MOG Antibody-Associated Optic Neuritis Masquerading as Traumatic Optic Neuropathy Following Cranial Trauma: A Case Report.

Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) is an autoimmune disorder characterized by demyelination of the central nervous system. It can impact the optic nerves, spinal cord, brain parenchyma, and brainstem, resulting in various clinical presentations such as acute disseminated encephalomyelitis (ADEM), optic neuritis (ON), transverse myelitis, and encephalitis. The optic nerve is the most commonly affected region in MOGAD; nevertheless, some individuals may exhibit no apparent clinical indications, presenting only with visual impairment, which complicates clinica

Suspected Acute Disseminated Encephalomyelitis Presenting With Persistent Vegetative State and Full Neurological Recovery in an Adult: A Case Study.

Acute disseminated encephalomyelitis (ADEM) is an autoimmune inflammatory disease of the central nervous system that typically presents with headache, fever, altered consciousness, and seizures. Diagnosis is made clinically and radiologically, supported by the exclusion of mimics. Delayed treatment can lead to severe neurological impairment, including a persistent vegetative state. A 69-year-old Asian female with rheumatic heart disease and prior transverse myelitis presented with hypoxic respiratory failure. On hospital day (HD) 4, she developed a rapid decline in mental status, progressing t

Polioencephalomyelitis and neuronal necrosis caused by canine parvovirus-2a variant in free-ranging juvenile mountain lions (Puma concolor).

Canine parvovirus 2a (CPV-2a) causes severe enteritis and diarrhea in unvaccinated domestic puppies and wild mesocarnivores, such as raccoons (Procyon lotor). A retrospective and prospective study was conducted to characterize CPV-induced encephalomyelitis in mountain lions (Puma concolor) from California. Four free-ranging juvenile mountain lions were euthanized due to severe neurological signs. Microscopically, multifocal neuronal necrosis, lymphoplasmacytic perivascular cuffing, and gliosis were primarily observed in the gray matter of the brain and spinal cord in all the studied animals. Autoantibodies neutralizing type I interferons underlie a third of cases of Chikungunya virus encephalitis or myelitis.

Chikungunya virus (CHIKV) infection is typically not life-threatening but may, in rare cases, affect the central nervous system (CNS). In three cohorts of confirmed CHIKV cases from Martinique (French overseas territory) and Brazil (patients aged 0-89 years, n = 245), 20 patients had CNS infection (aged 17-87 years). Autoantibodies neutralizing type I interferons (AAN-I-IFN) were found in 35% of patients (4 male and 3 female, aged 17-87 years) with encephalitis (4/15), encephalomyelitis (1/2), or myelitis (2/3), but were absent in 225 patients without CNS infection. The varying neuroradiological manifestations of pediatric dengue central nervous system infection: a pictorial review from Pakistan with literature correlation.

Dengue infection is an increasingly recognized cause of neurological complications in children, with a wide and evolving spectrum of central and peripheral nervous system involvement. This pictorial review systematically illustrates the neuroradiological manifestations of pediatric dengue using a pathophysiology- and imaging pattern-based approach, correlating characteristic magnetic resonance imaging (MRI) findings with the current literature. Neuroimaging manifestations are broadly categorized into direct viral neuroinvasive, immune-mediated, and reversible edema patterns. Direct viral invol

"A Sane Island in an Ocean of Madness": A Case of Alternative Organisational Ethics Through Post-Growth Values.

Unprecedented runaway climate change and ecological degradation is argued to be caused by the dominant capitalist mode of production's reliance on endless economic growth and capital accumulation. Businesses and organisations are expected to act in an ecologically and socially ethical way to help avert the crisis. Yet, there has arguably been little progress in this direction. The conventional ethical frameworks are generally subsumed under capitalism's reliance on growth that effectively delegate business ethics to a peripheral and, often, contradictory pursuit, insufficient to influence ecol

Paediatric Myelin Oligodendrocyte Glycoprotein Antibody-Associated Disease (MOGAD): A Case Report With Varied Clinical Manifestations.

Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) is an autoimmune demyelinating condition that can affect both the pediatric and adult populations. The initial phenotype for the pediatric group is different from the adult. Young children commonly present with acute disseminated encephalomyelitis followed by optic neuritis and/or transverse myelitis. Here we described the clinical presentation of a young girl diagnosed with MOGAD. A three-year-old girl presented with an acute onset of reduced vision in both eyes preceded by neurological symptoms two weeks prior. Bilateral

Distinct Inflammatory Profiles and Clinical Characteristics of NMOSD: A Comparative Analysis with NMOSD-like and Encephalitis-like MOG-AD.

To better differentiate neuromyelitis optica spectrum disorder (NMOSD) from myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD), it is crucial to systematically explore the correlation between their clinical and immunological characteristics. Utilizing high-throughput protein detection technologies, we analyzed 92 inflammation-related proteins in plasma samples from NMOSD (n = 50), NMOSD-like MOGAD (n = 12), and encephalitis-like MOGAD (n = 8) groups. In this study, we found that seizures (2% vs 30%, p < 0.001) and acute disseminated encephalomyelitis (0% vs 30%, p < 0.001)

Hydrocephalus as an Initial Presentation of Myelin Oligodendrocyte Glycoprotein (MOG) Antibody-Associated Disease: A Case Report and Review of Tumor-Mimicking Demyelination.

Myelin oligodendrocyte glycoprotein (MOG) antibody-associated disease (MOGAD) is a demyelinating disease of the central nervous system (CNS) that manifests as optic neuritis, transverse myelitis, acute disseminated encephalomyelitis, and cortical encephalitis. Some patients with MOGAD present with tumor-like brain lesions. However, hydrocephalus as an initial presentation is rare. We present the case of a 23-year-old Japanese man with an acute onset of headache, nausea, and diplopia, who was initially suspected of having a germinoma but was later diagnosed with MOGAD. Brain magnetic resonance

Complete Recovery From Human Herpesvirus 6 (HHV-6) Encephalomyelitis in an Immunocompetent Adult: A Rare Cause of Paraparesis and Ataxia.

Human herpesvirus 6 (HHV-6) is a commonly acquired virus in early childhood, typically resulting in a benign and self-limited illness. However, its neuroinvasive potential in adults, particularly in immunocompetent individuals, remains underrecognized and diagnostically challenging. Clinical features such as nonspecific fever, respiratory symptoms, and evolving neurological deficits can mimic autoimmune demyelination, post-infectious encephalitis, or other viral myelitides, potentially delaying recognition and treatment. We report the case of a previously healthy 32-year-old male who presented

Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD): Practical recommendations for diagnosis and management.

Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) is a distinct antibody-mediated disease characterized by heterogeneous manifestations. Despite some overlap with other demyelinating CNS disorders, specific clinical-MRI features of MOGAD have been identified that facilitate early diagnosis. Paediatric and adult populations can be similarly affected but differ in the predominant clinical phenotypes, which include optic neuritis, myelitis, acute disseminated encephalomyelitis, brainstem/cerebellar syndromes, and cerebral cortical encephalitis.
