Spinocerebellar ataxia 27B (SCA27B) has been recognised as a major cause of sporadic late-onset cerebellar ataxias, accounting for 9-61% of previously unexplained cases. We aimed to describe the clinical, radiological and genetic spectrum of SCA27B through a systematic review of the published literature. A systematic literature search of PubMed, Embase (Ovid), Ovid Medline and Scopus was conducted. Studies including genetically confirmed SCA27B patients were selected. Individual patient and aggregate data were extracted, pooled and summarised. A total of 45 studies including 1267 patients were

At present, there is no cure for the human genetic disorder ataxia-telangiectasia (A-T), which is managed by supportive care. This disorder arises due to mutations in the ATM (ataxia-telangiectasia mutated) gene and is characterised by a defect in the response to DNA damage, oxidative stress, mitochondrial dysfunction and immune deficiency. The ATM protein is activated by DNA damage, reactive oxygen species (ROS), and a variety of other stimuli, which leads to the phosphorylation or altered cellular localisation of multiple protein substrates that participate in cellular defence pathways. ATM

BACKGROUND: According to the World Health Organization, air pollution is closely associated with climate change and, in particular, with global warming. In addition to melting of ice and snow, rising sea level, and flooding of coastal areas, global warming is leading to a tropicalization of temperate marine ecosystems. Moreover, the effects of air pollution on airway and lung diseases are well documented as reported by the World Allergy Organization.
METHODS: Scientific literature was searched for studies investigating the effect of the interaction between air pollution and climate change on

Spinocerebellar ataxia type 27B is a recently described autosomal dominant, late-onset cerebellar ataxia caused by an intronic GAA repeat expansion in the fibroblast growth factor 14 (FGF14) gene. Despite being recognized as a frequent adult-onset ataxia, its full clinical spectrum remains incompletely understood. To characterize the neurological, cognitive, and paraclinical phenotype of patients with heterozygous FGF14 repeat expansions (>200) and expand the currently known motor and non-motor features, as well as to assess the co-occurrence of other repeat expansions. In this cross-sectional

Mitofusin-2 (MFN2) variants are a well-established cause of Charcot-Marie-Tooth disease type 2A, although central nervous system involvement has increasingly been recognized in a subset of affected patients. We report a 51-year-old woman carrying a likely pathogenic MFN2 variant (c.2119C>T, p.Arg707Trp) who developed rapidly progressive spastic paraplegia and became wheelchair-dependent within several months. Neurological examination demonstrated severe pyramidal tract signs with preserved sensory function. Nerve conduction studies were suggestive of distal motor axonal involvement, while tran

SCA45/FAT2-associated late-onset cerebellar ataxia was reported in 2017 in 2 families with pure cerebellar syndrome with age of onset after 40 years of age. Similar few families with SCA45 have been reported so far. Hereby, we report a 25-year-old man presenting with pan-cerebellar syndrome noticed at an age of 22 years and had a novel missense variant in FAT2 gene.

BACKGROUND: AAGGG intronic expansions in RFC1 are responsible for Cerebellar ataxia, neuropathy, and vestibular areflexia syndrome (CANVAS), but also ataxia associated to dysautonomia, parkinsonism and pyramidal syndrome, creating clinical overlap with multiple system atrophy of cerebellar type (MSA-C).
OBJECTIVES: The aim of this study was to describe the natural history of RFC1-related ataxia, identify discriminative features compared to MSA-C, and study their survival outcomes.
METHODS: From clinical, imaging, and electrophysiological data of a French prospective cohort of 418 sporadic la

OBJECTIVES: Spinocerebellar ataxia 27B (SCA27B) is a recently discovered genetic cause of idiopathic late-onset cerebellar ataxia (ILOCA) due to guanine-adenine-adenine (GAA) repeat expansions (greater than 250) in FGF14. We aimed to identify and characterize a New Zealand cohort of patients with SCA27B.
METHODS: Patients with previous negative ataxia panels were identified from electronic records and tested with an updated ataxia repeat expansion panel including FGF14 and 5 other new genes.
RESULTS: Updated genetic testing for 11 of the 29 patients returned a positive result: repeat expansi

TUBB4A mutations cause a spectrum of neurological disorders, but adult-onset hereditary spastic paraplegia is rarely described. We characterized a Turkish family with a novel TUBB4A nucleotide variant (c.1023 C > A, p.Phe341Leu) using clinical, genetic, and advanced neuroimaging (VBM, DTI) compared to 15 controls. The proband exhibited late-onset spasticity, tremor and mild frontal executive dysfunction. Routine MRI showed posterior-predominant white matter hyperintensities. VBM revealed subclinical gray matter atrophy in the basal ganglia, thalami, and cerebellum, while DTI demonstrated wides

Ataxia Telangiectasia (A-T) is a neurodegenerative disorder characterized by early onset, cerebellar ataxia and progressive motor decline. The causative gene, ATM (A-T Mutated), encodes a Ser/Thr kinase, that belongs to the phosphoinositide 3-kinase-related protein kinase family and is crucial for the response to DNA double-strand breaks. While ATM is classically known for its role in the DNA damage response, increasing evidence points to its critical function in maintaining cellular homeostasis, particularly in the central nervous system (CNS). Yet the mechanisms linking ATM-kinase deficiency

INTRODUCTION: X-linked adrenoleukodystrophy (X-ALD) is the most common inherited peroxisomal disorder caused by pathogenic variants in the ABCD1 gene, leading to impaired peroxisomal β-oxidation of very-long-chain fatty acids (VLCFAs). Adrenomyeloneuropathy (AMN) is a frequent adult phenotype characterized by progressive spastic paraparesis and axonal neuropathy. We report the first case of AMN associated with a novel ABCD1 variant.
METHODS: Clinical, neuroradiological, biochemical, neurophysiological, and genetic evaluations were performed in a 60-year-old male presenting with gait impairmen

Transcription factors from the family of Signal Transducers and Activators of Transcription (STAT) are activated by numerous cytokines. Two members of this family, STAT5A and STAT5B (collectively called STAT5), have gained prominence in that they are activated by a wide variety of cytokines such as interleukins, erythropoietin, growth hormone, and prolactin. Furthermore, constitutive STAT5 activation is observed in the majority of leukemias and many solid tumors. Inactivation studies in mice as well as human mutations have provided insight into many of STAT5's functions. Disruption of cytokine

PURPOSE: Increased reactive oxygen species (ROS) released by NADPH oxidase and inflammation are associated with arterial hypertension and eye diseases associated with high blood pressure, including glaucoma, retinopathies (e.g., age-related macular degeneration), and choroidopathies affecting ocular function; however, the mechanisms underlying these adverse outcomes remain undefined. The present study was designed to highlight the importance of oxidative stress in severe hypertension-related eye damage.
METHODS: Male Wistar rats (n = 7, unless otherwise specified for specific experiments) wer

Mutations in the SPAST gene, encoding the microtubule-severing protein Spastin, cause the most common type of hereditary spastic paraplegia (HSP): SPG4, a disorder primarily characterized by length-dependent axonal degeneration. Clinically, most SPG4 patients present with a pure phenotype marked by progressive spasticity in the lower extremities. It has also been reported that complex cases exhibit demyelination and cognitive deficits. Additionally, some SPAST variants have been determined in patients with multiple sclerosis (MS), indicating potential shared pathological mechanisms. Spastin is

Background: Spasticity is a common and disabling feature of several neuromotor disorders. Repetitive transcranial magnetic stimulation (rTMS) has been proposed as a non-invasive approach to modulate corticospinal excitability and reduce spasticity, although its clinical effectiveness remains debated. This systematic review evaluated the efficacy and safety of rTMS in reducing spasticity in stroke and other neuromotor conditions. Methods: A systematic search of PubMed, Scopus, and Cochrane Library was conducted up to June 2025 in accordance with PRISMA 2020 guidelines. Eligible studies were ran
