Electroencephalographic findings in Rett syndrome.

Rett syndrome is a progressive neurologic condition, affecting only girls and characterized by acquired microcephaly, dementia, seizures, autistic behavior, spontaneous hyperventilation, spasticity, hyperreflexia and a peculiar characteristic stereotypic movement disorder. A review of 35 EEGs (obtained over 0-8.5 years of follow-up) in 9 such patients revealed a striking age-related change in the electroencephalographic pattern. Often initially normal, the EEG shows a variety of epileptiform abnormalities but intact background activity between 3 and 5 years. Between 5 and 10 years of age, the

Rett syndrome: lack of association with fragile site Xp22 and strategy for genetic mapping of X-linked new mutations.

The hypothesis of X-linked new mutations which might cause early abortions of hemizygous male fetuses and a dominant phenotype in heterozygous females seems the most likely genetic explanation of the Rett syndrome. This hypothesis can be reconciled with the normal sex ratio observed in sibships of patients and with the rare recurrence of this disorder in sibs or half-sibs. The latter observation can be explained by germinal mosaicism in one of the two parents. Since in 14 patients no association was found with any particular fragile site or chromosome rearrangement, we propose to map the mutat

The genetics of Rett syndrome: the consequences of a disorder where every case is a new mutation.

The genetic data on Rett syndrome suggest a mutant gene acting as an X-linked dominant with lethality in the male and reproductive lethality in the female. Thus, all cases represent new mutations. The presence of two affected sisters in two of 600 families indicates the ratio of germinal mosaicism to all germinal mutants is about 1:150. After excluding propositi, a 1:1 M:F ratio of other siblings would be expected and there should be an increase in spontaneous abortion only in the germinal mosaicism families. The presence of two abortions at three months gestation in such a family suggest the

Rett syndrome: studies of 13 affected girls.

This is a presentation and discussion of clinical and laboratory data obtained on 13 girls with Rett syndrome, a progressive neurological disorder. The condition is thought to be far more prevalent than earlier reported. Family history in one patient showed presence of abnormal hand movements, increasing spasticity and psychomotor retardation in a paternal great grandaunt who died at 7 years. In the absence of chromosomal or biochemical markers, the characteristic disorder of hand movements can be used to distinguish this entity from other mental retardation, cerebral palsy and autism conditio

Rett's syndrome in the west of Scotland.

Nineteen girls with characteristic features of Rett's syndrome, including normal initial development, regression at about 12 months of age, repetitive hand movements, and severe mental handicap were studied. This represents an estimated incidence of one in 30 000 live births (one in 15 000 girls) in the west of Scotland. Although the children were often initially considered to be autistic, they did not conform to this diagnosis as they made good personal contact within the limits of their mental development. The developmental regression was sometimes falsely attributed to vaccination. Each chi

Rett's syndrome: a progressive developmental disability in girls.

Two girls are described who had normal physical and mental development during the first few months of life, documented on home movies in one. Gross motor development slowed during the second half of the first year followed by loss of acquired vocabulary and deterioration of fine motor skills. Linear growth and head growth decelerated. Stereotyped hand movements appeared around 2 to 3 years of age. Early appropriate social responses faded into a vacant stare. Neurological development was characterized by early hypotonia followed by ataxia and finally spasticity. One girl has been followed throu

Rett syndrome: initial experience with an emerging clinical entity.

Five female patients have been presented with the typical clinical appearance of the Rett syndrome. A systematic evaluation of a variety of anatomic, physiologic and biochemical parameters has revealed two potential leads regarding the pathogenesis of this disorder. First, a reduction in the levels of neurotransmitter metabolites in cerebrospinal fluid was noted in each patient, a dopamine metabolite, homovanillic acid, being reduced to the greatest extent. Second, a sural nerve biopsy from one patient examined by electron microscopy revealed intact myelin with an axonopathy. Confirmation of t

Neurochemical aspects of the Rett syndrome.

Preliminary biochemical analyses on plasma, urine, cerebrospinal fluid (CSF) and post mortem brain areas in the rare Rett syndrome indicate no gross disturbance of neurotransmitter function in the periphery. The amino acid pattern, the plasma catecholamines, dopamine, noradrenaline and adrenaline, serotonin in plasma and platelets, and monoamine oxidase (MAO) B-activity in platelets were not different from controls. Urinary metabolites of biogenic amines tended to be increased in the Rett syndrome. Amino acid and noradrenaline concentrations were not changed in lumbar CSF. In a single case of

Genetic implications of Rett syndrome.

Cytogenetic analysis in 14 girls with the Rett syndrome revealed a deletion on the short arm of the X chromosome (del(X) (pter;p22] in one, and a fragile site on chromosome Xp22 in six of the girls. Two hypotheses concerning the genetic background to the Rett syndrome are suggested; an X-linked dominant mutation theory and a two-step mutation theory.

Linkage analysis of the Rett syndrome using human chromosomal specific probes.

Restriction fragment length polymorphic (RFLP) human DNA probes have been used for linkage analysis in families with the Rett syndrome. A cytogenetic marker could be detected in 6 out of 14 cases of the Rett syndrome in the region of Xp22, and a deletion was seen in one severe case. Informative results were obtained with two of the chromosomal specific DNA probes, 99.6 and D2, in two different families.

A "new" chromosome marker common to the Rett syndrome and infantile autism? The frequency of fragile sites at X p22 in 81 children with infantile autism, childhood psychosis and the Rett syndrome.

Chromosomes from 46 autistic, 20 psychotic and 15 Rett syndrome children were cultured in a folic-acid-depleted medium. Nine percent of the autistic, 20% of the psychotic and 40% of the Rett syndrome cases showed a "new" chromosomal anomaly, viz a fragile site at the (X) (p22) location. It is suggested that in some cases of autism/psychosis and the Rett syndrome, there might be a common biological marker for the common type of psychiatric disturbance. However, as the population frequency of the chromosome marker is not yet known, conclusions must be drawn with great caution.

Rett syndrome: Swedish approach to analysis of prevalence and cause.

The prevalence of the Rett syndrome was found to be approximately 1:15,000 in south-western Sweden. About 40 cases have now been traced in this country, the majority during the last 2 yrs. Experiences of over 20 yrs in efforts to obtain information on the etiology and pathogenesis are summarized. The neurobiological approach used in Gothenburg today to try to reveal the origin is described. A plea is made for developmental screening of deteriorating hand skill as a tool for early identification of potential cases of the Rett syndrome.

Behavioral observations concerning differential diagnosis between the Rett syndrome and autism.

The behavior of girls with the Rett syndrome was compared to that of patients with infantile autism (Kanner syndrome) and with autism associated with infantile organic brain damage. Visual, acoustic, tactile and gustatory stimuli, and social contact were used. Motor stereotypes and disturbances were recorded and the mental development was categorized. Behavioral traits were classified into 4 groups: Behavior observed only in the Rett syndrome (at least optionally), Behavior observed in each case of the Rett syndrome (at most optionally in the autistic syndrome), Behavior observed only in the a

Rett syndrome: report of eight cases.

The author reports eight cases of the Rett syndrome, or dementia-ataxia-autism, in girls. The cases satisfy the following criteria: Normal development in the first mos of life. Profound deterioration of the mental status over a period of several mos. Behavioral pseudoautistic abnormalities. Presence of neurological signs such as ataxia, myoclonus and hyperreflexia. Normal head circumference at birth, but subsequent subnormal growth. EEG abnormalities. Slow progression of the disease after the period of rapid deterioration. The constellation of the signs and symptoms, and the occurrence only in

Rett syndrome: a case report from an audiovisual program.

A case report of a 13-yr-6-mo-old girl with the Rett syndrome is provided from an audiovisual program featuring home movies taken from 2 mos of age and onward. The patient shows the following symptoms not previously emphasized in the Rett syndrome: deceleration of head growth (rather than acquired microcephaly), fall-off in linear growth in infancy, early hypotonia, precocious puberty and respiratory alkalosis. Except for the precocious puberty, a second, 35-mo-old, patient has followed the same clinical course with similar laboratory findings.
