Clinical Characteristics and Risk Factors for Syphilis Among People Living With Human Immunodeficiency Virus: A Nationwide Population-Based Cohort Study in Korea.

BACKGROUND: The incidence of syphilis, a re-emerging infection linked to human immunodeficiency virus (HIV) through shared transmission routes, is increasing among people living with human immunodeficiency virus (PLH) in Korea. This study evaluate the prevalence, temporal trends, and risk factors for syphilis among PLH in the Republic of Korea. METHODS: Data from the National Health Insurance Service of Korea between 2002 and 2021 were analyzed to determine the incidence of syphilis in PLH. Syphilis was defined by the assigned Korean Standard Classification of Diseases code, with prescription

Syphilis in Poland in 2023.

BACKGROUND: In 2023 there was a significant increase in the number of syphilis cases in Poland, more than 50% new cases were reported compare to previous year. There is still a problem with the clinical differentiation of reported cases, i.e. new infections, reinfections, and residual positive serological tests confirming the presence of antibodies against Treponema pallidum in previously successfully treated individuals. OBJECTIVE: The aim of the study was to assess the epidemiological situation of syphilis cases in Poland in 2023 in comparison to previous years. MATERIAL AND METHODS: Analy

Neurosyphilis Presenting With Subtle Clinical Features: A Diagnostic and Therapeutic Challenge.

We report the case of a 36-year-old male who presented with an acute onset of headache, vomiting, and feverish sensation with a history of prior unsafe sexual activity. VDRL and TPHA were reactive in both the blood and cerebrospinal fluid. A diagnosis of neurosyphilis was thus made, and we successfully treated the patient with ceftriaxone and doxycycline. With the increasing global trend of syphilis, the involvement of the central nervous system, neurosyphilis, is an increasing concern too. Due to the vague nonspecific symptoms and variable timing of presentation, it presents as a diagnostic c

Clinician awareness, attitudes and prescribing practices relating to doxycycline post-exposure prophylaxis (DoxyPEP) in Ireland: Cross-sectional survey of sexual health professionals.

BackgroundDoxycycline post-exposure prophylaxis (DoxyPEP) has emerged as a biomedical strategy to reduce bacterial sexually transmitted infections (STIs), yet implementation remains contested due to antimicrobial resistance concerns and limited clinical guidance in many European settings. Evidence on clinician readiness to prescribe DoxyPEP is limited. This study examined awareness, attitudes and prescribing practices among sexual health clinicians in the Republic of Ireland.MethodsA cross-sectional anonymous online survey was conducted among sexual health clinicians in early 2026. Optimal testing frequency for sexually transmitted infections among men who have sex with men and transgender women who use HIV pre-exposure prophylaxis in Australia, Brazil and Thailand: a cost-effectiveness analysis.

BACKGROUND: Pre-exposure prophylaxis (PrEP) is highly effective in preventing HIV, but not other sexually transmitted infections (STIs) in men who have sex with men (MSM) and transgender women (TGW). PrEP users are offered periodic STI testing for early detection of STIs, including Chlamydia trachomatis (CT), Neisseria gonorrhoeae (NG) and syphilis. However, the optimal and most cost-effective frequency for separate and combined STI testing in PrEP users has yet to be established. METHODS: We constructed dynamic transmission models and simulated the epidemic trajectories of three STIs (CT, NG

Syphilitic Tonsillitis: A Case Report and Review of the Literature.

Syphilitic tonsillitis is an uncommon manifestation of Treponema pallidum infection that may present with nonspecific oropharyngeal symptoms and mimic other infectious or neoplastic processes. We report a case of a 30-year-old man who presented with a two-month history of sore throat, dysphagia, low-grade fever, and hoarseness. Clinical examination revealed bilateral tonsillar hypertrophy with ulcerated papules and cervical lymphadenopathy. A tonsillectomy was performed due to poor treatment response. Histopathology showed mucosal ulceration with architectural effacement due to marked paracort

A Case of Neuro- and Otosyphilis Successfully Managed with a Sequential Regimen Including Ceftriaxone.

Neurosyphilis is caused by the invasion of Treponema pallidum into the central nervous system (CNS), which is characterized with complex and varied clinical manifestations and lacks early symptom specificity. It usually occurs when syphilis is untreated or poorly treated and can occur in any stage of syphilis, potentially causing severe neurological damage. Otosyphilis, characterized by auditory-vestibular dysfunction, may manifest independently without cerebrospinal fluid (CSF) abnormalities, or coexist with neurosyphilis, as a part of the neurological symptoms of neurosyphilis, and is partic

Syphilitic Panuveitis and Rhegmatogenous Retinal Detachment: Diagnostic Pitfalls and Treatment Considerations.

Syphilitic panuveitis is a severe and diagnostically highly challenging manifestation of ocular syphilis. Its predominant posterior-segment involvement and its tendency to mimic noninfectious or viral uveitis may delay etiologic recognition and increase the risk of permanent vision loss. Rhegmatogenous retinal detachment (RRD) is a rare but vision-threatening complication that likely reflects advanced, inflammation-induced disruption of the vitreoretinal interface. A narrative literature review was conducted using the PubMed, Scopus, and Web of Science databases (January 2000 to 10 September 2

A retrospective review of screening for HIV, syphilis and anaemia on first antenatal care visits at a midwife-run obstetric unit in South Africa.

BACKGROUND: Early enrolment for antenatal care (ANC) is an important strategy in improving perinatal and maternal outcomes. Treatable conditions in pregnancy, such as anaemia, human immunodeficiency virus (HIV), and syphilis can be identified and treated in the early antenatal period, thereby improving maternal and foetal outcomes. However, late enrolment for ANC is a concerning problem, especially in high HIV-prevalence settings, such as South Africa. The aim of this study was to evaluate screening of pregnant women fro HIV, syphilis and anaemia on the first antenatal visit at a midwife-run o

Mpox in HIV-positive patients: A retrospective case series from Saudi Arabia highlighting clinical characteristics and outcomes.

BACKGROUND: Mpox has re-emerged globally since 2022, disproportionately affecting people living with HIV (PLHIV). While infection in immunocompetent individuals is usually mild and self-limited, severe and occasionally fatal disease has been reported among individuals with advanced immunosuppression. Regional data describing mpox-HIV coinfection in the Middle East remain limited. METHODS: We conducted a retrospective descriptive case series at a tertiary care center in Riyadh, Saudi Arabia. Adults diagnosed with laboratory-confirmed mpox and newly identified HIV infection between July 2022 an

[Neurosyphilis].

Neurosyphilis is a central nervous system infection caused by Treponema pallidum and can occur at any stage of syphilis, presenting with diverse neurological and psychiatric manifestations. Although once considered a disease of the past, the recent global resurgence of syphilis has renewed the clinical relevance of neurosyphilis. Early manifestations include meningitis, cranial neuropathies, and meningovascular disease, whereas late-stage disease is characterized by parenchymal involvement, such as general paresis and tabes dorsalis. Diagnosis requires clinical suspicion and is based on combin

Syphilitic proctitis presenting as rectal bleeding.

Syphilitic proctitis is an uncommon presentation of Treponema pallidum infection and can closely resemble other anorectal conditions. Due to its non-specific symptoms, it is often underdiagnosed, particularly in individuals without systemic manifestations. A male patient in his 30s presented with isolated rectal bleeding. Sigmoidoscopy revealed multiple superficial ulcers over the folds of the anal canal, along with erythematous rectal mucosa and focal erosions. Given the patient's history of unprotected sexual intercourse, serological testing was performed. The Venereal Disease Research Labor
