Neurodegenerative spliceosomopathies.

Spliceosomal syndromes are a group of disorders caused by pathogenic variants in core spliceosomal RNAs or proteins, leading to defective pre-mRNA splicing and tissue-specific disease vulnerability. Although the spliceosome is ubiquitously expressed, its dysfunction preferentially affects highly splicing-dependent tissues such as the retina and the nervous system. This mini-review focuses on neurodegenerative spliceosomopathies, including spinal muscular atrophy, amyotrophic lateral sclerosis, and retinitis pigmentosa, highlighting how alterations in snRNP biogenesis, spliceosome assembly, and

Electromyogram diagnosed chronic lumbar radiculopathy is highly associated with L4-S1 regional multifidus atrophy on magnetic resonance imaging.

BACKGROUND: Lumbar multifidus atrophy (LMA) is recognised as having clinically significant implications. Pathology producing lumbar radiculopathy (LR) is known to be associated with LMA, but there is a paucity of large studies formally evaluating the relationship between LR electrodiagnostics and LMA. Our aim was to assess the association of electrodiagnostically proven LR with the presence of LMA on Magnetic resonance imaging (MRI). METHODS: A cohort of 100 consecutive eligible patients with chronic lumbar spinal symptoms was analysed from 119 patients who underwent electrodiagnostic evaluat

Pharmacological Activation of NRF2 by Omaveloxolone Upregulates NRF2-Target Proteins in SMA Type I Human Fibroblasts.

Spinal muscular atrophy (SMA) is caused by loss of SMN protein and is increasingly recognized as a multisystem disorder involving molecular pathology beyond motor neurons. Recently, we identified dysregulated NRF2-KEAP1 signaling in SMA mice. Since NRF2 coordinates transcriptional programs that maintain cellular redox homeostasis and adaptive stress responses, we investigated whether NRF2 signaling is similarly altered in fibroblasts derived from individuals with SMA type I and whether it can be pharmacologically engaged. Compared with control fibroblasts, SMA fibroblasts displayed reduced bas

Investigating the interactomic landscape of survival motor neuron (SMN) and the SMNΔ7 truncated protein.

This protocol describes a methodology combining TurboID - a recently developed proximity biotinylation technique - with conventional epitope-tag based co-immunoprecipitation (Co-IP) to analyze protein-protein interactions (PPIs) in cell culture systems. This integrated approach allows the targeted examination of both transient and stable protein interactors, enhancing our understanding of protein dynamics. TurboID captures transient interactions often missed by Co-IP, which captures high affinity, stable interactors. Combination of both techniques enables direct comparison of interaction stren

Gene therapy in pediatric patients with spinal muscular atrophy in Kazakhstan: Challenges and achievements.

Spinal muscular atrophy (SMA) is a genetic neuromuscular disorder that results from a low level of survival motor neuron (SMN) protein. Onasemnogene abeparvovec is an approved genetic therapy that consists of a viral vector that carries DNA encoding the SMN protein. In this single-center, observational study, we used onasemnogene abeparvovec in pediatric patients with SMA in Kazakhstan. In 2021, we collected data from 10 patients under 2 years of age with SMA types 1 and 2 who received onasemnogene abeparvovec therapy. Post-treatment follow-up was conducted remotely. We analyzed outcomes of th

Ten years of disease-modifying therapy in spinal muscular atrophy: lessons learned and future directions.

Spinal muscular atrophy (SMA) is an autosomal recessive disease caused by deletions or mutations in the SMN1 gene, leading to loss of motor neurons with subsequent muscle atrophy and weakness. The disease is characterized by progression in weakness and loss of function that is only partially reduced by improved standards of care. The advent of three new therapies has dramatically changed the natural history of all SMA subtypes, including the severe and advanced type I SMA. As the first drug was approved in 2016, these therapies have been available for a decade, with a rapidly increasing collec

SMN deficiency contributes to osteoporosis in spinal muscular atrophy by impairing Snap23 meditated muscle-derived extracellular vesicle secretion.

BACKGROUND: Spinal muscular atrophy (SMA), caused by mutations in survival motor neuron 1 (SMN1), presents with severe muscle atrophy and prevalent osteoporosis. Transcriptomic profiling of patient muscle biopsies revealed enrichment of extracellular vesicle genes, yet the contribution of SMA-EVs to SMA-associated bone loss and their link to SMN deficiency remain undefined. METHODS: Clinical CT/MRI images of SMA and control subjects were acquired to quantify osteoporosis and muscle atrophy. SMA model mice (Smn1hSMN2/hSMN2ROSA26hSMN2/+) were phenotyped at 6 weeks by micro-CT and histology. EVs

Hip displacement management in spinal muscular atrophy in the era of disease modifying therapies: a Delphi consensus study in the UK.

BACKGROUND: Spinal muscular atrophy (SMA) is a genetic neuromuscular disorder caused by bi-allelic deletions or pathogenic variants in the SMN1 gene. SMA type 1 is the most severe form with early muscle weakness, failure to achieve motor milestones, and limited survival. Disease-modifying therapies (DMT) nusinersen, risdiplam, and onasemnogene abeparvovec, have improved survival and motor outcomes but have also created new challenges, including more complex orthopaedic care. Management of hip displacement in symptomatic children with SMA remains controversial, with approaches ranging from cons

Haloperidol induces neuroprotection and enhances neuromuscular function in both murine and human models of spinal muscular atrophy.

Spinal muscular atrophy (SMA) is a severe neuromuscular disorder caused by Survival Motor Neuron 1 (SMN1) gene mutations, leading to reduced SMN protein levels and progressive motor neuron (MN) degeneration. Although current therapies aim to restore SMN expression, limitations highlight the need for alternative strategies. We investigated haloperidol (HALO), a classical antipsychotic, as a potential therapeutic based on its ability to enhance SMN2 splicing and SMN expression. Using the delta 7 SMA mouse model, we assessed effects of HALO on survival, motor function, neuroprotection, and neuroi

Longitudinal multi-omics profiling of spinal muscular atrophy.

Spinal muscular atrophy (SMA) is an autosomal recessive neuromuscular disorder caused by SMN1 gene variants, leading to the degeneration of anterior horn cells in the spinal cord. It is a disabling disease with varying severity. Nusinersen, the first approved in France, has dramatically transformed SMA management. However, the significant variability in patient response and disease progression highlights a critical need for objective, measurable indicators. This study aims to identify biomarkers in cerebrospinal fluid (CSF) and plasma associated with the clinical status of treatment-naive pati

KIF5A downregulation in spinal muscular atrophy links axonal regeneration defects with ALS.

Spinal muscular atrophy (SMA) is a devastating neuromuscular disorder caused by mutations in the survival motor neuron 1 (SMN1) gene leading to decreased SMN protein levels and motor neuron dysfunction. SMN-restoring therapies offer clinical benefit, but the downstream molecular consequences of SMN reduction remain incompletely understood. SMN deficiency resulted in downregulation of kinesin heavy chain isoform 5A (KIF5A) in human neurons and in a mouse model of SMA. SMN associated with KIF5A mRNA and contributed to its stability. Reduced SMN levels impaired axon regeneration, which was rescue

A Translational Roadmap for Neurological Nonsense Mutation Disorders.

Nonsense mutations, responsible for ~11% of gene lesions causing human monogenic diseases, introduce premature termination codons (PTCs) that lead to truncated proteins and nonsense-mediated mRNA decay (NMD). In the central nervous system (CNS), these mutations drive severe, progressive neurological conditions such as spinal muscular atrophy, Rett syndrome, and Duchenne muscular dystrophy. Readthrough therapies-strategies to override PTCs and restore full-length protein expression-have evolved from early aminoglycosides to modern precision tools including suppressor tRNAs, RNA editing, and CRI

Efficient induction of motor neuron disease in transgenic G93A SOD1 mice by prion-like seeding.

Mutations in superoxide dismutase 1 (SOD1) cause paralysis in familial amyotrophic lateral sclerosis and promote its misfolding into neurotoxic aggregates. Previous studies have shown that mice expressing the ALS-causing G85R variant of SOD1 develop paralysis much faster after intraspinal injection of spinal homogenates from paralysed G85R SOD1 mice. These findings, and other studies in cell models, established the prionoid templating properties of misfolded mutant SOD1. Previously, however, we noted that the widely used Gur1-G93A SOD1 mice, which express at high levels and develop paralysis b
