HIV-1 diagnosis during pre-exposure prophylaxis: A case report.

We report a case of HIV diagnosis in a 36-year-old man who has sex with men receiving daily oral PrEP (emtricitabine/tenofovir) in Argentina, where viral load or HIV PCR testing is not routinely available before or during prophylaxis. In 2022, PrEP was initiated after a diagnosis of primary syphilis, with a non-reactive fourth-generation HIV test performed 13 days earlier (outside the recommended window), so very early infection already present at initiation cannot be excluded. Self-reported adherence was >95%. Serology was negative at day 23 and became reactive at day 73, with an indeterminat

Syphilis Screening in Pregnancy During Emergency Department Gonorrhea and Chlamydia Testing.

INTRODUCTION: Congenital syphilis is rising in the United States, especially among infants born to women with limited prenatal care. Emergency departments (ED) offer opportunities for early detection, particularly among patients evaluated for urogenital sexually transmitted infection (STI) concerns. In October 2019, our health system implemented an ED STI order set that promoted HIV co-testing and included a preselected option for syphilis testing. In this study, we evaluated change in syphilis testing during the post-implementation period among ED encounters with both pregnancy and gonorrhea/

One Dose versus Three Doses of Benzathine Penicillin G in Early Syphilis in People Living with HIV: A Systematic Review and Meta-Analysis.

Despite the current recommendations of the CDC and IUSTI, controversy persists regarding the adequate dose and duration of benzathine penicillin G (BPG) for early syphilis in people living with HIV (PLWH). PubMed, Embase, and Cochrane were searched from inception to October 2025 for studies comparing a single-dose versus a 3-dose BPG regimen in PLWH with early syphilis. A random-effects meta-analysis was performed using risk ratios (RRs) with 95% confidence intervals (CIs). The primary outcome included serologic cure, defined as a ≥4-fold (2-dilution) decline from baseline in antibody titers m

Demographic and clinical factors associated with Syphilis treatment initiation in a tribal health system: a retrospective cohort study.

BACKGROUND: American Indian/Alaska Native (AI/AN) individuals are disproportionately burdened by syphilis compared with other racial and ethnic groups. Early syphilis treatment is important to reduce transmission and prevent negative health outcomes. Understanding the characteristics of individuals who test positive and the factors associated with initiating treatment will support syphilis prevention and treatment efforts. METHODS: Cherokee Nation conducted a retrospective cohort study of individuals who tested positive for syphilis between 2016 and 2023. Data were collected from electronic h

Australian Gonococcal Surveillance Programme Annual Report, 2025.

The Australian Gonococcal Surveillance Programme (AGSP) has continuously monitored antimicrobial resistance in Neisseria gonorrhoeae (NG) for more than 45 years through the system of jurisdictional Neisseria reference laboratories, the National Neisseria Network (NNN). In 2025, a total of 10,204 isolates across Australia, from public and private sectors, were tested for in vitro antimicrobial susceptibility by standardised methods. In 2025, the AGSP captured antimicrobial susceptibility data for 23.9% of all gonococcal infection notifications nationally. The current treatment recommendation fo

Congenital syphilis as a failure of the maternal-fetal prevention cascade: Clinical implications for pregnant migrant and structurally vulnerable women in high-income settings - an ESCMID study group for infections in Travellers and migrants (ESGITM) perspective.

BACKGROUND: Congenital syphilis remains a preventable but persistent cause of fetal loss, neonatal morbidity, and early-life complications. In high-income settings, its occurrence rarely reflects the absence of diagnostic tools or effective therapy; but more often exposes missed opportunities across antenatal care, sexually transmitted infection prevention, maternal treatment, partner management, and neonatal follow-up. This review investigates congenital syphilis from the viewpoint of the possible failures of the maternal-fetal prevention pathway, with a specific focus on pregnant migrant and

A Lip Mass Revealing Syphilis: A Case Report With a Review of the Literature.

BACKGROUND: Syphilis is a chronic systemic infection caused by Treponema pallidum and is well known for its wide spectrum of clinical presentations. Oral manifestations are uncommon and often mimic benign or reactive lesions, making diagnosis challenging, particularly in the absence of overt systemic symptoms. CASE PRESENTATION: We report a case of a 26-year-old male who presented with a nonpainful swelling of the lower lip that had persisted for 2 months, with a differential diagnosis including both reactive and neoplastic processes, and underwent a biopsy for definitive diagnosis. Histopath

Mpox and sexually transmitted infections: syndemic drivers of transmission and disease outcomes-A Narrative Review.

The worldwide outbreak of mpox (formerly monkeypox) in 2022 emerged as a public health emergency posing greater threat to persons with sexually transmitted infections (STIs). This outbreak featured an increase in overlap between mpox and STIs reflecting evolving transmission dynamics in sexual networks. Epidemiological data reveal that STIs such as human immunodeficiency virus (HIV), Chlamydia trachomatis, herpes simplex virus, syphilis have been frequently associated with mpox infection highlighting the role of sexual contact in its transmission. A narrative review was conducted using PubMed

Immunoglobulin deposition and plasma cell infiltration define distinct immunopathological patterns in skin diseases.

Immunoglobulin deposition in the skin is an important diagnostic and etiological feature in many skin diseases. However, the relationship between deposited immunoglobulins and the local presence of antibody-producing cells, such as B cells and plasma cells, remains poorly understood. In several autoimmune diseases, including systemic lupus erythematosus and bullous pemphigoid, marked immunoglobulin deposition is observed despite limited B-cell infiltration into the skin, suggesting that the deposited antibodies primarily originate as circulating autoantibodies. In contrast, diseases such as di

[Atypical presentation of ocular syphilis].

We report the case of a 55-year-old man who presented with a sudden decrease in visual acuity in the left eye, revealing ocular syphilis associated with asymptomatic neurosyphilis. Ophthalmological examination showed mild anterior uveitis of the left eye associated with focal peripheral retinitis displaying a ground-glass appearance, multiple creamy-white superficial retinal precipitates, and obliterative retinal arteritis. Multimodal imaging, particularly optical coherence tomography (OCT), demonstrated full-thickness retinal involvement with retinal layer disorganization, retrohyaloid inflam

Associations Between Maternal Mental Health and Congenital Syphilis Birth Outcomes: Analysis of Georgia Surveillance Data, 2018-2023.

BACKGROUND: Congenital syphilis (CS) is a preventable yet persistent public health problem in the United States. Maternal mental health conditions (MMHC) during pregnancy may contribute to missed prevention opportunities, but their association with CS outcomes is unclear. METHODS: Georgia sexually transmitted infections (STI) surveillance data (2018-2023) were analyzed to assess prebirth MMHC mentions among syphilis-affected pregnancies, their association with CS, and variation by maternal household composition as a proxy for immediate social support. MMHC mentions were identified using rule-

Clinical and serological outcomes of syphilis: an 18-year retrospective cohort study on predictors of treatment response and seroreversion in Türkiye.

PURPOSE: Despite established protocols, the global resurgence of syphilis remains a public health challenge, with outcomes influenced by disease stage and host factors. This study described 18-year epidemiological trends and identified predictors of serological treatment response and long-term seroreversion. METHODS: We retrospectively analyzed 503 adults diagnosed with syphilis between 2005 and 2022 and classified disease stages according to CDC criteria. Treatment response was defined as a ≥ 4-fold decline in rapid plasma reagin (RPR) titers within 12 months for early-stage and 24 months fo

Syphilis Screening Among Commercially-Insured Pregnant Women in the United States.

BACKGROUND: Congenital syphilis (CS) incidence increased by 81.7% in the United States (US) between 2020 and 2024. Syphilis screening is recommended at the first prenatal visit and at delivery, but poor compliance may lead to inadequate screening and preventable CS cases. METHODS: Using a US-wide collection of longitudinal healthcare claims data, we identified a cohort of commercially-insured pregnant women aged 15-44 years. Women were included if they had a pregnancy resulting in a live birth, stillbirth, or spontaneous abortion during 2017-2021. We described patterns of syphilis screening,

CDC AIDS Project 72: The Prostitute Study.

In the early years of the AIDS epidemic, the occurrence and potential for heterosexual transmission of human immunodeficiency virus type 1 (HIV 1) in the United States were uncertain. To better understand infection among women and the possible role of prostitution in the emerging epidemic, the Centers for Disease Control and Prevention established CDC AIDS Project 72, a multisite study of 1 410 women with recent histories of prostitution recruited from eight areas of the United States during 1986 and 1987. Participants were interviewed, examined, and tested for serologic markers of HIV 1, hepa

Congenital Syphilis: Strategies for Prevention.

Once nearly eradicated, rates of syphilis among pregnant people have increased over 200% and rates of congenital syphilis have increased over 750% since the 2010s. Syphilis disproportionately affects historically marginalized populations, including racial and ethnic minorities, and those with barriers to prenatal care. Syphilis is caused by the spirochete Treponema pallidum and is spread through sexual contact or through vertical transmission in utero. Syphilis during pregnancy can lead to negative outcomes for the pregnant person, adverse pregnancy outcomes, and congenital syphilis for the ch

Positive Syphilis Serology With Concurrent Disseminated Talaromycosis marneffei in a Patient With HIV: A Case Report.

BACKGROUND Talaromyces marneffei (TM) infection is an opportunistic infection that typically affects individuals with compromised immune systems. TM is an invasive fungal infection associated with high mortality in patients with advanced human immunodeficiency virus (HIV) disease. Syphilis, caused by Treponema pallidum, is a re-emerging infectious disease with diverse clinical manifestations. While primary syphilis often goes unnoticed, secondary syphilis is characterized by systemic symptoms and variable skin lesions, earning it the epithet "the great imitator." In areas where TM is non-endem

Longitudinal quantitative treponemal-specific immunoassay patterns in syphilis reinfection among MSM living with HIV.

OBJECTIVES: Syphilis reinfection is increasing among men who have sex with men (MSM) living with HIV and is frequently asymptomatic, complicating timely diagnosis and management. Current screening strategies rely on multistep serological algorithms that require confirmation with non-treponemal testing, often delaying clinical decision-making. The chemiluminescent microparticle immunoassay for Treponema pallidum (TP-CMIA) is widely used as an initial screening assay and provides quantitative signal-to-cut-off values; however, it is currently designated for qualitative interpretation. We aimed t

An Adenoviral-Vectored Tp0326 Vaccine Elicits Robust Functional Antibodies to Prevent Treponema pallidum Dissemination in a Rabbit Model.

Vaccination can prevent congenital infections by inducing antibodies that block transplacental pathogen transmission. For syphilis, which is caused by Treponema pallidum (T. pallidum), no vaccine is yet available to halt its vertical spread to fetuses. Here, we designed an adenovirus-vectored vaccine expressing the T. pallidum outer membrane protein TP0326 and evaluated its immunogenicity and protective efficacy in animal models. The vaccine elicited robust humoral and Th2‑biased cellular immune responses in both mice and rabbits, with resulting antibodies exhibiting potent anti‑adhesion, opso
