Botulinum Toxin Treatment in Musician's Dystonia: Long-Term-Effects and Influencing Factors.

Musician's Dystonia (MD) is a task-specific movement disorder that occurs while playing an instrument and severely limits a musician's ability to perform. While currently no causal therapies are available, symptomatic therapies, particularly the injection of botulinum toxin (BoNT), offer a therapeutic option. The present study presents a 30-year longitudinal evaluation of the clinical course of 156 patients with MD treated with BoNT at a specialist outpatient facility. The evaluation took into account known risk factors, such as family history, time to treatment, initial severity of symptoms,

Clinical outcomes after pallidotomy for dystonia: A pilot study of lesion volume and topography.

BACKGROUND: Deep Brain Stimulation(DBS) of the globus pallidus internus(GPi) is the current surgical standard for medically refractory dystonia; pallidotomy offers a cost-effective alternative. OBJECTIVES: To evaluate clinical outcomes, safety, and radiological correlates of radiofrequency pallidotomy in patients with medically refractory generalised or segmental dystonia. METHODS: Twenty-one patients who underwent unilateral or bilateral radiofrequency pallidotomy (2017-2024) were included in an ambispective analysis. Outcomes were measured using Burke-Fahn-Marsden Dystonia Rating Scale mot

The heterogeneity of thalamic tremor: report of two cases.

BACKGROUND: Thalamic lesions are a well-recognized cause of secondary tremor, most commonly manifesting as a classic Holmes tremor (HT). This phenotype typically combines postural, kinetic, and rest components and may be accompanied by dystonic features, reflecting disruption of thalamo-cortical motor networks. CASE REPORTS: We report two patients with tremor following unilateral thalamic vascular lesions showing different motor phenotypes. The first patient developed a low-frequency, large-amplitude tremor of the left upper limb after a right lateral thalamic ischemic lesion, consistent with

Child Neurology: Early-Onset, Rapidly Progressive, and Refractory DYT-TOR1A Status Dystonicus.

Primary (genetic) pediatric dystonia, characterized by sustained or intermittent muscle contractions that cause abnormal movements or postures, affects 16.4 in 100,000 children worldwide. It can be caused by various factors, including pathogenic variants in genes such as TOR1A, associated with DYT-TOR1A dystonia. In this report, we present the case of a toddler with DYT-TOR1A treated with deep brain stimulation (DBS) because of his early-onset, rapidly progressive, and refractory status dystonicus. Genetic testing confirmed a maternally inherited pathogenic variant in TOR1A, c.907_909del (p.Gl

Intellectual disability, neuroregression and adult-onset progressive dystonia due to a DLG4 pathogenic variant.

We report a 34-year-old male with childhood developmental delay, severe intellectual disability in adulthood, episodes of agitation with a previous diagnosis of schizoaffective disorder and adult-onset cognitive regression who developed progressive generalised dystonia due to a de novo DLG4 pathogenic loss-of-function variant. This case expands the phenotypic spectrum of recognised movement disorder manifestations associated with DLG4-related synaptopathy.

The dystonia-associated Torsin1A sustains CLCC1 function in membrane fusion of the nuclear envelope for NPC biogenesis.

DYT1 dystonia is an incurable movement disorder caused by a loss-of-function mutation in Torsin1A, an endoplasmic reticulum (ER)-resident AAA+ ATPase. Here, we use Drosophila and human cells to shed light on Torsins' mode of action. Fly germ cells lacking dTorsin arrest in development with defects in nuclear pore complex (NPC) biogenesis due to impaired nuclear envelope membrane fusion. We identify the conserved membrane protein Chloride Channel CLIC-like protein 1 (CLCC1) as a Torsin1A interaction partner whose absence phenocopies membrane fusion defects caused by Torsin deletion. CLCC1 is en

Facio-Brachial Dystonic Seizures as an Atypical Presentation of Anti-IgLON5 Disease.

Anti-IgLON5 disease is a rare neurological disorder with overlapping autoimmune and neurodegenerative mechanisms and a strong HLA haplotype association. It typically presents with sleep disturbances, though bulbar and neuropsychiatric symptoms contribute to clinical heterogeneity. The definitive diagnostic hallmark is the detection of anti-IgLON5 IgG antibodies. A 74-year-old female was initially admitted with pneumonia and persistent encephalopathy. Continuous video EEG monitoring captured multiple brief episodes of facial contraction with dystonic posturing of the left upper extremity, consi

Three-Day Bi-Hemispheric tDCS With Motor Training Shows No Improvement in Pianist's With Musician's Dystonia: A Pilot Study.

Focal hand dystonia in musicians is a task-specific movement disorder that can impair professional performance, and effective rehabilitative treatments remain limited. Prior work suggested that bi-hemispheric transcranial direct current stimulation (tDCS) paired with bimanual mirror-movement training may transiently improve fine motor control in affected pianists. This case-control pilot study tested whether extending this approach to three consecutive days would produce detectable and sustained benefit. Ten professional pianists with unilateral musician's dystonia and 10 matched healthy piani

Abnormal head movements in neurological conditions: a knowledge-based dataset with application to cervical dystonia.

Abnormal head movements (AHMs) manifest across a broad spectrum of neurological disorders, however the absence of a multi-condition resource integrating kinematic measurements, clinical severity scores, and patient demographics constitutes a persistent barrier to the development of AI-driven diagnostic tools. To address this gap, this study introduces NeuroPose-AHM, a knowledge-based dataset of neurologically induced AHMs constructed through a multi-LLM extraction framework applied to 1,430 peer-reviewed publications. The dataset contains 2,763 patient-group-level records spanning 57 neurologi

Emergency Department Presentation of Duloxetine-induced Acute Extrapyramidal Symptoms: A Case Report.

INTRODUCTION: Duloxetine, a serotonin-norepinephrine reuptake inhibitor, has been associated with extrapyramidal symptoms and tardive syndromes; however, such adverse reactions are rare and remain sparsely documented, particularly in the setting of non-psychiatric use. CASE REPORT: A young, healthy, active-duty military service member developed acute extrapyramidal symptoms-restlessness, dystonic movements, and acute dystonia-shortly after initiating duloxetine for postsurgical neuropathic pain. CONCLUSION: This case highlights the need for vigilance regarding movement disorders in patients

[Genetic Dystonias].

Dystonia is a movement disorder characterized by abnormal, sustained or intermittent movements and/or postures, which are often exacerbated by voluntary action and associated with overflow phenomena. The current classification is structured along two axes: a clinical axis, which considers features such as age at onset and pattern of distribution, and an etiological axis, which integrates genetic, acquired and anatomical causes. Genetic dystonias constitute a broad and heterogeneous group, frequently beginning in childhood or adolescence, and may present as isolated, combined or complex forms.

Oculogyric Crisis During Chronic Aripiprazole Therapy: A Diagnostic Challenge in the Emergency Department.

Oculogyric crisis can occur even during chronic, stable aripiprazole therapy without recent dose escalation. In patients with acute upward eye deviation and an otherwise normal neurologic examination, medication review is key to recognizing drug-induced dystonia and avoiding unnecessary neurologic workup.

Identifying High-Risk Medications for Drug-Induced Dystonia: A 20-Year Retrospective Real-World Pharmacovigilance Study Based on FAERS.

BACKGROUND AND AIMS: Drug-induced dystonia is a serious, potentially disabling adverse event (AE) associated with certain medications. Despite its clinical relevance, the existing literature is largely limited to case reports or analyses of individual drugs, and a systematic evaluation of medications implicated in dystonia remains lacking. This study undertook one of the first comprehensive screenings and risk signal rankings of drugs linked to dystonia using data from the FDA adverse event reporting system (FAERS), aiming to inform clinical drug safety. METHODS: Reports of dystonia-related A

Exploring Risk Drugs and Mechanisms in Dystonia: Insights from Pharmacovigilance and Proteogenomics.

INTRODUCTION: Dystonia is a movement disorder (MD), which is the third most common MD after Parkinson's disease and essential tremor. Although drugs are one of the main risk factors for dystonia, they are often not fully recognized. This study aims to identify drugs related to dystonia and explore the potential molecular mechanism of drug-induced dystonia. METHODS: We used disproportionality analysis to analyze the data of the FDA Adverse Event Reporting System (FAERS) to identify risk drugs associated with dystonia. The molecular target information of these drugs comes from the DrugBank data
