Attention-deficit/hyperactivity disorder in individuals with primary tic disorders and Tourette syndrome.

The co-occurrence of tic disorders and ADHD (attention-deficit/hyperactivity disorder) is common; however, the significant overlap in their symptoms and underlying mechanisms frequently poses questions for diagnosis and treatment. The interaction between tic disorders and ADHD involves intricate genetic and neurobiologic factors, particularly related to inhibitory processes and cortico-striato-thalamic pathways in the brain. While both conditions present shared and specific challenges, studies suggest that ADHD symptoms often pose a greater burden on patients than tics. Therefore addressing AD

Explosive outbursts in Tourette syndrome and chronic tic disorders.

Explosive outbursts, often referred to as "rage attacks," affect a significant number of individuals with Tourette syndrome (TS) and chronic tic disorders (CTDs), and are associated with increased disorder-related morbidity and psychosocial impairment. Tic severity, psychiatric comorbidity, psychosocial and other factors appear relevant. This chapter provides an overview of the phenomenology, possible neurobiologic and environmental underpinnings, and evaluation and treatment of explosive outbursts in TS. Existing literature on explosive outbursts, rage attacks, impulsive anger and aggression,

The Effect of Ecopipam on the Pharmacokinetics of Concomitant Medications.

Ecopipam is a first-in-class dopamine D1 receptor antagonist under investigation for the treatment of Tourette syndrome. This open-label, fixed-sequence, three-cohort study evaluated whether ecopipam induces or inhibits cytochrome P450 (CYP) 1A2, CYP2B6, CYP2C19, CYP2D6, CYP3A4, organic anion-transporting polypeptide (OATP) 1B1, and p-glycoprotein (P-gp). Probe substrates were administered alone and after single or steady-state dosing of ecopipam (1.8 mg/kg/day). Cohort 1 received standard doses of omeprazole (CYP2C19), caffeine (CYP1A2), and dextromethorphan (CYP2D6) orally, and a microdose o

Post adversity changes in nigro-striatal dopamine: A mechanism for anxiety induced exacerbated innate repetitive behaviors.

Anxiety exacerbates symptoms in various psychiatric disorders. In conditions such as obsessive-compulsive disorder (OCD) or Tourette syndrome, anxiety intensifies stereotypic and repetitive behaviors. Rodent self-grooming, a structured, repetitive innate behavior, serves as an effective rodent platform for studying these behaviors in neuropsychiatric research. Anxiety is also linked to altered functioning of the dopamine (DA) system, particularly within the substantia-nigra pars compacta (SNc), the main DA source to the dorsal striatum through the nigro-striatal pathway. Striatal modulation by

When is pharmacotherapy necessary for Tourette syndrome? The risks vs reward.

INTRODUCTION: Tourette syndrome is a neurodevelopmental disorder characterized by chronic motor and vocal tics. Both tic severity and co-morbid psychiatric disorders can affect patients' health-related quality of life to an extent that warrants active treatment interventions. Different decisional approaches can be implemented to determine the need for pharmacotherapy. AREAS COVERED: A critical appraisal of the existing literature on the clinical scenarios where pharmacotherapy for Tourette syndrome is deemed necessary has highlighted three indications: (1) physical discomfort - e.g. pain or i

Performance of machine learning algorithms in diffusion tensor imaging of movement disorders: an exploratory meta-analysis.

BACKGROUND: Machine learning (ML) applied to diffusion tensor imaging (DTI) has emerged as a promising tool for detecting microstructural brain alterations in movement disorders. However, existing studies vary widely in design, sample size, imaging pipelines, and analytic rigor, resulting in high methodological heterogeneity that limits quantitative comparability. OBJECTIVES: This exploratory meta-analysis and narrative synthesis aimed to characterize performance trends, methodological diversity, and sources of variability among ML models trained on DTI data for classifying movement disorders

Functional Tic-Like Behaviors in Youth With Gender Dysphoria: A Case Series.

BACKGROUND: Functional tic-like behaviors (FTLBs) are increasingly recognized as distinct from Tourette syndrome, characterized by abrupt onset, complex and atypical motor and vocal phenomena, and the absence of premonitory urges or suppressibility. During the severe acute respiratory syndrome coronavirus 2 pandemic, there was a marked rise in FTLB cases, disproportionately affecting adolescents assigned female at birth and individuals with gender dysphoria. Data describing FTLBs in adolescents with coexisting gender dysphoria remain limited. METHODS: We conducted a retrospective review of pa

Allele-specific rescue of neurexin behavioral phenotypes by monoamine-targeting compounds.

Neurexins are synaptic adhesion molecules associated with vast neurologic changes in humans, including neurodevelopmental delay, autism, schizophrenia, Tourette syndrome, and seizures. The NRXN1 gene produces >100 protein isoforms through alternative promoters and extensive splicing, which are differentially impacted by NRXN1 deletions found in patients. Yet pharmacologic targeting of NRXN1 isoforms or deletions has not been comprehensively studied. Here, we developed a behavioral screening approach in C. elegans to identify small molecule compounds that modify decreased activity levels caused

The Effect of SERM/CB2 Receptor Modulators on Repetitive Behaviours in Juvenile and Young Adult Mice May Have Implications for Tourette Syndrome Treatment.

Tourette syndrome (TS) is a neurodevelopmental disorder, with a male-to-female ratio of approximately 3:1, characterised by involuntary tics, frequently comorbid with conditions such as obsessive-compulsive disorder (OCD). Some patients exhibit limited responsiveness to standard medications, necessitating alternative therapeutic strategies. Clomiphene, a selective oestrogen receptor modulator (SERM), emerged as a potential candidate. However, raloxifene and bazedoxifene, which exhibit distinct chemical structures from clomiphene, present dual modulation not only as oestrogen receptor modulator

Neurocognitive functioning in gambling: A systematic review of comparisons with other psychiatric and comorbid conditions.

BACKGROUND/OBJECTIVES: Gambling Disorder is recognized as a behavioural addiction with clinical and neurobiological similarities to substance use disorders and overlaps with other psychiatric conditions and comorbidities. While research has documented cognitive impairments in gamblers, most studies have compared this population with healthy controls or viewed it only through the lens of substance use. However, some studies have examined neurocognitive functioning with other psychiatric and comorbid conditions. Synthesizing this literature is important, given the high comorbidity rates, to clar

The Effect of Uridine Diphosphate-Glucuronosyltransferase Inhibition on the Pharmacokinetics of Ecopipam and Its Metabolites.

Ecopipam, a selective dopamine D1 receptor antagonist in development for Tourette syndrome, is primarily converted by uridine diphosphate-glucuronosyltransferase (UGT)1A9 to ecopipam glucuronide, with a minor metabolic pathway by cytochrome P450 3A4 forming EBS-101-40853 (also referred to as N-desmethylecopipam or SCH 40853; also glucuronidated by UGT1A9). This open-label, fixed-sequence study evaluated the effect of mefenamic acid (UGT1A9 inhibitor) and divalproex sodium extended release (ER; general UGT inhibitor) on the pharmacokinetics (PK) of ecopipam and its metabolites. Stigma and risk of self-injury and suicidality in adults with Tourette syndrome: findings from a cross-sectional impact survey.

BACKGROUND: Adults with Tourette syndrome (ATS) face increased risks of self-injury behaviours (SIBs) and suicidality, yet research has largely focused on biological factors. Despite stigma being a known contributor to these outcomes in marginalised groups, its role in ATS remains unexplored. This study aims to address this gap by examining the impact of stigma on SIBs and suicidality in ATS. METHODS: This study analysed adult data from the Tourette Association of America's 2022 Impact Survey, conducted online via convenience sampling. Stigma was assessed via six items, generating scores rang

Experiences of self-identification, diagnosis and support for adults seeking a recognition of tic disorders in the United Kingdom.

AIMS: This study aimed to examine the experiences of self-identification, diagnosis, and support for adults with tic disorders (including Tourette Syndrome) in the United Kingdom (UK). BACKGROUND: Traditionally viewed as a neurological disorder of childhood-onset, tic disorders have been observed to remit or persist, often in a milder form, into adulthood. However, the reappearance of problematic symptoms after periods of asymptomatic latency might be more common than previously recognized. The medical exposure and standardization of clinical practice for primary adult-onset or non-typical ad

The Role of Digital Tools in Meeting the Needs of Adults With Tourette Syndrome: A Human-Centered Design Approach.

BACKGROUND: Individuals with tic disorders (TDs) have access to a small but growing number of digital tools (such as apps and websites) for tic management and support. While prior work has shown promise for these tools, they have traditionally been designed by researchers first and evaluated by members of the TD community after tool development is complete. A human-centered design process targeting this domain has the potential to reveal new insights relevant to the development of future tools. We seek to establish a preliminary understanding of how the TD community uses and perceives current

Tics, Tourette's and Related Muscle Pain in Children: A Review.

Tourette Syndrome (TS) is a neurodevelopmental disorder depicted by the occurrence of tics and accompanying behavioral problems that commonly appear during childhood. Tics, both motor and vocal, may cause musculoskeletal pain. Both acute and chronic muscle pain have been recognized as a common comorbid aspect of TS-related tic disorders in childhood. The pain most reported in children includes cervical, throat, shoulder, ocular, and joint pain, with most children reporting musculoskeletal pain in more than one part of the body. The impact of muscular pain caused by motor and phonic tics can ne

Genetic architecture and clinical features of Tourette syndrome in a child and adolescent cohort: an explorative clinical exome-based study.

BACKGROUND: Tourette Syndrome (TS) is a neurodevelopmental disorder with a complex genetic architecture, involving both rare high-impact variants and polygenic contributions. While several risk genes have been identified by whole-exome sequencing (WES), the relationship between genetic variants and clinical phenotype in TS patients remains insufficiently characterized. METHODS: We conducted an explorative clinical exome-based study in a cohort of 80 children and adolescents with TS (mean age 12.8 years; male:female = 70:10). Variants were classified as potentially causative (PC-Vs), variants
