Differentiating early Parkinson's disease from multiple system atrophy on multimodal PET/MRI using atlas-guided segmentation and vision transformer classification.

INTRODUCTION: Early differentiation between Parkinson's disease (PD) and multiple system atrophy-parkinsonian type (MSA-P) remains clinically challenging because of overlapping motor symptoms and subtle imaging abnormalities. METHODS: We developed a fully automated multimodal PET/MRI framework for early PD-MSA-P differentiation that integrates atlas-guided subject-specific label generation, nnU-Net segmentation of the caudate nuclei and putamina, multimodal image fusion, and Vision Transformer classification with multi-slice voting (ViT+MSV). A total of 155 early-stage patients, including 121

Pearls & Oy-sters: Reversible Leukoencephalopathy and Parkinsonism Due to CNS Involvement in Cryoglobulinemia.

We present a case of reversible leukoencephalopathy and parkinsonism associated with type III mixed cryoglobulinemic vasculitis in a patient who initially presented with systemic vasculitic symptoms such as rash and arthralgias. Neither hepatitis C nor monoclonal gammopathy-typical triggers of cryoglobulinemia-was identified. The neurologic syndrome evolved with tremor, rigidity, significant cognitive decline, and extensive white matter abnormalities on MRI, mimicking primary neurodegenerative conditions such as multiple system atrophy. Diagnostic workup ruled out infectious, metabolic, autoim

Clinically advanced NLRP3 inhibitor modulates microglial transcriptome and alleviates α-synuclein-induced progression of parkinsonism.

BACKGROUND: Parkinson’s disease (PD), the second most common neurodegenerative disorder after Alzheimer’s disease, and the rare disorder multiple system atrophy (MSA), are both characterized by intracellular accumulation of α-synuclein fibrils and early, sustained microglial reactivity in parallel to the neurodegeneration. Activation of the NLRP3 inflammasome in disease-associated reactive microglia is increasingly recognized as a key pathogenic driver and a promising therapeutic target in synucleinopathies. Dapansutrile (OLT1177®) is a selective, orally bioavailable NLRP3 inhibitor with a fav

Cerebrospinal fluid HOMER1 and NPTX2 as candidate signatures in early-stage multiple system atrophy-parkinsonism.

Different morphological and pathological features of abnormal α-synuclein in synucleinopathies-multiple system atrophy (MSA) and Lewy body diseases, including Parkinson's disease (PD) and dementia with Lewy bodies (DLB)-may underlie disease-specific synaptic dysfunction. To compare synaptic profiles associated with abnormal α-synuclein, we examined temporal lobe tissues from patients with MSA (N = 4), DLB (N = 5), and controls (N = 9). We further analysed cerebrospinal fluid (CSF) samples from patients with early-stage MSA-parkinsonism (MSA-P: N = 8) and PD (N = 8) to determine whether synapti

Comparative transcriptomics reveal molecular convergence and divergence in parkinsonian disorders.

Neurodegenerative parkinsonisms are phenotypically diverse disorders including Parkinson's disease, dementia with Lewy bodies, multiple system atrophy, progressive supranuclear palsy, and corticobasal degeneration. These currently incurable diseases are characterized by overlapping but also distinct clinicopathological and molecular features, and largely unknown pathogenesis. Transcriptomic studies have offered insights, but are limited by small sample sizes, technical variability, and limited reproducibility. Here, we present a large comparative bulk transcriptomic analysis of parkinsonian sy

Peripheral Immunological Signatures of Parkinsonism: Insights from the Caribbean vs. North America.

The contribution of the peripheral immune system to Parkinson's disease (PD) and Atypical parkinsonism (AP), including multiple system atrophy, progressive supranuclear palsy, and Caribbean atypical parkinsonism, is increasingly recognized, albeit incompletely resolved. Our study aimed to compare the peripheral immunological profile of PD and AP patients from two distinct geographical areas, Guadeloupe and Montreal. We conducted a cross-sectional study on PD and AP patients and healthy controls from Guadeloupe and Montreal (total 57 subjects). We collected clinical assessments and arbovirus se

Amplitude of low-frequency fluctuation differences in resting-state functional magnetic resonance imaging between Parkinson's disease and multiple system atrophy-parkinsonian type.

BACKGROUND: Parkinson's disease (PD) and Multiple system atrophy-parkinsonian type (MSA-P) both belong to α-synucleinopathies. Their overlapping clinical manifestations often lead to misdiagnosis. This study aimed to identify objective neuroimaging biomarkers for differentiating PD from MSA-P. METHODS: 25 PD patients, 20 MSA-P patients, and 20 healthy controls (HC) were enrolled. All participants underwent resting-state functional magnetic resonance imaging (rs-fMRI). Differences in the amplitude of low-frequency fluctuation (ALFF) across brain regions between the PD and MSA-P groups were ana

The PET tracer [11C]MODAG-005 targets alpha-synuclein aggregates in the brain.

Synucleinopathies are neurodegenerative diseases characterized by the presence of brain inclusions containing the pathologically aggregated protein α-synuclein. The development of a positron emission tomography tracer to detect aggregates of misfolded α-synuclein could revolutionize early diagnosis, disease monitoring, and the evaluation of therapeutic efficacy. Here, we present the development, preclinical validation, and first-in-human evaluation of [11C]MODAG-005. In vitro binding experiments demonstrated subnanomolar binding affinity to recombinant α-synuclein fibrils and to α-synuclein in

Generation of an induced pluripotent stem cell line (NIMHi28-A) from a parkinsonian-type multiple system atrophy patient of Indian origin.

Multiple System Atrophy (MSA) is a rare, adult-onset, and rapidly progressive neurodegenerative disorder characterized by motor dysfunction and autonomic failure, with an estimated prevalence of 1.9-4.9 per 100,000 individuals per year. Clinically, MSA is broadly classified into two major phenotypes: cerebellar-dominant (MSA-C) and Parkinsonian-dominant (MSA-P). In this study, induced pluripotent stem cells (iPSCs) were generated from peripheral blood mononuclear cells of a clinically diagnosed MSA-P patient using Sendai virus-mediated reprogramming. Detailed characterization demonstrated robu

Structural MRI Volumetry Index for Differentiation of Progressive Supranuclear Palsy From Parkinson's Disease and Multiple System Atrophy by Automatic Segmentation: A Comparison With Magnetic Resonance Parkinsonism Index.

BACKGROUND: Although Parkinson's disease (PD) and Parkinsonism-Plus syndromes (PPS) exhibit distinct brain atrophy, current clinical differentiation heavily relies on manual, plane-specific 2D metrics, which are labor-intensive and observer-dependent. To fill this gap, this study developed an automated, software-independent metric-the parkinsonism volumetric index (PDSVI)-calculated as [(pons volume × third ventricle volume)/(midbrain volume × thalamus volume) × 10]. PURPOSE: To evaluate the performance of PDSVI in precisely differentiating progressive supranuclear palsy (PSP) from PD, multip

Substantia Nigra Susceptibility-To-Volume Ratio Derived From QSM as a Marker for Discrimination of Progressive Supranuclear Palsy From Parkinson's Disease and Multiple System Atrophy.

BACKGROUND: Progressive supranuclear palsy (PSP) is both an underdiagnosed and a frequently misdiagnosed disorder. To remedy these diagnostic limitations, MRI has been used to investigate brain morphology to differentiate PSP from Parkinson's disease (PD) and multiple system atrophy (MSA). However, while nigrostriatal degeneration and tau aggregation are prominent in PSP, and result in iron accumulation and atrophy in the region, substantia nigra (SN) atrophy remains an underexplored diagnostic marker. PURPOSE: To investigate the diagnostic utility of QSM-derived SN parameters for PSP. STUDY

Patterns of iron deposition and classification in parkinsonian syndromes: a quantitative susceptibility mapping study.

Differential diagnosis of parkinsonian syndromes remains challenging because of overlapping clinical symptoms. We characterized brain iron deposition using whole-brain quantitative susceptibility mapping (QSM) and developed internally validated exploratory machine learning (ML) classifiers. This single-center retrospective study included 119 patients with Parkinson's disease (PD), 62 with multiple system atrophy-parkinsonian type (MSA-P), 25 with progressive supranuclear palsy (PSP), and 56 healthy controls (HC). Voxel-wise and region-of-interest (ROI) analyses were performed, and classifiers

Residential radon in Peru: challenges for health surveillance and lung cancer prevention.

The Journal of Genetics, started by William Bateson in 1910, played a distinguished role in the early years of genetics. However, it stopped publishing in 1978. The Indian Academy of Sciences revived it in 1985, and has published it regularly since then. To commemorate this landmark, I highlight one of the 17 articles published that year.
