Multiple system atrophy (MSA) is a progressive neurodegenerative disease characterized by parkinsonism, cerebellar ataxia, and autonomic dysfunction. Recently, a close relationship of neuroinflammation to MSA pathogenesis was identified. Studies in mouse models of MSA have indicated that peripheral-type myeloid cells and endogenous microglia promote neuroinflammation in MSA. However, their distinct roles in MSA pathogenesis remain unknown. We performed immunohistochemical analyses of brain tissues obtained at autopsy from patients with MSA and age-matched controls to examine the putamen and ce

Spinocerebellar ataxia type 41 (SCA41) is a rare autosomal dominant cerebellar ataxia caused by mutations in the transient receptor potential canonical 3 (TRPC3) gene. We report a case of a patient with SCA41 whose clinical manifestations were initially suspected to be multiple system atrophy cerebellar-type (MSA-C). Whole-exome sequencing (WES) revealed a c.1955A>G (p.K652R) mutation in the TRPC3 gene of this patient. After treatment with transcranial magnetic stimulation and rehabilitation training, the patient reported a slight improvement in unsteady gait compared with before. This is the

Hypomyelination and atrophy of basal ganglia and cerebellum (H-ABC) is a rare leukodystrophy associated with causal variants in β-tubulin 4A (TUBB4A). The recurring variant p.Asp249Asn (D249N) presents in infancy with dystonia, communication deficits, and loss of ambulation during the first decade of life. In this study, we characterized a genetic murine series (Tubb4aKO/KO, Tubb4aD249N/+, Tubb4aD249N/KO, and Tubb4aD249N/D249N) to demonstrate that disease severity correlates with the expression of mutant Tubb4a and relative preservation of wild-type tubulin. To further evaluate the translation

INTRODUCTION: The thyrotropin-releasing hormone (TRH) analog protirelin has short-term benefits for cerebellar ataxia (CA), but its long-term efficacy and the subtype-specific responsiveness of spinocerebellar degeneration (SCD) patients remain unclear.
METHODS: We retrospectively reviewed the records of 92 SCD patients (273 courses) treated with protirelin between July 2011 and October 2025, including patients with idiopathic CA (IDCA), multiple system atrophy with predominant CA (MSA-C) or parkinsonism (MSA-P), and genetic subtypes including spinocerebellar ataxia (SCA) type 6, SCA31 and de

BACKGROUND: Pontocerebellar hypoplasia type 2A (PCH2A) is a rare autosomal recessive neurodegenerative disease caused by a specific pathogenic variant in the TSEN54 gene (p.A307S). Affected children show early but initially unspecific symptoms, diagnosed primarily through postnatal magnetic resonance imaging (MRI), with confirmation by genetic testing. This study examines the diagnostic process and key considerations for accurate diagnosis.
METHODS: We retrospectively collected data from 65 children (33 girls, 32 boys) with genetically confirmed PCH2A as part of a Natural History Study. Data

OBJECTIVE: To systematically evaluate the efficacy and safety of mesenchymal stem cell (MSC) therapy for patients with Multiple System Atrophy (MSA) by synthesising available clinical trial evidence and clarifying signals of disease modification.
BACKGROUND: MSA is a rapidly progressive and fatal neurodegenerative disorder for which no disease-modifying therapies exist. MSC therapy has emerged as a potential treatment, with mechanisms centered on neuroprotection and clinical benefit through anti-inflammatory and trophic effects rather than direct cell replacement.
METHODS: We systematically

BACKGROUND: A two-year-old female patient was referred to our clinic due to speech and gait disturbances, strabismus, vacant staring, truncal hypotonia, and spasticity in the extremities. The patient had a history of a complicated delivery resulting in perinatal asphyxia. Electrocardiogram (ECG), echocardiogram (ECHO) and abdominal ultrasound findings reported no abnormalities. Previously performed spinal muscular atrophy (SMA) test, chromosomal microarray and karyotype analyses yielded normal results. On clinical examination, facial dysmorphic features included: prominently low-set ears, stra

This report details a case of a patient in his 60s who exhibited ataxic gait, frequent falls, limb incoordination, tremors and rigidity, leading to a diagnosis of progressive supranuclear palsy with cerebellar predominance (PSP-C). Clinical assessment indicated slurred speech, increased muscle tone, deep tendon reflexes that were mildly reduced asymmetrically on the left side and reduced strength, while sensations remained intact. PSP-C is an uncommon variant of PSP often misidentified as multiple system atrophy-cerebellar type (MSA-C) because of shared characteristics. The patient participate

BACKGROUND AND PURPOSE: The glymphatic system is a glial-dependent perivascular clearance pathway that maintains central nervous system homeostasis through cerebrospinal fluid and interstitial fluid exchange. Emerging evidence suggests that glymphatic dysfunction contributes to the pathophysiology of neurodegenerative disorders involving cerebellar networks. This review summarizes current evidence linking glymphatic dysfunction to cerebellar neurodegeneration, with a particular focus on spinocerebellar ataxia type 3, multiple system atrophy, progressive supranuclear palsy, and essential tremor

BACKGROUND: No disease-modifying treatments exist for the cerebellar subtype of multiple system atrophy (MSA-C). Because transcranial alternating current stimulation (tACS) has demonstrated safety and efficacy in spinocerebellar ataxia type 3, a cerebellar disorder, we tested whether tACS improves clinical outcomes in MSA-C.
METHODS: We conducted a randomized, double-blind, placebo-controlled trial of tACS in MSA-C. Participants received daily 40-min sessions of 70 Hz, 2 mA tACS or sham stimulation, 5 days/week for 2 weeks. The primary outcome was the change from baseline to 2 weeks in the Un

INTRODUCTION: Parkinson's disease (PD), pure autonomic failure (PAF), and multiple system atrophy (MSA) feature intracellular deposition of alpha-synuclein and catecholamine deficiency in the putamen or heart. This retrospective, cross-sectional, observational study assessed relationships of cerebrospinal fluid alpha-synuclein seed amplification assay (CSF SAA) data with catecholamine deficiency indicated by positron emission tomography (PET).
METHODS: In groups with PD, PAF, parkinsonian MSA (MSA-P), or cerebellar MSA (MSA-C) SAAs were conducted by Rocky Mountain Laboratories (RML) and Ampri

BACKGROUND: Parkinson's disease (PD) and multiple system atrophy (MSA) are both characterized by postural instability, increasing the risk of falls and impairing quality of life. This study aimed to compare intermittent control strategies between PD and MSA during static and dynamic standing, focusing on changes in active controller parameters that simulate central nervous system (CNS) responses.
METHOD: Postural stability was evaluated in 17 healthy controls, 61 PD patients, 40 MSA-C (cerebellar type) patients, and 26 MSA-P (parkinsonian type) patients using an intermittent control model und

BACKGROUND: Parkinson's disease (PD) and Multiple system atrophy-parkinsonian type (MSA-P) both belong to α-synucleinopathies. Their overlapping clinical manifestations often lead to misdiagnosis. This study aimed to identify objective neuroimaging biomarkers for differentiating PD from MSA-P.
METHODS: 25 PD patients, 20 MSA-P patients, and 20 healthy controls (HC) were enrolled. All participants underwent resting-state functional magnetic resonance imaging (rs-fMRI). Differences in the amplitude of low-frequency fluctuation (ALFF) across brain regions between the PD and MSA-P groups were ana

Synucleinopathies are neurodegenerative diseases characterized by the presence of brain inclusions containing the pathologically aggregated protein α-synuclein. The development of a positron emission tomography tracer to detect aggregates of misfolded α-synuclein could revolutionize early diagnosis, disease monitoring, and the evaluation of therapeutic efficacy. Here, we present the development, preclinical validation, and first-in-human evaluation of [11C]MODAG-005. In vitro binding experiments demonstrated subnanomolar binding affinity to recombinant α-synuclein fibrils and to α-synuclein in

Multiple System Atrophy (MSA) is a rare, adult-onset, and rapidly progressive neurodegenerative disorder characterized by motor dysfunction and autonomic failure, with an estimated prevalence of 1.9-4.9 per 100,000 individuals per year. Clinically, MSA is broadly classified into two major phenotypes: cerebellar-dominant (MSA-C) and Parkinsonian-dominant (MSA-P). In this study, induced pluripotent stem cells (iPSCs) were generated from peripheral blood mononuclear cells of a clinically diagnosed MSA-P patient using Sendai virus-mediated reprogramming. Detailed characterization demonstrated robu

BACKGROUND: Although Parkinson's disease (PD) and Parkinsonism-Plus syndromes (PPS) exhibit distinct brain atrophy, current clinical differentiation heavily relies on manual, plane-specific 2D metrics, which are labor-intensive and observer-dependent. To fill this gap, this study developed an automated, software-independent metric-the parkinsonism volumetric index (PDSVI)-calculated as [(pons volume × third ventricle volume)/(midbrain volume × thalamus volume) × 10].
PURPOSE: To evaluate the performance of PDSVI in precisely differentiating progressive supranuclear palsy (PSP) from PD, multip

BACKGROUND: Progressive supranuclear palsy (PSP) is both an underdiagnosed and a frequently misdiagnosed disorder. To remedy these diagnostic limitations, MRI has been used to investigate brain morphology to differentiate PSP from Parkinson's disease (PD) and multiple system atrophy (MSA). However, while nigrostriatal degeneration and tau aggregation are prominent in PSP, and result in iron accumulation and atrophy in the region, substantia nigra (SN) atrophy remains an underexplored diagnostic marker.
PURPOSE: To investigate the diagnostic utility of QSM-derived SN parameters for PSP.

INTRODUCTION: Idiopathic late-onset cerebellar ataxia (ILOCA) is a degenerative, non-hereditary form of adult-onset cerebellar ataxia. A subset of patients with ILOCA eventually develop multiple system atrophy of the cerebellar type (MSA-C), but imaging biomarkers that predict this transition remain unidentified. This study aimed to identify imaging biomarkers that may predict phenoconversion from ILOCA to MSA-C.
METHODS: A retrospective cohort of patients diagnosed with ILOCA who underwent baseline T1-weighted and diffusion-weighted magnetic resonance imaging (MRI) was analyzed. Patients wer
