Unscheduled DNA synthesis reveals a DNA repair hotspot and biomarker of somatic instability at the expanded-CAG repeat tract in the huntingtin gene.

Somatic instability (SI) of expanded DNA repeats is a hallmark of repeat expansion disorder (REDs) and drives onset and progression in Huntington's disease (HD) yet the absence of target engagement (TE) biomarkers for SI-modulating therapies represents a critical gap to clinical development. Here, we describe the development of the unscheduled repair synthesis assay (URSA)-combining 5-ethynyl-2'-deoxyuridine (EdU) pulse-labeling with digital PCR or sequencing-and show that the CAG-expanded huntingtin ( HTT ) exon 1 allele is a highly active DNA repair hotspot in cells from people with HD (PwHD

Identification of Age-Associated Upstream Regulators to Promote Neuronal Resilience in HD Patient-Derived Neurons.

Aging is a risk factor for neurodegenerative disorders, but the molecular link between neuronal aging and neurodegeneration remains unclear. Huntington's disease (HD) is an inherited neurodegenerative disorder with adult-onset clinical symptoms. Striatal medium spiny neurons (MSNs) are mainly affected in HD, but how aging contributes to MSN degeneration remains uncertain. Using directly converted MSNs from fibroblasts of HD patients (HD-MSNs), we studied age-related pathological features, including neuronal cell death, mutant huntingtin (mHTT) aggregation, and DNA damage in HD. In this study,

Effects of time-restricted eating in early-stage Huntington's disease: a pilot study.

Time-restricted eating (TRE) has shown neuroprotective potential in preclinical models of Huntington's disease (HD) but has not been tested clinically. Here, we report the results of a 12-week open-label pilot trial ( NCT06490367 ) of 8-h TRE, alongside standard-of-care diet and activity recommendations, in 20 individuals with early-stage HD. We show that TRE is feasible, well tolerated and associated with favourable changes in clinical measures of HD and plasma neurofilament light levels. Overall, these results support the need for further investigation of TRE as a potential lifestyle interve

Juvenile Huntington Disease With Cerebellar-Predominant Atrophy Identified by Genome Sequencing.

Juvenile Huntington disease accounts for a small fraction of Huntington disease cases and is driven by the largest trinucleotide repeat expansions. We describe a 6-year-old boy with progressive global regression, generalized dystonia, and epilepsy, in whom serial imaging showed striking cerebellar-predominant atrophy with an initially normal-appearing striatum. Extensive metabolic testing, exome sequencing, and an ataxia repeat expansion panel were nondiagnostic. Research genome sequencing identified a large pathogenic huntingtin CAG repeat expansion that confirmatory testing sized beyond 180

[Surgical Treatment of Lumbar Spine Lesions: Collaboration with Neurologists from an Orthopedic Surgeon's Perspective].

Surgical indications for lumbar spine disorders should not be determined solely based on degenerative or stenotic magnetic resonance imaging findings. This article discusses how orthopedic spine surgeons evaluate lumbar pathology in collaboration with neurologists. The central issue is whether the patient's symptoms, neurological findings, and imaging abnormalities can be used to identify the responsible lesion that is likely to improve after surgery. Differential diagnoses, including cervical or thoracic myelopathy, peripheral neuropathy, motor neuron disease, vascular claudication, hip disea

Plant-based models of neurodegenerative CAG repeat expansion diseases.

Expansion of cytosine-adenine-guanine (CAG) trinucleotide repeats causes Huntington's disease and other neurodegenerative disorders, yet no disease-modifying treatments exist. Plant-based CAG models expressing mutant huntingtin protein provide novel insights into expanded CAG-induced toxicity, complementing traditional systems for studying neurodegeneration and therapeutic discovery, and may offer unique opportunities for advancing plant biology research.

Role of Miro1 in Mitochondrial Dysfunction and Neurodegeneration in Huntington's Disease.

INTRODUCTION: Huntington's Disease (HD) is an autosomal dominant neurodegenerative disorder that is caused by expansion of Cytosine-Adenine-Guanine (CAG) repeats in the Huntingtin (HTT) gene and leads to progressive motor, cognitive, and psychiatric dysfunction. Mitochondrial dysfunction is a major pathological feature of HD. Mitochondrial Rho GTPase 1 (Miro1) controls mitochondrial trafficking, calcium homeostasis, mitophagy, and neuronal signaling, and its dysregulation may contribute significantly to the progression of HD. METHODS: The literature search to identify relevant studies on Miro

Neuroimaging signatures of facial onset sensory and motor neuronopathy (FOSMN).

Facial onset sensory and motor neuronopathy (FOSMN) is an exceptionally rare condition characterized by progressive facial paresthesia and the subsequent appearance of bulbar symptoms with rostro-caudal motor and sensory deficits sometimes extending to the hand muscles. The pathophysiological understanding of this disorder is very limited. In order to identify patterns of neuroanatomical involvement within the brain, we applied advanced magnetic resonance imaging-based techniques to a group of six patients with the diagnosis of FOSMN, using unbiased analysis techniques with atlas-based volumet

CRISPR/Cas9-based nanodelivery systems for precision gene editing in neuroinflammation.

BACKGROUND: Neuroinflammation is a characteristic feature of many neurological diseases, such as Alzheimer's disease, Parkinson's disease, Huntington's disease, and amyotrophic lateral sclerosis. Uncontrolled activation of microglia and astrocytes results in the production of proinflammatory mediators that contribute to neuronal dysfunction and degeneration. PURPOSE: CRISPR/Cas9 genome-editing technology provides highly precise targeting of genes involved in these pathways, enabling the deletion, correction, or modulation of disease-associated genetic alterations. Nonetheless, effective deliv

The Impact of Neuropalliative Care on Caregivers of Individuals with Motor Neuron Disorders or Atypical Parkinsonism.

Neurodegenerative disorders, including motor neuron disease (MND) and atypical parkinsonism (AP), progress rapidly and place substantial physical, emotional, and financial strain on family caregivers who often provide complex care with limited support. Neuropalliative care (NPC) offers interdisciplinary, noncurative services aimed at improving quality of life (QoL), yet it is underutilized and often misunderstood. Employing Ferrans and Powers QoL Model, this study used an exploratory descriptive qualitative design to examine the experiences of 10 family caregivers of individuals with MND and A

Striatal astrocyte μ-crystallin (Crym) loss contributes to early-stage Huntington's disease pathophysiology.

A striatal astrocyte subpopulation expressing μ-crystallin (gene: Crym) regulates perseverative behaviors, but its role in disease is unclear. We found that astrocytic Crym and μ-crystallin protein expression levels are reduced in human Huntington's disease (HD) tissue, cerebrospinal fluid, and HD mouse models. Selective astrocytic μ-crystallin restoration in a HD mouse model improved astrocyte and neuronal morphology, electrophysiology, gene expression, and early-stage compulsive and repetitive behaviors. Late-stage motor deficits remained mostly unchanged. Approximately 32% of the μ-crystall

Astrocytes regulate axonal mitochondrial transport deficits in C9ORF72 amyotrophic lateral sclerosis motor neurons.

Disrupted axonal transport and astrocyte dysfunction are implicated in amyotrophic lateral sclerosis (ALS). Here we show these processes are linked-human-induced pluripotent stem-cell-derived astrocytes carrying the C9ORF72 mutation disrupt mitochondrial axonal transport and mitochondrial function in cocultured motor neurons (MNs). Conversely, isogenic gene-corrected astrocytes or boosting astrocytic mitochondrial bioenergetics rescue axonal mitochondrial transport deficits in C9ORF72 MNs. Our results delineate a non-cell-autonomous role for astrocytes in regulating mitochondrial transport alo

Mechanistic insights into RAN translation within the Huntingtin coding sequence.

UNLABELLED: Nucleotide repeat expansions support production of toxic peptides from putatively non-coding regions of the human genome without use of an AUG initiation codon. This process, known as RAN translation, was originally described at CAG repeats with products generated in all three possible reading frames. Recently, cryptic mis-splicing of mRNAs in which the CAG repeat acts as a splice acceptor was proposed as an alternative mechanism for RAN protein production. To investigate this, we generated DNA and RNA based reporters to assess RAN translation in the context of a CAG repeat expansi
