastic increase in muscle tone. Blood chemistries, including ammonia

Plasma endorphins in Rett syndrome: preliminary data.

Plasma levels of beta-endorphin (beta-EP) and prolactin (PRL) were measured in 5 girls with Rett syndrome and in a control group before and after giving 10 mg metoclopramide i.v. beta-lipotropin (beta-LPH) was only measured in basal conditions. Basal values of beta-EP and beta-LPH were lower than in control individuals. The responses of plasma beta-EP to metoclopramide in Rett syndrome patients were less intense than in control individuals, while the PRL increase in girls with Rett syndrome was significantly higher than in control subjects. These preliminary data suggest a derangement of the d

Autism and Rett syndrome: some notes on differential diagnosis.

Rett syndrome and infantile autism share the common symptom of autism, especially in the early stages of the disorders. In the later development of Rett syndrome autism sometimes becomes less prominent and there may also be clear differences as regards the quality of autistic behaviour in Rett syndrome and infantile autism. However, in infancy, the syndromes may be indistinguishable. The present paper deals briefly with this issue and suggests that in many girls referred in infancy for autistic symptoms Rett syndrome should be considered seriously as a primary diagnostic alternative.

Shift to righthandedness in Rett syndrome around age 7.

This paper confirms Nomura's finding (Nomura et al, 1984) that almost all of those children with Rett syndrome under 7 who grasp objects are lefthanded. It is suggested that the lefthandedness in Rett syndrome up to age 7 comes from a functional lateralization of the cerebral hemispheres similar to that which Birkmayer and Rett proposed for normal infants up to age 1 (Birkmayer and Rett, 1954). However, examination of children over 7 showed that almost all of those patients who grasp objects prefer the right hand and show symptoms of disturbance of the functions of the pyramidal tract that are

Motivational conflicts in Rett syndrome.

Some of the peculiar symptoms of Rett syndrome including stereotypic wringing and washing hand movements, staring, social withdrawal, bouts of hyperpnea, are interpreted as the expression of motivational conflicts occurring in severely brain damaged patients unable to perform useful hand movements. These behaviours can be considerably reduced, if external 'requests' are substantially diminished and sources of keen interest (stories, music, face to face interactions) are activated. These preliminary observations were derived from 34 patients seen by the author.
